1Effects of glutamine on intestinal permeability and bacterial translocation in TPN-rats with endotoxemia显示文摘AIM: To evaluate the protective effect and mechanism ofglutamine on the intestinal barrier function in totalparenteral nutrition (TPN) rats with trauma or endotoxemia. METHODS: To perform prospective, randomized andcontrolled animal experimentation of rats with surgicaltrauma, TPN and endotoxemia, thirty-four male, adultSprague Dawley rats were divided into four groups: control group (n=8), TPN group (n=9), trauma and endotoxemia group (LPS, n=8) and trauma plus endotoxemia supplemented with glutamine in TPN solution group (Gin.group, n=9). All groups except the control group were given TPN solutions in 7-day experimental period. For Gin group, 1 000 mg/kg/d of glutamine was added to TPN solution during day 1-6. On the 7th day all the animals were gavaged with lactulose (66 mg) and mannitol (50 mg)in 2 mi of normal saline. Then 24 h urine with preservative was collected and kept at -20 ℃. On day 8, under intraperitoneal anesthesia using 100 mg/kg ketamin, the intestine, liver, mesenteric lymph nodes and blood were taken for examination. RESULTS: The body weight of LPS group decreased most among the four groups. The structure of small intestinal mucosa in TPN group, LPS group and Gln group showed impairments of different degrees, and the damage of small intestinal mucosa in Gln group was remarkably alleviated.The concentrations of interleukins in small intestine mucosa were lower (for IL-4 and IL-6) or the lowest (IL-10) in Gln group. The IgA level in the blood plasma and the mucosa of Gln group was the highest among all of the groups. The urine lactulose/mannitol test showed that the intestinal permeability in LPS group was lower than that in TPN group (P<0.001), but there was no difference between LPS group and Gln group. The rate of bacterial translocation in Gln group was lower than that in LPS group (P<0.02). 2Experimental study on antitumor effect of arsenic trioxide in combination with cisplatin or doxorubicin on hepatocellular carcinoma显示文摘INTRODUCTIONThe main component of a traditional Chinese drug 'Pishuang'. arsenic trioxide (As2O3), has obviously selective anti-tumor effect on human hepatocellular carcinoma (HCC)in both in vitro and in vivo studies[1-5]. Due to limited effectiveness when any anti-carcinogen is used alone and obviously increased toxicity when the dose is raised, there is no exception for As2O3. Furthermore, combined chemotherapy contributes to improve therapeutic effectiveness, disperse toxicity and surmount drug-resistance,in which the combination of traditional Chinese and modern medicine has more advantages and characteristics. 3Clinical effects of continuous high volume hemofiltration on severe acute pancreatitis complicated with multiple organ dysfunction syndrome显示文摘AIM: To investigate the efficiency of continuous high volume hemofiltration (HVHF) in the treatment of severe acute pancreatitis (SAP) complicated with multiple organ dysfunction syndrome (MODS).METHODS: A total of 28 SAP patients with an average of 14.36±3.96 APACHE Ⅱ score were involved. Diagnostic criteria for SAP standardized by the Chinese Medical Association and diagnostic criteria for MODS standardized by American College of Chest Physicians (ACCP) and Society of Critical Care Medicine (SCCM) were applied for inclusion. HVHF was started 6.0±6.1 (1-30) days after onset of the disease and sustained for at least 72 hours, AN69 hemofilter (1.2 m2)was changed every 24 hours. The ultrafiltration rate during HVHF was 4 000 mi/h, blood flow rate was 250-300 mi/min,and the substitute fluid was infused with pre-dilution. Low molecular weight heparin was used for anticoagulation.RESULTS: HVHF was well tolerated in all the patients, and lasted for 4.04±3.99 (3-24) days. 20 of the patients survived,6 patients died and 2 of the patients quited for financial reason.The ICU mortality was 21.4%. Body temperature, heart rate and breath rate decreased significantly after HVHF.APACHE Ⅱ score was 14.4±3.9 before HVHF, and 9.9±4.3after HVHF, which decreased significantly (P<0.01). Partial pressure of oxygen in arterial blood before HVHF was 68.5±19.5 mmHg, and increased significantly after HVHF,which was 91.9±25 mmHg (P<0.01). During HVHF the hemodynamics was stable, and serum potassium, sodium,chlorine, glucose and pH were at normal level.CONCLUSION: HVHF is technically possible in SAP patients complicated with MODS. 4Intestinal permeability in patients after surgical trauma and effect of enteral nutrition versus parenteral nutrition显示文摘AIM: To study the intestinal permeability (IP) following stress of abdominal operation and the different effects on IP of enteral nutrition (EN) and parenteral nutrition (PN).METHODS: Forty patients undergoing abdominal surgery were randomized into EN group and PN group. Each group received nutritional support of the same nitrogen and calorie from postoperative day (POD) 3 to POD 11. On the day before operation (POD-1), POD 7 and POD 12, 10 g of lactulose and 5 g of mannitol were given orally, and urine was collected for 6 hours. Urine excretion ratios of lactulose and mannitol (L/M) were measured.RESULTS: L/M ratios of EN group on POD-1, POD 7 and POD 12 were 0.026±0.017, 0.059±0.026, 0.027±0.017,respectively, and those of PN group were 0.025±0.013,0.080±0.032, 0.047±0.021, respectively. Patients of both groups had elevated L/M ratios on POD 7 vs. POD-1. 5Influence of acute hyperglycemia in human sepsis on inflammatory cytokine and counterregulatory hormone concentrations显示文摘AIM: In human sepsis, a prominent component of the hypermetabolite is impaired glucose tolerance (IGT) and hyperglycemia. Elevations in plasma glucose concentration impair immune function by altering cytokine production from macrophages. We assessed the role of glucose in the regulation of circulating levels of insulin, glucagon, cortisol,IL-6 and TNF-α in human sepsis with normal or impaired glucose tolerance.METHODS: According to the results of intravenous glucose tolerance test, forty patients were classified into two groups: control group (n=20) and IGT group (n=20).Plasma glucose levels were acutely raised in two groups and maintained at 15 mmol/L for 3 hours. Plasma insulin,glucagon and cortisol levels were measured by radioimmunoassay, the levels of TNF-α and IL-6 were detected by ELISA.RESULTS: In IGT group, the fasting concentrations of plasma glucose, insulin, glucagon, cortisol, IL-6 and TNFα levels were significantly higher than those in control group (P<0.05). During clamp, the control group had a higher average amount of dextrose infusion than the IGT group (P<0.01). In control group, plasma insulin levels rose from a basal value to a peak at an hour (P<0.05) and maintained at high levels. Plasma glucagon levels descended from a basal value to the lowest level within an hour (P<0.01)and low levels were maintained throughout the clamp. In IGT group, plasma insulin was more significantly elevated (P<0.01), and plasma glucagon levels were not significantly declined. Plasma cortisol levels were not significantly changed in two groups. In control group, plasma IL-6 and TNF-α levels rose (P<0.01) within 2 hours of the clamp and returned to basal values at 3 hours. In IGT group, increased levels of plasma cytokine lasted longer than in control group (3 hours vs. 2 hours, P<0.05), and the cytokine peaks of IGT group were higher (P<0.05) than those of control group.CONCLUSION: Acute hyperglycemia pricks up hyperinsulinemia and increases circulating cytokine concentrations and these effects are more pronounced in sepsis with IGT. 6Ureaplasma urealyticum infection and apoptosis of spermatogenic cells显示文摘Aim: To study the relationship between ureaplasma urealyticum (UU) infection and apoptosis of human spennato-genie cells. Methods: Spermatogenic cells were observed under light microscope with Wright-Giemsa staining andby means of terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate (dUTP)-biotin nick-end labeling(TUNEL) technique. Results: Apoptotic rate of UU-infected males ( 15.5%±6.8% ) was significantly higherthan that of controls (5.2%±2.3 % ). 7Role of laparoscopic subtotal cholecystectomy in the treatment of complicated cholecystitis显示文摘BACKGROUND: Laparoscopic cholecystectomy (LC) has become the 'gold standard' in treating benign gallbladder diseases. Increasing laparoscopic experience and techniques have made laparoscopic subtotal cholecystectomy (LSC) a feasible option in more complex procedures. In recent years, few studies with a few cases of LSC have reported good results in patients with various types of cholecystitis. This study was designed to evaluate the feasibility, indications, characteristics and benefits of LSC in patients with complicated cholecystitis. METHODS: Altogether, 3485 patients were scheduled to receive LC during the past 4 years at our institute. Among them, 168 patients with various complicated forms of cholecystitis were treated by LSC. Meanwhile, the other 3317 patients who received standard LC were enrolled as the control group. Perioperative data from the two groups were collected and retrospectively analyzed. RESULTS: In the LSC group, 135 patients suffered from acute calculic cholecystitis, 18 from chronic calculic cholecystitis with cirrhotic portal hypertention, and 15 from chronic calculic atrophy cholecystitis with severe fibrosis. These patients constituted 4.8% of the total patients who underwent LC (168/3485) in the same period at our institute. In 122 patients, the cystic duct and artery were clipped before division. In another 46 patients, the gallbladder was initially incised at Hartmann’s pouch. Five patients (3.0%) were converted to open subtotal cholecystectomy. The median operation time for LSC was 65.5±15.2 minutes, estimated operative blood loss was 71.5±15.5 ml, and the time to resume diet was 20.4±6.3 hours. Thirteen patients (7.7%) had local complications. The mean postoperative hospital stay was 4.2±2.6 days. In the LC group, 2887 had chronic calculic cholecystitis, 312had acute calculic cholecystitis, 47 had chronic calculic atrophy cholecystitis, and 71 had polypus. Seventeen patients (0.5%) were converted to open cholecystectomy. The median operation time was 32.6±10.2 minutes, the estimated operative blood loss was 24.5±8.5 ml, and the time to resume diet was 18.3±4.5 hours. Thirty- nine patients (1.2%) had local complications. Mean postoperative hospital stay was 3.8±1.4 days. There was no bile duct injury or mortality in either group. CONCLUSIONS: LSC for patients with complicated cholecystitis is difficult, with a longer operation time, more operative blood loss and higher conversion and complication rates than LC. However, it is feasible and relatively safe. LSC is advantageous over open surgery, but it remains a non-routine choice. 8Application of laparoscopic cholecystectomy in patients with cirrhotic portal hypertension显示文摘BACKGROUND: Laparoscopic cholecystectomy (LC) has been widely adopted in treating benign gallbladder disea- ses. Cirrhosis and cirrhotic portal hypertension (CPH) are contraindicated for LC in its early period. In recent years, several studies have reported liberal use of LC in patients with cirrhosis. But its benefits and successful use in patients with CPH are less documented. This study was designed to evaluate the feasibility, safety and technical characteristics of LC in CPH patients. METHODS: In 38 patients with symptomatic gallbladder disease and CPH, 19 belonged to Child A class, 15 Child B class and 4 Child C class. Perioperative data of these pa- tients were collected and analyzed. RESULTS: LC was successfully performed in 36 patients, and 2 patients (5.3%) were converted to open cholecystec- tomy (OC) for difficulty in management of bleeding under laparoscopy and dense adhesion of Calot’ s triangle. The sur- gical time was 62.6±15.2 minutes. The estimated amount of intraoperative hemorrhage was 75.5±15.5 ml. No blood transfusion was necessary. The time to resume diet was 18.3 ±6.5 hours. Seven postoperative complications in 5 patients (13.2%) included port-site infection (1 patient), respiratory infection (2), upper digestive tract bleeding (1), slight hepatic encephalopathy (1) and increased ascites (2). All patients were cured and discharged from the hospital within 5.6±2.4 days after LC. CONCLUSIONS: Despite LC is difficult for CPH patients, it is feasible and relatively safe. 9Prognosis of microscopic polyangiitis with renal involvement:report of 60 Chinese patients显示文摘With the widespread clinical application of renal biopsy and serum antineutrophil cytoplasmic antibody (ANCA) testing, the percentage of renal injuries caused by systemic small vessel vasculitis (SVV) including microscopic polyangiitis (MPA) and Wegener’ s granulomatosis (WG) is on the rise in China. According to our previous report, SVV constituted 1.09% of diagnoses from all renal biopsies (5. 8% in secondary nephritis) and 14. 4% of that from renal biopsies from patients with chronic renal,failure, among which MPA is the most common type. Many studies reported that SVV with rapid progressive glomerulonephritis had poor prognosis with a 5-year renal survival less than 75%. However, there is a lack of large-sample statistical analysis about the long-term prognosis of SVV in China. Indeed, the clinical manifestations of SVV in China are quite different from those reported in other countries. 11Pathogenesis and treatment of parenteral nutrition-associated liver disease显示文摘BACKGROUND: Parenteral nutrition-associated liver disease (PNALD) has been common in patients who require long-term parenteral nutrition. PNALD develops in 40%-60% of infants on long-term parenteral nutrition compared with 15%-40% of adults on home parenteral nutrition for intestinal failure. The pathogenesis of PNALD is multifactorial and remains unclear There is no specific treatment. Management strategies for its prevention and treatment depend on an understanding of many risk factors. This review aims to provide an update on the pathogenesis and treatment of this disease. DATA SOURCES: A literature search was performed on the MEDLINE and Web of Science databases for articles published up to October 2011, using the keywords: parenteral nutrition associated liver disease, intestinal failure associated liver disease lipid emulsions and fish oil. The available data reported in the relevant literatures were analyzed. RESULTS: The literature search provided a huge amount of evidence about the pathogenesis and management strategies on PNALD. Currently, lack of enteral feeding, extended duration of parenteral nutrition, recurrent sepsis, and nutrient deficiency or excess may play important roles in the pathogenesis of PNALD. Recent studies found that phytosterols present as contaminants in soy-based lipid emulsions, are also an important factor in the pathogenesis. Moreover, the treatment of PNALD is discussed. CONCLUSIONS: The use of lipid emulsions, phytosterols in particular, is associated with PNALD. Management strategies for the prevention and treatment of PNALD include consideration of early enteral feeding, the use of specialized lipid emulsions such as fish oil emulsions, and isolated small bowel or combined liver and small bowel transplantation. A greater understanding of the pathogenesis of PNALD has led to promising interventions to prevent and treat this condition. 12A study on pathogenicity of hepatitis G virus显示文摘AIM To study the pathogenicity of hepatitis G virus (HGV) and observe the genesis and pathological process of hepatitis G.METHODS HGV-RNA in serum was detected by RT-PCR assay. The immunohistochemical assays of liver tissue were performed with HGV monocoloned antibody (McAb)expressed from the region of HGV NS5 nucleic acid sequence. The clinical and pathological data of 52 patients with hepatitis G were discussed. In animal experiment,the Chinese Rhesus monkeys were infected with the serum of a patient with HGV infection. And the dynamic changes in serology and liver histology of animals were observed.RESULTS One hundred and fifty-four patients with HGVRNA positive were selected from 1552 patients with various kinds of hepatitis. Of 154 patients with HGV infection, 52 were infected with HGV only, which accounted for 33.8% (52/154) and 102 with positive HGVRNA were super-infected with other hepatitis viruses,which accounted for 66.2% (102/154). The clinical and pathological observation showed that the acute and chronic hepatitis could be induced by HGV. 13The effects of the formula of amino acids enriched BCAA on nutritional support in traumatic patients显示文摘AIM: To investigate the formula of amino acid enriched BCAA on nutritional support in traumatic patients after operation.METHODS: 40 adult patients after moderate or large abdominal operations were enrolled in a prospective,randomly and single-blind-controlled study, and total parenteral nutrition (TPN) with either formula of amino acid (AA group, 20 cases) or formula of amino acid enriched BCAA (BCAA group, 20 cases). From the second day after operation, total parenteral nutrition was infused to the patients in both groups with equal calorie and equal nitrogen by central or peripheral vein during more than 12 hours per day for 6 days. Meanwhile, nitrogen balance was assayed by collecting 24 hours urine for 6 days. The markers of protein metabolism were investigated such as amino acid patterns, levels of total protein, albumin, prealbumin,transferrin and fibronectin in serum.RESULTS: The positive nitrogen balance in BCAA group occurred two days earlier than that in AA group. The serum levels oftotal protein and albumin in BCAA group were increased more obviously than that in AA group. 14The study of chemiluminescence in gastric and colonic carcinoma cell lines treated by anti-tumor drugs显示文摘AIM: To study the influence of chemotherapy on proliferationactivation of tumor cell by observing the change ofchemiluminescence (CL) and cell cycle in various tumor celllines after mitomycin C treated.METHODS: BGC823 and LoVocell lines were all cultured inRPMI-1640, and then were adjusted to a concentration of1x105 cells/ml in fresh media.and incubated for 24 h.Mitomycin C (100 ng@ml-1) was added to each bottle. Allindeses were examined after 24 h. No Mitomycin C wasadded in control group. Each group contained 8 samples.Flow cytometric analysis and luminol-dependent CL wereused to investigate the effect of mitomycin C on twogastrointestinal carcinoma cell lines.RESULTS: BGC823 and LoVo cell lines incubated with MMCfor 24 h. We discovered that the emergence of peak of CLstimulated by PHA was postponed significantly (BGC823:12.63±3.21 vs 4.50±1.04, LoVo: 13.25+2.96 vs 5.12±1.36,P<0.01) and the peak intension of CL was reduced significantly(BGC823:120.25±16.61 vs248.38±29.17, LoVo: 98.13±10.49vs267.50±18.56, P<0.01).The PI of cell lines was decreasedsignificantly (BGC823:51.87±4.82 vs 25.44±2.26, LoVo:47.11±1.04 vs 24.23±0.37, P<0.01) and the apoptoticfractions changed by contraries (BGC823:26.25+5.29 vs9.83±2.51, LoVo: 33.50±3.68 vs9.63±1.44, P<0.01).CONCLUSION: CL can be used to measure activation oftumor cells. We discovered that the ground CL intensions oftwo cell lines were not high but increased rapidly afterstimulation of PHA. The CL peak ranged from 4-5 minutes,and then decreased gradually. The results were not reportedbefore. CL of tumor cell has close correlativity with thedynamics of cell cycle and can reflect the feature of oxidationmetabolism and proliferation activation of tumor cell. 15Study on Retinal Ganglion Cell Apoptosis after Explosive Injury of Eyeballs in Rabbits显示文摘Purpose: To explore the pathological changes of retinal ganglion cell apoptosis and its relation to the value of glutamate concentration in vitreous body after explosive injury of eyeballs in the rabbits.Method: Similar explosive injury models of eyeballs in 10 adult grey rabbits were made.The rabbits were killed on scheduled time. The retinal tissues of studied eyes and control eyes were obtained for the pathological examination with TUNEL method respectively.The value of glutamate in vitreum of injured eyes was measured and was compared with that of contralateral eyes. Statistical comparison analysis on the experiment data was performed.Result: The value of glutamate in vitreum of injured eyes was significantly higher than that of contralateral eyes in all rabbits in the study. A lot of TUNEL positive cells were observed in the injured eyes. 16Effect of Seawater Soaking on Explosive Cornea Injury显示文摘Objective: To observe the changes in corneal tissue after explosive corneal injury and the effect of seawater soaking on the healing of cornea after explosive injury with the help ofoptical microscope. Methods: Make 10 similar explosive injury models of rabbit′s eyeball using 10 adult greyrabbits. For each rabbit, both eyes are artificially injured through explosion; its right eye is the comparison eye and after the injury the left eye is soaked in seawater for 30 minutes.Conduct slit lamp examinations, fluorescein dyeing, and ultrasonic cornea thickness metering on the corneas at intervals, that is, before the injury, 1 , 2, 3, 4, 5, 7, 9, 12, 15days after the injury respectively. Examine the corneas under the optical microscope andmake comparisons.Results: The corneal thickness of the experiment eye becomes apparently thicker than thatof the comparison eye after the injury. The corneal clouding of the former apparentlyaggravates compared with the latter. The healing of corneal epithelium in the injured eye isslower compared with that in the comparison eye. Comparative examination under the opticalmicroscope shows: after the injury, the corneal tissue of the experiment eye changesdistinctly, its healing is delayed and the vascularization degree becomes high in cornealstroma.Conclusion: Scars and vascularization of various degrees will appear in corneal stroma afterexplosive injuries. 17Use of endoscopic naso-pancreatic drainage in the treatment of severe acute pancreatitis显示文摘AIM: To review the experience on the use of endoscopic nasopancreatic drainage (ENPD) in the treatment of severe acute pancreatitis (SAP).METHODS: Since March 1998, under the regular management of SAP with non-operative method, ENPD has been randomly used in 14 patients. The average age of the patients was 41.3±15.9 (years), with 8 males and 6 females.The time from onset to admission was 32.9±22.8 (hours). 8cases were found to have gallbladder stone. The daily output of pancreatic fluid was measured. The body temperature,heart rate, WBC count, blood glucose, blood calcium, PaO2,blood and urine levels of amylase were detected on the fifth day and compared with their respective data on the first day.Therapeutic results and hospitalization times were recorded.RESULTS: The time of drainage was 7.3±4.0 days. The daily drainage outputs of the first five days were 236.4±176.6,287.1±164.7, 284.6±216.4, 435.0±357.8 and 377.8±223.8 ml,respectively. The decreases in body temperature, heart rate,WBC counts, blood and urine levels of amylase and the increase in PaO2 were significant on the fifth day when compared with those on the first day. Infection of pancreatic necrosis was found in one patient and controlled by antiinfectives. 6 out of 8 patients with gallbladder stone were operated during hospital stay. All patients were cured and diischarged and the average hospital stay was 28.1±11.6 days.CONCLUSION: ENPD is an effective method for the drainage of pancreatic fluid and might have an important role in the treatment of SAP. 18Color Doppler Imaging in the Study of Retrobulbar Hemodynamic Changes of Primary Angle-closure Glaucoma显示文摘Purpose: To investigate the hemodynamic changes of ophthalmic and central retinal artery (OA,CRA) in Primary Angle-closure Glaucoma (PACG) and the effects of intraocular pressure (TOP) on the retrobulbar hemodynamics.Methods: The hemodynamic changes of OA, CRA in PACG were explored with color Doppler imaging (CDI) technique. Peak systolic blood flow velocity (PSV), end-diastolic blood flow velocity (EDV),pulsatile and resistive indices (PI,RI) were recorded and compared with those of the normal subjects. There were 20 patients (40 eyes) with PACG who were divided into two groups according to their IOP levels; ocular hypertension (15 eyes) and normal tension (25 eyes) .Results : Compared with the normal subjects, the hypertension group of PACG showed significant reduction in the EDV and increases in PI, RI of CRA; normal tension group of PACG also showed remarkable increase in RI of CRA. 19A novel hepatitis B virus mutant with A-to-G at nt551 in the surface antigen gene显示文摘AIM: Hepatitis B surface antigen (HBsAg) mutant of hepatitisB virus (HBV) is one of the important factors that result inimmune escape and cause failure of immunization. In thisstudy we reported and characterized a novel HBV mutantwith A-to-G at nt551 and intended to provide theoreticaldata for prevention of HBV infection in China.METHODS: A methodology comprising polymerase chainreaction (PCR) amplifying, M13 bacteriophage cloning andnucleotide sequencing was used to analyze the sera of thepediatric patient who was hepatitis B (HB) immune failure.Expression plasmids containing the mutant S gene and awild-type (adr) S gene were constructed respectively andthe recombinant HBsAg were expressed in COS-7 cells underthe regulation of SV40 early promoter. The recombinantproteins were investigated for their immunological reactivitywith different monoclonal antibodies (mAb) against 'a'determinant and vaccine-raised human neutralizingantibodies.RESULTS: It was found that there was a new point mutationat nt551 of the HBV (adr) genome from A to G, leading to asubstitution of methionine (Met) to valine (Val) at position133 in the 'a' determinant of HBsAg.
