| 1 | Functional analysis and drug response to zinc and D-penicillamine in stable ATP7B mutant hepatic cell lines显示文摘AIM: To study the effect of anti-copper treatment for survival of hepatic cells expressing different ATP7 B mutations in cell culture. METHODS: The most common Wilson disease(WD) mutations p.H1069 Q, p.R778 L and p.C271*, found in the ATP7 B gene encoding a liver copper transporter, were studied. The mutations represent major genotypes of the United States and Europe, China, and India, respectively. A human hepatoma cell line previously established to carry a knockout of ATP7 B was used to stably express WD mutants. m RNA and protein expression of mutant ATP7 B, survival of cells, apoptosis, and protein trafficking were determined.RESULTS: Low temperature increased ATP7 B protein expression in several mutants. Intracellular ATP7 B localization was significantly impaired in the mutants. Mutants were classified as high, moderate, and no survival based on their viability on exposure to toxic copper. Survival of mutant p.H1069 Q and to a lesser extent p.C271* improved by D-penicillamine(DPA) treatment, while mutant p.R778 L showed a pronounced response to zinc(Zn) treatment. Overall, DPA treatment resulted in higher cell survival as compared to Zn treatment; however, only combined Zn + DPA treatment fully restored cell viability. | 2 | Epidemiological link between obesity,type 2 diabetes mellitus and cancer显示文摘There exists a complex interaction between obesity,type 2 diabetes mellitus(T2DM)and cancer,and an increase in the incidence of cancer is expected with the growing obesity-diabetes pandemic.The association of cancer with diabetes mellitus and obesity appears to be site-specific,the highest risk being for postmenopausal breast cancer,endometrial cancer,and colorectal cancer.Moreover,there is worsening of hyperglycaemia with the onset of cancer,evidencing a bidirectional link between cancer and diabetes mellitus and the need for monitoring for diabetes in cancer survivors.In this review,we look at the epidemiological evidence from observational studies and Mendelian randomization studies linking obesity,diabetes,and cancer,as well as the complex pathophysiological mechanisms involved,including insulin resistance with associated hyperinsulinaemia,the effect of chronic low-grade inflammation,and the effect of various adipokines that are associated with obesity and T2DM.Additionally,we describe the novel therapeutic strategies,based on their role on the discrete pathophysiological mechanisms involved in the tumourigenesis. | Cornelius J Fernandez Annu Susan George Nikhila A Subrahmanyan Joseph M Pappachan | 2021 | World Journal of Methodology2021,11,3: | 2 |

| 13 | Evaluating role of bone marrow-derived stem cells in dry age-related macular degeneration using multifocal electroretinogram and fundus autofluorescence imaging显示文摘AIM:To evaluate the role of bone marrow-derived stem cells in the treatment of advanced dry age-related macular degeneration(AMD) using multifocal electroretinogram(mf-ERG) and fundus autofluorescence imaging.METHODS:Thirty patients(60 eyes) with bilateral central geographic atrophy(GA) were recruited.Worse eye of each patient received autologous bone marrow-derived hematopoietic stem cells(BM-HSCs)(group 1) and the fellow eye with better visual acuity served as control(group 2).The effect of stem cell therapy was determined in terms of visual acuity,amplitude and implicit time in mf-ERG and size of GA on fundus autofluorescence imaging.These tests were performed at presentation and first,third and sixth month follow up.Adverse events(if any) were also monitored.RESULTS:At 6 mo follow-up there was no statistically significant improvement in median log MAR best corrected visual acuity(BCVA) in either group.Mf-ERG revealed significant improvement in amplitude and implicit time in the intervention group.A significant decrease was also noted in greatest linear dimension(GLD) of GA in the eyes receiving stem cells [6.78±2.60 mm at baseline to 6.56±2.59 mm at 6 mo(P=0.021)].However,no such improvement was noted in the control group.CONCLUSION:Electrophysiological and anatomical improvement in the intervention group sheds light on the therapeutic role of BM-HSCs.Further studies are required to determine the stage of disease at which the maximal benefit can be achieved and to standardize the dose and frequency of stem cell injection. | Atul Kumar Neha Midha Sujata Mohanty Annu Chohan Tulika Seth Varun Gogia Shikha Gupta | 2017 | International Journal of Ophthalmology(English edition)2017,10,10: | 1 |

| 15 | Gastroenteropancreatic neuroendocrine neoplasms:A clinical snapshot显示文摘Our understanding about the epidemiological aspects,pathogenesis,molecular diagnosis,and targeted therapies of neuroendocrine neoplasms(NENs)have drastically advanced in the past decade.Gastroenteropancreatic(GEP)NENs originate from the enteroendocrine cells of the embryonic gut which share common endocrine and neural differentiation factors.Most NENs are welldifferentiated,and slow growing.Specific neuroendocrine biomarkers that are used in the diagnosis of functional NENs include insulin,glucagon,vasoactive intestinal polypeptide,gastrin,somatostatin,adrenocorticotropin,growth hormone releasing hormone,parathyroid hormone-related peptide,serotonin,histamine,and 5-hydroxy indole acetic acid(5-HIAA).Biomarkers such as pancreatic polypeptide,human chorionic gonadotrophin subunits,neurotensin,ghrelin,and calcitonin are used in the diagnosis of non-functional NENs.5-HIAA levels correlate with tumour burden,prognosis and development of carcinoid heart disease and mesenteric fibrosis,however several diseases,medications and edible products can falsely elevate the 5-HIAA levels.Organ-specific transcription factors are useful in the differential diagnosis of metastasis from an unknown primary of well-differentiated NENs.Emerging novel biomarkers include circulating tumour cells,circulating tumour DNA,circulating micro-RNAs,and neuroendocrine neoplasms test(NETest)(simultaneous measurement of 51 neuroendocrine-specific marker genes in the peripheral blood).NETest has high sensitivity(85%-98%)and specificity(93%-97%)for the detection of gastrointestinal NENs,and is useful for monitoring treatment response,recurrence,and prognosis.In terms of management,surgery,radiofrequency ablation,symptom control with medications,chemotherapy and molecular targeted therapies are all considered as options.Surgery is the mainstay of treatment,but depends on factors including age of the individual,location,stage,grade,functional status,and the heredity of the tumour(sporadic vs inherited).Medical management is helpful to alleviate the symptoms,manage inoperable lesions,suppress postoperative tumour growth,and manage recurrences.Several molecular-targeted therapies are considered second line to somatostatin analogues.This review is a clinical update on the pathophysiological aspects,diagnostic algorithm,and management of GEP NENs. | Cornelius J Fernandez Mayuri Agarwal Biju Pottakkat Nisha Nigil Haroon Annu Susan George Joseph MPappachan | 2021 | World Journal of Gastrointestinal Surgery2021,13,3: | 1 |
