| 2 | Timely reperfusion for ST-segment elevation myocardial infarction:Effect of direct transfer to primary angioplasty on time delays and clinical outcomes显示文摘Primary percutaneous coronary intervention(PPCI) is the preferred reperfusion therapy for patients presenting with ST-segment elevation myocardial infarction(STEMI) when it can be performed expeditiously and by experienced operators. In spite of excellent clinical results this technique is associated with longer delays than thrombolysis and this fact may nullify the benefit of selecting this therapeutic option. Several strategies have been proposed to decrease the temporal delays to deliver PPCI. Among them,prehospital diagnosis and direct transfer to the cath lab,by-passing the emergency department of hospitals,has emerged as anattractive way of diminishing delays. | 3 | Focus on the gut-brain axis: multiple sclerosis, the intestinal barrier and the microbiome显示文摘The brain-gut axis serves as the bidirectional connection between the gut microbiome, the intestinal barrier and the immune system that might be relevant for the pathophysiology of inflammatory demyelinating diseases. People with multiple sclerosis have been shown to have an altered microbiome, increased intestinal permeability and changes in bile acid metabolism. Experimental evidence suggests that these changes can lead to profound alterations of peripheral and central nervous system immune regulation. Besides being of pathophysiological interest, the brain-gut axis could also open new avenues of therapeutic targets. Modification of the microbiome, the use of probiotics, fecal microbiota transplantation, supplementation with bile acids and intestinal barrier enhancers are all promising candidates. | 4 | Transarterial chemoembolization using 40 μm drug eluting beads for hepatocellular carcinoma显示文摘AIM To assess the safety and efficacy of transarterial chemoembolization(TACE) of hepatocellular carcinoma(HCC) using a new generation of 40 μm drug eluting beads in patients not eligible for curative treatment.METHODS Drug eluting bead TACE(DEB-TACE) using a new generation of microspheres(embozene tandem,40 μm) preloaded with 100 mg of doxorubicin was performed on 48 early or intermediate HCC patients with compensatedcirrhosis.Response to therapy was assessed with Response Evaluation Criteria in Solid Tumors(RECIST) and modified RECIST(m RECIST) guidelines applied to computed tomography or magnetic resonance imaging.Eleven out of the 48 treated patients treated progressed on to receive liver orthotopic transplantation(OLT).This allowed for histological analysis on the treated explanted nodules.RESULTS DEB-TACE with 40 μm showed a good safety profile without major complications or 30-d mortality.The objective response rate of treated tumors was 72.6% and 26.7% according to m RECIST and RECIST respectively.Histological examination in 11 patients assigned to OLT showed a necrosis degree > 90% in 78.6% of cases.The overall time to progression was 13 mo(11-21).CONCLUSION DEB-TACE with 40 μm particles is an effective treatment for the treatment of HCC in early-intermediate patients(Barcelona Clinic Liver Cancer stage A/B) with a good safety profile and good results in term of objective response rate and necrosis. | Giorgio Greco Tommaso Cascella Antonio Facciorusso Roberto Nani Rodolfo Lanocita Carlo Morosi Marta Vaiani Giuseppina Calareso Francesca G Greco Antonio Ragnanese Marco A Bongini Alfonso V Marchianò Vincenzo Mazzaferro Carlo Spreafico | 2017 | World Journal of Radiology2017，9，5： | 3 |

| 6 | Analysis of the therapeutic evolution in the management of airway infantile hemangioma显示文摘AIM: To analyze the evolution in the management of airway infantile hemangioma(AIH) and to report the results from 3 pediatric tertiary care institutions.METHODS: A retrospective study of patients with diagnosis of AIH and treated in 3 pediatric tertiary care institutions from 1996 to 2014 was performed. RESULTS: Twenty-three patients with diagnosis of AIH were identified. Mean age at diagnosis was 6 mo(range, 1-27). Single therapy was indicated in 16 patients and 7 patients received combined therapy. Two therapeutic groups were identified: Group A included 14 patients who were treated with steroids, interferon, laser therapy and/or surgery; group B included 9 patients treated with oral propranolol. In group A, oral corticosteroids were used in 9 patients with a good response in 3 cases(no requiring other therapeutic option), the other patients required additional treatment options. Cushing syndrome was observed in 3 patients. One patient died of a fulminant sepsis. Open surgical excision and endoscopic therapy were performed in 11 patients(in 5 of them as a single treatment) with a response rateof 54.5%. Stridor persisted in 2 cases, and one patient died during the clinical course of bronchial aspiration. In group B, oral propranolol was used in 9 patients(in 8 of them as a single treatment) with a response rate of 100%, with an mean treatment duration of 7 mo(range, 5-10); complications were not observed. | 7 | Chemical and spectroscopic characterization of humic substances from sediment and riparian soil of a highly polluted urban river (Suquía River, Cordoba, Argentina)显示文摘The Suquía River,the largest urban river in C ordoba(Argentina),has been severely polluted for decades.Actions must be taken to restore its environmental quality by managing riparian zones for increased water-self purification.The current study aimed to characterize organic matter(OM)dynamics and humic substances(HS)spectrochemical properties along the lower-middle basin of the Suquía River.Riparian soil(0e20 cm)and sediment(0e10 cm)samples were collected from a reference location(S1)and four polluted sites(S2eS5)during a low-flow period.The contents of soil and sedimentary OM and HS fractions were analyzed by wet oxidation,as well as HS Fourier transform infrared(FT-IR)and ultravioletevisible(UVeVis)spectrochemical properties.The OM and HS fractions from riparian soil were high upstream of C ordoba City(S1 and S2,50.2e50.4 g/kg OM)and within a 50 km downstream location(S5,30.9 g/kg OM)owing to a surplus of fresh plant biomass-carbon(C)inputs.Highly heterogeneous sediment samples did not show any significant differences among sites(P>0.05).The lowest values of the ratio of absorbances at 465 and 665 nm(E4/E6)(1.78)and the Δ log K(0.15)coefficient(a measure of HS maturity degree)were obtained downstream of C ordoba City,for both riparian soil and sediment,indicating that HS were enriched by more condensed aromatic structures within highly degraded portions of the river.All samples exhibited similar IR spectra,implying overlapping recalcitrant-C structures at the functional group level,but with different absorbance intensity.Data from the current study constitute a baseline for understanding the chemical nature of HS from sediment and riparian soil along the Suquía River and can be used as a reference for future studies tracking OM compositional changes over time. | Carolina Merlo Carolina Vázquez Ana Graciela Iriarte Carlos Matías Romero | 2020 | International Journal of Sediment Research2020，35，3： | 3 |

| 12 | Lymphoepithelial cysts and cystic lymphangiomas: Underrecognized benign cystic lesions of the pancreas显示文摘AIM: To identify their diagnostic and prognostic clinical characteristics in a large series.METHODS: Retrospective review of clinicopathologic and imaging characteristics of patients diagnosed with lymphoepithelial cysts and cystic lymphangiomas of the pancreas at Massachusetts General Hospital.RESULTS: Twelve patients were identified between 1/1/1997 and 8/1/2007. Their median age was 55.5 years(range 19-78 years), and 6 were females. The le-sion was incidentally discovered in half of the patients.Contrast enhanced computed tomography demonstrat-ed that the cysts had thin walls, without calcifications, pancreatic duct dilation or pancreatic parenchyma inva-sion. Endoscopic ultrasound with fine needle aspiration(EUS/FNA) confirmed the diagnosis of a lymphoepithe-lial cyst in 3 patients, one of whom was spared an op-eration and continues to do well after 6 years. Eleven patients had a resection: 3 pancreaticoduodenecto-mies, 7 distal pancreatectomies, and 1 enucleation. The median size of the cysts was 3 cm(range 2-20 cm). At a median follow-up of 57 mo no recurrences or other pancreas-related conditions occurred.CONCLUSION: Lymphoepithelial cysts and cystic lymphangiomas of the pancreas can be diagnosed with a combination of contrast-enhanced computed tomog-raphy scans and EUS/FNA.
