| 2 | Thalidomide-based multidisciplinary treatment for patients with advanced hepatocellular carcinoma:A retrospective analysis显示文摘AIM:To evaluate the efficacy of thalidomide in combination with other therapies to treat patients with advanced hepatocellular carcinoma (HCC).METHODS:We performed a retrospective analysis of all patients with HCC who were treated with thalidomide for at least two months.The medical records of patients with HCC who were treated at our institution between April 2003 and March 2008 were reviewed.Image studies performed before and after treatment,tumor response,overall survival,and the decrease in α-fetoprotein (AFP) levels were evaluated.RESULTS:A total of 53 patients with HCC received either 100 or 200 mg/d of thalidomide.The patient population consisted of 9 women and 44 men with a median age of 61 years.Thirty patients (56.6%) were classified as Child-Pugh A,and 12 patients (22.6%) were classified as Child-Pugh B.Twenty-six patients had portal vein thrombosis (49.1%),and 25 patients had extrahepatic metastasis (47.1%).The median duration of thalidomide treatment was 6.0 mo.Six of the 53 patients achieved a confirmed response (11.3%),one achieved a complete response (1.9%) and 5 achieved a partial response (9.4%).The disease control rate (CR + PR + SD) was 28.3% (95% CI:17.8-42.4),and the median overall survival rate was 10.5 mo.The 1-and 2-year survival rates were 45% and 20%,respectively.Only one complete response patient showed an improved overall survival rate of 66.8 mo.Sixteen patients (30.2%) showed more than a 50% decrease in their serum AFP levels from baseline,indicating a better response rate (31.3%),disease control rate (43.8%),and overall survival time (20.7 mo).The therapy was well tolerated,and no significant toxicities were observed.CONCLUSION:Thalidomide was found to be safe for advanced HCC patients,demonstrating anti-tumor activity including response,survival,and AFP decreases of greater than 50% from baseline. | Yang-Yuan Chen Hsu-Heng Yen Kun-Ching Chou Shun-Sheng Wu | 2012 | World Journal of Gastroenterology2012，18，5： | 25 |

| 4 | Efficacy and safety of transnasal butorphanol for pain relief after anal surgery显示文摘AIM:To compare the analgesic properties and efficacy of transnasal butorphanol with intramuscular meperidine after anal surgery.METHODS: Sixty patients who underwent fistulectomy were enrolled in the study from January 2006 to December 2007. They were randomly divided into transnasal butorphanol (n=30) or intramuscular meperidine (n=30) treatment groups. Assessment of postoperative pain was made using a visual analogue scale (VAS). The VAS score was recorded 6 h after the completion of surgery, before receiving the first dose of analgesic, 60 min after analgesia and the next morning. Any adverse clinical effects such as somnolence, dizziness, nausea or vomiting were recorded. Satisfaction with narcotic efficacy, desire to use the particular analgesic in the future and any complaints were recorded by patients using questionnaires before being discharged.RESULTS: Forty-two men and eighteen women were included in the study. There were no significant differences in VAS scores between the groups within 24 h. Length of hospital stay and the incidence of adverse effects between the groups were similar. In addition, most patients were satisfied with butorphanol nasal spray and wished to receive this analgesic in the future, if needed.CONCLUSION: Butorphanol nasal spray is effective for the relief of pain after fistulectomy. | 6 | Upper gastrointestinal carcinoid tumors incidentally found by endoscopic examinations显示文摘AIM: This study shares Asian clinical experiences of carcinoid tumors that originated in the upper gastrointestinal tract.METHODS: From May 1987 to June 2002, we had found only 13 cases of histologically confirmed carcinoid tumors in the upper gastrointestinal tract by endoscopic examinations. There were eight males and five females.The mean age was 53.16±20.51 years that ranged from 26 to 82 years. Each of their clinical presentations,locations, tumor morphology, and size and the treatment outcome were analyzed and discussed.RESULTS: One patient had a polypoid lesion at the lower esophagus, nine were stomach lesions and three located at the duodenum. All patients with polypoid and submucosal tumor types were of small size (<1.7 cm) and all patients survived after simple excision or polypectomy.Four of the five patients in tumor mass forms died and the tumors were more than 2.0 cm in size.CONCLUSION: Carcinoid tumors rarely originated from the upper gastrointestinal tract and are usually found accidentally after endoscopic study. | 8 | The status evaluation of the permafrost environment along the Chaida'er-Muli Railway in southern Qilian Mountains in northern Qinghai Province,China显示文摘Engineering construction has major influence on the permafrost environment.This paper analyzes the interaction between engineering construction and permafrost environment along the Chaidaer-Muli Railway(simply,CMR) based on the press-state-response(PSR) framework.The permafrost environmental system is divided into three subsystems,consisting of permafrost thermal stability,proneness to the freeze-thawing erosion and permafrost ecological fragility.Each subsystem considers its most important influencing factors.Catastrophe Progression Method(CPM) is applied to calculate the current environment condition along the railway.The result indicates that:(1) as far as the thermal stability is concerned,most sections along the CMR are mainly concentrated in rank Ⅲ(fair situation),and a few in Ⅱ(good situation) and Ⅳ(bad situation),respectively;(2) for the proneness tothe freeze-thawing erosion,the entire railway route falls largely in rank Ⅱ(good situation);(3) along the CMR,the ecological fragility of the permafrost environment is in rank Ⅱ(good situation),or slightly fragile;(4) overall,the permafrost environments along the CMR are in rank Ⅲ(fair situation) or Ⅱcondition(good situation).In general,the permafrost environment along the CMR is fair.It is mainly because a series of active measures of protecting permafrost were taken for stabilizing the CMR foundation soils.On the one hand,we should try our best to minimize the influences that engineering activities have exerted on ecology and environment,on the other hand,the positive measures have made improvements to prevent the permafrost environment from deterioration. | CAO Wei SHENG Yu CHOU Ya-ling WU Ji-chun LI Jing WANG Sheng-ting | 2016 | Journal of Mountain Science2016，13，12： | 5 |

| 10 | Gene expression profile of peripheral blood in colorectal cancer显示文摘AIM:Optimal molecular markers for detecting colorectal cancer(CRC)in a blood-based assay were evaluated.METHODS:A matched(by variables of age and sex)case-control design(111 CRC and 227 non-cancer samples)was applied.Total RNAs isolated from the338 blood samples were reverse-transcribed,and the relative transcript levels of candidate genes were analyzed.The training set was made of 162 random samples of the total 338 samples.A logistic regression analysis was performed,and odds ratios for each gene were determined between CRC and non-cancer.The samples(n=176)in the testing set were used to validate the logistic model,and an inferred performance(generality)was verified.By pooling 12 public microarray datasets(GSE 4107,4183,8671,9348,10961,13067,13294,13471,14333,15960,17538,and 18105),which included 519 cases of adenocarcinoma and 88 controls of normal mucosa,we were able to verify the selected genes from logistic models and estimate their external generality.RESULTS:The logistic regression analysis resulted in the selection of five significant genes(P<0.05;MDM2,DUSP6,CPEB4,MMD,and EIF2S3),with odds ratios of 2.978,6.029,3.776,0.538 and 0.138,respectively.The five-gene model performed stably for the discrimination of CRC cases from controls in the training set,with accuracies ranging from 73.9%to 87.0%,a sensitivity of 95%and a specificity of 95%.In addition,a good performance in the test set was obtained using the discrimination model,providing 83.5%ac-curacy,66.0%sensitivity,92.0%specificity,a positive predictive value of 89.2%and a negative predictive value of 73.0%.Multivariate logistic regressions analyzed 12 pooled public microarray data sets as an external validation.Models that provided similar expected and observed event rates in subgroups were termed well calibrated.A model in which MDM2,DUSP6,CPEB4,MMD,and EIF2S3 were selected showed the result in logistic regression analysis(H-L P=0.460,R2=0.853,AUC=0.978,accuracy=0.949,specificity=0.818 and sensitivity=0.971).CONCLUSION:A novel gene expression profile was associated with CRC and can potentially be applied to blood-based detection assays. | Yu-Tien Chang Chi-Shuan Huang Chung-Tay Yao Sui-Lung Su Harn-Jing Terng Hsiu-Ling Chou Yu-Ching Chou Kang-Hua Chen Yun-Wen Shih Chian-Yu Lu Ching-Huang Lai Chen-En Jian Chiao-Huang Lin Chien-Ting Chen Yi-Syuan Wu Ke-Shin Lin Thomas Wetter Chi-Wen Chang Chi-Ming Chu | 2014 | World Journal of Gastroenterology2014，20，39： | 4 |
| 11 | Impact of diabetes on promoting the growth of breast cancer显示文摘Background:Type Ⅱ diabetes mellitus(DM2)is a significant risk factor for cancers,including breast cancer.However,a proper diabetic breast cancer mouse model is notwell-established for treatment strategy design.Additionally,the precise diabetic signaling pathways that regulate cancer growth remain unresolved.In the present study,we established a suitable mouse model and demonstrated the pathogenic role of diabetes on breast cancer progression.Methods:We successfully generated a transgenic mouse model of human epidermal growth factor receptor 2 positive(Her2^(+) or ERBB2)breast cancer with DM2 by crossing leptin receptor mutant(Lepr^(db/+))mice with (MMTV-ErbB2/neu)mice.Themousemodelswere administrated with antidiabetic drugs to assess the impacts of controlling DM2 in affecting tumor growth.Magnetic resonance spectroscopic imaging was employed to analyze the tumor metabolism.Results:Treatment with metformin/rosiglitazone in MMTV-ErbB2/Lepr^(db/db) mousemodel reduced serum insulin levels,prolonged overall survival,decreased cumulative tumor incidence,and inhibited tumor progression.Anti-insulin resistance medications also inhibited glycolytic metabolism in tumors in vivo as indicated by the reduced metabolic flux of hyperpolarized ^(13)C pyruvate-to-lactate reaction.The tumor cells from MMTV-ErbB2/Lepr^(db/db) transgenic mice treated with metformin had reprogrammed metabolism by reducing levels of both oxygen consumption and lactate production.Metformin decreased the expression of Myc and pyruvate kinase isozyme 2(PKM2),leading to metabolism reprogramming.Moreover,metformin attenuated the mTOR/AKT signaling pathway and altered adipokine profiles.Conclusions:MMTV-ErbB2/Lepr^(db/db) mouse model was able to recapitulate diabetic HER2^(+) human breast cancer.Additionally,our results defined the signaling pathways deregulated in HER2^(+) breast cancer under diabetic condition,which can be intervened by anti-insulin resistance therapy. | Ping-Chieh Chou Hyun Ho Choi Yizhi Huang Enrique Fuentes-Mattei Guermarie Velazquez-Torres Fanmao Zhang Liem Phan Jaehyuk Lee Yanxia Shi James A.Bankson Yun Wu Huamin Wang Ruiying Zhao Sai-Ching Jim Yeung Mong-Hong Lee | 2021 | Cancer Communications2021，41，5： | 4 |
| 12 | A trend based investment decision approach using clustering and heuristic algorithm显示文摘This paper proposes a stock price trend clustering and trend investment decision model by using a genetic algorithm to search for optimal solutions and the best investment strategies for different stock price trends.The new price trend clustering model identifies three types of stock price movements:uptrends,sideways trends,and downtrends.Unfortunately,trends discovered through stock price movements or technical indicator graphs are typically subjective and unquantifiable.This paper takes daily stock prices and trading volume data from the China Shanghai Stock Exchange Composite Index(SSECI)from January 2,1997 to August31,2012,to examine the performance of the proposed trend clustering model.The proposed model is also compared to other popular stock market investment strategies to verify its validity.Research result shows that the proposed trend clustering model correctly identifies three different trends in the stock market.Furthermore,the trend investment strategy model developed by using genetic algorithm methodology performs better than other investment strategies,namely,Granville’s rule,the KD indicator strategy,the buys and holds strategy,and GMA rules,in both bull and bear market periods.Research results prove the proposed new model to be a stable and valid investment strategy. | WU ChungMin CHOU ShengChun LIAW HorngTwu | 2014 | Science China(Information Sciences)2014，57，9： | 4 |
| 13 | Understanding Sulfur Redox Mechanisms in Different Electrolytes for Room-Temperature Na-S Batteries显示文摘This work reports influence of two different electrolytes,carbonate ester and ether electrolytes,on the sulfur redox reactions in room-temperature Na-S batteries.Two sulfur cathodes with different S loading ratio and status are investigated.A sulfur-rich composite with most sulfur dispersed on the surface of a carbon host can realize a high loading ratio(72%S).In contrast,a confined sulfur sample can encapsulate S into the pores of the carbon host with a low loading ratio(44%S).In carbonate ester electrolyte,only the sulfur trapped in porous structures is active via‘solid-solid’behavior during cycling.The S cathode with high surface sulfur shows poor reversible capacity because of the severe side reactions between the surface polysulfides and the carbonate ester solvents.To improve the capacity of the sulfur-rich cathode,ether electrolyte with NaNO_(3) additive is explored to realize a‘solid-liquid’sulfur redox process and confine the shuttle effect of the dissolved polysulfides.As a result,the sulfur-rich cathode achieved high reversible capacity(483 mAh g^(−1)),corresponding to a specific energy of 362 Wh kg^(−1) after 200 cycles,shedding light on the use of ether electrolyte for high-loading sulfur cathode. | Hanwen Liu Wei-Hong Lai Qiuran Yang Yaojie Lei Can Wu Nana Wang Yun-Xiao Wang Shu-Lei Chou Hua Kun Liu Shi Xue Dou | 2021 | Nano-Micro Letters2021，13，8： | 3 |

| 17 | Five-year sequential changes in secondary antibiotic resistance of Helicobacter pylori in Taiwan显示文摘AIM: To determine changes in the antibiotic resistance of Helicobacter pylori(H. pylori) in southern Taiwan after failure of first-line standard triple therapy.METHODS: We analyzed 137 H. pylori-infected isolates from patients who experienced eradication failure after standard first-line triple therapy from January2010 to December 2014. The H. pylori strains were tested for susceptibility to amoxicillin, clarithromycin,levofloxacin, metronidazole and tetracycline using the E-test method. The minimal inhibitory concentration(MIC) was determined by the agar dilution test.MIC values of ≥ 0.5, ≥ 1, ≥ 1, ≥ 4 and ≥ 8 mg/L were considered to be the resistance breakpoints for amoxicillin, clarithromycin, levofloxacin, tetracycline and metronidazole, respectively.RESULTS: A high resistance rate was found for clarithromycin(65%-75%) and metronidazole(30%-40%)among patients who failed first-line standard therapy.The resistance levels to amoxicillin and tetracycline remained very low; however, levofloxacin resistance was as high as 37.5% in 2010 but did not increase any further during the past 5 years. The rates of resistance to these antibiotics did not show a statistically significant upward or downward trend.CONCLUSION: Antibiotic resistance of H. pylori remains a problem for the effective eradication of this pathogen and its associated diseases in Taiwan. High clarithromycin resistance indicated that this antibiotic should not be prescribed as a second-line H. pylori eradication therapy. | 18 | An association between the location of white matter changes and the behavioral and psychological symptoms of dementia in Alzheimer's disease patients显示文摘Objective:The frontal lobe may be involved in circuits associated with depression,apathy,aggression,and other psychiatric symptoms.Although white matter changes(WMC)are related to the severity of behavioral and psychological symptoms of dementia(BPSD)in patients with Alzheimer’s disease(AD),it is unclear which part of the WMC may play the most important role in BPSD.This study was designed to investigate the relationship between the location of WMC and the severity of BPSD in AD patients.Methods:Among patients diagnosed with Alzheimer’s disease between 2009 and2014,387 patients were retrospectively reviewed after those with pre‐existing organic brain syndrome,psychiatric diseases,or toxic‐metabolic encephalopathy were excluded.Patients’demographic and laboratory data,WMC measured with brain computed tomography and scored using the age‐related white matter changes(ARWMC)scale,and neuropsychological tests,including the cognitive abilities screening instrument(CASI),the Mini‐Mental State Examination(MMSE),the clinical dementia rating scale with sum‐box(CDR‐SB),and the neuropsychiatric inventory(NPI)were analyzed.Results:There was no significant difference in the NPI between patients with and without a history of stroke,hypertension,and diabetes.No significant difference in the NPI was identified between different sexes or different Apolipoprotein E(APOE)alleles.The NPI score was significantly correlated with the duration of education(r=–0.4515,P=0.0172),CASI(r=–0.2915,P<0.0001),MMSE(r=–0.8476,P<0.0001),and CDR‐SB(r=2.2839,P<0.0001).WMC in the right frontal lobe showed a significant difference in NPI in comparison to those without WMC(P=0.0255).After adjusting for age,duration of education,and CASI,WMC in the right frontal lobe remained significantly associated with the NPI score(β=3.8934,P=0.042).Conclusions:WMC involving the right frontal lobe may play an important role in the BPSD in AD patients during their dementia diagnosis.Further studies are necessary to confirm whether controlling the risk factors of WMC can slow the progression of BPSD. | Tzuchou Lin Yihui Lin Linli Kao Yihui Kao Yuanhan Yang Pingsong Chou Mengni Wu | 2016 | Translational Neuroscience and Clinics2016，2，1： | 3 |
