目的:在超重但不肥胖(体重指数为25—30)的人群中研究限制热量6个月伴/不伴运动对其产生的影响。
设计、地点及参试者:于2002年3月至2004年8月在位于路易斯安娜州首府巴顿鲁治的研究中心对48名不好动的健康人进行随机对照研究。
干预:参试者在6个月内随机分为4个组:对照组(饮食可维持体重);限制热量组(限制基线所需能量的25%);限制热量+运动组(限制12.5%的热量+运动增加12.5%的能耗);极低热量饮食组(每日摄入890keal,直至体重减少15%,随后采用维持体重的饮食)。
主要观测指标:机体组成;硫酸脱氢表雄酮(dehydroepiandrostemne sulfate,DHEAS)、葡萄糖及胰岛素水平;蛋白羰基化合物;DNA损伤;24小时能耗;核心体温。
结果:6个月时各组体重变化的均值(标准误)为:对照组-1.0%(1.1%);限制热量组-10.4%(0.9%);限制热量+运动组-10.0%(0.8%);极低热量饮食组-13.9%(0.7%)。6个月时,各干预组空腹血糖水平较基线明显降低(P均〈0.01),而DHEAS和葡萄糖水平没有改变。限制热量组及限制热量+运动组核心体温有所下降(P均〈0.05)。对机体组成进行校正后发现,对照组24小时静息能耗无变化,但限制热量组(-135kcal/d[42kcal/d])、限制热量+运动组(-117kcal/d[52kcal/d])和极低热量饮食组(-125kcal/d[35kcal/d])24小时静息能耗有所下降(P均〈0.008)。上述“代谢调节”(超出预计值的-6%)与对照组相比存在显著差异(P〈0.05)。6个月时,各组蛋白羰基化合物的浓度较基线时无变化,而各干预组DNA损伤有所减少(P〈0.005)。

| 4 | Retrograde-viewing device improves adenoma detection rate in colonoscopies for surveillance and diagnostic workup显示文摘AIM:To determine which patients might benefit most from retrograde viewing during colonoscopy through subset analysis of randomized,controlled trial data.METHODS:The Third Eye Retroscope Randomized Clinical Evaluation(TERRACE) was a randomized,controlled,multicenter trial designed to evaluate the efficacy of a retrograde-viewing auxiliary imaging device that is used during colonoscopy to provide a second video image which allows viewing of areas on the proximal aspect of haustral folds and flexures that are difficult to see with the colonoscope's forward view.We performed a post-hoc analysis of the TERRACE data to determine whether certain subsets of the patient population would gain more benefit than others from use of the device.Subjects were patients scheduled for colonoscopy for screening,surveillance or diagnostic workup,and each underwent same-day tandem examinations with standard colonoscopy(SC) and Third Eye colonoscopy(TEC),randomized to SC followed by TEC or vice versa.RESULTS:Indication for colonoscopy was screening in 176/345 subjects(51.0%),surveillance after previous polypectomy in 87(25.2%) and diagnostic workup in 82(23.8%).In 4 subjects no indication was specified.Previously reported overall results had shown a net additional adenoma detection rate(ADR) with TEC of 23.2% compared to SC.Relative risk(RR) of missing adenomas with SC vs TEC as the initial procedure was 1.92(P = 0.029).Post-hoc subset analysis shows additional ADRs for TEC compared to SC were 4.4% for screening,35.7% for surveillance,55.4% for diagnostic and 40.7% for surveillance and diagnostic combined.The RR of missing adenomas with SC vs TEC was 1.11(P = 0.815) for screening,3.15(P = 0.014) for surveillance,8.64(P = 0.039) for diagnostic and 3.34(P = 0.003) for surveillance and diagnostic combined.Although a multivariate Poisson regression suggested gender as a possibly significant factor,subset analysis showed that the difference between genders was not statistically significant.Age,bowel prep quality and withdrawal time did not significantly affect the RR of missing adenomas with SC vs TEC.Mean sizes of adenomas detected with TEC and SC were similar at 0.59 cm and 0.56 cm,respectively(P = NS).CONCLUSION:TEC allows detection of significantly more adenomas compared to SC in patients undergoing surveillance or diagnostic workup,but not in screening patients(ClinicalTrials.gov Identifier:NCT01044732). | Peter D Siersema Amit Rastogi Anke M Leufkens Paul A Akerman Kassem Azzouzi Richard I Rothstein Frank P Vleggaar Alessandro Repici Giacomo Rando Patrick I Okolo Olivier Dewit Ana Ignjatovic Elizabeth Odstrcil James East Pierre H Deprez Brian P Saunders Anthony N Kalloo Bradley Creel Vikas Singh Anne Marie Lennon Daniel C DeMarco | 2012 | World Journal of Gastroenterology2012，18，26： | 6 |

| 8 | Species delimitation within the Glaucidium brodiei owlet complex using bioacoustic tools显示文摘Background:The taxonomy of the Collared Owlet(Glaucidium brodiei)species complex is confused owing to great individual variation in plumage colouration seemingly unrelated to their distribution.Although generally recognised as a single species,vocal differences among the subspecies have been noted by field recordists.However,there is no study assessing the vocal differences among these four subspecies.Methods:We obtained 76 sound recordings of the G.brodiei species complex comprising all four subspecies.We conducted bioacoustic examinations using principal component analysis and the Isler criterion to quantitatively test species boundaries within the G.brodiei complex.In addition,we compared plumage colouration among 13 specimens of the G.brodiei complex deposited at the Natural History Museum at Tring,UK and the Lee Kong China Natural History Museum,Singapore to ascertain the presence of plumage differences across taxa.Results:We found the Bornean and Sumatran populations vocally similar to each other,but distinctly different from the mainland and Taiwan populations.The vocal pattern seems to corroborate plumage distinctions in the colouration of neck collars:the Bornean and Sumatran taxa share a white neck collar,whereas the continental and Taiwan taxa share a rufous neck collar.Conclusions:We propose the taxonomic elevation of the Sumatran and Bornean populations to species level as Sunda Owlet G.sylvaticum,with one subspecies on Sumatra(G.s.sylvaticum)and Borneo(G.s.borneense)each.Our study corroborates the importance of bioacoustics in ascertaining species boundaries in non-passerines,and emphasises the significance of incorporating multiple species delimitation approaches when making taxonomic decisions. | Chyi Yin Gwee James AEaton Elize YXNg Frank ERheindt | 2019 | Avian Research2019，10，4： | 3 |

| 15 | Acute care needs in an Indian emergency department: A retrospective analysis显示文摘BACKGROUND: Emergencies such as road traffic accidents(RTAs), acute myocardial infarction(AMI) and cerebrovascular accident(CVA) are the most common causes of death and disability in India. Robust emergency medicine(EM) services and proper education on acute care are necessary. In order to inform curriculum design for training programs, and to improve the quality of EM care in India, a better understanding of patient epidemiology and case burden presenting to the emergency department(ED) is needed.METHODS: This study is a retrospective chart review of cases presenting to the ED at Kerala Institute of Medical Sciences(KIMS), a private hospital in Trivandrum, Kerala, India, from November 1, 2011 to April 21, 2012 and from July 1, 2013 to December 21, 2013. De-identified charts were systematically sampled and reviewed.RESULTS: A total of 1 196 ED patient charts were analyzed. Of these patients, 55.35%(n=662) were male and 44.7%(n=534) were female. The majority(67.14%, n=803) were adults, while only 3.85%(n=46) were infants. The most common chief complaints were fever(21.5%, n=257), renal colic(7.3%, n=87), and dyspnea(6.9%, n=82). The most common ED diagnoses were gastrointestinal(15.5%, n=185), pulmonary(12.3%, n=147), tropical(11.1%, n=133), infectious disease and sepsis(9.9%, n=118), and trauma(8.4%, n=101).CONCLUSION: The patient demographics, diagnoses, and distribution of resources identifi ed by this study can help guide and shape Indian EM training programs and faculty development to more accurately refl ect the burden of acute disease in India. | Elizabeth G.
