1世界家庭医生组织(WONCA)研究论文摘要汇编——以家庭为中心的抗抑郁疗法治疗老年男性抑郁患者遇到的挑战和解决方案:一项对利益相关者的研究显示文摘背景家庭成员在老年男性抑郁患者的生活中起到了非常重要的作用,其经常陪同患者去社区医院就诊。然而,对如何最有效地让患者家庭成员参与患者的治疗过程相关研究较少。由于一些原因,如抑郁的'病耻心'与家庭成员间的冲突,导致家庭成员很难参与患者的抗抑郁治疗。本研究旨在探究家庭成员参与老年男性抑郁患者的治疗所遇到的困难及其措施。方法本项横断面定性描述性访谈研究在位于加利福尼亚中央谷地的'安全网'联邦合格健康服务中心进行。37例研究对象中包含15例老年(≥60岁)男性抑郁患者(由邮件招募、候诊室招募和转诊患者组成)、12例患者家庭成员(在参与研究的患者家庭成员中招募),以及10例全科医务人员(通过目的抽样抽取)。对研究对象关于把家庭干预列为治疗老年男性抑郁的全科疗法之一的看法,进行了半结构式的访谈。访谈内容被录音、记录。结果访谈结果表明,家庭成员参与老年男性抑郁患者的治疗所遇到的主要困难有:让患者在适当的时机接受治疗;保持患者的自主性;处理患者个人隐私问题;处理家庭成员之间的冲突。对于如何解决这些困难,研究对象也提出了可行性的建议。结论尽管以家庭为中心的抗抑郁疗法是一项治疗老年男性抑郁患者有效的全科疗法,但是实施起来尚有一定困难。此疗法的继续开展以及临床应用需要研制出解决这些困难的方案和指南。HINTON L SCIOLLA A F UNüTZER J 本刊编辑部 2017中国全科医学2017,20,32:4
