| 2 | Comparison of different diagnostic methods in infants with Cholestasis显示文摘AIM: To evaluate different methods in differentiating idiopathic neonatal hepatitis from biliary atresia. METHODS: Sixty-five infants with cholestatic jaundice and final diagnosis of idiopathic neonatal hepatitis and biliary atresia were studied prospectively from September 2003 to March 2006. A thorough history and physical examination were undertaken and the liver enzymes were examined. All cases underwent abdominal ultrasonography, hepatobiliary scintigraphy, and percutaneous liver biopsy. The accuracy, sensitivity, specificity and predictive values of these various methods were compared. RESULTS: There were 34 girls and 31 boys, among them 46 subjects had idiopathic neonatal hepatitis (age, 61 ± 17 d) and 19 had biliary atresia (age, 64 ± 18 d). The mean age at onset of jaundice was significantly lower in cases of biliary atresia when compared to idiopathic neonatal hepatitis cases (9 ± 13 d vs 20 ± 21 d; P = 0.032). The diagnostic accuracy of different methods was as follows: liver biopsy, 96.9%; clinical evaluation, 70.8%; ultrasonography, 69.2%; hepatobiliary scintigraphy, 58.5%; and liver enzymes, 50.8%.CONCLUSION: Our results indicate that clinical evaluation by an experienced pediatric hepatologist and a biopsy of the liver are considered as the most reliable methods to differentiate idiopathic neonatal hepatitis and biliary atresia. | Seyed Mohsen Dehghani Mahmood Haghighat Mohammad Hadi Imanieh Bita Geramizadeh | 2006 | World Journal of Gastroenterology2006,12,36: | 14 |
| 3 | Cyclic vomiting syndrome in children: Experience with 181 cases from southern Iran显示文摘AIM: To evaluate the clinical presentation, response to prophylactic therapy and outcome of children with cyclic vomiting syndrome (CVS) in Shiraz, Iran.METHODS: During a period of 11 years (March 1994 to March 2005), 181 consecutive children with a final diagnosis of CVS were evaluated, treated and followed in our center. Patients were randomized to receive either amitriptyline or propranolol as prophylactic treatments. RESULTS: There were 88 boys and 93 girls with mean age of onset of symptoms of 4.9 ± 3.3 years (range, neonatal period to 14 years), the mean age at final diagnosis was 6.9 years (range, 1.5 to 14), and the mean duration between the onset of the first attack and the final diagnosis of CVS was 2 ± 1.81 years (range, 1/6 to 8). The mean duration of each attack was 4.26 days (range, from few hours to 10 d) and the mean interval between the attacks was 1.8 mo (range, 1 wk to 12 mo). The time of onset of the attacks was midnight to early morning in about 70% of cases. Amitriptyline was effective in 46 out of 81 (56%) patients (P < 0.001). Propranolol appeared to have a superior action and was effective in 74 out of 83 (92%) patients (P < 0.0001).CONCLUSION: There is a significant lag time between the onset of clinical symptoms and the final diagnosis of CVS in our area. In patients with typical clinical presentations of CVS, who are examined by an experienced physician, invasive workup is not necessary. | 4 | Transplantation of stem cells from human exfoliated deciduous teeth for bone regeneration in the dog mandibular defect显示文摘AIM: To investigate the effect of stem cells from human exfoliated deciduous teeth(SHED) transplanted for bone regeneration in the dog mandibular defect.METHODS: In this prospective comparative study, SHEDs had been isolated 5 years ago from human exfoliated deciduous teeth. The undifferentiated stem cells were seeded into mandibular bone through-andthrough defects of 4 dogs. Similar defects in control group were filled with cell-free collagen scaffold. After 12 wk, biopsies were taken and morphometric analysis was performed. The percentage of new bone formation and foreign body reaction were measured in each case. The data were subject to statistical analysis using the Mann-Whitney U and Kruskalwalis statistical tests. Differences at P < 0.05 was considered as significant level.RESULTS: There were no significant differences between control and SHED-seeded groups in connective tissue(P = 0.248), woven bone(P = 0.248) and compact bone(P = 0.082). | 5 | Intrusion Detection System Using Voting-Based Neural Network显示文摘Several security solutions have been proposed to detect network abnormal behavior. However, successful attacks is still a big concern in computer society. Lots of security breaches, like Distributed Denial of Service(DDoS),botnets, spam, phishing, and so on, are reported every day, while the number of attacks are still increasing. In this paper, a novel voting-based deep learning framework, called VNN, is proposed to take the advantage of any kinds of deep learning structures. Considering several models created by different aspects of data and various deep learning structures, VNN provides the ability to aggregate the best models in order to create more accurate and robust results. Therefore, VNN helps the security specialists to detect more complicated attacks. | 6 | Organisms causing spontaneous bacterial peritonitis in children with liver disease and ascites in Southern Iran显示文摘AIM: To determine the causative agents of spontaneous bacterial peritonitis (SBP) in children with liver disease and ascites in our center.METHODS: During a 2.5 year period, from September 2003 to March 2006, 12 patients with 13 episodes of SBP were studied. In all cases at the time of admission serum albumin and glucose, urinalysis and urine culture was performed. Analysis [white blood cell (WBC) count with differential, albumin, glucose], gram stain, culture by BACTEC method and antibiogram was done on ascitic fluids. Abdominal paracentesis was repeated after 48 h of antibiotic therapy for bacteriologic assay. The patients were followed for at least three months in a gastroenterology clinic. RESULTS: There were 7 girls (58%) and 5 boys (42%) with a median age of 5.2 years (range, 6 mo to 16 years). All cases had positive ascitic fluid culture. Gram stain was positive in 5 (38.5%) of them. The isolated organisms were S. pneumoniae in 5 (38.5%), E. coli in 2 (15.3%), S. viridans in 2 (15.3%), and K. pneumoniae, H. influenza, Enterococci, and nontypable Streptococcus each in one (7.7%). All of them except Enterococci were sensitive to ciprofloxacin and ceftriaxone. All ascitic fluid cultures were negative after 48 h of antibiotic therapy. | 7 | Solitary rectal ulcer syndrome in children:A literature review显示文摘Solitary rectal ulcer syndrome(SRUS) is a benign and chronic disorder well known in young adults and less in children.It is often related to prolonged excessive straining or abnormal defecation and clinically presents as rectal bleeding,copious mucus discharge,feeling of incomplete defecation,and rarely rectal prolapse.SRUS is diagnosed based on clinical symptoms and endoscopic and histological findings.The current treatments are suboptimal,and despite correct diagnosis,outcomes can be unsatisfactory.Some treatment protocols for SRUS include conservative management such as family reassurance,regulation of toilet habits,avoidance of straining,encouragement of a high-fiber diet,topical treatments with salicylate,sulfasalazine,steroids and sucralfate,and surgery.In children,SRUS is relatively uncommon but troublesome and easily misdiagnosed with other common diseases,however,it is being reported more than in the past.This condition in children is benign;however,morbidity is an important problem as reflected by persistence of symptoms,especially rectal bleeding.In this review,we discuss current diagnosis and treatment for SRUS. | Seyed Mohsen Dehghani Abdorrasoul Malekpour Mahmood Haghighat | 2012 | World Journal of Gastroenterology2012,18,45: | 3 |

| 19 | Determination of critical parameters in the analysis of road tunnel fires显示文摘The analysis of the fluid characteristics downstream of a fire source in transportation tunnels is one the most important factor in the emergency response, evacuation, and the rescue service studies. Some crucial parameters can affect the fluid characteristics downstream of the fire. This research develops a statistical analysis on the computational fluid dynamics(CFD) data of the road tunnel fire simulations in order to quantify the significance of tunnel dimensions, inlet air velocity, heat release rate, and the physical fire size(fire perimeter) on the fluid characteristics downstream of the fire source. The selected characteristics of the fluid(response variables) were the average temperature, the average density, the average viscosity, and the average velocity. The prediction of the designed statistical models was assessed; then the significant parameters' effects and the parameters interactive effects on different response variables were determined individually. Next, the effect of computational domain length on the selection of the significant parameters downstream of the fire source was analyzed. In this statistical analysis, the linear models were found to provide the statistically good prediction.
