| 1 | 基于IMRT时代的第八版AJCC/UICC鼻咽癌临床分期建议显示文摘目的准确的分期系统对癌症的治疗至关重要。随着癌症分期和治疗方法的演变,需要不断评价分期的适用性和改进性。方法基于第7版AJCC/UICC分期回顾性分析香港和中国大陆2个肿瘤中心收治的1609例接受调强放射治疗的首诊无转移鼻咽癌患者临床资料,所有患者治疗前均行核磁共振分期评估。结果无其他T3、T4期解剖结构受侵患者中,伴有咀嚼肌间隙(翼内肌和/或翼外肌)侵犯、椎前肌侵犯及咽旁间隙侵犯的三组患者之间0s相近。伴广泛软组织(上述侵犯结构以外的软组织)受侵患者OS与伴有颅内侵犯或颅神经侵犯相似。仅2%患者锁骨上窝以上淋巴结转移者直径〉6cm,其0s率与下颈淋巴结转移者类似。用下颈(环状软骨尾侧缘水平以下)代替锁骨上窝并不影响N分期之间的风险差异性。采用推荐的T、N分期,T。N。、T。N,期0s相近。结论经AJCC/UICC分期筹备委员会审阅后,建议第8版分期应将翼内肌/翼外肌从T。降到T:期,增加椎前肌为L期,用下颈取代锁骨上窝,将淋巴结最大直径〉6cm合并归为N,期,将T。、N,期统一归为Ⅳ。期。这些改变不仅使得相邻分期间风险差异性更好,而且使得临床实践性与全球适用性之间达到最佳平衡。 | 潘建基 Wai Tong Ng 宗井凤 Lucy L.K.Chan Brian O' Sullivan 林少俊 Henry C.K.Sze 陈韵彬 Horace C.W.Choi 郭巧娟 Wai Kuen Kan 肖友平 Xu Wei Quynh Thu Le Christine M.Glastonbury A.Dimtrios Colevas Randal S.Weber Jatin P.Shah Anne W.M.Lee | 2016 | 中华放射肿瘤学杂志2016，25，3： | 65 |
| 2 | Hepatic steatosis is associated with an increased risk of carotid atherosclerosis显示文摘AIM: Although an association between helatic steatosis and vascular risk factors has been described, direct relationships between fatty liver and atherosclerosis have not yet been investigated. The aim of the present study has been to investigate those relationships.METHODS: The Study of Health in Pomerania examined a random population sample aged between 20 and 79 years.A study population of 4 222 subjects without hepatitis B and C infections and without liver cirrhosis was available for the present analysis. Hepatic steatosis was defined sonographically and intima-media thickness (IMT) as well as plaque prevalence were estimated by carotid ultrasound.RESULTS: The prevalence rate of hepatic steatosis was 29.9%. Among subjects aged ≥45 years, an association between hepatic steatosis and IMT of the carotid arteries was found in bivariate analysis, but not after adjustment for atherosclerotic risk factors. Individuals with fatty liver had more often carotid plaques than persons without fatty liver (plaque prevalence rate 76.8% vs 66.6%; P<0.001).This association persisted after adjustment for confounding factors and was predominantly present in subjects with no to mild alcohol consumption.CONCLUSION: There is an independent association between hepatic steatosis and carotid atherosclerotic plaques. | 3 | Multi-length scale modeling of carburization,martensitic microstructure evolution and fatigue properties of steel gears显示文摘Multi-length scale modeling is performed to(i)predict the carburized case depth of SAE8620 steel gears by solving the Fick’s second law of diffusion,(ii)model the martensitic microstructure evolution in a grain inside the carburized case as well as to study the effect of stress cycling on retained austenite(RA)and martensite using a 3D phase-field model,(iii)simulate the effect of carburization and different RA contents on macroscale fatigue behavior of SAE8620 steel spur gear using the finite element method.The diffusion model predicts that the case depth increases with increasing heat treatment time and temperature.The phase-field simulations show that RA can transform to martensite during fatigue loading,where the extent of the transformation will depend on the type of stresses applied,i.e.stresses in a high stress regime or low stress regime of fatigue loading.Reverse transformation of martensite to austenite is also observed in low RA sample under high stress regime.The macroscale simulations show that the carburized case with high RA gives rise to better fatigue life compared to that with low RA. | Edward Charles Henry Crawford O’Brien Hemantha Kumar Yeddu | 2020 | Journal of Materials Science & Technology2020，47，14： | 5 |

| 5 | WONCA研究论文摘要汇编——初级医疗连续性服务及其与老年人生存的关系:一项17年的前瞻性队列研究显示文摘背景尽管连续性服务在基层医疗中是被广泛接受的核心原则,但是关于其优势的证据仍然不足。目的探讨全科医学中连续性服务是否与老年人长寿相关。设计与场所数据来源于阿姆斯特丹纵向衰老研究,对荷兰老年人进行不间断队列研究。研究样本包括1 712例年龄≥60岁的老年人,进行3年随访,周期长达17年,随访其死亡率直至2013年。方法连续性服务被定义为患者和全科医生之间不间断的治疗关系的持续过程。采用赫芬达尔-赫希曼指数计算服务的连续性(COC)。COC指数为1代表最大连续性。将COC指数〈1的患者分为3组,COC最大值的参与者为第4组。采用Cox回归分析探讨COC和存活时间之间的关联。结果 742例参与者(43.3%)COC取得最大值。在生存17年的759例参与者中,251例(33.1%)仍有相同的全科医生服务。COC最小值组(指数〉0~0.500)死亡率高于COC最大值组,差异有统计学意义〔HR=1.20,95%CI(1.01,1.42)〕。目前无混杂因素影响该HR。结论该研究表明全科医学中低连续性服务与高死亡风险相关,应强化该案例以鼓励连续性服务。 | MAARSINGH O R HENRY Y VAN DE VEN P M 本刊编辑部 | 2016 | 中国全科医学2016，19，27： | 4 |
