1Circulating tumor and cancer stem cells in hepatitis C virusassociated liver disease显示文摘AIM:To assess the role of circulating tumor cells(CTCs)and cancer stem cells(CSCs)in hepatitis C virus(HCV)-associated liver disease.METHODS:Blood and/or tissue samples were obtained from HCV(genotype 4)-associated hepatocellularcarcinoma patients(HCC;n=120),chronic hepatitis C patients(CH;n=30)and 33 normal control subjects(n=33).Serum levels of alpha-fetoprotein(AFP),alkaline phosphatase,and alanine and aspartate aminotransferases were measured.Cytokeratin 19(CK19)monoclonal antibody was used to enumerate CTCs,and CD133 and CD90 were used to enumerate CSCs by flow cytometry.The expression levels of the CSCs markers(CD133 and CD90)as well as telomerase,melanoma antigen encoding gene 1(MAGE1)and MAGE3 were assessed by RTPCR and quantitative real-time polymerase chain reactions.The number of CTCs and/or the expression levels of CK19,CD133,telomerase,MAGE1 and MAGE3 were correlated to the standard clinicopathologic prognostic factors and disease progression.RESULTS:Levels of AFP,alkaline phosphatase and aspartate aminotransferase were significantly different among the HCC,CH and control groups(P<0.001),whereas alanine aminotransferase differed significantly between patient(HCC and CH)and control groups(P<0.001).At the specified cutoff values determine by flow cytometry,CK19(49.8),CD90(400)and CD133(73)were significantly higher in the blood of HCC patients compared to those in the CH and control groups(P<0.001).On the other hand,CD133 at a 69.5 cutoff was significantly higher in the CH compared to the control group(P≤0.001).Telomerase,MAGE1 and MAGE3RNA were expressed in 55.71%,60.00%and 62.86%of the HCC patients,respectively,but were not detected in patients in the CH or control groups,which were statistically significant(Ps<0.001).The expression levels of telomerase,CD90,MAGE3,CD133 and CK19 were all significantly associated with high tumor grade and advanced stage in HCC patients(all P s<0.05).CONCLUSION:CTC counts and AFP,CK19,telomerase,and MAGE1/MAGE3 expression predict disease progression in patients with HCV,whereas telomerase,MAGE3,CD90,CD133 and CK19 are prognostic mark-Abeer A Bahnassy Abdel-Rahman N Zekri Ahmed El-Bastawisy Amal Fawzy Marwa Shetta Nehal Hussein Dalia Omran Abdallah A S Ahmed Samir S El-Labbody 2014World Journal of Gastroenterology2014,20,48:9
2Interprofessional, multiple step simulation course improves pediatric resident and nursing staff management of pediatric patients with diabetic ketoacidosis显示文摘AIM To investigate the use of a multidisciplinary, longitudinal simulation to educate pediatric residents and nurses on management of pediatric diabetic ketoacidosis.METHODS A multidisciplinary, multiple step simulation course was developed by faculty and staff using a modified Delphi method from the Pediatric Simulation Center and pediatric endocrinology department. Effectiveness of the simulation for the residents was measured with a pre- and post-test and a reference group not exposed to simulation. A follow up post-test was completed 3-6 mo after the simulation. Nurses completed a survey regarding the education activity. RESULTS Pediatric and medicine-pediatric residents(n = 20) and pediatric nurses(n = 25) completed the simulation course. Graduating residents(n = 16) were used as reference group. Pretest results were similar in the control and intervention group(74% ± 10% vs 76% ± 15%, P = 0.658). After completing the intervention, participants improved in the immediate post-test in comparison to themselves and the control group(84% ± 12% post study; P < 0.05). The 3-6 mo follow up post-test results demonstrated knowledge decay when compared to their immediate post-test results(78% ± 14%, P = 0.761). Long-term prophylaxis with single daily oral antibiotic has been shown to be cost effective in delaying a recurrence but only for a short time[3].What has never been tested in this population is the cycling of antibiotics. 4Association between thrombotic risk factors and extent of fibrosis in patients with non-alcoholic fatty liver diseases显示文摘AIM: To evaluate the prevalence of genetic and acquired prothrombotic risk factors and their association with the extent of fibrosis and fatty infiltration in patients with non-alcoholic fatty liver disease (NAFLD).METHODS: Forty-four patients with chronic hepatitis (28 men and 16 women, with mean age of 45±11 and 49±12 years, respectively) constituted the patient population of this study. The groups were divided as follows: 15 patients with fatty liver (FL); 15 with non-alcoholic steatohepatitis (NASH); 14 with chronic viral hepatitis (CH) diagnosed by histology and liver technetium scan or ultrasound; and 10 healthy individuals. Thrombophilic, coagulation factors and genetic mutations were diagnosed by standard hemostatic and molecular coagulation assays.RESULTS: Activated protein C (APC) resistance and protein S were the most prevalent thrombotic risk factors (6% and 10% in NAFLD vs 21% and 14% in CH; P<0.01 and P<0.05, respectively). One thrombotic risk factor was identified in 41% of patients (23% mild fibrosis, 18% severe fibrosis) and two thrombotic risk factors in 6% of patients with NAFLD and severe fibrosis. While no differences in APC ratio, lupus anticoagulant, fibrinogen, factor V Leiden,prothrombin, and MTHFR mutation were found. Protein S levels were significantly lower in NASH patients than in patients with FL alone (92±19 vs106±2, P<0.01). Protein C levels were markedly higher in patients with NAFLD and mild or severe fibrosis as compared to the patients with CH, respectively (128±40 vs96±14, P<0.001 or 129±36 vs 88±13, P<0.01).CONCLUSION: Up to 46% of patients with NAFLD may have thrombotic risk factors, and the presence of thrombotic risk factors is correlated with the extent of hepatic fibrosis,suggesting a crucial role of the coagulation system in the pathogenesis of hepatic fibrosis.N Assy I Bekirov Y Mejritsky L Solomon S Szvalb O Hussein 2005World Journal of Gastroenterology2005,11,37:3
