1Recurrence after endoscopic piecemeal mucosal resection for large sessile colorectal polyps显示文摘AIM: To evaluate the safety and outcomes of endoscopic piecemeal mucosal resection (EPMR) for large sessile colorectal polyps. METHODS: The patients enrolled in this study were 47 patients with 50 large sessile polyps (diameter, 2 cm or greater) who underwent EPMR using a submucosal saline injection technique between December 2002 and October 2005. All medical records, including characteristics of the patients and polyps, complications, and recurrences, were retrospectively reviewed. The first follow-up endoscopic examination was performed at 3-6 mo after initial endoscopic resection, and the second at 12 mo postEPMR. Subsequent surveillance colonoscopic examinations were individualized, taking risk factors into account. RESULTS: The patients were 23 men and 24 women,with a mean age of 60 years. Mean polyp size was 30.1 mm. Of 50 polyps identified, 34 (68%) were benign and 16 (32%) were malignant. There were 6 (12%) cases with EPMR-related bleeding: 5 intra-procedural and 1 early post-procedural bleeding. All bleeding episodes were managed by endoscopic clipping or argon beam coagulation. There were no perforations. Recurrence was identified in 5 cases (12.2%): 4 local recurrences detected at 3 mo post-EPMR and 1 local recurrence detected at 14 mo post-EPMR. The recurrence rate after EPMR was 3.1% for benign polyps and 33.3% for malignant polyps (P < 0.05). Median follow-up time was 37 mo. CONCLUSION: EPMR is safe, but should be applied carefully in malignant polyps. 2Outcome after endoscopic submucosal dissection for early gastric cancer in Korea显示文摘Endoscopic treatment,such as endoscopic mucosal resection(EMR) and endoscopic submucosal dissection(ESD),has been established as one of the treatment options for selected cases with early gastric cancer(EGC).Most studies on this topic have been carried out by researchers in Japan.Recently,the experience in EMR/ESD for EGC outside Japan is increasingly reported.In Korea,gastric cancer is the most common malignant disease,and the second leading cause of cancer death.Currently,EMR for EGC is widely performed in many centers in Korea.Early results with a shortterm follow-up period are very promising in Korea.Thecomplete resection rate of EMR was 37.8%-94.3%,and that of ESD was 77.4%-93.1%.In this review,we will provide an overview of the outcomes of endoscopic treatments in Korea.Jun Haeng Lee Su Jin Hong Jae Young Jang Seong Eun Kim Sang Young Seol 2011World Journal of Gastroenterology2011,17,31:8
3Isolation of cabbage exosome-like nanovesicles and investigation of their biological activities in human cells显示文摘There are extensive studies on the applications of extracellular vesicles(EVs)produced in cell culture for therapeutic drug development.However,large quantities of EVs are needed for in vivo applications,which requires high production costs and time.Thus,the development of new EV sources is essential to facilitate their use.Accordingly,plant-derived exosome-like nanovesicles are an emerging alternative for culture-derived EVs.Until now,however,few studies have explored their biological functions and uses.Therefore,it is necessary to elucidate biological activities of plant-derived exosome-like nanovesicles and harness vesicles for biomedical applications.Herein,cabbage and red cabbage were used as nanovesicle sources owing to their easy cultivation.First,an efficient method for nanovesicle isolation from cabbage(Cabex)and red cabbage(Rabex)was developed.Furthermore,isolated nanovesicles were characterized,and their biological functions were assessed.Both Cabex and Rabex promoted mammalian cell proliferation and,interestingly,suppressed inflammation in immune cells and apoptosis in human keratinocytes and fibroblasts.Finally,therapeutic drugs were encapsulated in Cabex or Rabex and successfully delivered to human cells,demonstrating the potential of these vesicles as alternative drug delivery vehicles.Overall,the current results provide strong evidence for the wide application of Cabex and Rabex as novel therapeutic biomaterials.Jae Young You Su Jin Kang Won Jong Rhee 2021Bioactive Materials2021,6,12:7

5Gastric metastasis from primary lung adenocarcinoma mimicking primary gastric cancer显示文摘Gastric metastases from lung adenocarcinoma are rare. Because gastric metastasis grossly resembles advanced gastric cancer, it is difficult to diagnose gastric metastasis especially when the histology of the primary lung cancer is adenocarcinoma. We describe a case of gastric metastasis from primary lung adenocarcinoma mimicking Borrmann type Ⅳ primary gastric cancer. A 68-year-old man with known lung adenocarcinoma with multiple bone metastases had been experiencing progressive epigastric pain and dyspepsia over one year. Esophagogastroduodenoscopy revealed linitis plasticalike lesions in the fundus of the stomach. Pathologic examination revealed a moderately differentiated adenocarcinoma with submucosal infiltration. Positive immunohistochemical staining for thyroid transcription factor-1(TTF-1) and napsin A(Nap-A) confirmed that the metastasis was pulmonary in origin. The patient had been treated with palliative chemotherapy for the lung cancer and had lived for over fifteen months after the diagnosis of gastric metastasis. 6Prevalence of upper gastrointestinal bleeding risk factors among the general population and osteoarthritis patients显示文摘AIM To assess the prevalence of possible risk factors of upper gastrointestinal bleeding(UGIB) and their agegroup specific trend among the general population and osteoarthritis patients.METHODS We utilized data from the National Health InsuranceService that included claims data and results of the national health check-up program. Comorbid conditions(peptic ulcer, diabetes, liver disease, chronic renal failure, and gastroesophageal reflux disease), concomitant drugs(aspirin, clopidogrel, cilostazol, non-steroidal anti-inflammatory drugs, steroid, anticoagulants, and SSRI), personal habits(smoking, and alcohol consumption) were considered as possible UGIB risk factors. We randomly imputed the prevalence of infection in the data considering the age-specific prevalence of Helicobacter pylori(H. pylori) infection in Korea. The prevalence of various UGIB risk factors and the age-group specific trend of the prevalence were identified. Prevalence was compared between osteoarthritis patients and others.RESULTS A total of 801926 subjects(93855 osteoarthritis patients) aged 20 and above were included. The prevalence of individual and concurrent multiple risk factors became higher as the age increased. The prevalence of each comorbid condition and concomitant drug were higher in osteoarthritis patients. Thirty-five point zero two percent of the overall population and 68.50% of osteoarthritis patients had at least one or more risk factors of UGIB. The prevalence of individual and concurrent multiple risk factors in younger age groups were also substantial. Furthermore, when personal habits(smoking, and alcohol consumption) and H. pylori infection were included, the prevalence of concurrent multiple risk factors increased greatly even in younger age groups.CONCLUSION Prevalence of UGIB risk factors was high in elderly population, but was also considerable in younger population. Patient with osteoarthritis was at higher UGIB risk than those without osteoarthritis. 7Double layered self-expanding metal stents for malignant esophageal obstruction, especially across the gastroesophageal junction显示文摘AIM: To evaluate the clinical outcomes of double-layered self-expanding metal stents (SEMS) for treatment of malignant esophageal obstruction according to whether SEMS crosses the gastroesophageal junction (GEJ). METHODS: Forty eight patients who underwent the SEMS insertion for malignant esophageal obstruction were enrolled. Patients were classified as GEJ group (SEMS across GEJ, 18 patients) and non-GEJ group (SEMS above GEJ, 30 patients) according to SEMS position. Double layered (outer uncovered and inner covered stent) esophageal stents were placed. RESULTS: The SEMS insertion and the clinical improvement were achieved in all patients in both groups. Stent malfunction occurred in seven patients in the GEJ group and nine patients in the non-GEJ group. Tumor overgrowth occurred in five and eight patients, respectively, food impaction occurred in one patient in each group, and stent migration occurred in one and no patient, respectively. There were no significant differences between the two groups. Reflux esophagitis occurred more frequently in the GEJ group (eight vs five patients, P = 0.036) and was controlled by proton pump inhibitor. Aspiration pneumonia occurred in zero and five patients, respectively, and tracheoesophageal fistula occurred in zero and two patients, respectively. CONCLUSION: Double-layered SEMS are a feasible and effective treatment when placed across the GEJ for malignant esophageal obstruction. 10Ternary MOF‑Based Redox Active Sites Enabled 3D‑on‑2D Nanoarchitectured Battery‑Type Electrodes for High‑Energy‑Density Supercapatteries显示文摘Designing rationally combined metal-organic frameworks(MOFs)with multifunctional nanogeometries is of significant research interest to enable the electrochemical properties in advanced energy storage devices.Herein,we explored a new class of binderfree dual-layered Ni-Co-Mn-based MOFs(NCM-based MOFs)with three-dimensional(3D)-on-2D nanoarchitectures through a polarityinduced solution-phase method for high-performance supercapatteries.The hierarchical NCM-based MOFs having grown on nickel foam exhibit a battery-type charge storage mechanism with superior areal capacity(1311.4μAh cm^−2 at 5 mA cm^−2),good rate capability(61.8%;811.67μAh cm^−2 at 50 mA cm^−2),and an excellent cycling durability.The superior charge storage properties are ascribed to the synergistic features,higher accessible active sites of dual-layered nanogeometries,and exalted redox chemistry of multi metallic guest species,respectively.The bilayered NCM-based MOFs are further employed as a battery-type electrode for the fabrication of supercapattery paradigm with biomass-derived nitrogen/oxygen doped porous carbon as a negative electrode,which demonstrates excellent capacity of 1.6 mAh cm^−2 along with high energy and power densities of 1.21 mWh cm^−2 and 32.49 mW cm^−2,respectively.Following,the MOF-based supercapattery was further assembled with a renewable solar power harvester to use as a self-charging station for various portable electronic applications.Goli Nagaraju SChandra Sekhar Bhimanaboina Ramulu SkKhaja Hussain DNarsimulu Jae Su Yu 2021Nano-Micro Letters2021,13,1:3
11Comparison between uncovered and covered self-expandable metal stent placement in malignant duodenal obstruction显示文摘AIM:To compare the clinical outcomes of uncovered and covered self-expandable metal stent placements in patients with malignant duodenal obstruction.METHODS:A total of 67 patients were retrospectivelyenrolled from January 2003 to June 2013.All patients had symptomatic obstruction characterized by nausea,vomiting,reduced oral intake,and weight loss.The exclusion criteria included asymptomatic duodenal obstruction,perforation or peritonitis,concomitant small bowel obstruction,or duodenal obstruction caused by benign strictures.The technical and clinical success rate,complication rate,and stent patency were compared according to the placement of uncovered(n = 38) or covered(n = 29) stents.RESULTS:The technical and clinical success rates did not differ between the uncovered and covered stent groups(100% vs 96.6% and 89.5% vs 82.8%).There were no differences in the overall complication rates between the uncovered and covered stent groups(31.6% vs 41.4%).However,stent migration occurred more frequently with covered than uncovered stents [20.7%(6/29) vs 0%(0/38),P < 0.05].Moreover,the overall cumulative median duration of stent patency was longer in uncovered than in covered stents [251 d(95%CI:149.8 d-352.2 d) vs 139 d(95%CI:45.5 d-232.5 d),P < 0.05 by log-rank test] The overall cumulative median survival period was not different between the uncovered stent(70 d) and covered stent groups(60 d).CONCLUSION:Uncovered stents may be preferable in malignant duodenal obstruction because of their greater resistance to stent migration and longer stent patency than covered stents.Ji Won Kim Ji Bong Jeong Kook Lae Lee Byeong Gwan Kim Dong Won Ahn Jae Kyung Lee Su Hwan Kim 2015World Journal of Gastroenterology2015,21,5:2

13Carbon-Coated Three-Dimensional MXene/Iron Selenide Ball with Core–Shell Structure for High-Performance Potassium-Ion Batteries显示文摘Two-dimensional(2D)MXenes are promising as electrode materials for energy storage,owing to their high electronic conductivity and low diffusion barrier.Unfortunately,similar to most 2D materials,MXene nanosheets easily restack during the electrode preparation,which degrades the electrochemical performance of MXene-based materials.A novel synthetic strategy is proposed for converting MXene into restacking-inhibited three-dimensional(3D)balls coated with iron selenides and carbon.This strategy involves the preparation of Fe_(2)O_(3)@carbon/MXene microspheres via a facile ultrasonic spray pyrolysis and subsequent selenization process.Such 3D structuring effectively prevents interlayer restacking,increases the surface area,and accelerates ion transport,while maintaining the attractive properties of MXene.Furthermore,combining iron selenides and carbon with 3D MXene balls offers many more sites for ion storage and enhances the structural robustness of the composite balls.The resultant 3D structured microspheres exhibit a high reversible capacity of 410 mAh g^(−1) after 200 cycles at 0.1 A g^(−1) in potassium-ion batteries,corresponding to the capacity retention of 97% as calculated based on 100 cycles.Even at a high current density of 5.0 A g^(−1),the composite exhibits a discharge capacity of 169 mAh g^(−1).Su Hyun Yang Yun Jae Lee Heemin Kang Seung-Keun Park Yun Chan Kang 2022Nano-Micro Letters2022,14,1:2
14Multifunctional core-shell-like nanoarchitectures for hybrid supercapacitors with high capacity and long-term cycling durability显示文摘Transition metal oxide/hydroxide with multif unctional hierarchical nano structures has attracted widespread attention in supercapacitors(SCs)because of their large accessible surface area,high electrochemical activity and superior redox chemistry.Herein,core-shell-like copper(Cu)hydroxide nano tube arrays grafted nickel aluminum layered double hydroxide n anosheets were facilely synthesized on porous Cu foam(CH NTAs@NiAI LDH NSs)for use as an efficient battery-type electrode in hybrid SCs.With the synergistic effects of NiAl LDH NSs on well-adhered CH NTAs/CF,the core-shell-like composite(prepared for 24 h)delivered a maximum areal capacity of 334.3 pAh/cm2 at a current density of 3 mA/cm2 in 2 M KOH electrolyte,which is comparatively higher than other samples synthesized at different growth times.Moreover,the core-shell-like CH NTAs@NiAI LDH NSs-24 demonstrated an outstanding cycling stability of 134.3%after 10,000 cycles.Utilizing high capacity and stability of CH NTAs@NiAI LDH NSs-24,a pouch-type hybrid SC was further assembled with core-shell-like composite as a positive electrode and reduced graphene oxide as a negative electrode with a filter paper as a separator in aqueous alkaline electrolyte.The hybrid SC showed a high areal capacity of 250 pAh/cm2 at 2 mA/cm2 with maximum areal energy and power densities of 181.9 pWh/cm2 and 24,991.5 pW/cm2,respectively.Successfully harvest!ng the solar energy via solar cell panel and subsequently delivering the stored en ergy to switching and proximity applicati ons also dem on strated the real-time applicability of our hybrid SCs.S.Chandra Sekhar Goli Nagaraju Bhimanaboina Ramulu Sk.Khaja Hussain D.Narsimulu Jae Su Yu 2019Nano Research2019,12,10:2

16Cancer-associated fibroblast-derived secreted phosphoprotein 1 contributes to resistance of hepatocellular carcinoma to sorafenib and lenvatinib显示文摘Background:Cancer-associated fibroblasts(CAFs)play an important role in the induction of chemo-resistance.This study aimed to clarify the mechanism underlying CAF-mediated resistance to two tyrosine kinase inhibitors(TKIs),sorafenib and lenvatinib,and to identify a novel therapeutic target for overcoming TKI resistance in hepatocellular carcinoma(HCC).Methods:We performed a systematic integrative analysis of publicly available gene expression datasets and whole-transcriptome sequencing data from 9 pairs of CAFs and para-cancer fibroblasts isolated from human HCC and para-tumor tissues,respectively,to identify key molecules that might induce resistance to TKIs.We then performed in vitro and in vivo experiments to validate selected targets and related mechanisms.The associations of plasma secreted phosphoprotein 1(SPP1)expression levels before sorafenib/lenvatinib treatment with progression-free survival(PFS)and overall survival(OS)of 54 patients with advanced HCC were evaluated using Kaplan-Meier and Cox regression analysis.Results:Bioinformatic analysis identified CAF-derived SPP1 as a candidate molecule driving TKI resistance.SPP1 inhibitors reversed CAF-induced TKI resistance in vitro and in vivo.CAF-derived SPP1 activated rapidly accelerated fibrosarcoma(RAF)/mitogen-activated protein kinase(MAPK)and phosphatidylinositol 3-kinase(PI3K)/protein kinase B(AKT)/mammalian target of rapamycin(mTOR)through the integrin-protein kinase C-alpha(PKCα)signaling pathway and promoted epithelial-to-mesenchymal transition(EMT).A high plasma SPP1 level before TKI treatment was identified as an independent predictor of poor PFS(P=0.026)and OS(P=0.047)in patients with advanced HCC after TKI treatment.Conclusions:CAF-derived SPP1 enhances TKI resistance in HCC via bypass activation of oncogenic signals and EMT promotion.Its inhibition represents a promising therapeutic strategy against TKI resistance inHCC.Moreover,plasma SPP1 level before TKI treatment represents a potential biomarker for treatment response prediction.JungWoo Eun Jung Hwan Yoon Hye Ri Ahn Seokhwi Kim Young Bae Kim Su Bin Lim Won Park TaeWook Kang Geum Ok Baek Moon Gyeong Yoon Ju A Son Ji HyangWeon Soon Sun Kim Hyo Jung Cho Jae Youn Cheong 2023Cancer Communications2023,43,4:2

20Two-week bismuth-containing quadruple therapy and concomitant therapy are effective first-line treatments for Helicobacter pylori eradication: A prospective open-label randomized trial显示文摘BACKGROUND Increasing levels of antibiotic resistance have reduced the Helicobacter pylori(H.pylori)eradication rates afforded by the standard triple therapy.Thus,2-wk firstline four-drug regimens must be considered.AIM To analyze the eradication rates of modified bismuth-containing quadruple therapy(mBCQT)and concomitant therapy(CT),the associated adverse events,and compliance.METHODS Patients infected with H.pylori were prospectively randomized to receive mBCQT or CT for 2 wk.mBCQT featured a proton pump inhibitor(PPI),bismuth,metronidazole,and tetracycline,taken twice daily.CT included a PPI,clarithromycin,metronidazole,and amoxicillin,taken twice daily.The 13C-urea breath test was performed no earlier than 4 wk after therapy concluded to confirm eradication.If either the histological or rapid urease test was positive,H.pylori infection was diagnosed.RESULTS The demographic characteristics of 68 patients who received mBCQT and 68 who received CT did not differ significantly.On intention-to-treat analysis,the eradication rate was 88.2%(60/68)in the mBCQT group and 79.4%(54/68)in the CT group(P=0.162).By per-protocol analysis,the respective eradication rates were 98.4%(60/61)and 93.1%(54/58)(P=0.199).More CT than mBCQT patients experienced adverse events[33.8%(23/68)mBCQT vs 51.5%(35/58)CT patients,respectively,P=0.037].All patients showed good compliance[85.3%(58/68)mBCQT vs 82.4%(56/68)CT patients,P=0.641].CONCLUSION The H.pylori eradication rates of the 2-wk mBCQT and CT regimens are high.Most patients show good compliance,and more CT than mBCQT patients experience adverse events.So Jeong Kim Jun-Won Chung Hyun Sun Woo Su Young Kim Jung Ho Kim Yoon Jae Kim Kyoung Oh Kim Kwang An Kwon Dong Kyun Park 2019World Journal of Gastroenterology2019,25,46:2
