Glutamate reduces experimental intestinal hyperpermeability and facilitates glutamine support of gut integrity显示文摘AIM:To assess whether glutamate plays a similar role to glutamine in preserving gut wall integrity.METHODS:The effects of glutamine and glutamate on induced hyperpermeability in intestinal cell lines were studied.Paracellular hyperpermeability was induced in Caco2.BBE and HT-29CL.19A cell lines by adding phorbol-12,13-dibutyrate(PDB) apically, after which the effects of glutamine and glutamate on horseradish peroxidase(HRP) diffusion were studied. An inhibitor of glutamate transport(L-trans-pyrrolidine-2,4-dicarboxylic acid:trans-PDC) and an irreversible blocker(acivicin) of the extracellular glutamine to glutamate converting enzyme,γ-glutamyltransferase,were used.RESULTS:Apical to basolateral HRP flux increased significantly compared to controls not exposed to PDB (n=30,P<0.001).Glutamine application reduced hyperpermeability by 19%and 39%in the respective cell lines.Glutamate application reduced hyperpermeability by 30%and 20%,respectively.Incubation of HT29CL.19A cells with acivicin and subsequent PDB and glutamine addition increased permeability levels.Incubation of Caco2.BBE cells with trans-PDC followed by PDB and glutamate addition also resulted in high permeability levels.CONCLUSION:Apical glutamate-similar to glutaminecan decrease induced paracellular hyperpermeability.Extracellular conversion of glutamine to glutamate and subsequent uptake of glutamate could be a pivotal step in the mechanism underlying the protective effect of glutamine. | Mechteld AR Vermeulen Jeffrey de Jong Mathijs J Vaessen Paul AM van Leeuwen Alexander PJ Houdijk | 2011 | World Journal of Gastroenterology2011,17,12: | 9

Clinical course of subepithelial lesions detected on upper gastrointestinal endoscopy显示文摘AIM:To evaluate the natural history of subepithelial lesions.METHODS:We reviewed the medical records of 104 159 patients who underwent upper gastrointestinal endoscopy at the Center for Health Promotion of Samsung Medical Center between 1996 and 2003.Subepithelial lesions were detected in 795 patients(0.76%);252 patients were followed using upper gastrointestinal endoscopy for 82.5 ± 29.2 mo(range,12-160 mo;median,84 mo;1st quartile,60 mo;3rd quartile,105 mo).The median interval of follow-up endoscopy was 12 mo(range,6-105 mo;1st quartile,12 mo;3rd quartile,24 mo).RESULTS:The mean patient age was 53 years(range,22-80 years),and the male-to-female ratio was 2.36:1(177/75).The lesion size at initial measurement averaged 8.9 mm(range,2-25 mm;median,8 mm;1st quartile,5 mm;3rd quartile,10 mm).Of the 252 lesions,244(96.8%) were unchanged and 8(3.2%) were significantly increased in size(from 12.9 ± 6.0 to 21.2 ± 12.2 mm) after a mean interval of 59.1 ± 27.5 mo(range,12-86 mo).Surgical resection of lesions was performed when the lesions were ≥ 3 cm in diameter. Two lesions were diagnosed as gastrointestinal stromal tumors with an intermediate or high risk of malignancy and one lesion was classified as a schwannoma.CONCLUSION:Most small subepithelial lesions do not change as shown by endoscopic examination, and regular follow-up with endoscopy may be considered in small, subepithelial lesions, especially lesions < 1 cm in size. | Yeun Jung Lim Hee Jung Son Jong-Soo Lee Young Hye Byun Hyun Joo Suh Pool Lyul Rhee Jae J Kim Jong Chul Rhee | 2010 | World Journal of Gastroenterology2010,16,4: | 6
m TOR signaling in liver regeneration: Rapamycin combined with growth factor treatment显示文摘AIM: To investigate the effects of mammalian target of rapamycin(mT OR) inhibition on liver regeneration and autophagy in a surgical resection model.METHODS: C57BL/6 mice were subjected to a 70% partial hepatectomy(PH) and treated intraperitoneally every 24 h with a combination of the m TOR inhibitor rapamycin(2.5 mg/kg per day) and the steroid dexamethasone(2.0 mg/kg per day) in phosphate bufferedsaline(PBS) or with PBS alone as vehicle control. In the immunosuppressant group, part of the group was treated subcutaneously 4 h prior to and 24 h after PH with a combination of human recombinant interleukin 6(IL-6; 500 μg/kg per day) and hepatocyte growth factor(HGF; 100 μg/kg per day) in PBS. Animals were sacrificed 2, 3 or 5 d after PH and liver tissue and blood were collected for further analysis. Immunohistochemical staining for 5-Bromo-2'-deoxyuridine(Brd U) was used to quantify hepatocyte proliferation. Western blotting was used to detect hepatic microtubule-associated protein 1 light chain 3(LC3)-Ⅱ protein expression as a marker for autophagy. Hepatic gene expression levels of proliferation-, inflammation- and angiogenesisrelated genes were examined by real-time reverse transcription-polymerase chain reaction and serum bilirubin and transaminase levels were analyzed at the clinical chemical core facility of the Erasmus MC-University Medical Center.RESULTS: m TOR inhibition significantly suppressed regeneration, shown by decreased hepatocyte proliferation(2% vs 12% Brd U positive hepatocyte nuclei at day 2, P < 0.01; 0.8% vs 1.4% at day 5, P = 0.02) and liver weight reconstitution(63% vs 76% of initial total liver weight at day 3, P = 0.04), and furthermore increased serum transaminase levels(aspartate aminotransferase 641 U/L vs 185 U/L at day 2, P = 0.02). Expression of the autophagy marker LC3-Ⅱ, which was reduced during normal liver regeneration, increased after mT OR inhibition(46% increase at day 2, P = 0.04). Hepatic gene expression showed an increased inflammation-related response [tumor necrosis factor(TNF)-α 3.2-fold upregulation at day 2, P = 0.03; IL-1Ra 6.0-fold upregulation at day 2 and 42.3-fold upregulation at day 5, P < 0.01] and a reduced expression of cell cycle progression and angiogenesis-related factors(HGF 40% reduction at day 2; vascular endothelial growth factor receptor 2 50% reduction at days 2 and 5; angiopoietin 1 60% reduction at day 2, all P ≤ 0.01). Treatmentwith the regeneration stimulating cytokine IL-6 and growth factor HGF could overcome the inhibitory effect on liver weight(75% of initial total liver weight at day 3, P = 0.02 vs immunosuppression alone and P = 0.90 vs controls) and partially reversed gene expression changes caused by rapamycin(TNF-α and IL-1Ra levels at day 2 were restored to control levels). However, no significant changes in hepatocyte proliferation, serum injury markers or autophagy were found.CONCLUSION: mT OR inhibition severely impairs liver regeneration and increases autophagy after PH. These effects are partly reversed by stimulation of the IL-6 and HGF pathways. | Suomi MG Fouraschen Petra E de Ruiter Jaap Kwekkeboom Ron WF de Bruin Geert Kazemier Herold J Metselaar Hugo W Tilanus Luc JW van der Laan Jeroen de Jonge | 2013 | World Journal of Transplantation2013,3,3: | 6

On tolerability and safety of a maintenance treatment with 6-thioguanine in azathioprineor 6-mercaptopurine intolerant IBD patients显示文摘AIM: To determine the tolerability and safety profile of a low-dose maintenance therapy with 6-TG in azathioprine (AZA) or 6-mercaptopurine (6-MP) intolerant inflammatory bowel disease (IBD) patients over a treatment period of at least 1 year.METHODS: Database analysis.RESULTS: Twenty out of ninety-five (21%) patients discontinued 6-TG (mean dose 24.6 mg; mean 6-TGN level 540 pmol/8×108 RBC) within 1 year. Reasons for discontinuation were GI complaints (31%), malaise (15%)and hepatotoxicity (15%). Hematological events occurred in three patients, one discontinued treatment. In the 6-TG-tolerant group, 9% (7/75) could be classified as hepatotoxicity. An abdominal ultrasound was performed in 54% of patients, one patient had splenomegaly.CONCLUSION: The majority of AZA or 6-MP-intolerant IBD patients (79%) is able to tolerate maintenance treatment with 6-TG (dosages between 0.3 and 0.4 mg/kg per d). 6-TG may still be considered as an escape maintenance immunosuppressant in this difficult to treat group of patients, taking into account potential toxicity and efficacy of other alternatives. The recently reported hepatotoxicity is worrisome and 6-TG should therefore be administered only in prospective trials. | Nanne KH de Boer Luc JJ Derijks Lennard PL Gilissen Daniel W Hommes Leopold GJB Engels Sybrand Y de Boer Gijsbertus den Hartog Piet M Hooymans Anja BU M(?)kelburg Barend D Westerveld Anton HJ Naber Chris JJ Mulder Dirk J de Jong | 2005 | World Journal of Gastroenterology2005,11,35: | 4

Prevention of overuse: A view on upper gastrointestinal endoscopy显示文摘Many upper gastrointestinal(GI) endoscopies worldwide are performed for inappropriate indications. This overuse of healthcare negatively affects healthcare quality and puts pressure on endoscopy services. Dyspepsia is one of the most common inappropriate indications for upper GI endoscopy as diagnostic yield is low. Reasons for untimely referral are: unfamiliarity with dyspepsia guidelines, uncertainty about etiology of symptoms, and therapy failure. Unfiltered openaccess referrals feed upper GI endoscopy overuse. This review highlights strategies applied to diminish use of upper GI endoscopies for dyspepsia. First,we describe the impact of active guideline implementation. We found improved guideline adherence, but resistance was encountered in the process. Secondly, we show several forms of clinical assessment. While algorithm use reduced upper GI endoscopy volume, effects of referral assessment of individual patients were minor. A third strategy proposed Helicobacter pylori test and treat for all dyspeptic patients. Many upper GI endoscopies can be avoided using this strategy, but outcomes may be prevalence dependent. Lastly, empirical treatment with Proton pump inhibitors achieved symptom relief for dyspepsia and avoided upper GI endoscopies in about two thirds of patients. Changing referral behavior is complex as contributing factors are manifold. A collaboration of multiple strategies is most likely to succeed. | Judith J de Jong Marten A Lantinga Joost PH Drenth | 2019 | World Journal of Gastroenterology2019,25,2: | 3

STATISTICS显示文摘Revised September 2013 by G.Cowan(RHUL).This chapter gives an overview of statistical methods used in high-energy physics.In statistics,we are interested in using a given sample of data to make inferences about a probabilistic model,e.g.,to assess the model's validity or to determine the values of its parameters.There are two main approaches to statistical inference,which we may call frequentist and Bayesian.In frequentist statistics,probability is interpreted as the frequency of the outcome of a repeatable experiment.The most important tools in this framework are parameter estimation,covered in Section 38.2. | K.A.Olive K.Agashe C.Amsler M.Antonelli J.-F.Arguin D.M.Asner H.Baer H.R.Band R.M.Barnett T.Basaglia C.W.Bauer J.J.Beatty V.I.Belousov J.Beringer G.Bernardi S.Bethke H.Bichsel O.Biebe E.Blucher S.Blusk G.Brooijmans O.Buchmueller V.Burkert M.A.Bychkov R.N.Cahn M.Carena A.Ceccucci A.Cerr D.Chakraborty M.-C.Chen R.S.Chivukula K.Copic G.Cowan O.Dahl G.D'Ambrosio T.Damour D.de Florian A.de Gouvea T.DeGrand P.de Jong G.Dissertor B.A.Dobrescu M.Doser M.Drees H.K.Dreiner D.A.Edwards S.Eidelman J.Erler V.V.Ezhela W.Fetscher B.D.Fields B.Foster A.Freitas T.K.Gaisser H.Gallagher L.Garren H.-J.Gerber G.Gerbier T.Gershon T.Gherghetta S.Golwala M.Goodman C.Grab A.V.Gritsan C.Grojean D.E.Groom M.Grnewald A.Gurtu T.Gutsche H.E.Haber K.Hagiwara C.Hanhart S.Hashimoto Y.Hayato K.G.Hayes M.Heffner B.Heltsley J.J.Hernandez-Rey K.Hikasa A.Hocker J.Holder A.Holtkamp J.Huston J.D.Jackson K.F.Johnson T.Junk M.Kado D.Karlen U.F.Katz S.R.Klein E.Klempt R.V.Kowalewski F.Krauss M.Kreps B.Krusche Yu.V.Kuyanov Y.Kwon O.Lahav J.Laiho P.Langacker A.Liddle Z.Ligeti C.-J.Lin T.M.Liss L.Littenberg K.S.Lugovsky S.B.Lugovsky F.Maltoni T.Mannel A.V.Manohar W.J.Marciano A.D.Martin A.Masoni J.Matthews D.Milstead P.Molaro K.Monig F.Moortgat M.J.Mortonson H.Murayama K.Nakamura M.Narain P.Nason S.Navas M.Neubert P.Nevski Y.Nir L.Pape J.Parsons C.Patrignani J.A.Peacock M.Pennington S.T.Petcov Kavli IPMU A.Piepke A.Pomarol A.Quadt S.Raby J.Rademacker G.Raffel B.N.Ratcliff P.Richardson A.Ringwald S.Roesler S.Rolli A.Romaniouk L.J.Rosenberg J L.Rosner G.Rybka C.T.Sachrajda Y.Sakai G.P.Salam S.Sarkar F.Sauli O.Schneider K.Scholberg D.Scott V.Sharma S.R.Sharpe M.Silari T.Sjostrand P.Skands J.G.Smith G.F.Smoot S.Spanier H.Spieler C.Spiering A.Stahl T.Stanev S.L.Stone T.Sumiyoshi M.J.Syphers F.Takahashi M.Tanabashi J.Terning L.Tiator M.Titov N.P.Tkachenko N.A.Tornqvist D.Tovey G.Valencia G.Venanzoni M.G.Vincter P.Vogel A.Vogt S.P.Wakely W.Walkowiak C.W.Walter D.R.Ward G.Weiglein D.H.Weinberg E.J.Weinberg M.White L.R.Wiencke C.G.Wohl L.Wolfenstein J.Womersley C.L.Woody R.L.Workman A.Yamamoto W.-M.Yao G.P.Zeller O.V.Zenin J.Zhang R.-Y.Zhu F.Zimmermann P.A.Zyla G.Harper V.S.Lugovsky P.Schaffner | 2014 | Chinese Physics C2014,38,9: | 3
Consensus on the definition of colorectal anastomotic leakage: A modified Delphi study显示文摘BACKGROUND Despite the emerging knowledge about colorectal anastomotic leakage(CAL)through the increasing number of clinical and experimental studies, there is no generally accepted definition of CAL. Because of the wide variety of definitions used in literature, comparison of study outcomes and quality of care is complicated.AIM To reach consensus on the definition of CAL using a modified Delphi method.METHODS The RAND/UCLA appropriateness method was used. The expert panel consisted of international colorectal surgeons and researchers who had published three or more articles about CAL. The consensus process consisted of two online distributed questionnaires and a third round with a recommendation. In the questionnaires participants were asked to rate the appropriateness of statements using a 1-9 Likert scale. Consensus was defined as a panel median between 1-3 or 7-9 without disagreement. In the final round a recommendation was formed regarding the definition of CAL and the expert panel was asked if they agreed or disagreed.RESULTSTwenty-three authors participated in the first round and twenty-one finished the second round. After two rounds consensus was reached on 37 items(80%) in nine different categories. The International Study Group of Rectal Cancer definition is the most frequently advised general definition by our panel. Consensus was reached regarding the clinical symptoms of CAL, which serum markers contributes to the suspicion of CAL, which radiological and perioperative findings should be considered as CAL, which grading system is appropriate and if there should be a range of postoperative days in the definition. Eventually, 19 experts completed all three rounds of which 16(84%) agreed with our final recommendations for the definition of CAL.CONCLUSION A consensus-based recommendation for the definition of CAL was formed using our modified Delphi method that can be widely incorporated in the field. | Claire PM van Helsdingen Audrey CHM Jongen Wouter J de Jonge Nicole D Bouvy Joep PM Derikx | 2020 | World Journal of Gastroenterology2020,26,23: | 3
