2Measurement and Prediction of Insertion Force for the Mosquito Fascicle Penetrating into Human Skin显示文摘Mosquitoes are exceptional in their ability to pierce into human skin with a natural ultimate painless microneedle, namedfascicle. Here the structure of the Aedes albopictus mosquito fascicle is obtained using a Scanning Electron Microscope (SEM),and the whole process of the fascicle inserting into human skin is observed using a high-speed video imaging technique. Directmeasurements of the insertion force for mosquito fascicle to penetrate into human skin are reported. Results show that themosquito uses a very low force (average 18 μN) to penetrate into the skin. This force is at least three orders of magnitude smallerthan the reported lowest insertion force for an artificial microneedle with an ultra sharp tip to insert into the human skin. In orderto understand the piercing mechanism of mosquito fascicle tip into human multilayer skin tissue, a numerical simulation isconducted to analyze the insertion process using a nonlinear finite element method. 3Grain Size and Wettability of TiO_2/SiO_2 Photocatalytic Composite Thin Filing显示文摘The uniform transparent TiO2/SiO2 photocatalytic composite thin films are prepared by sol-gel method on the soda lime glass substrates, and characterized by UV-visible spectroscopy, X-ray diffraction (XRD), transmission electron microscopy (TEM), BET surface area, FTIR spectroscopy and X-ray photoelectron spectroscopy (XPS). It was found that the addition of SiO2 to TiO2 thin films can suppress the grain growth of TiO2 crystal, increase the hydroxyl content on the surface of TiO2 films, lower the contact angle for water on TiO2 films and enhance the hydrophilic property of TiO2 films. 4A Novel Semiconductor CIGS Photovoltaic Material and Thin-Film ED Technology显示文摘In order to achieve low cost high efficiency thin film solar cells,a novel Semiconductor Photovoltaic (PV) active material CuIn 1-x Ga x Se 2 (CIGS) and thin film Electro Deposition (ED) technology is explored.Firstly,the PV materials and technologies is investigated,then the detailed experimental processes of CIGS/Mo/glass structure by using the novel ED technology and the results are reported.These results shows that high quality CIGS polycrystalline thin films can be obtained by the ED method,in which the polycrystalline CIGS is definitely identified by the (112),(204,220) characteristic peaks of the tetragonal structure,the continuous CIGS thin film layers with particle average size of about 2μm of length and around 1 6μm of thickness.The thickness and solar grade quality of CIGS thin films can be produced with good repeatability.Discussion and analysis on the ED technique,CIGS energy band and sodium (Na) impurity properties,were also performed.The alloy CIGS exhibits not only increasing band gap with increasing x ,but also a change in material properties that is relevant to the device operation.The beneficial impurity Na originating from the low cost soda lime glass substrate becomes one prerequisite for high quality CIGS films.These novel material and technology are very useful for low cost high efficiency thin film solar cells and other devices.ZHENG Guang fu 1,YANG Hong xing 1,MAN Cheuk ho 1,WONG Wing lok 2, AN Da wei 1 and John BURNETT 1(1 Centre for Development of Solar Energy Technology,Department of Building Services Engineering, The Hong Kong Polytechnic University,Hong Kong,C 2001Journal of Semiconductors2001,22,11:10
5High-resolution microendoscopy for esophageal cancer screening in China: A cost-effectiveness analysis显示文摘AIM:To study the cost-effectiveness of high-resolution microendoscopy(HRME)in an esophageal squamous cell carcinoma(ESCC)screening program in China.METHODS:A decision analytic Markov model of ESCC was developed.Separate model analyses were conducted for cohorts consisting of an averagerisk population or a high-risk population in China.Hypothetical 50-year-old individuals were followed until age 80 or death.We compared three different strategies for both cohorts:(1)no screening;(2)standard endoscopic screening with Lugol’s iodine staining;and(3)endoscopic screening with Lugol’s iodine staining and an HRME.Model parameters were estimated from the literature as well as from GLOBOCAN,the Cancer Incidence and Mortality Worldwide cancer database.Health states in the model included non-neoplasia,mild dysplasia,moderate dysplasia,high-grade dysplasia,intramucosal carcinoma,operable cancer,inoperable cancer,and death.Separate ESCC incidence transition rates were generated for the average-risk and high-risk populations.Costs in Chinese currency were converted to international dollars(I$)and were adjusted to 2012dollars using the Consumer Price Index.RESULTS:The main outcome measurements for this study were quality-adjusted life years(QALYs)and incremental cost-effectiveness ratio(ICER).For the average-risk population,the HRME screening strategy produced 0.043 more QALYs than the no screening strategy at an additional cost of I$646,resulting in an ICER of I$11808 per QALY gained.Standard endoscopic screening was weakly dominated.Among the high-risk population,when the HRME screening strategy was compared with the standard screening strategy,the ICER was I$8173 per QALY.For both the high-risk and average-risk screening populations,the HRME screening strategy appeared to be the most cost-effective strategy,producing ICERs below the willingness-topay threshold,I$23500 per QALY.One-way sensitivity analysis showed that,for the average-risk population,higher specificity of Lugol’s iodine(>40%)and lower specificity of HRME(<70%)could make Lugol’s iodine screening cost-effective.For the high-risk population,the results of the model were not substantially affected by varying the follow-up rate after Lugol’s iodine screening,Lugol’s iodine test characteristics(sensitivity and specificity),or HRME specificity.CONCLUSION:The incorporation of HRME into an ESCC screening program could be cost-effective in China.Larger studies of HRME performance are needed to confirm these findings.Chin Hur Sung Eun Choi Chung Yin Kong Gui-Qi Wang Hong Xu Alexandros D Polydorides Li-Yan Xue Katherine E Perzan Angela C Tramontano Rebecca R Richards-Kortum Sharmila Anandasabapathy 2015World Journal of Gastroenterology2015,21,18:8

7Primary squamous cell carcinoma of the rectum: An update and implications for treatment显示文摘AIM: To provide an update on the aetiology, pathogenesis, diagnosis, staging and management of rectal squamous cell carcinoma(SCC).METHODS: A systematic review was conducted according to the preferred reporting items for Systematic Reviews and Meta-Analyses guidelines. A comprehensive search of Ovid MEDLINE was performed with the reference list of selected articles reviewed to ensure all relevant publications were captured. The search strategy was limited to the English language, spanning from 1946 to 2015. A qualitative analysis was undertaken examining patient demographics, clinical presentation, diagnosis, staging, treatment and outcome. The quantitaive analysis was limited to data extracted on treatment and outcomes including radiological, clinical and pathological complete response where available. The narrative and quantitative review were synthesised in concert.RESULTS: The search identified 487 articles in total with 79 included in the qualitative review. The quantitative analysis involved 63 articles, consisting of 43 case reports and 20 case series with a total of 142 individual cases. The underlying pathogenesis of rectal SCC while unclear, continues to be defined, with increasing evidence of a metaplasia-dysplasia-carcinoma sequence and a possible role for human papilloma virus in this progression. The presentation is similar to rectal adenocarcinoma, with a diagnosis confirmed by endoscopic biopsy. Many presumed rectal SCC's are in fact an extension of an anal SCC, and cytokeratin markers are a useful adjunct in this distinction. Staging is most accurately reflected by the tumour-nodemetastasis classification for rectal adenocarcinoma. It involves examining locoregional disease by way of magnetic resonance imaging and/or endorectal ultrasound, with systemic spread excluded by way of computed tomography. Positron emission tomography is integral in the workup to exclude an external siteof primary SCC with metastasis to the rectum. While the optimal treatment remains as yet undefined, recent studies have demonstrated a global shift away from surgery towards definitive chemoradiotherapy as primary treatment. Pooled overall survival was calculated to be 86% in patients managed with chemoradiation compared with 48% for those treated traditionally with surgery. 10成人脊柱畸形环形微创手术矫形分期治疗方案显示文摘为降低开放手术并发症的发生风险,过去十余年人们一直致力于成人脊柱畸形(adult spinal deformity,ASD)微创手术(minimally invasive surgery,MIS)方法的研究。在众多微创术式中,环形MIS(circumferential MIS,cMIS)相对于常规开放手术,可有效发挥微创优势。我们采用最新cMIS手术方案治疗1例严重ASD患者,并对其术前评估、手术规划及优化术式选择进行了总结。术前评估包括详细病史采集、体格检查及影像学检查等。其中影像学检查有站立位全脊柱X线检查(测量患者畸形程度和脊柱骨盆参数)、MRI检查(评估椎管狭窄情况,确保无椎间盘碎片游离于椎管内),如疑有节段融合,则需行CT扫描,以观察关节面融合或椎体间融合情况,指导椎间融合器的合理应用,实现对脊柱矫形手术的有效干预。cMIS手术分两期进行,一期经斜外侧微创入路行腰椎椎间融合,融合范围包括参与测量Cobb角的所有节段。上端椎水平位于X线片及MRI中第一个正常平行椎间盘的下位椎体;远端如有局部椎管狭窄,椎体不稳、倾斜或退行性变,矢状面失衡超过10 cm或骨质疏松,则可将融合范围延长至L5~S1水平。患者一期手术结束后当天即可下地行走,术后2 d或3 d复查站立位X线片,可根据新的影像学测量参数适当调整手术方案。一期术后2~3 d开始做二期手术,对涉及的所有节段行经皮椎弓根螺钉固定,置钉同时经肌间隙显露相应的椎弓根峡部及关节突关节,对其进行去皮质操作,以实现胸椎节段的后路融合。Anand N Kong C Fessler RG 梁达轩 吴增晖 2017中国骨科临床与基础研究杂志2017,9,6:4

14The Novel Mechanical Property of Tongue of a Woodpecker显示文摘Biomaterials such as bone,teeth,nacre and silk are known to have superior mechanical properties due to their specificnanocomposite structures.Here we report that the woodpecker’s tongue exhibits a novel strength and flexibility due to its specialcomposite micro/nanostructure.The tongue consists of a flexible cartilage-and-bone skeleton covered with a thin layer tissue ofhigh strength and elasticity.At the interface between the cartilage-and-bone skeleton and the tissue layer,there is a hierarchicalfiber-typed connection.It is this special design of the tongue that makes the woodpeckers efficient in catching the insects insidetrees.The special micro/nanostructures of the woodpecker’s tongue show us a potential method to enhance the interfacialconnection between soft and hard material layers for bio-inspired composite system designs.P Zhou X Q Kong C W Wu Z Chen 2009Journal of Bionic Engineering2009,6,3:3
