1Adjuvant sorafenib after heptectomy for Barcelona Clinic Liver Cancer-stage C hepatocellular carcinoma patients显示文摘AIM: To investigate the efficacy and safety of adjuvant sorafenib after curative resection for patients with Barcelona Clinic Liver Cancer(BCLC)-stage C hepatocellular carcinoma(HCC).METHODS: Thirty-four HCC patients, classified as BCLC-stage C, received adjuvant sorafenib for highrisk of tumor recurrence after curative hepatectomy at a tertiary care university hospital. The study group was compared with a case-matched control group of 68 patients who received curative hepatectomy for HCC during the study period in a 1:2 ratio.RESULTS: The tumor recurrence rate was markedly lower in the sorafenib group(15/34, 44.1%) than in the control group(51/68, 75%, P = 0.002). The median disease-free survival was 12 mo in the study group and 10 mo in the control group. Tumor number more than 3, macrovascular invasion, hilar lymph nodes metastasis, and treatment with sorafenib were significant factors of disease-free survival by univariate analysis. Tumor number more than 3 and treatment with sorafenib were significant risk factors of diseasefree survival by multivariate analysis in the Cox proportional hazards model. The disease-free survival and cumulative overall survival in the study group were significantly better than in the control group(P = 0.034 and 0.016, respectively). 5Complete response to sorafenib in a patient with recurrent hepatocellular carcinoma显示文摘Partial hepatectomy is still the treatment of choice aiming at a cure for patients with hepatocellular carcinoma(HCC), provided that the patient can tolerate the treatment. For patients with multiple recurrent HCC after partial hepatectomy which cannot be treated by re-hepatectomy or local ablative therapy, the prognosis is extremely poor. sorafenib is a molecular-targeted agent which has been demonstrated in two global phase III randomized controlled trials to show survival benefit for advanced HCC. Here, we present a 56-yearold patient with HCC who showed complete clinical response after sorafenib was used for tumor recurrence which developed 3 mo after partial hepatectomy. 6A GLP-1 analog lowers ER stress and enhances protein folding to ameliorate homocysteine-induced endothelial dysfunction显示文摘Hyperhomocysteinemia(HHcy)is an independent risk factor for cardiovascular diseases and increases mortality in type 2 diabetic patients.HHcy induces endoplasmic reticulum(ER)stress and oxidative stress to impair endothelial function.The glucagon-like peptide 1(GLP-1)analog exendin-4 attenuates endothelial ER stress,but the detailed vasoprotective mechanism remains elusive.The present study investigated the beneficial effects of exendin-4 against HHcy-induced endothelial dysfunction.Exendin-4 pretreatment reversed homocysteine-induced impairment of endothelium-dependent relaxations in C57BL/6 mouse aortae ex vivo.Four weeks subcutaneous injection of exendin-4 restored the impaired endothelial function in both aortae and mesenteric arteries isolated from mice with diet-induced HHcy.Exendin-4 treatment lowered superoxide anion accumulation in the mouse aortae both ex vivo and in vivo.Exendin-4 decreased the expression of ER stress markers(e.g.,ATF4,spliced XBP1,and phosphorylated eIF2α)in human umbilical vein endothelial cells(HUVECs),and this change was reversed by cotreatment with compound C(CC)(AMPK inhibitor).Exendin-4 induced phosphorylation of AMPK and endothelial nitric oxide synthase in HUVECs and arteries.Exendin-4 increased the expression of endoplasmic reticulum oxidoreductase(ERO1α),an important ER chaperone in endothelial cells,and this effect was mediated by AMPK activation.Experiments using siRNA-mediated knockdown or adenoviral overexpression revealed that ERO1αmediated the inhibitory effects of exendin-4 on ER stress and superoxide anion production,thus ameliorating HHcy-induced endothelial dysfunction.The present results demonstrate that exendin-4 reduces HHcy-induced ER stress and improves endothelial function through AMPK-dependent ERO1αupregulation in endothelial cells and arteries.AMPK activation promotes the protein folding machinery in endothelial cells to suppress ER stress.Chak Kwong Cheng Jiang-Yun Luo Chi Wai Lau William Chi-shing Cho Chi Fai Ng Ronald Ching Wan Ma Xiao Yu Tian Yu Huang 2021Acta Pharmacologica Sinica2021,42,10:6
7Consensus on the digestive endoscopic tunnel technique显示文摘With the digestive endoscopic tunnel technique(DETT), many diseases that previously would have been treated by surgery are now endoscopically curable by establishing a submucosal tunnel between the mucosa and muscularis propria(MP). Through the tunnel, endoscopic diagnosis or treatment is performed for lesions in the mucosa, in the MP, and even outside the gastrointestinal(GI) tract.At present, the tunnel technique application range covers the following:(1)Treatment of lesions originating from the mucosal layer, e.g., endoscopic submucosal tunnel dissection for oesophageal large or circular early-stage cancer or precancerosis;(2) treatment of lesions from the MP layer, per-oral endoscopic myotomy, submucosal tunnelling endoscopic resection, etc.; and(3) diagnosis and treatment of lesions outside the GI tract, such as resection of lymph nodes and benign tumour excision in the mediastinum or abdominal cavity. With the increasing number of DETTs performed worldwide, endoscopic tunnel therapeutics, which is based on DETT, has been gradually developed and optimized. However, there is not yet an expert consensus on DETT to regulate its indications, contraindications, surgical procedure, and postoperative treatment.The International DETT Alliance signed up this consensus to standardize the procedures of DETT. 9Intrahepatic Glissonian approach and outflow vascular occlusion during partial hepatectomy显示文摘Under ultrasound guidance, a blunt suture needle was inserted around the Glissonian pedicle and then sutured. This technique significantly reduced the blood loss and facilitated the procedure of partial hepatectomy. We applied this technique in 182 patients who needed partial hepatectomy. 10含高钙和维生素D的强化奶能有效提高健康的绝经后的中国女性体内的维生素D水平并减少骨吸收显示文摘目的钙摄入水平低以及体内维生素D不足是导致亚洲女性罹患骨质疏松的危险因素。此项研究的目的是评估高钙维生素D强化牛奶(HCM)对健康的中国绝经后妇女的体内维生素D状态和骨吸收标志物进行干预的影响。方法本试验对象为55岁以上,绝经至少5年的女性。共有124名女性进行了总体健康状况和骨密度检查筛选。63名女性被随机分为对照组和试验组,试验组在12w内每日饮用两次高钙/维生素D强化奶(含900 mg钙、96 mg镁、2.4 mg锌和6.4μg维生素D)。2组分别在试验开始、试验2 w、8 w和12 w时测量血矿物质、25(OH)维生素D3和Ⅰ型胶原C端肽(CTX)水平。结果对照组女性平均年龄为63±4.6岁,高钙强化奶(HCM)组女性平均年龄为62±3.8岁。所测得的每日钙摄入量的基线值为HCM组260~482 mg,对照组为252~692 mg。12 w后HCM组的平均身体质量指数(BMI)显著降低(P<0.05)。HCM组的血25(OH)维生素D3水平显著提高(33.13~39.49 nmol/L),而对照组则维持不变(29.27~28.21 nmol/L)。HCM组的血CTX水平在0 w和2 w之间下降了25%,并保持不变直至第12w。从第2w开始HCM组和对照组相比血CTX水平就出现了显著差异(P<0.001)。结论通过饮用含高钙和维生素D的强化奶(HCM),在2至4w内能显著提高中国绝经妇女体内维生素D水平,并减少骨吸收破坏。如果长期持续地饮用含高钙和维生素D的强化奶(HCM),能减少骨吸收和骨量丢失的风险,从而减少患骨质疏松和由此引起骨折的风险。Marlena C Kruger PengCheng Ha Joanne M Todd Barbara Kuhn-Sherlock Linda M Schollum Jiliang Ma George Qin Edith Lau 2013中国骨质疏松杂志2013,19,2:2
