1Single center experience of capsule endoscopy in patients with obscure gastrointestinal bleeding显示文摘AIM:To identify optimum timing to maximize diagnostic yield by capsule endoscopy (CE) in patients with obscure gastrointestinal bleeding (OGIB).METHODS:We identified patients who underwent CE at our institution from August 2003 to December 2009.Patient medical records were reviewed to determine type of OGIB (occult,overt),CE results and complications,and timing of CE with respect to onset of bleeding.RESULTS:Out of 385 patients investigated for OGIB,284 (74%) had some lesion detected by CE.In 222 patients (58%),definite lesions were detected that could unequivocally explain OGIB.Small bowel ulcer/erosions secondary to Crohn's disease,tuberculosis or non-steroidal anti-inflammatory agent use were the commonest lesions detected.Patients with overt GI bleeding for < 48 h before CE had the highest diagnostic yield (87%).This was significantly greater (P < 0.05) compared to that in patients with overt bleeding prior to 48 h (68%),as well as those with occult OGIB (59%).CONCLUSION:We established the importance of early CE in management of OGIB.CE within 48 h of overt bleeding has the greatest potential for lesion detection.Mahesh Kumar Goenka Shounak Majumder Sanjeev Kumar Pradeepta Kumar Sethy Usha Goenka 2011World Journal of Gastroenterology2011,17,6:39
2Thermal decomposition and kinetics of plastic bonded explosives based on mixture of HMX and TATB with polymer matrices显示文摘This work describes thermal decomposition behaviour of plastic bonded explosives(PBXs) based on mixture of 1,3,5,7-tetranitro-1,3,5,7-tetrazocane(HMX) and 2,4,6-triamino-1,3,5-trinitrobenzene(TATB)with Viton A as polymer binder. Thermal decomposition of PBXs was undertaken by applying simultaneous thermal analysis(STA) and differential scanning calorimetry(DSC) to investigate influence of the HMX amount on thermal behavior and its kinetics. Thermogravimetric analysis(TGA) indicated that the thermal decomposition of PBXs based on mixture of HMX and TATB was occurred in a three-steps. The first step was mainly due to decomposition of HMX. The second step was ascribed due to decomposition of TATB, while the third step was occurred due to decomposition of the polymer matrices. The thermal decomposition % was increased with increasing HMX amount. The kinetics related to thermal decomposition were investigated under non-isothermal for a single heating rate measurement. The variation in the activation energy of PBXs based on mixture of HMX and TATB was observed with varying the HMX amount. The kinetics from the results of TGA data at various heating rates under non-isothermal conditions were also calculated by Flynn—Wall—Ozawa(FWO) and Kissinger-Akahira-Sunose(KAS)methods. The activation energies calculated by employing FWO method were very close to those obtained by KAS method. 3Current Status of Conventional and Molecular Interventions for Blast Resistance in Rice显示文摘Pyricularia oryzae anamorph of Magnaporthe oryzae is one of the most notorious fungal pathogens causing severe economic loss in rice production worldwide. Various methods, viz. cultural, biological and molecular approaches, are utilized to counteract this pathogen. Moreover, some tolerant or resistant rice varieties have been developed with the help of breeding programmes. Isolation and molecular characterization of different blast resistance genes now open the gate for new possibilities to elucidate the actual allelic variants of these genes via various molecular breeding and transgenic approaches. However, the behavioral pattern of this fungus breakups the resistance barriers in the resistant or tolerant rice varieties. This host-pathogen barrier will be possibly countered in future research by comparative genomics data from available genome sequence data of rice and M. oryzae for durable resistance. Present review emphasized fascinating recent updates, new molecular breeding approaches, transgenic and genomics approaches(i.e. mi RNA and genome editing) for the management of blast disease in rice. 4细菌性角膜溃疡的医院流行病学,诱因和微生物诊断(英文)显示文摘目的:确定细菌性角膜溃疡的发病诱因,临床和微生物特性。方法:本研究涉及300例(300眼)临床上疑似微生物感染的角膜溃疡患者。通过病史回顾和裂隙灯检查获得病例资料。采用标准技术完成角膜刮片检查。取部分刮片样本加入100g/L的氢氧化钾液后,以及采用革兰氏和吉姆萨染色,再直接在显微镜下观察是否存在真菌、细菌和棘阿米巴。另一部分刮片样本直接接种在血琼脂、麦康基琼脂、巧克力琼脂和萨布罗琼脂表面。通过角膜刮片检查,细菌性角膜溃疡被确诊为化脓性角膜溃疡浸润和表层上皮细胞缺失伴细菌感染。对患者应用抗菌治疗。结果:在随访期间,因丢失而被排除的有60个病例。在剩下的240人中,被确诊为细菌性角膜溃疡的156人(65%),大多数为男性(102眼),年龄分布14~74岁。角膜溃疡位于角膜中央的96人(61.5%),位于外周的60人(38.5%)。溃疡深度<1/3角膜厚度的82人(52.6%)。前房炎症为:Tyndall现象和浮游细胞1+~2+64人。角膜涂片中分离出细菌的有125人(80%),其中69%为革兰氏阳性,39%为革兰氏阴性。革兰氏阴性菌感染患者伴有重症前房炎症(P=0.003)并且深度>2/3角膜厚度(P=0.001)。分离得到的最常见致病微生物为金黄色葡萄球菌。治疗后40%的患者视力恢复到入院时的水平甚至更好,其余60%的患者预后差。结论:细菌性角膜溃疡是一种严重的眼部感染性疾病,对眼部存在视力威胁,在治疗上仍具挑战性。快速分离细菌和眼部抗生素的应用是治疗此类疾病的关键。Ashok Kumar Narsani Shafi Muhammad Jatoi Mahesh Kumar Lohana Syed Asher Dabir Siddiqa Gul Mahtab Alam Khanzada 2009国际眼科杂志2009,9,3:6

6Role of Joshi's external stabilization system with percutaneous screw fixation in high-energy tibial condylar fractures associated with severe soft tissue injuries显示文摘Purpose: The treatment of high-energy tibial condylar fractures which are associated with severe soft tissue injuries remains contentious and challenging. In this study, we assessed the results of Joshi's external stabilization system (JESS) by using the principle of ligamentotaxis and percutaneous screw fixation for managing high-energy tibial condylar fractures associated with severe soft tissue injuries. Methods: Between June 2008 and June 2010, 25 consecutive patients who were 17-71 years (mean, 39.7), underwent the JESS fixation for high-energy tibial condylar fractures associated with severe soft tissue injuries. Out of 25 patients, 2 were lost during follow-up and in 1 case early removal of frame was done, leaving 22 cases for final follow-up. Among them, 11 had poor skin condition with abrasions and blisters and 2 were open injuries (Gustilo-Anderson grade I & 11). The injury mechanisms were motor vehicle accidents (n - 19), fall from a height (n = 2) and assault (n - 1). The fractures were classified according to Schatzker classification system. Results: There were 7 type-V, 14 type-Vl and 1 type-IV Schatzker's tibial plateau fractures. The average interval between the injury and surgery was 6.8 days (range 2-13). The average hospital stay was 13 days (range, 7-22). The average interval between the surgery and full weight bearing was 13.6 weeks (range 11-20). The average range of knee flexion was 121° (range 105°-135°). The normal extension of the knee was observed in 20 patients, and an extensor lag of 5°-8° was noted in 2 patients. The complications included superficial pin tract infections (n 4) with no knee stiffness. Conclusion: JESS with lag screw fixation combines the benefit of traction, external fixation, and limited internal fixation, at the same time as allowing the ease of access to the soft tissue for wound checks, pin care, dressing changes, measurement of compartment pressure, and the monitoring of the neurovascular status. 7Effect of Low Level Subchronic Microwave Radiation on Rat Brain显示文摘Objective The present study was designed to investigate the effects of subchronic low level microwave radiation(MWR) on cognitive function,heat shock protein 70(HSP70) level and DNA damage in brain of Fischer rats.Methods Experiments were performed on male Fischer rats exposed to microwave radiation for 90 days at three different frequencies: 900,1800,and 2450 MHz.Animals were divided into 4 groups: Group I: Sham exposed,Group II: animals exposed to microwave radiation at 900 MHz and specific absorption rate(SAR) 5.953 × 10-4 W/kg,Group III: animals exposed to 1800 MHz at SAR 5.835 × 10-4 W/kg and Group IV: animals exposed to 2450 MHz at SAR 6.672 × 10-4 W/kg.All the animals were tested for cognitive function using elevated plus maze and Morris water maze at the end of the exposure period and subsequently sacrificed to collect brain tissues.HSP70 levels were estimated by ELISA and DNA damage was assessed using alkaline comet assay.Results Microwave exposure at 900-2450 MHz with SAR values as mentioned above lead to decline in cognitive function,increase in HSP70 level and DNA damage in brain.Conclusion The results of the present study suggest that low level microwave exposure at frequencies 900,1800,and 2450 MHz may lead to hazardous effects on brain.Pravin Suryakantrao Deshmukh Kanu Megha Namita Nasare Basu Dev Banerjee Rafat Sultana Ahmed Mahesh Pandurang Abegaonkar Ashok Kumar Tripathi Pramod Kumari Mediratta 2016Biomedical and Environmental Sciences2016,29,12:4

9Molecular docking studies of anti-cancerous candidates in Hippophae rhamnoides and Hippophae salicifolia显示文摘Actinorhizal plants contain numerous antioxidants that may play a crucial role in preventing the formation of tumors.H-Ras p21,a member of the Ras-GTPase family,is a promising target to treat various kinds of cancers.An in silico docking study was carried out to identify the inhibitory potential of compounds of these plants against H-Ras by using Discovery Studio 3.5 and by using Autodock 4.2.Docking studies revealed that four compounds,isorhamnetin-7-rhamnoside,quercetin-3-glucoside-7-rhamnoside(present in H.rhamnoides),zeaxanthin,and translutein(present in H.salicifolia) significantly bind with binding energies-17.1534,-14.7936,-10.2105 and-17.2217 Kcal/mol,respectively,even though they slightly deviate from Lipinski's rule.Absorption,distribution,metabolism,excretion and toxicity(ADME/tox) analyses of these compounds and their stereoisomers showed that they were less toxic and non-mutagenic.Amongst them,isorhamntein-7-rhamnoside showed hepatotoxicity.Hence,these compounds can be further investigated in vivo to optimize their formulation and concentration and to develop potential chemical entities for the prevention and treatment of cancers.Talambedu Usha Sushil Kumar Middha Arvind Kumar Goyal Mahesh Karthik DA Manoj Syed Faizan Peyush Goyal HP Prashanth Veena Pande 2014The Journal of Biomedical Research2014,28,5:3
10Endotherapy of leaks and fistula显示文摘Perforations, leaks and fistula involving gastrointestinal(GI) tract are increasing encountered in clinical practice. There is a changing paradigm for their management with surgical approach being replaced by conservative approach including endoscopic therapy. Clips(through the scope and over the scope) and covered stent are front runners for endotherapy for GI leaks and fistula.Over the scope clips introduced recently, can treat larger defects compared to through the scope clips. Covered stents are suited for larger defects and those associated with luminal narrowing. However cervical esophagus, gastro-esophageal junction, stomach and right colonic lesions may be better for clip therapy rather than stenting. Recent developments in this field include use of endovac therapy which consists of a sponge with suction device, biodegradable stent, use of fibrin glue and some endo-suturing device. Conservative therapy with no surgical or endoscopic intervention, may be suitable for a small subset of patients. 13Methanolic extract of Momordica cymbalaria enhances glucose uptake in L6 myotubes in vitro by up-regulating PPAR-γ and GLUT-4显示文摘The present study was undertaken to evaluate the influence of the methanolic fruit extract of Momordica cymbalaria(MFMC) on PPARγ(Peroxisome Proliferator Activated Receptor gamma) and GLUT-4(Glucose transporter-4) with respect to glucose transport. Various concentrations of MFMC ranging from 62.5 to 500 μg·mL-1 were evaluated for glucose uptake activity in vitro using L6 myotubes, rosiglitazone was used as a reference standard. The MFMC showed significant and dose-dependent increase in glucose uptake at the tested concentrations, further, the glucose uptake activity of MFMC(500 μg·mL-1) was comparable with rosigilitazone. Furthermore, MFMC has shown up-regulation of GLUT-4 and PPARγ gene expressions in L6 myotubes. In addition, the MFMC when incubated along with cycloheximide(CHX), which is a protein synthesis inhibitor, has shown complete blockade of glucose uptake. This indicates that new protein synthesis is required for increased GLUT-4 translocation. 18Analysis of groundwater level trend in Jakham River Basin of Southern Rajasthan显示文摘Groundwater accounts for about half of the water use for irrigation in India.The fluctuation pattern of the groundwater level is examined by observing rainfall replenishment and monitoring wells.The southern part of Rajasthan has experienced abrupt changes in rainfall and has been highly dependent on groundwater over decades.This study presents the impact of over-dependence on groundwater usage for irrigation and other purposes,spatially and temporally.Hence,the objective of this study is to examine the groundwater level trend by using statistical analysis and geospatial technique.Rainfall factor was also studied in groundwater level fluctuation during 2009-2019.To analyze the influence of each well during recharge or withdrawal of groundwater,thiessien polygonswere generated from them.In the Jakham River basin,75 wells have been identified for water level trend study using the Mann-Kendall statistical test.The statistics of trend analysis show that 15%wells are experiencing water level decline in pre-monsoon,while very low percentage of wells have such trend during post-monsoon season.The average rate of water level decline is 0.245 m/a in pre-monsoon and 0.05 m/a in post-monsoon.The aquifer recharge potential is also decreasing by year.it is expected that such type of studies will help the policy makers to adopt advanced management practices to ensure sustainable groundwater resource management.Vinay Kumar Gautam Mahesh Kothari P.K.Singh S.R.Bhakar K.K.Yadav 2022Journal of Groundwater Science and Engineering2022,10,1:2
