1Moving from control to elimination of schistosomiasis in sub-Saharan Africa:time to change and adapt strategies显示文摘Schistosomiasis is a water borne parasitic disease of global importance and with ongoing control the disease endemic landscape is changing.In sub-Saharan Africa,for example,the landscape is becoming ever more heterogeneous as there are several species of Schistosoma that respond in different ways to ongoing preventive chemotherapy and the inter-sectoral interventions currently applied.The major focus of preventive chemotherapy is delivery of praziquantel by mass drug administration to those shown to be,or presumed to be,at-risk of infection and disease.In some countries,regional progress may be uneven but in certain locations there are very real prospects to transition from control into interruption of transmission,and ultimately elimination.To manage this transition requires reconsideration of some of the currently deployed diagnostic tools used in surveillance and downward realignment of existing prevalence thresholds to trigger mass treatment.A key challenge will be maintaining and if possible,expanding the current donation of praziquantel to currently overlooked groups,then judging when appropriate to move from mass drug administration to selective treatment.In so doing,this will ensure the health system is adapted,primed and shown to be cost-effective to respond to these changing disease dynamics as we move forward to 2020 targets and beyond.Louis-Albert Tchuem Tchuente David Rollinson JRussell Stothard David Molyneux 2017Infectious Diseases of Poverty2017,6,1:8

3Economic cost analysis of malaria case management at the household level during the malaria elimination phase in The People’s Republic of China显示文摘Background:In China,malaria has been posing a significant economic burden on households.To evaluate malaria economic burden in terms of both direct and indirect costs has its meaning in improving the effectiveness of malaria elimination program in China.Methods:A number of study sites(eight counties in five provinces)were selected from the malaria endemic area in China,representing the different levels of malaria incidence,risk classification,economic development.A number of households with malaria cases(n=923)were surveyed during the May to December in 2012 to collect information on malaria economic burden.Descriptive statistics were used to characterize the basic profiles of selected malaria cases in terms of their gender,age group,occupation and malaria type.The malaria economic costs were evaluated by direct and indirect costs.Comparisons were carried out by using the chi-square test(or Z-test)and the Mann-Whitney U test among malaria cases with reference to local/imported malaria patients,hospitalized/out patients,and treatment hospitals.Results:The average cost of malaria per case was 1691.23 CNY(direct cost was 735.41 CNY and indirect cost was 955.82 CNY),which accounted for 11.1%of a household’s total income.The average costs per case for local and imported malaria were 1087.58 CNY and 4271.93 CNY,respectively.The average cost of a malaria patient being diagnosed and treated in a hospital at the county level or above(3975.43 CNY)was 4.23 times higher than that of malaria patient being diagnosed and treated at a village or township hospital(938.80 CNY).Conclusion:This study found that malaria has been posing a significant economic burden on households in terms of direct and indirect costs.There is a need to improve the effectiveness of interventions in order to reduce the impact costs of malaria,especially of imported infections,in order to eliminate the disease in China.Shang Xia Jin-Xiang Ma Duo-Quan Wang Shi-Zhu Li David Rollinson Shui-Sen Zhou Xiao-Nong Zhou 2016Infectious Diseases of Poverty2016,5,1:4
4Masirah-the other Oman ophiolite:A better analogue for mid-ocean ridge processes?显示文摘Oman has two ophiolites-the better known late Cretaceous northern Oman(or Semail)ophiolite and the lesser known and smaller,Jurassic Masirah ophiolite located on the eastern coast of the country adjacent to the Indian Ocean.A number of geological,geochronological and geochemical lines of evidence strongly suggest that the northern Oman ophiolite did not form at a mid-ocean ridge but rather in a supra-subduction zone setting by fast spreading during subduction initiation.In contrast the Masirah ophiolite is structurally part of a series of ophiolite nappes which are rooted in the Indian Ocean floor.There are significant geochemical differences between the Masirah and northern Oman ophiolites and none of the supra-subduction features typical of the northern Oman ophiolite are found at Masirah.Geochemically Masirah is MORB,although in detail it contains both enriched and depleted MORB reflecting a complex source for the lavas and dykes.The enrichment of this source predates the formation of the ophiolite.The condensed crustal section on Masirah(ca.2 km)contains a very thin gabbro sequence and is thought to reflect its genesis from a cool mantle source associated with the early stages of sea-floor spreading during the early separation of eastern and western Gondwana.These data suggest that the Masirah ophiolite is a suitable analogue for an ophiolite created at a mid-ocean ridge,whereas the northern Oman ophiolite is not.The stratigraphic history of the Masirah ophiolite shows that it remained a part of the oceanic crust for ca.80 Ma.The chemical variability and enrichment of the Masirah lavas is similar to that found elsewhere in Indian Ocean basalts and may simply reflect a similar provenance rather than a feature fundamental to the formation of the ophiolite.Hugh Rollinson 2017Geoscience Frontiers2017,8,6:2
5Towards interruption of schistosomiasis transmission in sub-Saharan Africa:developing an appropriate environmental surveillance framework to guide and to support‘end game’interventions显示文摘Schistosomiasis is a waterborne parasitic disease in sub-Saharan Africa,particularly common in rural populations living in impoverished conditions.With the scale-up of preventive chemotherapy,national campaigns will transition from morbidity-to transmission-focused interventions thus formal investigation of actual or expected declines in environmental transmission is needed as‘end game’scenarios arise.Surprisingly,there are no international or national guidelines to do so in sub-Saharan Africa.Our article therefore provides an introduction to key practicalities and pitfalls in the development of an appropriate environmental surveillance framework.In this context,we discuss how strategies need to be adapted and tailored to the local level to better guide and support future interventions through this transition.As detection of egg-patent infection in people becomes rare,careful sampling of schistosome larvae in freshwater and in aquatic snails with robust species-specific DNA assays will be required.Appropriate metrics,derived from observed prevalence(s)as compared with predetermined thresholds,could each provide a clearer insight into contamination-and exposure-related dynamics.Application could be twofold,first to certify areas currently free from schistosomiasis transmission or second to red-flag recalcitrant locations where extra effort or alternative interventions are needed.J.Russell Stothard Suzy J.Campbell Mike Y.Osei-Atweneboana Timothy Durant Michelle C.Stanton Nana-Kwadwo Biritwum David Rollinson Dieudonne R.Eloundou Ombede Louis-Albert Tchuem-Tchuente 2017Infectious Diseases of Poverty2017,6,1:2

9Elimination of schistosomiasis:the tools required显示文摘Background:Historically,the target in the schistosomiasis control has shifted from infection to morbidity,then back to infection,but now as a public health problem,before moving on to transmission control.Currently,all endemic countries are encouraged to increase control efforts and move towards elimination as required by the World Health Organization(WHO)roadmap for the global control of the neglected tropical diseases(NTDs)and the WHA65.21 resolution issued by the World Health Assembly.However,schistosomiasis prevalence is still alarmingly high and the global number of disability-adjusted life years(DALYs)due to this infection has in fact increased due to inclusion of some‘subtle’clinical symptoms not previously counted.Main body:There is a need to restart and improve efforts to reach the elimination goal.To that end,the first conference of the Global Schistosomiasis Alliance(GSA)Research Working Group was held in mid-June 2016 in Shanghai,People’s Republic of China.It reviewed current progress in schistosomiasis control and elimination,identified pressing operational research gaps that need to be addressed and discussed new tools and strategies required to make elimination a reality.The articles emanating from the lectures and discussions during this meeting,together with some additional invited papers,have been collected as a special issue of the‘Infectious Diseases of Poverty’entitled‘Schistosomiasis Research:Providing the Tools Needed for Elimination’,consisting of 26 papers in all.This paper refers to these papers and discusses critical questions arising at the conference related to elimination of schistosomiasis.Conclusion:The currently most burning questions are the following:Can schistosomiasis be eliminated?Does it require better,more highly sensitive diagnostics?What is the role of preventive chemotherapy at the elimination stage?Is praziquantel sufficient or do we need new drugs?Contemplating these questions,it is felt that the heterogeneity of the endemic areas in the world requires WHO policies to be upgraded instituting new,differentiated guidelines.Robert Bergquist Xiao-Nong Zhou David Rollinson Jutta Reinhard-Rupp Katharina Klohe 2017Infectious Diseases of Poverty2017,6,1:1
