1Diagnosis of pancreatic tumors by endoscopic ultrasound-guided fine-needle aspiration显示文摘AIM: To evaluate the diagnostic accuracy of endoscopic ultrasound-guided fi ne-needle aspiration (EUS-FNA) for pancreatic solid tumors larger or smaller than 3 cm, and cystic lesions. METHODS: From January/1997 to December/2006, 611 patients with pancreatic tumors were subjected to EUS-FNA. The fi nal diagnosis was obtained either by surgery (356 cases) or after a mean clinical follow-up of 11.8 mo in the remaining patients. RESULTS: There were 405 solid tumors, 189 cystic lesions and 17 mixed. Pancreatic specimens for cytological assessment were successfully obtained by EUS-FNA in 595 (97.4/) cases. There were 352 (57.6/) malignancies and 259 (42.4/) benign tumors. Among the malignancies, pancreatic adenocarcinomas accounted for 67/ of the lesions. Overall, the sensitivity, specifi city, positive and negative predictive values, and accuracy of EUS-FNA were, respectively, 78.4/, 99.2/, 99.3/, 77.2/ and 87.2/. Specif ically for solid tumors, the same parameters for neoplasms larger and smaller than 3 cm were, respectively, 78.8/ vs 82.4/, 100/ vs 98.4/, 100/ vs 99/, 54.8/ vs 74.1/ and 83.1/ vs 87.8/. For cystic lesions, the values were, respectively, 72.2/, 99.3/, 97.5/, 91/ and 92.2/. CONCLUSION: EUS-FNA can be used to sample pancreatic tumors in most patients. Even though the negative predictive value is inadequate for large solid tumors, the results are rather good for small solid tumors, especially concerning the sensitivity, negative predictive value and diagnostic accuracy. 2Fatty liver disease in severe obese patients:Diagnostic value of abdominal ultrasound显示文摘AIM:To evaluate the sensitivity and specificity of abdominal ultrasound (US) for the diagnosis of hepatic steatosis in severe obese subjects and its relation to histological grade of steatosis. METHODS: A consecutive series of obese patients, who underwent bariatric surgery from October 2004 to May 2005, was selected. Ultrasonography was performed in all patients as part of routine preoperative time and an intraoperative wedge biopsy was obtained at the beginning of the bariatric surgery. The US and histological findings of steatosis were compared, considering histology as the gold standard. RESULTS: The study included 105 patients. The mean age was 37.2 ± 10.6 years and 75.2% were female. The histological prevalence of steatosis was 89.5%. The sensitivity and specificity of US in the diagnosis of hepatic steatosis were, respectively, 64.9% (95% CI: 54.9-74.3) and 90.9% (95% CI: 57.1-99.5). The positive predictive value and negative predictive value were, respectively, 98.4% (95% CI: 90.2-99.9) and 23.3% (95% CI: 12.3-39.0). The presence of steatosis onUS was associated to advanced grades of steatosis on histology (P = 0.016). CONCLUSION: Preoperative abdominal US in our series has not shown to be an accurate method for the diagnosis of hepatic steatosis in severe obese patients. 3Biodegradable stent for the treatment of a colonic stricture in Crohn's disease显示文摘Biodegradable polydioxanone stents were developed for the treatment of refractory benign esophageal strictures but have been suggested as a new therapeutic option for intestinal strictures.The primary advantage of biodegradable stents over self-expandable metallic stents is that removal is not required.There are,however,few data available on their use in the small or large bowel.We herein describe the case of a 33-yearold patient with long-standing Crohn's disease(CD) who developed a fibrotic stricture of the sigmoid too long to be amenable to balloon dilation.The use of a biodegradable polydioxanone stent was chosen to avoid surgery.Combined endoscopic and fluoroscopic placement of the stent was technically simple,safe and clinically successful,and no recurrence of obstructive symptoms occurred during a 16-mo follow-up.Further studies are needed to evaluate the long-term efficacy and safety of biodegradable stents in the treatment of intestinal strictures,particularly in the context of CD.Catarina Rodrigues Ana Oliveira Liliana Santos Eduardo Pires Joo Deus 2013World Journal of Gastrointestinal Endoscopy2013,5,5:9
4Impaired T-cell differentiation in diabetic foot ulceration显示文摘Foot ulceration is one of the most debilitating complications associated with diabetes,but its cause remains poorly understood.Several studies have been undertaken to understand healing kinetics or find possible therapies to enhance healing.However,few studies have been directed at understanding the immunological alterations that could influence wound healing in diabetes.In this study,we analysed the T-cell receptor(TCR)repertoire diversity in TCR-αβ^(+)T cells.We also analysed the distribution and phenotype of T cells obtained from the peripheral blood of healthy controls and diabetic individuals with or without foot ulcers.Our results showed that diabetic individuals,especially those with foot ulcers,have a significantly lower naive T-cell number and a poorer TCR-Vβrepertoire diversity.We also showed that the reduced TCR-Vβrepertoire diversity in diabetic individuals was mainly owing to the accumulation of effector T cells,the major source of tumour necrosis factor-αproduction,which was even more pronounced in patients with acute foot ulceration.Moreover,the expression of several inflammatory chemokine receptors was significantly reduced in diabetic patients.In conclusion,effector T-cell accumulation and TCR repertoire diversity reduction appear to precede the development of foot ulcers.This finding may open new immunological therapeutic possibilities and provide a new prognostic tool in diabetic wound care.João Moura João Rodrigues Marta Gonçalves Cláudia Amaral Margarida Lima Eugénia Carvalho 2017Cellular & Molecular Immunology2017,14,9:6

6Metal alginates for polyphenol delivery systems: Studies on crosslinking ions and easy-to-use patches for release of protective flavonoids in skin显示文摘Incorporation of bioactive natural compounds like polyphenols is an attractive approach for enhanced functionalities of biomaterials.In particular flavonoids have important pharmacological activities,and controlled release systems may be instrumental to realize the full potential of these phytochemicals.Alginate presents interesting attributes for dermal and other biomaterial applications,and studies were carried here to support the development of polyphenol-loaded alginate systems.Studies of capillary viscosity indicated that ionic medium is an effective strategy to modulate the polyelectrolyte effect and viscosity properties of alginates.On gelation,considerable differences were observed between alginate gels produced with Ca^2+,Ba^2+,Cu^2+,Fe^2+,Fe^3+and Zn^2+as crosslinkers,especially concerning shrinkage and morphological regularity.Stability assays with different polyphenols in the presence of alginate-gelling cations pointed to the choice of calcium,barium and zinc as safer crosslinkers.Alginate-based films loaded with epicatechin were prepared and the kinetics of release of the flavonoid investigated.The results with calcium,barium and zinc alginate matrices indicated that the release dynamics is dependent on film thicknesses,but also on the crosslinking metal used.On these grounds,an alginate-based system of convenient use was devised,so that flavonoids can be easily loaded at simple point-of-care conditions before dermal application.This epicatechin-loaded patch was tested on an ex-vivo skin model and demonstrated capacity to deliver therapeutically relevant concentrations on skin surface.Moreover,the flavonoid released was not modified and retained full antioxidant bioactivity.The alginate-based system proposed offers a multifunctional approach for flavonoid controllable delivery and protection of skin injured or under risk.João Silva Pavlo Vanat Dorinda Marques-da-Silva Joaquim Rui Rodrigues Ricardo Lagoa 2020Bioactive Materials2020,5,3:2

8Challenges in the diagnosis of intestinal neuronal dysplasia type B:A look beyond the number of ganglion cells显示文摘Intestinal neuronal dysplasia type B(IND-B)is a controversial condition among gastrointestinal neuromuscular disorders.Constipation is its most common clinical manifestation in patients.Despite intense scientific research,there are still knowledge gaps regarding the diagnostic criteria for IND-B in the histopathological analysis of rectal biopsies.The guidelines published in the past three decades have directed diagnostic criteria for quantifying the number of ganglion cells in the nervous plexus of the enteric nervous system.However,it is very complex to distinguish numerically what is pathological from what is normal,mainly because of the difficulty in determining a reliable control group composed of healthy children without intestinal symptoms.Thus,a series of immunohistochemical markers have been proposed to assist in the histopathological analysis of the enteric nervous system.Several of these markers facilitate the identification of other structures of the enteric nervous system,in addition to ganglion cells.These structures may be related to the etiopathogenesis of IND-B and represent new possibilities for the histopathological diagnosis of this disease,providing a view beyond the number of ganglion cells.This review critically discusses the aspects related to the disease definitions and diagnostic criteria of this organic cause of constipation.Simone Antunes Terra Anderson Cesar Gonçalves Pedro Luiz Toledo de Arruda Lourenção Maria Aparecida Marchesan Rodrigues 2021World Journal of Gastroenterology2021,27,44:2
9NT157 has antineoplastic effects and inhibits IRS1/2 and STAT3/5 in JAK2^(V617F)-positive myeloproliferative neoplasm cells显示文摘Recent data indicate that IGF1R/IRS signaling is a potential therapeutic target in BCR-ABL1-negative myeloproliferative neoplasms(MPN);in this pathway,IRS2 is involved in the malignant transformation induced by JAK2^(V617F),and upregulation of IGF1R signaling induces the MPN phenotype.NT157,a synthetic compound designed as an IGF1R-IRS1/2 inhibitor,has been shown to induce antineoplastic effects in solid tumors.Herein,we aimed to characterize the molecular and cellular effects of NT157 in JAK2^(V617F)positive MPN cell lines(HEL and SET2)and primary patient hematopoietic cells.In JAK2^(V617F)cell lines,NT157 decreased cell viability,clonogenicity,and cell proliferation,resulting in increases in apoptosis and cell cycle arrest in the G2/M phase(p<0.05).NT157 treatment inhibited IRS1/2,JAK2/STAT,and NFκB signaling,and it activated the AP-1 complex,downregulated four oncogenes(CCND1,MYB,WT1,and NFKB1),and upregulated three apoptotic-related genes(CDKN1A,FOS,and JUN)(p<0.05).NT157 induced genotoxic stress in a JAK2/STAT-independent manner.NT157 inhibited erythropoietin-independent colony formation in cells from polycythemia vera patients(p<0.05).These findings further elucidate the mechanism of NT157 action in a MPN context and suggest that targeting IRS1/2 proteins may represent a promising therapeutic strategy for MPN.Bruna Alves Fenerich Jaqueline Cristina Fernandes Ana Paula Nunes Rodrigues Alves Juan Luiz Coelho-Silva Renata Scopim-Ribeiro Priscila Santos Scheucher Christopher A.Eide Cristina E.Tognon Brian J.Druker Eduardo Magalhães Rego João Agostinho Machado-Neto Fabiola Traina 2020Signal Transduction and Targeted Therapy2020,5,1:2
