1肿瘤部位或决定结肠癌DNA错配修复功能缺陷的预后影响显示文摘目前对于接受氟尿嘧啶、亚叶酸钙及奥沙利铂(FOLFOX)辅助化疗方案治疗的结肠癌患者,DNA错配修复功能缺陷(dMMR)与预后间的联系尚不明了。美国梅奥诊所的Frank A.Sinicrope博士等人进行了一项临床试验,研究人员在2,580份肿瘤患者的蛋白芯片中,检测到314份(12%)肿瘤患者的蛋白芯片存在 dMMR,其中分别有49.3%及10.6%存在BRAFV600E或KRAS突变。尽管从整体而言,MMR状态并非预后预测因素,但研究同时发现,MMR与原发肿瘤部位及淋巴结分期( N1 v N2)间存在显著的相互作用。对突变情况及协变量校正后,研究人员发现,与准确进行MMR邻近肿瘤相对的dMMR可获得良好的DFS结局,但远端肿瘤的dMMR与良好的DFS结局无关。 N2肿瘤患者不能通过dMMR取得生存获益。BRAFV600 E突变或KRAS突变均为较差DFS的独立相关因素。根据肿瘤部位相互作用观察到的MMR得到了III期结肠癌患者独立队列的验证。Sinicrope FA Mahoney MR Smyrk TC 2013中华结直肠疾病电子杂志2013,2,4:27
