| 1 | Dysfunctional stem and progenitor cells impair fracture healing with age显示文摘Successful fracture healing requires the simultaneous regeneration of both the bone and vasculature;mesenchymal stem cells (MSCs) are directed to replace the bone tissue, while endothelial progenitor cells (EPCs) form the new vasculature that supplies blood to the fracture site. In the elderly, the healing process is slowed, partly due to decreased regenerative function of these stem and progenitor cells. MSCs from older individuals are impaired with regard to cell number, proliferative capacity, ability to migrate, and osteochondrogenic differentiation potential. The proliferation, migration and function of EPCs are also compromised with advanced age. Although the reasons for cellular dysfunction with age are complex and multidimensional, reduced expression of growth factors, accumulation of oxidative damage from reactive oxygen species, and altered signaling of the Sirtuin-1 pathway are contributing factors to aging at the cellular level of both MSCs and EPCs. Because of these geriatric-specific issues, effective treatment for fracture repair may require new therapeutic techniques to restore cellular function. Some suggested directions for potential treatments include cellular therapies, pharmacological agents, treatments targeting age-related molecular mechanisms, and physical therapeutics. | 2 | Dietary approaches following endoscopic retrograde cholangiopancreatography: A survey of selected endoscopists显示文摘AIM: To describe the dietary recommendations of experienced endoscopists for patients who have undergone endoscopic retrograde cholangiopancreatography (ERCP) and the factors that influence these recommendations. METHODS: Selected U.S. endoscopists with ERCP experience were surveyed by e-mail. A questionnaire with three hypothetical ERCP cases of patients at low, medium and high risk for development of post-ERCP pancreatitis (PEP) was shown. For each scenario, respondents were asked to recommend a post-procedure diet and time to first oral intake. Respondents were also asked about the effect of various clinical factors on their recommendations, including risk of PEP.RESULTS: 97/187 selected ASGE members (51.9%) responded. When risk of PEP was either low, medium or high, 53%, 88% and 96% recommended a diet of clear liquids/NPO respectively, and 2%, 5% and 18% recommended delaying first oral intake until the following day. About 88% of respondents gave the same type of diet to patients at high as those with moderate-risk of PEP (P = 0.04). However, 37% and 43% of respondents gave different types of diet to patients at low vs moderate-risk and low-risk vs high-risk of PEP respectively (P < 0.001). No statistically significant associations were found regarding the effect of other clinical factors or respondent demographics.CONCLUSION: Most experienced endoscopists limit diet to NPO/clear liquids after ERCP for patients at high or moderate risk of post-ERCP pancreatitis. | 3 | Hypertonicity:Clinical entities,manifestations and treatment显示文摘Hypertonicity causes severe clinical manifestations and is associated with mortality and severe short-term and longterm neurological sequelae. The main clinical syndromes of hypertonicity are hypernatremia and hyperglycemia.Hypernatremia results from relative excess of body sodium over body water. Loss of water in excess of intake,gain of sodium salts in excess of losses or a combination of the two are the main mechanisms of hypernatremia.Hypernatremia can be hypervolemic,euvolemic or hypovolemic. The management of hypernatremia addresses both a quantitative replacement of water and,if present,sodium deficit,and correction of the underlying pathophysiologic process that led to hypernatremia.Hypertonicity in hyperglycemia has two components,solute gain secondary to glucose accumulation in the extracellular compartment and water loss through hyperglycemic osmotic diuresis in excess of the losses of sodium and potassium. Differentiating between these two components of hypertonicity has major therapeutic implications because the first component will be reversed simply by normalization of serum glucose concentration while the second component will require hypotonic fluid replacement. | 4 | Stability of fruit quality traits in diverse watermelon cultivars tested in multiple environments显示文摘Lycopene is a naturally occurring red carotenoid compound that is found in watermelon.Lycopene has antioxidant properties.Lycopene content,sugar content and hollowheart resistance are subject to significant genotype×environment interaction(G×E),which makes breeding for these fruit quality traits difficult.The objectives of this study were to(i)evaluate the influence of years and locations on lycopene content,sugar content and hollowheart resistance for a set of watermelon genotypes,and(ii)identify genotypes with high stability for lycopene,sugar,and hollowheart resistance.A diverse set of 40 genotypes was tested over 3 years and 8 locations across the southern United States in replicated,multi-harvest trials.Lycopene was tested in a subset of 10 genotypes.Data were analyzed using univariate and multivariate stability statistics(BLUP-GGE biplot)using SASGxE and RGxE programs.There were strong effects of environment as well as G×E interaction on watermelon quality traits.On the basis of stability measures,genotypes were classified as stable or unstable for each quality trait.'Crimson Sweet'is an inbred line with high quality trait performance as well as trait stability.'Stone Mountain','Tom Watson','Crimson Sweet'and'Minilee'were among the best genotypes for lycopene content,sugar content and hollowheart resistance.We developed a stability chart based on marketable yield and average ranking generated from different stability measures for yield attributes and quality traits.The chart will assist in choosing parents for improvement of watermelon cultivars.See http://cuke.hort.ncsu.edu/cucurbit/wmelon/wmelonmain.html. | Mahendra Dia Todd C Wehner Penelope Perkins-Veazie Richard Hassell Daniel S Price George E Boyhan Stephen M Olson Stephen R King Angela R Davis Gregory E Tolla Jerome Bernier Benito Juarez | 2016 | Horticulture Research2016,3,1: | 2 |
