| 9 | High Risk Factors of Pancreatic Carcinoma显示文摘Over the past decades, cancer has become one of the toughest challenges for health professionals. The epidemiologists are increasingly directing their research efforts on various malignant tumor worldwide. Of note, incidence of cancers is on the rise more quickly in developed countries. Indeed, great endeavors have to be made in the control of the life-threatening disease. As we know it, pancreatic cancer(PC) is a malignant disease with the worst prognosis. While little is known about the etiology of the PC and measures to prevent the condition, so far, a number of risk factors have been identified. Genetic factors, pre-malignant lesions, predisposing diseases and exogenous factors have been found to be linked to PC. Genetic susceptibility was observed in 10% of PC cases, including inherited PC syndromes and familial PC. However, in the remaining 90%, their PC might be caused by genetic factors in combination with environmental factors. Nonetheless, the exact mechanism of the two kinds of factors, endogenous and exogenous, working together to cause PC remains poorly understood. The fact that most pancreatic neoplasms are diagnosed at an incurable stage of the disease highlights the need to identify risk factors and to understand their contribution to carcinogenesis. | 10 | Predictors of Clostridium difficile infection severity in patients hospitalised in medical intensive care显示文摘AIM:To describe and analyse factors associated with Clostridium difficile infection(CDI)severity in hospitalised medical intensive care unit patients.METHODS:We performed a retrospective cohort study of 40 patients with CDI in a medical intensive care unit(MICU)at a French university hospital.We include patients hospitalised between January 1,2007and December 31,2011.Data on demographics characteristics,past medical history,CDI description was collected.Exposure to risk factors associated with CDI within 8 wk before CDI was recorded,including previous hospitalisation,nursing home residency,antibiotics,antisecretory drugs,and surgical procedures.RESULTS:All included cases had their first episode of CDI.The mean incidence rate was 12.94 cases/1000admitted patients,and 14.93,8.52,13.24,19.70,and8.31 respectively per 1000 admitted patients annually from 2007 to 2011.Median age was 62.9[interquartile range(IQR)55.4-72.40]years,and 13(32.5%)were women.Median length of MICU stay was 14.0d(IQR 5.0-22.8).In addition to diarrhoea,the clinical symptoms of CDI were fever(>38℃)in 23 patients,abdominal pain in 15 patients,and ileus in 1 patient.The duration of diarrhoea was 13.0(8.0-19.5)d.In addition to diarrhoea,the clinical symptoms of CDI were fever(>38℃)in 23 patients,abdominal pain in 15 patients,and ileus in 1 patient.Prior to CDI,38patients(95.0%)were exposed to antibiotics,and 12(30%)received at least 4 antibiotics.Fluoroquinolones,3rdgeneration cephalosporins,coamoxiclav and tazocillin were prescribed most frequently(65%,55%,40%and 37.5%,respectively).The majority of cases were hospital-acquired(n=36,90%),with 5 cases(13.9%)being MICU-acquired.Fifteen patients had severe CDI.The crude mortality rate within 30 d after diagnosis was 40%(n=16),with 9 deaths(9 over 16;56.3%)related to CDI.Of our 40 patients,15(37.5%)had severe CDI.Multivariate logistic regression showed that male gender[odds ratio(OR):8.45;95%CI:1.06-67.16,P=0.044],rising serum C-reactive protein levels(OR=1.11;95%CI:1.02-1.21,P=0.021),and previous exposure to fluoroquinolones(OR=9.29;95%CI:1.16-74.284,P=0.036)were independently associated with severe CDI.CONCLUSION:We report predictors of severe CDI not dependent on time of assessment.Such factors could help in the development of a quantitative score in ICU’s patients. | Nagham Khanafer Abdoulaye Touré Cécile Chambrier Martin Cour Marie-Elisabeth Reverdy Laurent Argaud Philippe Vanhems | 2013 | World Journal of Gastroenterology2013,19,44: | 2 |

| 12 | Effect of Graphene Nanoribbons (TexasPEG) on locomotor function recovery in a rat model of lumbar spinal cord transection显示文摘A sharply transected spinal cord has been shown to be fused under the accelerating influence of membrane fusogens such as polyethylene glycol(PEG)(GEMINI protocol). Previous work provided evidence that this is in fact possible. Other fusogens might improve current results. In this study, we aimed to assess the effects of PEGylated graphene nanoribons(PEG-GNR, and called 'Texas PEG' when prepared as 1 wt% dispersion in PEG600) versus placebo(saline) on locomotor function recovery and cellular level in a rat model of spinal cord transection at lumbar segment 1(L_1) level. In vivo and in vitro experiments(n = 10 per experiment) were designed. In the in vivo experiment, all rats were submitted to full spinal cord transection at L_1 level. Five weeks later, behavioral assessment was performed using the Basso Beattie Bresnahan(BBB) locomotor rating scale. Immunohistochemical staining with neuron marker neurofilament 200(NF200) antibody and astrocytic scar marker glial fibrillary acidic protein(GFAP) was also performed in the injured spinal cord. In the in vitro experiment, the effects of Texas PEG application for 72 hours on the neurite outgrowth of SH-SY5 Y cells were observed under the inverted microscope. Results of both in vivo and in vitro experiments suggest that Texas PEG reduces the formation of glial scars, promotes the regeneration of neurites, and thereby contributes to the recovery of locomotor function of a rat model of spinal cord transfection. | C-Yoon Kim William K. | 13 | Prevalence of and risk factors for malaria,filariasis,and intestinal parasites as single infections or co-infections in different settlements of Gabon,Central Africa显示文摘Background:Malaria,filariasis,and intestinal parasitic infections(IPIs)are common and frequently overlap in developing countries.The prevalence and predictors of these infections were investigated in three different settlements(rural,semi-urban,and urban)of Gabon.Methods:During cross-sectional surveys performed from September 2013 to June 2014,451 individuals were interviewed.In addition,blood and stool samples were analysed for the presence of Plasmodium,filarial roundworm,intestinal protozoan,and helminth infections.Results:Intestinal parasitic infections(61.1%),including intestinal protozoa(56.7%)and soil-transmitted helminths(STHs)(22.2%),predominated,whereas Plasmodium falciparum(18.8%),Loa loa(4.7%),and Mansonella perstans(1.1%)were less prevalent.Filariasis and STHs were mainly found in rural settlements,whereas a higher plasmodial infection prevalence rate was observed in the periurban area.The most common IPI was blastocystosis(48.6%),followed by ascaridiasis(13.7%),trichuriasis(11.8%),amoebiasis(9.3%),giardiasis(4.8%),and strongyloidiasis(3.7%).Hookworm was detected in one adult from rural Dienga.Adults had a higher prevalence of Blastocystis hominis and STHs,whereas Giardia duodenalis was more frequently observed among children aged below 5 years(P<0.01).The polyparasitism rate was 41.5%,with 7.0%Plasmodium-IPIs and 1.8%Plasmodium-STH co-infections.The multivariate analysis showed that living in a suburban area,belonging to the age group of 5-15 years,having none or a secondary education,or having an open body water close to home were significant risk factors for malaria(P≤0.01).For STH infections,identified risk factors were drinking untreated water and living in a rural area(P≤0.04).No significant predictors were identified for IPIs and malaria-IPI co-infection.Conclusions:This study reports a high prevalence of IPIs and intestinal protozoa,but a low rate of malaria-IPI co-infections in the study sites.Improvements in the living conditions of the population such as adequate water supply and proper health education and sanitation should be integrated into control strategies for malaria,STHs,and IPIs. | Noe Patrick M’bondoukwe Eric Kendjo Denise Patricia Mawili-Mboumba Jeanne Vanessa Koumba Lengongo Christelle Offouga Mbouoronde Dieudonne Nkoghe Fousseyni Toure Marielle Karine Bouyou-AkotetAuthor details | 2018 | Infectious Diseases of Poverty2018,7,1: | 2 |
| 14 | Interactions between Soil Characteristics, Environmental Factors, and Plant Species Abundance: A Case Study in the Karst Mountains of Longhushan Nature Reserve, Southwest China显示文摘The ability to manage and restore plant communities in the face of human-induced landscape change may rely on our ability to predict how species respond to environmental variables.Understanding this response requires examining factors or their interactions that have influence on plant and resource availability.Our objective was to analyze the relationships between changes in plant abundance and the interaction among environmental habitat factors including soil, geological(rock type), and other environmental variables in the Longhushan karst mountains ecosystem.Species density and dominance were examined using ANOVA, ANCOVA,and Generalized Linear Models to establish the single or combined effects of these groups of factors.The results showed that trends in abundance were mainly affected by rock type(related to the percentage content of dolomite and calcite), soil characteristics in association with topography.Both plant indices were higher in dolomite dominated areas and varied positively with moisture, and elevation, but negatively with organic matter, while density also increased with slope degree.The results demonstrate that significant variations in species abundance was produced with the combination of variables from soil, geological, andenvironmental factors, suggesting their interaction influence on plants.We postulate that spatial variations in plant abundance in karst ecosystem depends on the carbonate rock type in addition to water and nutrient availability which are mainly controlled by topography and other factors such as soil texture and temperature.The study suggests that in karst areas carbonate rock type, in addition to local environmental variables, should be taken into account when analyzing the factors that have impact on plant communities. | Dado TOURE GE ji-wen ZHOU Jian-wei | 2015 | Journal of Mountain Science2015,12,4: | 2 |
