1Validation of the moderate severity category of acute pancreatitis defined by determinant-based classification显示文摘BACKGROUND: Recent international multidisciplinary consultation proposed the use of local(sterile or infected pancreatic necrosis) and/or systemic determinants(organ failure) in the stratification of acute pancreatitis. The present study was to validate the moderate severity category by international multidisciplinary consultation definitions.METHODS: Ninety-two consecutive patients with severe acute pancreatitis(according to the 1992 Atlanta classification) were classified into(i) moderate acute pancreatitis group with the presence of sterile(peri-) pancreatic necrosis and/or transient organ failure; and(ii) severe/critical acute pancreatitis group with the presence of sterile or infected pancreatic necrosis and/or persistent organ failure. Demographic and clinical outcomes were compared between the two groups.RESULTS: Compared with the severe/critical group(n=59), the moderate group(n=33) had lower clinical and computerized tomographic scores(both P<0.05). They also had a lower incidence of pancreatic necrosis(45.5% vs 71.2%, P=0.015),infection(9.1% vs 37.3%, P=0.004), ICU admission(0% vs27.1%, P=0.001), and shorter hospital stay(15±5 vs 27±12days; P<0.001). A subgroup analysis showed that the moderate group also had significantly lower ICU admission rates, shorter hospital stay and lower rate of infection compared with the severe group(n=51). 4Biological conduit small gap sleeve bridging method for peripheral nerve injury: regeneration law of nerve fibers in the conduit显示文摘The clinical effects of 2-mm small gap sleeve bridging of the biological conduit to repair peripheral nerve injury are better than in the traditional epineurium suture, so it is possible to replace the epineurium suture in the treatment of peripheral nerve injury. This study sought to identify the regeneration law of nerve fibers in the biological conduit. A nerve regeneration chamber was constructed in models of sciatic nerve injury using 2-mm small gap sleeve bridging of a biodegradable biological conduit. The results showed that the biological conduit had good histocompatibility. Tissue and cell apoptosis in the conduit apparently lessened, and regenerating nerve fibers were common. The degeneration regeneration law of Schwann cells and axons in the conduit was quite different from that in traditional epineurium suture. During the prime period for nerve fiber regeneration(2–8 weeks), the number of Schwann cells and nerve fibers was higher in both proximal and distal ends, and the effects of the small gap sleeve bridging method were better than those of the traditional epineurium suture. 5Prediction of the severity of acute pancreatitis on admission by urinary trypsinogen activation peptide: A meta-analysis显示文摘AIM: To undertake a meta-analysis on the value of urinary trypsinogen activation peptide (uTAP) in predicting severity of acute pancreatitis on admission. METHODS: Major databases including Medline, Embase, Science Citation Index Expanded and the Cochrane Central Register of Controlled Trials in the Cochrane Library were searched to identify all relevant studies from January 1990 to January 2013. Pooled sensitivity, specificity and the diagnostic odds ratios (DORs) with 95%CI were calculated for each study and were compared to other systems/biomarkers if mentioned within the same study. Summary receiver-operating curves were conducted and the area under the curve (AUC) was evaluated. RESULTS: In total, six studies of uTAP with a cut-off value of 35 nmol/L were included in this meta-analysis. Overall, the pooled sensitivity and specificity of uTAP for predicting severity of acute pancreatitis, at time of admission, was 71% and 75%, respectively (AUC = 0.83, DOR = 8.67, 95%CI: 3.70-20.33). When uTAP was compared with plasma C-reactive protein, the pooled sensitivity, specificity, AUC and DOR were 0.64 vs 0.67, 0.77 vs 0.75, 0.82 vs 0.79 and 6.27 vs 6.32, respectively. Similarly, the pooled sensitivity, specificity, AUC and DOR of uTAPvs Acute Physiology and Chronic Health Evaluation Ⅱ within the first 48 h of admission were found to be 0.64 vs 0.69, 0.77 vs 0.61, 0.82 vs 0.73 and 6.27vs 4.61, respectively. 6Biodegradable chitin conduit tubulation combined with bone marrow mesenchymal stem cell transplantation for treatment of spinal cord injury by reducing glial scar and cavity formation显示文摘We examined the restorative effect of modified biodegradable chitin conduits in combination with bone marrow mesenchymal stem cell transplantation after right spinal cord hemisection injury. Immunohistochemical staining revealed that biological conduit sleeve bridging reduced glial scar formation and spinal muscular atrophy after spinal cord hemisection. Bone marrow mesenchymal stem cells survived and proliferated after transplantation in vivo, and differentiated into cells double-positive for S100(Schwann cell marker) and glial fibrillary acidic protein(glial cell marker) at 8 weeks. Retrograde tracing showed that more nerve fibers had grown through the injured spinal cord at 14 weeks after combination therapy than either treatment alone. Our findings indicate that a biological conduit combined with bone marrow mesenchymal stem cell transplantation effectively prevented scar formation and provided a favorable local microenvironment for the proliferation, migration and differentiation of bone marrow mesenchymal stem cells in the spinal cord, thus promoting restoration following spinal cord hemisection injury.Feng Xue Er-jun Wu Pei-xun Zhang Li-ya A Yu-hui Kou Xiao-feng Yin Na Han 2015Neural Regeneration Research2015,10,1:7
7Age and menopausal status are important factors influencing the serum human epididymis secretory protein 4 level:a prospective cross-sectional study in healthy Chinese people显示文摘Background::Human epididymis secretory protein 4(HE4)is a new ovarian cancer biomarker.The factors influencing HE4 levels are not clear,and the reference data in China are limited.Here,we aim to evaluate the effects of menopause and age on HE4 levels and to provide a possible reference value for HE4 in healthy Chinese people.Methods::A total of 2493 healthy females aged 40 years or older were recruited from March 2013 to March 2017 with the cooperation of four medical institutions across Beijing,China.The serum levels of HE4 and cancer antigen 125(CA125)were measured by enzyme-linked immunosorbent assay.The Wilcoxon rank-sum test of variance and a stratified analysis were used to analyze the relationships among age,menopausal status,and levels of HE4 or CA125.Confidence intervals(5%-95%)were determined for reference ranges in different populations.Results::There was a statistically significant difference in median HE4 levels between the post-menopausal(n=2168)and premenopausal groups(n=325)(36.46 vs.24.04 pmol/L,Z=-14.41,P<0.001).HE4 increased significantly with age in the post-menopausal groups(H=408.18,P<0.001)but not in the pre-menopausal subjects(Z=-0.43,P=0.67).The upper 95th percentile of HE4 levels were 44.63 pmol/L for pre-menopausal women,78.17 pmol/L for post-menopausal women,and 73.3 pmol/L for all women.In the post-menopausal population,the HE4 reference ranges were 13.15 to 47.31,14.31 to 58.04,17.06 to 73.51,24.50 to 115.25,and 35.71 to 212.37 pmol/L for different age groups from forty divided by decade.The CA125 level was affected mainly by menopausal status and not age.Conclusions::Menopausal status and age were both important factors influencing the level of HE4,and age affected HE4 levels mainly in post-menopausal women.The HE4 level was higher in the post-menopausal population than in the pre-menopausal population and increased with age.Hong-Yan Cheng Lin Zeng Xue Ye Rui-Qiong M a Zhi-Jian Tang Hong-Ling Chu Yi-M ing Zhao Li-Rong Zhu Yu-Nong Gao Xiao-Hong Chang Heng Cui 2020Chinese Medical Journal2020,,11:6
8Posterior gut development in Drosophila:a model system for identifying genes controlling epithelial morphogenesis显示文摘The posterior gut of the Drosophila embryo, consisting of hindgut and Malpighian tubules, provides a simple,well-defined system where it is possible to use a genetic approach to define components essential for epithelial morphogenesis.We review here the advantages of Drosophila as a model genetic organism, the morphogenesis of the epithelial structures of the posterior gut, and what is known about the genetic requirements to form these structures.In overview, primordia are patterned by expression of hierarchies of transcription factors; this leads to localized expression of cell signaling molecules, and finally, to the least understood step: modulation of cell adhesion and cell shape. 9Experimental porcine model of complex fistula-in-ano显示文摘AIM To establish and evaluate an experimental porcine model of fistula-in-ano.METHODS Twelve healthy pigs were randomly divided into two groups. Under general anesthesia, the experimental group underwent rubber band ligation surgery, and the control group underwent an artificial damage technique. Clinical magnetic resonance imaging(MRI) and histopathological evaluation were performed on the 38 th d and 48 th d after surgery in both groups, respectively. RESULTS There were no significant differences between the experimental group and the control group in general characteristics such as body weight, gender, and the number of fistula(P > 0.05). In the experimental group, 15 fistulas were confirmed clinically, 13 complex fistulas were confirmed by MRI, and 11 complex fistulas were confirmed by histopathology. The success rate in the porcine complex fistula model establishment was 83.33%. Among the 18 fistulas in the control group, 5 fistulas were confirmed clinically, 4 complex fistulas were confirmed by MRI, and 3 fistulas were confirmed by histopathology. The success rate in the porcine fistula model establishment was 27.78%. Thus, the success rate of the rubber band ligation group was significantly higher than the control group(P < 0.05). CONCLUSION Rubber band ligation is a stable and reliable method to establish complex fistula-in-ano models. 10Urinary trypsinogen-2 for diagnosing acute pancreatitis:a meta-analysis显示文摘BACKGROUND: Currently, serum amylase and lipase are the most popular laboratory markers for early diagnosis of acute pancreatitis with reasonable sensitivity and specificity. Urinary trypsinogen-2 (UT-2) has been increasingly used but its clinical value for the diagnosis of acute pancreatitis and post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis has not yet been systematically assessed. DATA SOURCES: A comprehensive search was carried out using PubMed (MEDLINE), Embase, and Web of Science for clinical trials, which studied the usefulness of UT-2 as a diagnostic marker for acute pancreatitis. Sensitivity, specificity and the diagnostic odds ratios (DORs) with 95% confidence interval (CI) were calculated for each study and were compared with serum amylase and lipase. Summary receiver-operating curves were conducted and the area under the curve (AUC) was evaluated. RESULTS: A total of 18 studies were included. The pooled sensitivity and specificity of UT-2 for the diagnosis of acute pancreatitis were 80% and 92%, respectively (AUC=0.96, DOR=65.63, 95% CI: 31.65-139.09). The diagnostic value of UT-2 was comparable to serum amylase but was weaker than serum lipase. The pooled sensitivity and specificity for the diagnosis of post-ERCP pancreatitis were 86% and 94%, respectively (AUC=0.92, DOR=77.68, 95% CI: 24.99-241.48).CONCLUSIONS: UT-2 as a rapid test could be potentially used for the diagnosis of post-ERCP pancreatitis and to an extent, acute pancreatitis. 12Shoot and root traits in drought tolerant maize(Zea mays L.) hybrids显示文摘This study aimed to investigate the differences in shoot and root traits, and water use and water use efficiency(WUE) in drought tolerant(DT) maize(Zea mays L.) hybrids under full and deficit irrigated conditions. A two-year greenhouse study was conducted with four hybrids(one conventional hybrid, 33D53AM, two commercial DT hybrids, P1151 AM, N75H, and an experimental hybrid, Exp HB) grown under two water regimes(I_(100) and I_(50), referring to 100 and 50% of evapotranspiration requirements). Under water stress, the hybids P1151 AM, N75, and Exp HB showed more drought tolerance and had either greater shoot dry weight or less dry weight reduction than the conventional hybrid(33 D53 AM). However, these three hybrids responded to water stress using different mechanisms. Compared with the conventional hybrid, the two commercial DT hybrids(P1151 AM and N75H) had a smaller leaf area, shoot dry weight, and root system per plant. As a result, these hybrids used less water but had a higher WUE compared with the conventional hybrid. In contrast, the experimental hybrid(ExpHB) produced more shoot biomass by silking stage at both irrigation levels than all other hybrids, but it had relatively lower WUE. The hybrids demonstrated different drought response mechanisms that may require different irrigation management strategies.
