| 1 | Intrahepatic biliary cystic neoplasms:Surgical results of 9 patients and literature review显示文摘AIM: To investigate the eligible management of the cystic neplasms of the liver. METHODS: The charts of 9 patients who underwent surgery for intrahepatic biliary cystic liver neoplasms between 2003 and 2008 were reviewed retrospectively. Informed consent was obtained from the patients and approval was obtained from the designated review board of the institution. RESULTS: All patients were female with a median (range) age of 49 (27-60 years). The most frequent symptom was abdominal pain in 6 of the patients. Four patients had undergone previous laparotomy (with other diagnoses) which resulted in incomplete surgery or recurrences. Liver resection (n = 6) or enucleation (n = 3) was performed. The final diagnosis was intrahepaticbiliary cystadenoma in 8 patients and cystadenocarcinoma in 1 patient. All symptoms resolved after surgery. There has been no recurrence during a median (range) 31 (7-72) mo of follow up. CONCLUSION: In spite of the improvement in imaging modalities and increasing recognition of biliary cystadenoma and cystadenocarcinoma, accurate preoperative diagnosis may be difficult. | 2 | Prognostic efficacy of inflammation-based markers in patients with curative colorectal cancer resection显示文摘AIM To evaluate the prognostic significance of neutrophilto-lymphocyte ratio(NLR), platelet-to-lymphocyte ratio(PLR), lymphocyte-to-monocyte ratio(LMR), and prognostic nutritional index(PNI) and other clinicopathological factors in patients undergoing curative resection of colon cancer.METHODS183 patients with histologically proven colorectal cancer who had undergone potentially curative resection between 2010 and 2016 at Ankara Numune Training and Research Hospital were retrospectively analyzed and clinicopathological characteristics included age, sex, tumor type, grade, size and localization, the number of metastatic and total number of lymph nodes removed, vascular and perineural invasion of the tumor, TNM stages, tumor marker levels(CEA, CA19-9, AFP, CA-125, CA15-3), complete blood counts, albumin levels, overall survival(months), NLR, PLR, LMR and PNI ratios were retrospectively reviewed and analyzed from the electronic database. The primary outcome measure was overall survival.RESULTS Regarding overall survival, on univariate analysis the following variables were significantly associated with poor outcome following resection: T-stage(P = 0.037), lymph node invasion(P = 0.037), cancer stage(P = 0.034), CEA(P = 0.042), CA19-9(P = 0.004), and PNI(P = 0.001). To evaluate the independent prognostic value, multivariate Cox proportional hazard analysis to control for other prognostic factors was used. Using cancer-specific death as an end point for NLR, PLR, LMR, PNI and CA19-9 the optimal cut off values were calculated by ROC analysis. Regarding overall survival, on multivariate analysis high CA19-9(HR = 1.001, 95%CI: 1.00-1.002, P = 0.012) and low PNI(HR = 0.938, 95%CI: 0.891-0.987, P = 0.014) were the only variables independently associated with shortened overall survival. Patients with a PNI < 35 had a median OS of 52.25 mo. In contrast, patients with an PNI > 35 had a median OS of 66 mo. | 3 | Clinical and economic consequences of pancreatic fistula after elective pancreatic resection显示文摘BACKGROUND:Postoperative pancreatic fistula is the main cause of morbidity after pancreatic resection.This study aimed to quantify the clinical and economic consequences of pancreatic fistula in a medium-volume pancreatic surgery center.METHODS:Hospital records from patients who had undergone elective pancreatic resection in our department were identified.Pancreatic fistula was defined according to the International Study Group on Pancreatic Fistula(ISGPF).The consequences of pancreatic fistula were determined by treatment cost,hospital stay,and out-patient follow-up until the pancreatic fistula was completely healed.All costs of the treatment are calculated in Euros.The cost increase index was calculated for pancreatic fistula of grades A,B,and C as multiples of the total cost for the no fistula group.RESULTS:In 54 months,102 patients underwent elective pancreatic resections.Forty patients(39.2%) developed pancreatic fistula,and 54 patients(52.9%) had one or more complications.The median length of hospital stay for the no fistula,grades A,B,and C fistula groups was 12.5,14,20,and 59 days,respectively.The hospital stay of patients with fistula of grades B and C was significantly longer than that of patients with no fistula(P<0.001).The median total cost of the treatment was 4952,4679,8239,and 30 820 Euros in the no fistula,grades A,B,and C fistula groups,respectively.CONCLUSIONS:The grading recommended by the ISGPF is useful for comparing the clinical severity of fistula and for analyzing the clinical and economic consequences of pancreatic fistula.Pancreatic fistula prolongs the hospital stay and increases the cost of treatment in proportion to the severity of the fistula. | Filip Ceka Bohumil Jon Zdeněk ubrt Alexander Ferko | 2013 | Hepatobiliary & Pancreatic Diseases International2013,12,5: | 7 |
| 4 | Comparing the removal of polycyclic aromatic hydrocarbons in soil after different bioremediation approaches in relation to the extracellular enzyme activities显示文摘A 120-day experiment was conducted to compare the removal of polycyclic aromatic hydrocarbons(PAHs) from agricultural soil after natural attenuation(NA), phytoremediation(P), mycoremediation(M), and plant-assisted mycoremediation(PAM) approaches in relation to the extracellular enzyme activities in soil. The NA treatment removed the total soil PAH content negligibly. The P treatment using maize(Zea mays) enhanced only the removal of low and medium molecular PAHs. The Pleurotus ostreatus cultivated on 30–50 mm wood chip substrate used in M treatment was the most successful in the removal of majority PAHs. Therefore,significantly(p < 0.05) highest total PAH removal by 541.4 μg/kg dw(dry weight)(36%) from all tested M treatments was observed. When using the same fungal substrate together with maize in PAM treatment, the total PAH removal was not statistically different from the previous M treatment. However, the maize-assisted mycoremediation treatment significantly boosted fungal biomass, microbial and manganese peroxidase activity in soil which strongly correlated with the removal of total PAHs. The higher PAH removal in that PAM treatment could be reflected in the following post-harvest time. | 5 | Pancreatic Castleman disease treated with laparoscopic distal pancreatectomy显示文摘BACKGROUND: Castleman disease is an uncommon lympho-proliferative disorder most frequently occurring in the mediastinum. Abdominal forms are less frequent, with pancreatic localization of the disease in particular being extremely rare. Only seventeen cases have been described in the world literature. METHOD: This report describes an interesting and unusual case of pancreatic Castleman disease treated with laparoscopic resection. RESULTS: A 48-year-old woman presented with epigastric pain. CT scan showed a well-encapsulated mass on the ventral border of the pancreas. Endosonography with fine needle aspiration biopsy was performed. Biopsy showed lymphoid elements and structures of a normal lymph node. The patient was treated with laparoscopic distal pancreatectomy. The pancreas was transected with a Ligasure device and the pancreatic stump was secured with a manual suture. One year after surgery the patient was complaint-free and showed no signs of recurrence of the disease. CONCLUSIONS: Laparoscopic distal pancreatectomy is a feasible and safe method for the treatment of lesions in the body and tail of the pancreas. | 6 | Long-term survival of a patient after resection of a gastrointestinal stromal tumor arising from the pancreas显示文摘BACKGROUND:Gastrointestinal stromal tumors (GISTs) may arise in any part of the gastrointestinal tract;extragastrointestinal locations are extremely rare.Only a few cases of extragastrointestinal stromal tumor arising from the pancreas were reported.None of the reports described a long-term follow-up of the patients.METHOD:This report describes an interesting and unusual case of GIST arising from the pancreas.RESULTS:A 74-year-old female presented with a palpable abdominal mass.CT scan showed a large mass 11×8×4 cm originating from the tail of the pancreas.Percutaneous biopsy revealed a GIST predominantly with spindle cells,but some parts also contained epitheloid cells.The patient was treated by distal pancreatic resection with splenectomy.Immunohistochemistry of the tumor showed a staining pattern characteristic of GIST.The patient has achieved a long-term survival of five years and six months without any sign of recurrence of the disease.CONCLUSION:This is the first reported case of an extragastrointestinal stromal tumor arising from the pancreas treated surgically,with a long-term survival. | Filip eka Bohumil Jon Alexander Ferko Zdeněk ubrt Dimitar H Nikolov Věra Tyová | 2011 | Hepatobiliary & Pancreatic Diseases International2011,10,3: | 3 |
| 7 | Novel approaches in schizophrenia-from risk factors and hypotheses to novel drug targets显示文摘Schizophrenia is a severe psychiatric disorder characterized by emotional,behavioral and cognitive disturbances, and the treatment of schizophrenia is oftencomplicated by noncompliance and pharmacoresistance. The search for thepathophysiological mechanisms underlying schizophrenia has resulted in theproposal of several hypotheses to explain the impacts of environmental, genetic,neurodevelopmental, immune and inflammatory factors on disease onset andprogression. This review discusses the newest insights into the pathophysiologyof and risk factors for schizophrenia and notes novel approaches in antipsychotictreatment and potential diagnostic and theranostic biomarkers. The currenthypotheses focusing on neuromediators (dopamine, glutamate, and serotonin),neuroinflammation, the cannabinoid hypothesis, the gut-brain axis model, andoxidative stress are summarized. Key genetic features, including small nucleotidepolymorphisms, copy number variations, microdeletions, mutations andepigenetic changes, are highlighted. Current pharmacotherapy of schizophreniarelies mostly on dopaminergic and serotonergic antagonists/partial agonists, butnew findings in the pathophysiology of schizophrenia have allowed theexpansion of novel approaches in pharmacotherapy and the establishment ofmore reliable biomarkers. Substances with promising results in preclinical andclinical studies include lumateperone, pimavanserin, xanomeline, roluperidone,agonists of trace amine-associated receptor 1, inhibitors of glycine transporters,AMPA allosteric modulators, mGLUR2-3 agonists, D-amino acid oxidase inhibitorsand cannabidiol. | 9 | Intra-abdominal drainage following pancreatic resection:A systematic review显示文摘AIM: To study all the aspects of drain management in pancreatic surgery.METHODS: We conducted a systematic review according to the PRISMA guidelines. We searched the Cochrane Central Registry of Controlled Trials,EMBASE,Web of Science,and Pub Med(MEDLINE) for relevant articles on drain management in pancreatic surgery. The reference lists of relevant studies were screened to retrieve any further studies. We included all articles that reported clinical studies on human subjects with elective pancreatic resection and that compared various strategies of intra-abdominal drain management,such as drain vs no drain,selective drain use,early vs late drain extraction,and the use of different types of drains. RESULTS: A total of 19 studies concerned with drain management in pancreatic surgery involving 4194 patients were selected for this systematic review. We included studies analyzing the outcomes of pancreatic resection with and without intra-abdominal drains,studies comparing early vs late drain removal and studies analyzing different types of drains. The majority of the studies reporting equal or superior results for pancreatic resection without drains were retrospective and observational with significant selection bias. One recent randomized trial reported higher postoperative morbidity and mortality with routine omission of intraabdominal drains. With respect to the timing of drain removal,all of the included studies reported superior results with early drain removal. Regarding the varioustypes of drains,there is insufficient evidence to determine which type of drain is more suitable following pancreatic resection. CONCLUSION: The prophylactic use of drains remains controversial. When drains are used,early removal is recommended.
