| 1 | Near-infrared fluorescence sentinel lymph node detection in gastric cancer: A pilot study显示文摘AIM: To investigate feasibility and accuracy of nearinfrared fluorescence imaging using indocyanine green: nanocolloid for sentinel lymph node(SLN) detection in gastric cancer.METHODS: A prospective, single-institution, phaseI feasibility trial was conducted. Patients suffering from gastric cancer and planned for gastrectomy were included. During surgery, a subserosal injection of 1.6 m L ICG:Nanocoll was administered around the tumor. NIR fluorescence imaging of the abdominal cavity was performed using the Mini-FLARE? NIR fluorescence imaging system. Lymphatic pathways and SLNs were visualized. Of every detected SLN, the corresponding lymph node station, signal-to-background ratio and histopathological diagnosis was determined. Patients underwent standard-of-care gastrectomy. Detected SLNs outside the standard dissection planes were also resected and evaluated.RESULTS: Twenty-six patients were enrolled. Four patients were excluded because distant metastases were found during surgery or due to technical failure of the injection. In 21 of the remaining 22 patients, at least 1 SLN was detected by NIR Fluorescence imaging(mean 3.1 SLNs; range 1-6). In 8 of the 21 patients, tumor-positive LNs were found. Overall accuracy of the technique was 90%(70%-99%; 95%CI), which decreased by higher p T-stage(100%, 100%, 100%, 90%, 0% for respectively Tx, T1, T2, T3, T4 tumors). All NIR-negative SLNs were completely effaced by tumor. Mean fluorescence signal-to-background ratio of SLNs was 4.4(range 1.4-19.8). In 8 of the 21 patients, SLNs outside the standard resection plane were identified, that contained malignant cells in 2 patients.CONCLUSION: This study shows successful use of ICG:Nanocoll as lymphatic tracer for SLN detection in gastric cancer. | 2 | Timely reperfusion for ST-segment elevation myocardial infarction:Effect of direct transfer to primary angioplasty on time delays and clinical outcomes显示文摘Primary percutaneous coronary intervention(PPCI) is the preferred reperfusion therapy for patients presenting with ST-segment elevation myocardial infarction(STEMI) when it can be performed expeditiously and by experienced operators. In spite of excellent clinical results this technique is associated with longer delays than thrombolysis and this fact may nullify the benefit of selecting this therapeutic option. Several strategies have been proposed to decrease the temporal delays to deliver PPCI. Among them,prehospital diagnosis and direct transfer to the cath lab,by-passing the emergency department of hospitals,has emerged as anattractive way of diminishing delays. | 3 | Thiopurine-methyltransferase variants in inflammatory bowel disease:Prevalence and toxicity in Brazilian patients显示文摘AIM:To analyze the prevalence of thiopurine-methyltransferase(TPMT)genotypes and their associationwith drug toxicity in inflammatory bowel disease(IBD)patients from southeastern Brazil.METHODS:A total of 219 consecutive patients with IBD,of which 146 had Crohn’s disease and 73 had ulcerative colitis,regularly seen at the outpatient unit of the Division of Gastroenterology at the University Hospital Pedro Ernesto of the State University of Rio de Janeiro,a tertiary referral center,were enrolled in this study from February 2009 to January 2011.We analyzed the presence of major TPMT genetic variants(TPMT*2,*3A,*3C)in IBD patients by means of a specific allele and RFLP-PCR.Genomic DNA was isolated from peripheral blood leukocytes by proteinase-K/Sodium Dodecyl Sulfate digestion and phenol-chloroform extraction.TPMT*2(C238G),TPMT*3A(G460A/A719G),and TPMT*3C(A719G)genotypes were detected by real-time polymerase chain reaction followed by direct sequencing with specific primers.Clinical data were systematically recorded,and correlated with the genotype results.RESULTS:The distribution of the selected TPMT gene polymorphism TPMT*2(C238G),TPMT*3A(G460A/A719G),and TPMT*3C(A719G)genotypes was 3.6%,5.4%,and 7.7%of the patients,respectively.Among the side effects recorded from patients taking azathioprine,14 patients presented with pancreatitis and/or an elevation of pancreatic enzymes,while 6 patients had liver toxicity,and 2 patients exhibited myelosuppression/neutropenia.TPMT polymorphisms were detected in 37/219 patients(8 heterozygous for*2,11 heterozygous for*3A,and 18 heterozygous for*3C).No homozygotic polymorphisms were found.Despite the prevalence of the TPMT*3C genotype,no differences among the genotype frequencies were significant.Although no association was detected regarding myelotoxicity or hepatotoxicity,a trend towards the elevation of pancreatic enzymes was observed for TPMT*2 and TPMT*3C genotypes.CONCLUSION:The prevalence of TPMT genotypes was high among Brazilian patients.Variants genes*2and*3C may be associated with azathioprine pancreatic toxicity in a IBD southeastern Brazilian population. | Ana Teresa P Carvalho Barbara C Esberard Renata S B Fróes Davy C M Rapozo Ana B Grinman Tatiana A Simo Juliana C V C Santos Antonio José V Carneiro Luis Felipe Ribeiro-Pinto Heitor S P de Souza | 2014 | World Journal of Gastroenterology2014，20，12： | 3 |

| 5 | A regionalized IO-model to value seasonal recreational ecosystem services in a mountain National Park in Spain显示文摘Recreational Ecosystem Services(RES)are among the most frequently evaluated ecosystem services.RES are seen as a major opportunity for sustainable development in areas of high ecological value resulting from the presence of emblematic species,habitats or scenery,often integrated in conservation areas,in particular in mountain areas affected by depopulation,rising environmental risks and poverty.Economically,the assessment of RES has been based on Contingent Valuation and Choice Experiments,methods with limitations related to their subjectivity.Alternatively,Input-Output(IO)models provide a very accurate and effective way of valuation of RES with regional information regarding interindustry transactions.Whilst data on nature tourist expenditure may be only available annually,tourism inflows have a strong seasonal behavior,which impacts the profitability of several local economic activities.In this paper,we firstly constructed a novel regionalized IO-model based on Cross-Industry Location Quotients using employment data and,secondly,based on Monte Carlo simulation,we estimated nature tourist expenditure monthly using data on nature tourism inflows.This method allowed for a more precise RES evaluation,estimating direct,indirect and induced monthly economic impacts of nature-based tourism.The method was applied to the Ordesa and Monte Perdido(OMP)National Park in the Spanish Pyrenees.To estimate the average tourist expenditure,we collected and analyzed spending on commerce,hospitality and restaurants,leisure,and transport of 385 visitors.Results suggest that using a regionalized IO model leads to a considerable reduction of over-estimation of the economic impact of tourist expenditure estimated by conventional methods.Taking into consideration the direct,indirect and induced effects of tourist expenditure in 2016-2018,the total annual output effect in the OMP National Park amounted to€208.8 million,while the total income effect amounted to€86.29 million.Also,a total of 2,429 jobs were created which indicates that for each thousand annual visitors to the National Park,3.8 jobs are created in the surrounding area.Overall,the proposed methodology can be easily applied to the remaining National Parks in Spain or other regions in the world,making it a valuable tool to estimate the value of RES in areas affects by strong seasonality,to set priorities and support regional policies for mountain sustainable development. | Víctor MARTIN BARROSO Mónica de CASTRO-PARDO Pascual FERNÁNDEZ MARTÍNEZ João C.AZEVEDO | 2022 | Journal of Mountain Science2022，19，1： | 2 |
