| 2 | 新型冠状病毒肺炎患者早期外周血淋巴细胞亚群及细胞因子特征显示文摘目的分析新型冠状病毒肺炎(COVID-19)早期(起病14天内)外周血免疫学特征及其与疾病严重程度的关系。方法收集2020年1月至3月广州医科大学附属市八医院收治的98例确诊COVID-19患者住院资料,分析COVID-19早期外周血淋巴细胞亚群[CD4^(+)T、CD8^(+)T细胞、B细胞、自然杀伤(NK)细胞、NKT细胞与调节性T细胞]和细胞因子[白细胞介素(IL)-2、IL-4、IL-6、IL-10、肿瘤坏死因子-α和干扰素-γ]变化特征,以及上述指标与患者病情严重程度的关系。年龄、起病时间、白细胞、中性粒细胞、淋巴细胞、淋巴细胞亚群和细胞因子水平为非正态分布的计量资料,3组患者间比较采用Kruskal-Wallis检验;性别、基础疾病、吸烟、武汉流行病学史、家庭成员被感染、症状、实验室指标异常比例为计数资料,采用Pearson卡方检验或Fisher确切概率检验;以Logistic回归分析影响疾病严重性的高危因素。结果根据病情严重程度将入组患者分为轻型组(17例)、普通型组(73例)和重型组(8例)。早期55.1%(54/98)患者CD4^(+)T细胞、40.8%(40/98)患者CD8^(+)T细胞和11.8%(10/85)患者B细胞计数下降。3组患者间CD4^(+)T细胞(P=0.010)、CD8^(+)T细胞(P=0.003)和B细胞(P=0.022)计数存在显著差异,以重型组水平最低。3组患者IL-6表达差异有统计学意义(P=0.001),以重型组水平最高,而其他细胞亚群及细胞因子水平则无显著差异(P均> 0.05)。Logistic回归分析显示,IL-6升高(OR=12.277、P=0.024)和年龄> 50岁(OR=21.823、P=0.030)分别为发生肺炎和重型化的独立预测因素。结论 COVID-19早期即明显破坏患者机体细胞和体液免疫应答,与疾病进展密切相关。 | 廖宝林 施海燕 刘艳霞 赵涵 焦倩 李丽雅 王信 曹意 何浩岚 | 2021 | 中华实验和临床感染病杂志(电子版)2021,15,3: | 8 |
| 3 | The signal pathways and treatment of cytokine storm in COVID-19显示文摘The Coronavirus Disease 2019(COVID-19)pandemic has become a global crisis and is more devastating than any other previous infectious disease.It has affected a significant proportion of the global population both physically and mentally,and destroyed businesses and societies.Current evidence suggested that immunopathology may be responsible for COVID-19 pathogenesis,including lymphopenia,neutrophilia,dysregulation of monocytes and macrophages,reduced or delayed type I interferon(IFN-I)response,antibody-dependent enhancement,and especially,cytokine storm(CS).The CS is characterized by hyperproduction of an array of pro-inflammatory cytokines and is closely associated with poor prognosis.These excessively secreted pro-inflammatory cytokines initiate different inflammatory signaling pathways via their receptors on immune and tissue cells,resulting in complicated medical symptoms including fever,capillary leak syndrome,disseminated intravascular coagulation,acute respiratory distress syndrome,and multiorgan failure,ultimately leading to death in the most severe cases.Therefore,it is clinically important to understand the initiation and signaling pathways of CS to develop more effective treatment strategies for COVID-19.Herein,we discuss the latest developments in the immunopathological characteristics of COVID-19 and focus on CS including the current research status of the different cytokines involved.We also discuss the induction,function,downstream signaling,and existing and potential interventions for targeting these cytokines or related signal pathways.We believe that a comprehensive understanding of CS in COVID-19 will help to develop better strategies to effectively control immunopathology in this disease and other infectious and inflammatory diseases. | Lan Yang Xueru Xie Zikun Tu Jinrong Fu Damo Xu Yufeng Zhou | 2021 | Signal Transduction and Targeted Therapy2021,6,8: | 7 |

| 6 | 新型冠状病毒肺炎患者实验室检查临床特点分析显示文摘目的:观察新型冠状病毒肺炎(coronavirus disease 2019,COVID-19)实验室检查指标与疾病严重程度的关系,并找出其发展成重型肺炎的危险因素。方法:纳入湖北省鄂州市第三人民医院综合楼隔离病房收治的COVID-19患者81例,其中普通型49例、重型32例,收集2组患者年龄、性别、慢性基础疾病、临床实验室检查指标进行统计对比分析,并使用受试者工作特征(receiver operating characteristic,ROC)曲线评估检查指标的预测价值。结果:81例COVID-19中,非重症组年龄(49.86±13.79)岁,重症组年龄(62.84±13.26)岁,重症组的年龄大于非重症组(P<0.05),合并有慢性基础疾病及白细胞计数(white blood cell count,WBC)、中性粒细胞计数(neutrophil count,NEU)、NEU百分比、C反应蛋白(C-reactive protein,CRP)、总胆红素(total bilirubin,TBIL)、肌酸激酶(creatine kinase,CK)、乳酸脱氢酶(lactate dehydrogenase,LDH)值越高的COVID-19患者越容易发展成重症肺炎(P<0.05);同时淋巴细胞计数(lymphocyte count,LYM)、LYM百分比、白蛋白值越低的COVID-19患者也容易发展成重症患者(P<0.05);对COVID-19患者重症风险的预测,患者年龄、WBC、NEU、NEU百分比、LYM、LYM百分比、ALB、CRP和LDH的ROC曲线下面积(area under the receiver-operating characteristic curve,AUC)分别为0.757、0.711、0.783、0.904、0.850、0.921、0.754、0.848和0.807。重复测量方差分析显示,NEU百分比、LYM百分比、CRP和LDH在不同病情组和治疗时间之间存在显著交互效应。结论:年龄大及合并慢性基础疾病的COVID-19患者更容易发展成重症,临床上可通过WBC、NEU、NEU百分比、LYM、LYM百分比、ALB、CRP和LDH评估COVID-19的严重程度,尤其是NEU百分比、LYM百分比结合患者年龄及肺部CT表现可预测其非重症患者进展成重症患者的危险性,以及时调整治疗方案。 | 汪晓凤 卢刚 | 2020 | 重庆医科大学学报2020,45,7: | 4 |

| 13 | 心肌损伤对新型冠状病毒肺炎危重型患者预后的影响及其原因分析显示文摘目的:探讨心肌损伤对新型冠状病毒肺炎(COVID-19)危重型患者预后的影响并分析其原因。方法:回顾性分析2787例COVID-19重症、危重症患者的临床资料。结果:2787例患者中,危重型患者392例,其中死亡236例(8.47%)。死亡患者中,78.81%的患者在入院时心肌损伤标志物血清心肌高敏肌钙蛋白(hs-cTnI)水平升高。根据入院时血清hs-cTnI水平分为hs-cTnI升高组(>34.2 pg/mL)210例,hs-cTnI正常组(≤34.2 pg/mL)182例。血清hs-cTnI升高组患者机械通气治疗的构成比更高(92.86%vs 64.29%,P<0.01)。血清hs-cTnI升高组14 d和30 d院内病死率明显高于正常组(65.71%vs 26.37%;84.28%vs 32.42%,均P<0.01)。患者血清hs-cTnI升高与14 d和30 d院内病死率相关(OR=2.34、3.15)。与hs-cTnI正常组比较,血清hs-cTnI升高组外周血淋巴细胞计数更低,而血清降钙素原、C-反应蛋白、白介素-2R、白介素-6、白介素-8、白介素-10及肿瘤坏死因子-α水平更高,提示病毒引发的炎症因子风暴可能是导致心肌损伤的原因。结论:心肌损伤是COVID-19危重型患者院内死亡的重要危险因素,其潜在机制可能是新型冠状病毒感染诱发的细胞因子风暴介导的炎症损伤。 | 汪璐芸 陈鹏 余波 李陈泽 周宁 汪道文 | 2020 | 内科急危重症杂志2020,26,6: | 2 |

| 20 | 轻重症及转归阶段的COVID-19患者免疫细胞的单细胞转录谱分析显示文摘目的构建轻重症及转归阶段的新型冠状病毒肺炎(corona virus disease 2019,COVID-19)患者和健康对照的外周血单个核细胞(peripheral blood mononuclear cell,PBMC)的单细胞转录图谱,分析疾病进展及恢复与宿主免疫反应之间的关系。方法从公共数据库中获取健康对照和COVID-19重症、轻症、恢复早期、恢复晚期患者PBMCs的单细胞转录组(single-cell RNA sequence,scRNA-seq)测序数据,根据每个细胞的表达谱将细胞进行聚类后,根据已知的细胞标志物确定每个聚类的细胞亚型,统计并比较每个细胞亚型的比例,并对轻重症及转归阶段细胞的差异表达基因进行分析。结果控制数据质量后得到的115334个PBMCs分成22个细胞亚群。CD14+单核细胞、炎症性单核细胞、na?ve B细胞和中间记忆B细胞在重症患者PBMCs中明显升高;增殖性CD8+T/NK细胞在重症患者T细胞中的比例明显升高;XCL1+NK细胞在重症患者PBMCs中明显降低;cDC2细胞、pDC细胞在恢复期患者PBMCs中明显升高。随着疾病的缓解与康复,PBMCs中NK细胞的比例逐步升高。疾病期与康复期的PBMCs中与白细胞活化、白细胞黏附相关的基因均有上调。结论轻重症及转归阶段的COVID-19患者PBMCs在细胞比例和表达谱上变化较大。 | 王正 张倩月 宋怀东 | 2022 | 复旦学报(医学版)2022,49,2: | 1 |
