| 1 | ^(40)Ar-^(39)Ar dating and geological implication of auriferous altered rocks from the middle-deep section of Q875 gold-quartz vein in Xiaoqinling area, Henan,China显示文摘The 40Ar-39Ar age method is employed in this work to analyze the auriferous altered rocks from the middle-deep section of the Q875 gold-quartz vein in the Xiao-qinling area, and the results show that the main gold deposition of the Q875 occurred in ca. 128-126 Ma. As a typical gold-bearing quartz vein in this gold-rich area, the age data obtained from the Q875 also constrain on the metallogenic time of the lode gold deposits developed in the same geological settings. | 2 | Current status of endoscopic retrograde cholangiopancreatography in patients with surgically altered anatomy显示文摘Endoscopic retrograde cholangiopancreatography(ERCP)in patients with surgically altered anatomy must be performed by a highly experienced endoscopist.The challenges are accessing the afferent limb in different types of reconstruction,cannulating a papilla with a reverse orientation,and performing therapeutic interventions with uncommon endoscopic accessories.The development of endoscopic techniques has led to higher success rates in this group of patients.Device-assisted ERCP is the endoscopic procedure of choice for high success rates in short-limb reconstruction;however,these success rate is lower in long-limb reconstruction.ERCP assisted by endoscopic ultrasonography is now popular because it can be performed independent of the limb length;however,it must be performed by a highly experienced and skilled endoscopist.Stent deployment and small stone removal can be performed immediately after ERCP assisted by endoscopic ultrasonography,but the second session is needed for other difficult procedures such as cholangioscopy-guided electrohydraulic lithotripsy.Laparoscopic-assisted ERCP has an almost 100%success rate in longlimb reconstruction because of the use of a conventional side-view duodenoscope,which is compatible with standard accessories.This requires cooperation between the surgeon and endoscopist and is suitable in urgent situations requiring concomitant cholecystectomy.This review focuses on the advantages,disadvantages,and outcomes of various procedures that are suitable in different situations and reconstruction types.Emerging new techniques and their outcomes are also discussed. | Chonlada Krutsri Mitsuhiro Kida Hiroshi Yamauchi Tomohisa Iwai Hiroshi Imaizumi Wasaburo Koizumi | 2019 | World Journal of Gastroenterology2019,25,26: | 23 |
| 3 | 放射生物学概念在鼻咽癌、食管癌及肺癌治疗中的应用显示文摘The term 'altered factionation' is relative to the conventonal fractionation, which is defined as follows: for curative treatment of most cancers, 180 to 200 cGy per fraction is given once a day, 5 times per week, to a total dose determined by the tumor being treated and the tolerance of critical normal tissues within the target volume, usually 60 to 70 Gy within 6~7 weeks. The basic rationales for altered fractionation can be summaried by (1) The late-responding tissues are much more sensitive to fraction size than the early-responding tissues, with multiple smaller fractions. The late responding tissues can tolerate an increased total dose and the effects on the tumor therefore may be increased by the higher total radiation dose with improved tumor control; (2) Reduction in overall treatment time decreases the opportunity of accelerated regeneration of tumor cells during treatment and therefore inceases the probability of tumor control for a given total dose. In general,altered fractionation schemes can be categoriclly divided in to the following:(1) Hypofraction(2) Split-course radiation therapy(3) Hyperfractionation(4) Accelerated fractionation(5) Accelerated hyperfractionationIn recently years, we have tested various altered fractionation schedules for the treatment of nasopharyngeal, esophageal and lung cancer. For the nasopharyngeal caricnoma, hyperfractionation was used firstly by 1.1 Gy/F, 2F/day with an interval of >6hrs, followed by concomitant boost to a total dose of 74 Gy within 6 weeks. In lung cancer, the program was 1.15~1.25 Gy per fraction, two fractions per day for a total dose of 70 Gy with overall treatment time about 6 to 7 weeks. Esophageal cancer, conventional fractionation was given to total dose about 41 Gy followed by a boost for an additional 28 Gy by 1.5 Gy per fraction twice daily with overall treatment time about 6.3 weeks.Our preliminary treatment data showed that the local tumor control rates were higher in these study groups than the conventional groups and the acute reactions were increased as expected but tolerable. | 6 | Risk factors for perforation during endoscopic retrograde cholangiopancreatography in post-reconstruction intestinal tract显示文摘BACKGROUND Endoscopic retrograde cholangiopancreatography(ERCP) in patients with surgically altered anatomy has been a major challenge to gastrointestinal endoscopists with low success rates for reaching the target site as well as high complication rates. The knowledge of ERCP-related risk factors is important for reducing unexpected complications.AIM To identify ERCP-related risk factors for perforation in patients with surgically altered anatomy.METHODS The medical records of 187 patients with surgically altered anatomy who underwent ERCP at our institution between April 2009 and December 2017 were retrospectively reviewed. An analysis of patient data, including age, sex, type of reconstruction, cause of surgery, aim of ERCP, success rate of reaching target site,success rate of procedure, adverse events, type of scope, time to reach the target site, and duration of procedure, was performed. In patients with Billroth-Ⅱ reconstruction, additional potential risk factors were the shape of the inserted scope and whether the anastomosis was antecolic or retrocolic.RESULTS All patients(n = 187) had surgical anatomy, such as Billroth-Ⅰ(n = 22), Billroth-Ⅱ(n = 33), Roux-en-Y(n = 54), Child, or Whipple reconstruction(n = 75). ERCP was performed for biliary drainage in 43 cases(23%), stone removal in 29 cases(16%),and stricture dilation of anastomosis in 59 cases(32%). The scope was unable to reach the target site in 17 cases(9%), and an aimed procedure could not be accomplished in 54 cases(29%). Adverse events were pancreatitis(3%),hyperamylasemia(10%), cholangitis(6%), cholestasis(4%), excessive sedation(1%), perforation(2%), and others(3%). Several operative interventions of both the gastrointestinal tract and the biliary and/or pancreatic system lead to altered anatomy, rendering ERCP more difficult or even impossible with a conventional side-viewing duodenoscope. Adapted endoscopes are avail-able to reach the biliopancreatic system and to perform ERCP in patients with altered anatomy. However, both technical difficulties and complications determine the procedure's success. Different technical approaches have been described and are highly dependent on local expertise and endoscopic equipment. Standard-ized practical guidelines are currently unavailable. This review focuses on the challenges encountered during ERCP in patients with altered anatomy and how to deal with them. The first challenge is reaching the papilla or the bilioenteric/pancreatoenteric anastomosis in the patient with postoperative altered anatomy. Initially, EUS was most commonly used for the diagnosis of pancreatobiliary diseases and tissue acquisition. EUS was first used for guided cholangiography in 1996, followed by EUS-guided biliary drainage in 2001. Advancements in equipment and endoscopic accessories have led to an expansion of EUS-guided procedures, which now include EUS-guided drainage of intra-abdominal abscesses or collections, intravascular treatment of refractory variceal and nonvariceal bleeding, transmural pancreatic drainage, common bile duct stone clearance, enteral feeding tube placement and entero-enteric anastomosis. Patients with surgically altered upper gastrointestinal anatomies have greatly benefited from EUS also. This systematic review describes and discusses EUS procedures performed in uncommon diseases and conditions, as well as applications on more vulnerable patients such as young children and pregnant women. In these cases, routine approaches do not always apply, and thus may require the use of innovative and unconventional techniques. | 15 | Recent trends in endoscopic management of achalasia显示文摘Esophageal achalasia is a chronic and progressive motility disorder characterized by absence of esophageal body peristalsis associated with an impaired relaxation of lower esophageal sphincter(LES) and usually with an elevated LES pressure, leading to an altered passage of bolus through the esophago-gastric junction. A definitive cure for achalasia is currently unavailable. Palliative treatment options provide only food and liquid bolus intake and relief of symptoms. Endoscopic therapy for achalasia aims to disrupt or weaken the lower esophageal sphincter. Intra-sphincteric injection of botulinum toxin is reserved for elderly or severely ill patients. Pneumatic dilation provides superior results than botulinum toxin injection and a similar mediumterm efficacy almost comparable to that attained after surgery. Per oral endoscopic myotomy is a promising option for treating achalasia, but it requires increased experience and further objective and long-term follow up. Alteration can not onlychange their texture and mineral assemblage, but also influence their geochemistry properties. The mostavailable data show that alteration will directly influence the content of Au in lamprophyre. For example,in the Superior area of Canada and the Yilgan area of Australia relatively fresh lamprophyres are bothlow in Au contents (3.9×10-9 and 1.7×10-9, respectively), but the contents of Au in altered lamprophyres from both areas are double increased (38×10-9 and 20×10-9 on average). Lamprophyres arewidely exposed in the Zhenyuan gold deposit, Yunnan Province and they are closely associated with goldmineralization both in space and in time. So the lamprophyres in the Zhenyuan gold deposit can be citedas the most typical example both at home and abroad.
