1Multiple methods of surgical treatment combined with primary IOL implantation on traumatic lens subluxation/dislocation in patients with secondary glaucoma显示文摘AIM:To describe clinical findings and complications from cases of traumatic lens subluxation/dislocation in patients with secondary glaucoma,and discuss the multiple treating methods of operation combined with primary intraocular lens(IOL)implantation.METHODS:Non-comparativeretrospectiveobservational case series.Participants:30 cases(30 eyes)of lens subluxation/dislocation in patients with secondary glaucoma were investigated which accepted the surgical treatment by author in the Ophthalmology of Xi'an No.4Hospital from 2007 to 2011.According to the different situations of lens subluxation/dislocation,various surgical procedures were performed such as crystalline lens phacoemulsification,crystalline lens phacoemulsification combined anterior vitrectomy,intracapsular cataract extraction combined anterior vitrectomy,lensectomy combined anterior vitrectomy though peripheral transparent cornea incision,pars plana lensectomy combined pars plana vitrectomy,and intravitreal cavity crystalline lens phacofragmentation combined pars plana vitrectomy.And whether to implement trabeculectomy depended on the different situations of secondary glaucoma.The posterior chamber intraocular lenses(PC-IOLs)were implanted in the capsular-bag or trassclerally sutured in the sulus decided by whether the capsular were present.Main outcome measures:visual acuity,intraocular pressure,the situation of intraocular lens and complications after the operations.RESULTS:The follow-up time was 11-36mo(21.4±7.13).Postoperative visual acuity of all eyes were improved;28 cases maintained IOP below 21 mm Hg;2cases had slightly IOL subluxation,4 cases had slightlytilted lens optical area;1 case had postoperative choroidal detachment;4 cases had postoperative corneal edema more than 1wk,but eventually recovered transparent;2 cases had mild postoperative vitreous hemorrhage,and absorbed 4wk later.There was no postoperative retinal detachment,IOL dislocation,and endophthalmitis.CONCLUSION:To take early treatment of traumatic lens subluxation/dislocation in patients with secondary glaucoma by individual surgical plan based on the different eye conditions would be safe and effective,which can effectively control the intraocular pressure and restore some vision.Rui Wang Chun-Chao Bi Chun-Ling Lei Wen-Tao Sun Shan-Shan Wang Xiao-Juan Dong 2014International Journal of Ophthalmology(English edition)2014,7,2:26

4DISLOCATION DISSOCIATIONS AND FAULTENERGIES IN Ni_3Al ALLOYS DOPED WITH PALLADIUM显示文摘Dislocation structures in polycrystalline Ni 3Al alloy doped with palladium deformed at room temperature have been investigated by transmission electron microscopy. The structure consists mainly of dislocations dissociated into a /2〈011〉 super partials bounding an anti phase boundary (APB). Dislocations dissociated into a /3〈112〉 super Shockley partials bounding a superlattice intrinsic stacking fault (SISF) are also common debris. The majority of the SISFs are truncated loops, i.e. the partials bounding the SISF are of similar Burgers vector. These faulted loops are generated from APB coupled dislocations, according to a mechanism for formation of SISFs proposed by Suzuki et al , and recently modified by Chiba et al . The APB energies for {111} and {010} slip planes are measured to be 144±20 mJ/m 2 and 102±11 mJ/m 2 respectively, and the SISF energy has been estimated to be 12 mJ/m 2 in this alloy. It is concluded that the dislocation structure in Ni 74.5 Pd 2Al 23.5 alloy deformed at room temperature is similar to that in binary Ni 3Al, and the difference in fault energies between these two alloys is small. Thus, it seems unlikely that the enhancement of ductility of Ni 74.5 Pd 2Al 23.5 results from only such a small decrease of the ordering energy of the alloy. 6Multiligament knee injuries with associated tibial plateau fractures: A report of two cases显示文摘The management of a combination of fracture and multiligament knee injury(MKI) in traumatic knee injury remains controversial, and there are evolving treatment recommendations. Currently, there are no studies focusing on older adult patients with MKI's in combination with tibia fractures. As a result, there is no well-established treatment algorithm for older adult patients with these complex injuries. We report two cases of MKI's with concomitant fractures in patients fifty years of age or older. Both patients were treated surgically for their associated tibial plateau fractures, but were managed with conservative treatment of the multiligamentous knee injuries. We also provide a review of the literature and guidelines for older adult patients with these types of complex traumatic injuries. Early to mid term acceptable outcomes were achieved for both patients through surgical fixation of the tibial plateau fracture and conservative treatment of the ligament injuries. 7New insights in the treatment of acromioclavicular separation显示文摘A direct force on the superior aspect of the shoulder may cause acromioclavicular(AC) dislocation or separation. Severe dislocations can lead to chronic impairment, especially in the athlete and high-demand manual laborer. The dislocation is classified according to Rockwood. Types Ⅰ?and Ⅱ are treated nonoperatively, while types Ⅳ, Ⅴ and Ⅵ are generally treated operatively. Controversy exists regarding the optimal treatment of type Ⅲ dislocations in the high-demand patient. Recent evidence suggests that these should be treated nonoperatively initially. Classic surgical techniques were associated with high complication rates, including recurrent dislocations and hardware breakage. In recent years, many new techniques have been introduced in order to improve the outcomes. Arthroscopic reconstruction or repair techniques have promising short-term results. 8Electron force-induced dislocations annihilation and regeneration of a superalloy through electrical in-situ transmission electron microscopy observations显示文摘What effect does electric current do on dislocation evolution of metals keeps being a confusing question to be answered and proved. To this end, the dislocation evolution of a superalloy with electric current was directly observed by electrical in-situ transmission electron microscopy in this work. Dislocations annihilation at first and then regeneration was found for the first time, which directly proves the existence of electron force during the electrically-assisted manufacturing. Dislocations regeneration would be driven by the electron force and the resistance softening by the local Joule heating effect. 9The‘skin effect’of subsurface damage distribution in materials subjected to high-speed machining显示文摘This paper proposes the‘skin effect’of the machining-induced damage at high strain rates.The paper first reviews the published research work on machining-induced damage and then identifies the governing factors that dominate damage formation mechanisms.Among many influential factors,such as stress-strain field,temperature field,material responses to loading and loading rate,and crack initiation and propagation,strain rate is recognized as a dominant factor that can directly lead to the‘skin effect’of material damage in a loading process.The paper elucidates that material deformation at high strain rates(>103 s−1)leads to the embrittlement,which in turn contributes to the‘skin effect’of subsurface damage.The paper discusses the‘skin effect’based on the principles of dislocation kinetics and crack initiation and propagation.It provides guidance to predicting the material deformation and damage at a high strain-rate for applications ranging from the armor protection,quarrying,petroleum drilling,and high-speed machining of engineering materials(e.g.ceramics and SiC reinforced aluminum alloys).Bi Zhang Jingfei Yin 2019International Journal of Extreme Manufacturing2019,1,1:11
10Recurrent anterior shoulder instability: Review of the literature and current concepts显示文摘The purpose of this review article is to discuss the clinical spectrum of recurrent traumatic anterior shoulder instability with the current concepts and controversies at the scientific level. Because of increasing participation of people from any age group of the population in sports activities, health care professionals dealing with the care of trauma patients must have a thorough understanding of the anatomy, patho-physiology, risk factors, and management of anterior shoulder instability. The risk factors for recurrent shoulder dislocation are young age, participation in high demand contact sports activities, presence of Hill-Sachs or osseous Bankart lesion, previous history of ipsilateral traumatic dislocation, ipsilateral rotator cuff or deltoid muscle insufficiency, and underlying ligamentous laxity. Achieving the best result for any particular patient depends onthe procedure that allows observation of the joint surfaces, provides the anatomical repair, maintains range of motion, and also can be applied with low rates of complications and recurrence. 12Experimental study about nano-deformation field near quasi-cleavage crack tip显示文摘Using the nano-moire method, we measure the near tip nanoscopic deformation on the {111 } plane of single crystal silicon with a loaded quasi-cleavage crack running in the [110] direction. The measured strain distribution ahead of the crack tip agrees with the linear elastic fracture mechanics prediction up to 10 nm from the crack tip. 13Sternoclavicular joint dislocation and its management: A review of the literature显示文摘Dislocations of the sternoclavicular joint(SCJ) occur with relative infrequency and can be classified into anterior and posterior dislocation, with the former being more common. The SCJ is inherently unstable due to its lack of articular contact and therefore relies on stability from surrounding ligamentous structures, such as the costoclavicular, interclavicular and capsular ligaments. The posterior capsule has been shown in several studies to be the most important structure in determining stability irrespective of the direction of injury. Posterior dislocation of the SCJ can be associated with life threatening complications such as neurovascular, tracheal and oesophageal injuries. Due to the high mortality associated with such complications, these injuries need to be recognised acutely and managed promptly. Investigations such as x-ray imaging are poor at delineating anatomy at the level of the mediastinum and therefore CT imaging has become the investigation of choice. Due to its rarity, the current guidance on how to manage acute and chronic dislocations is debatable. This analysis of historical and recent literature aims to determine guidance on current thinking regarding SCJ instability, including the use of the Stanmore triangle. 15幼儿发育性髋脱位髋臼前倾的三维CT研究显示文摘[目的]利用髋关节的三维CT影像,来探讨3岁以下幼儿发育性髋关节脱位(developmentaldislocationofthehip,DDH)髋臼前倾角(Acetabularanteversion,AA)的改变。[方法]对44例3岁以下单侧DDH患儿的3DCT影像资料进行分析,正常侧髋作为对照,测量髋臼的前倾角。[结果]正常侧髋臼AA为(10·0±4·7)°,脱位侧为(18·1±4·5)°,P<0·01。按年龄分组,正常侧髋臼AA:小于18个月组为(9·2±4·3)°,大于等于18个月组为(10·6±4·9)°,P=0·34。脱位侧髋臼AA:小于18个月组为(16·2±4·5)°,大于等于18个月组为(19·2±4·1)°,P=0·03。[结论]三维CT能清晰地显示幼儿DDH髋臼的形态。脱位后,髋臼前倾角明显增加,且脱位时间越长,髋臼前倾越明显。了解髋臼的前倾情况,对DDH的治疗具有重要意义。李连永 赵群 2005中国矫形外科杂志2005,13,17:8

17Corrosion-enhanced dislocation emission and motion resulting in initiation of stress corrosion cracking显示文摘A special constant deflection device for TEM has been designed, and then change of dislocation configuration ahead of a crack tip during stress corrosion cracking (SCC) of brass in water and of Ti-24Al-11Nb alloy in methanol and initiation of SCC can he observed in TEM In situ tensile test in TEM for brass was carried out for comparison The results show that anodic dissolution during SCC can facilitate dislocation emission, multiplication and motion, and a dislocation free zone (DFZ) is formed The stress at a particular site in the DFZ, which is an elastic zone and is thinned gradually through corrosion-enhanced dislocation emission and motion, is possibly up to the cohesive strength, resulting in initiating of a nanocrack of SCC in the DFZ or sometimes at the crack tip. 18Neurovascular complications due to the Hippocrates method for reducing anterior shoulder dislocations显示文摘In spite of the fact that the Hippocrates method hardly has been evaluated in a scientific manner and numerous associated iatrogenic complications have been reported, this method remains to be one of the most common techniques for reducing anterior shoulder dislocations. We report the case of a 69-year-old farmer under coumarin anticoagulant therapy who sustained acute first time anterior dislocation of his dominant right shoulder. By using the Hippocrates method with the patient under general anaesthesia, the brachial vein was injured and an increasing hematoma subsequently caused brachial plexus paresis by pressure. After surgery for decompression and vascular suturing, symptoms declined rapidly, but brachial plexus paresis still was not fully reversible after 3 mo of follow-up. The hazardousness of using the Hippocrates method can be explained by traction on the outstretched arm with force of the operator's body weight, direct trauma to the axillary region by the physician's heel, and the topographic relations of neurovascular structures and the dislocated humeral head. As there is a variety of alternative reduction techniques which have been evalu-ated scientifically and proofed to be safe, we strongly caution against the use of the Hippocrates method as a first line technique for reducing anterior shoulder dislocations, especially in elder patients with fragile vessels or under anticoagulant therapy, and recommend the scapular manipulation technique or the Milch technique, for example, as a first choice.Markus Regauer Hans Polzer Wolf Mutschler 2014World Journal of Orthopedics2014,5,1:7
19MICROSTRUCTURAL EVOLUTION AND MECHANISMS OF SUPERPLASTICITY IN LARGE GRAINED IRON ALUMINIDES显示文摘The superplastic behavior has been found in Fe 3Al and FeAl alloys with grain sizes of 100~600 μm. The large grained Fe 3Al and FeAl alloys exhibit all deformation characteristics of conventional fine grain size superplastic alloys. However, superplastic behavior was found in large grained iron aluminides without the usual prerequisites for the superplasticity of a fine grain size and grain boundary sliding. The metallographic examinations have shown that average grain size of large grained iron aluminides decreased during superplastic deformation. Transmission electron microscopy (TEM) observations have shown that there were a great number of subgrain boundaries which formed a network and among which the proportion of low and high angle boundaries increased with the increase of strain. The observed superplastic phenomenon is explained by continuous recovery and recrystallization. During superplastic deformation, an unstable subgrain network forms and these subboundaries absorb gliding dislocations and transform into low and high angle grain boundaries.
