| 3 | Effect of Comprehensive Therapy based on Chinese Medicine Patterns on Self-Efficacy and Effectiveness Satisfaction in Chronic Obstructive Pulmonary Disease Patients显示文摘Objective: To evaluate the effect of comprehensive therapy based on Chinese medicine(CM) patterns on self-efficacy and satisfaction with its effectiveness in patients with chronic obstructive pulmonary disease(COPD). Methods: A total of 216 patients were randomly divided into the trial group(n=108) and the control group(n=108) based on the stratified and block randomization design. Patients in the trial group were treated with conventional Western medicine combined with Bufei Jianpi Granules(补肺健脾颗粒), Bufei Yishen Granules(补肺益肾颗粒), and Yiqi Zishen Granules(益气滋肾颗粒) according to the CM patterns respectively, and patients in the control group were treated with conventional Western medicine. The COPD Self-Efficacy Scale(CSES) and the Effectiveness Satisfaction Questionnaire for COPD(ESQ-COPD) were employed in a 6-month treatment and in further 6 month follow-up visit. Results: Among the 216 patients, 191 patients(97 in the trial group and 94 in the control group) fully completed the study. After 12-month treatment and follow-up, the mean scores of the trial group all continued to increase over time, which were significantly higher than those of the control group(P<0.05), and the improvement in the following trial group domain: negative affect domain(12.13%), intense emotional arousal domain(12.21%), physical exertion domain(11.72%), weather/environmental domain(13.77%), behavioral risk domain(7.67%) and total score(10.65%). The trial group also exhibited significantly higher mean scores in the ESQ-COPD(P<0.05) and the improvement in the following domain: capacity for life and work domain(30.59%), clinical symptoms domain(53.52%), effect of therapy domain(35.95%), convenience of therapy domain(35.54%), and whole effect domain(52.47%). | 4 | Effectiveness and safety of SARS-CoV-2 vaccine in real-world studies: a systematic review and meta-analysis显示文摘Background: To date, coronavirus disease 2019 (COVID-19) becomes increasingly fierce due to the emergence of variants. Rapid herd immunity through vaccination is needed to block the mutation and prevent the emergence of variants that can completely escape the immune surveillance. We aimed to systematically evaluate the effectiveness and safety of COVID-19 vaccines in the real world and to establish a reliable evidence-based basis for the actual protective effect of the COVID-19 vaccines, especially in the ensuing waves of infections dominated by variants.Methods: We searched PubMed, Embase and Web of Science from inception to July 22, 2021. Observational studies that examined the effectiveness and safety of SARS-CoV-2 vaccines among people vaccinated were included. Random-effects or fixed-effects models were used to estimate the pooled vaccine effectiveness (VE) and incidence rate of adverse events after vaccination, and their 95% confidence intervals (CI).Results: A total of 58 studies (32 studies for vaccine effectiveness and 26 studies for vaccine safety) were included. A single dose of vaccines was 41% (95%CI: 28-54%) effective at preventing SARS-CoV-2 infections, 52% (31-73%) for symptomatic COVID-19, 66% (50-81%) for hospitalization, 45% (42-49%) for Intensive Care Unit (ICU) admissions, and 53% (15-91%) for COVID-19-related death;and two doses were 85% (81-89%) effective at preventing SARS-CoV-2 infections, 97% (97-98%) for symptomatic COVID-19, 93% (89-96%) for hospitalization, 96% (93-98%) for ICU admissions, and 95% (92-98%) effective for COVID-19-related death, respectively. The pooled VE was 85% (80-91%) for the prevention of Alpha variant of SARS-CoV-2 infections, 75% (71-79%) for the Beta variant, 54% (35-74%) for the Gamma variant, and 74% (62-85%) for the Delta variant. The overall pooled incidence rate was 1.5% (1.4-1.6%) for adverse events, 0.4 (0.2-0.5) per 10 000 for severe adverse events, and 0.1 (0.1-0.2) per 10 000 for death after vaccination.Conclusions: SARS-CoV-2 vaccines have reassuring safety and could effectively reduce the death, severe cases, symptomatic cases, and infections resulting from SARS-CoV-2 across the world. | 5 | Effect of hierarchical ZSM-5zeolite crystal size on diffusion and catalytic performance of n-heptane cracking显示文摘Hierarchical ZSM-5zeolite aggregates with different sizes of nanocrystals were synthesized using different amounts of the mesoporogen 3-aminopropyl- triethoxysilane.The effect of the crystal size on the catalytic cracking of n-heptane was investigated and the Thiele modulus and effectiveness factor were used to determine the reaction rate-limiting step.The crystal size affected the textual properties of the catalysts but not the acidic properties of the catalysts.The reaction rate was first order with respect to the n-heptane concentration.Cracking over hierarchical zeolites with nanocrystal sizes larger than about 50nm took place under transition-limiting conditions,whereas the reaction over hierarchical zeolites with nanocrystal sizes of 15 or 30nm proceeded under reaction control conditions.Hierarchical ZSM-5 zeolite aggregates with smaller nanocrystals had better selectivity for light olefins which can be ascribed to the shorter diffusion path lengths and lower diffusion resistance in these catalysts. | 9 | Laboratory model study of newly deposited dredger fills using improved multiple-vacuum preloading technique显示文摘Problems continue to be encountered concerning the traditional vacuum preloading method in field during the treatment of newly deposited dredger fills.In this paper,an improved multiple-vacuum preloading method was developed to consolidate newly dredger fills that are hydraulically placed in seawater for land reclamation in Lingang Industrial Zone of Tianjin City,China.With this multiplevacuum preloading method,the newly deposited dredger fills could be treated effectively by adopting a novel moisture separator and a rapid improvement technique without sand cushion.A series of model tests was conducted in the laboratory for comparing the results from the multiple-vacuum preloading method and the traditional one.Ten piezometers and settlement plates were installed to measure the variations in excess pore water pressures and moisture content,and vane shear strength was measured at different positions.The testing results indicate that water dischargeetime curves obtained by the traditional vacuum preloading method can be divided into three phases:rapid growth phase,slow growth phase,and steady phase.According to the process of fluid flow concentrated along tiny ripples and building of larger channels inside soils during the whole vacuum loading process,the fluctuations of pore water pressure during each loading step are divided into three phases:steady phase,rapid dissipation phase,and slow dissipation phase.An optimal loading pattern which could have a best treatment effect was proposed for calculating the water discharge and pore water pressure of soil using the improved multiple-vacuum preloading method.For the newly deposited dredger fills at Lingang Industrial Zone of Tianjin City,the best loading step was 20 kPa and the loading of 40-50 k Pa produced the highest drainage consolidation.The measured moisture content and vane shear strength were discussed in terms of the effect of reinforcement,both of which indicate that the multiple-vacuum preloading method has a better treatment effect not only in decreasing the moisture content and increasing the bearing capacity,but also in increasing the process uniformity at different depths of foundation. | Jingjin Liu Huayang Lei Gang Zheng Haizuo Zhou Xuelian Zhang | 2017 | Journal of Rock Mechanics and Geotechnical Engineering2017,9,5: | 7 |

| 13 | Effectiveness of Tenofovir Alafenamide in Chronic Hepatitis B Patients with Normal Alanine Aminotransferase and Positive Hepatitis B Virus DNA显示文摘Background and Aims:With an increasing understanding of hepatitis B,the antiviral indications have been broadening gradually.To evaluate the effectiveness of tenofovir alafena-mide(TAF)in chronic hepatitis B(CHB)patients with normal alanine aminotransferase(ALT)and detectable hepatitis B virus(HBV)DNA,those who are ineligible for broader anti-viral criteria from the Chinese CHB prevention guide(2019).Methods:A total of 117 patients were recruited and their data were collected from paper or electronic medical records.HBV DNA and liver function were measured at baseline and throughout the 24-week follow-up.The effectiveness end-point was complete virological response.The safety endpoint was the first occurrence of any clinical adverse event during the treatment.Results:Among the 117 patients,45 had normal ALT as well as detectable HBV DNA and they were not recommended for antiviral therapy according to Chinese Guidelines(2019).After TAF antiviral therapy,the rates of patients who achieved HBV DNA<20 IU/mL at 4,12 and 24 weeks were 77.1%,96.7%and 96.8%respectively.Among them,the undetectable rates of HBV DNA in patients with low baseline viral load at 4,12 and 24 weeks were 92.3%,100%and 100%,while the rates of those with high baseline viral load were 68.2%,94.1%and 94.4%.Compared with 71.4%,94.4%and 94.7%in the high baseline group,the undetectable rates of HBV DNA at 4,12 and 24 weeks in the low baseline liver stiffness group were 85.7%,100%and 100%.There was no statistical significance among the above groups.Conclusions:CHB patients who had normal ALT and detectable HBV DNA and did not meet“CHB prevention guide(2019)”,could achieve complete virological response in 24 weeks after antiviral treatment by TAF. | Wen-Kang Gao Yan-Yun Shu Yue Chen Yan Ai Xiao-Qian Yang Fan Du Jin Ye | 2022 | Journal of Clinical and Translational Hepatology2022,10,1: | 5 |

| 15 | Effectiveness of ozonated saline in the treatment of VX2 tumors in rabbits显示文摘Objective To investigate the efficacy, safety, and associated mechanisms of injected ozonated saline in the treatment of VX2 tumors. Methods A total of 90 rabbits bearing VX2 tumors on their left hind legs were randomly divided into three groups. The control group(A) received normal saline, while groups B and C received 20 μg/mL and 40 μg/mL O_3/O_2 ozonated saline, respectively. Rabbits were anesthetized and 2 mL of blood was drawn directly from the heart to measure serum concentrations of interleukin(IL-6) and tumor necrosis factor(TNF-α). The skin covering the VX2 tumor was cut in each rabbit and the maximum and vertical diameters of the tumors were measured under direct visualization. Several milliliters of saline, saline pre-treated with 20 μg/mL O_3/O_2, or saline pre-treated with 40 μg/mL O_3/O_2 were directly injected into the tumors of groups A, B, and C, respectively(injection volume(milliliter) =1/2 volume of the tumor, V = 1/2ab^2). On days 4, 8 and 12 following treatment, 10 rabbits were randomly selected from each group for blood sample collection, and serum IL-6 and TNF-α were measured. The tumor growth rate was calculated by measuring the maximum and vertical diameters of the VX2 tumors under direct visualization. All selected rabbits were euthanized and the tumors, livers, and lungs were removed for pathological examination. The tumor necrosis rate was calculated by cutting the tumors into half along the longitudinal axis and measuring the maximum diameters of the intratumoral necrotic regions. Results The average tumor volume in the three groups increased to different degrees at each time point; however, the average tumor growth rates in groups B and C were substantially lower than that in group A, exhibiting a statistically significant difference. The difference in the tumor growth rate between group B and group C was not statistically significant. The serum concentrations of IL-6 and TNF-α increased in the three groups at each time point, with larger increases occurring in groups B and C; however, the greater increases did not reach statistical significance. Although the diameters of the necrotic areas were larger in both groups B and C than that in group A, significant differences in necrotic area diameters were only found when comparing groups A and C on days 4 and 12 following treatment. Conclusion Direct injection of different concentrations of ozonated saline into VX2 tumors significantly increased intratumoral necrosis and reduced the tumor growth rate. Radiofrequency catheter ablation (RFCA) is an establishedtreatment option superior to antiarrhythmics (AAs). In this study, we investigated safety and efficacy of RFCA of AF in octogenarians.Methods From our database, we extracted procedural and follow-up data for patients 〉 80 years with symptomatic AF undergoing RFCAand compared this population to RFCA patients 〈 50 years. All patients underwent pulmonary vein isolation (PVI) supplemented by linearlesions in PVI-nonresponders. Arrhythmia-free survival was assessed using seven day Holter every three months post procedure. All patientscompleted their 12 months follow-up. Results Fifty patients aged ≥80 years (80.5 ± 1.6 years) were compared to 259 patients aged 〈 50years (43.5±5.5 years). The RFCA complication rate did not vary between groups. No differences in procedural characteristics were seenafter being analyzed by type of AF. Among patients with paroxysmal AF, 71.4% octogenarian vs. 84.7% young patients was free of anyarrhythmia, without AAs, after single procedure. For non-paroxysmal AF, arrhythmia-free survival without AAs, was considerably lower(58.6% octogenarians vs. 81.2% younger patients, P = 0.023). If AAs were used, arrhythmia-free survival for paroxysmal AF increased to90.5% and 92.1% in octogenarians and younger patients, respectively; and in non-paroxysmal AF it increased to 79.3% vs. 88.4%. ConclusionsRFCA is a safe and effective strategy to achieve normal sinus rhythm in a highly selected group of octogenarians. Paroxysmal AF ablation inoctogenarians has similar clinical effectiveness as that seen in much younger patients. | 20 | Effectiveness of Integrative Medicine Therapy on Coronary Artery Disease Prognosis: A Real-World Study显示文摘Objective: To evaluate the effectiveness of integrative medicine(IM) on patients with coronary artery disease(CAD) and investigate the prognostic factors of CAD in a real-world setting. Methods: A total of 1,087 hospitalized patients with CAD from four hospitals in Beijing, China were consecutively selected between August 2011 and February 2012. The patients were assigned to two groups based on the treatment: Chinese medicine(CM) plus conventional treatment, i.e., IM therapy(IM group); or conventional treatment alone(CT group). The endpoint was major adverse cardiac events [MACE; including cardiac death, myocardial infarction(MI), and revascularization]. Results: A total of 1,040 patients finished the 2-year follow-up. Of them, 49.4%(514/1,040) received IM therapy. During the 2-year follow-up, the total incidence of MACE was 11.3%. Most of the events involved revascularization(9.3%). Cardiac death/MI occurred in 3.0% of cases. For revascularization, logistic stepwise regression analysis revealed that age 65 years [odds ratio(OR), 2.224], MI(OR, 2.561), diabetes mellitus(OR, 1.650), multi-vessel lesions(OR, 2.554), baseline high sensitivity C-reactive protein level 3 mg/L(OR, 1.678), and moderate or severe anxiety/depression(OR, 1.849) were negative predictors(P<0.05); while anti-platelet agents(OR, 0.422), β-blockers(OR, 0.626), statins(OR, 0.318), and IM therapy(OR, 0.583) were protective predictors(P<0.05). For cardiac death/MI, age 65 years(OR, 6.389) and heart failure(OR, 7.969) were negative predictors(P<0.05), while statin use(OR, 0.323) was a protective predictor(P<0.05) and IM therapy showed a beneficial tendency(OR, 0.587), although the difference was not statistically significant(P=0.218).
