| 1 | Modified porous tantalum rod technique for the treatment of femoral head osteonecrosis显示文摘AIM: To study a modified porous tantalum technique for the treatment of osteonecrosis of the femoral head.METHODS: The porous tantalum rod was combined with endoscopy,curettage,autologous bone grafting and use of bone marrow aspirates from iliac crest aspiration in 49 patients(58 hips) with a mean age of 38 years.The majority of the patients had idiopathic osteonecrosis,followed by corticosteroid-induced osteonecrosis.Thirtyeight hips were of Steinberg stage Ⅱ disease and 20 hips were of stage Ⅲ disease.Patients were followed for 5 years and were evaluated clinically with the Merle D'Aubigne and Postel score and radiologically.The primary outcome of the study was survival based on the conversion to total hip arthroplasty(THA).Secondary outcomes included deterioration of the osteonecrosis to a higher disease stage at 5 years compared to the preoperative period and identification of factors that were associated with survival.The Kaplan-Meier survival analysis was performed to evaluate the survivorship ofthe prosthesis,and the Fisher exact test was performed to test associations between various parameters with survival.RESULTS: No patient developed any serious intraoperative or postoperative complication including implant loosening or migration and donor site morbidity.During the 5-year follow up,1 patient died,7 patients had disease progression and 4 hips were converted to THA.The 5-year survival based on conversion to THA was 93.1% and the respective rate based on disease progression was 87.9%.Stage Ⅱ disease was associated with statistically significant better survival rates compared to stage Ⅲ disease(P = 0.04).The comparison between idiopathic and non-idiopathic osteonecrosis and between steroid-induced and non-steroid-induced osteonecrosis did not showed any statistically significant difference in survival rates.The clinical evaluation revealed statistically significantly improved Merle d'Aubigne scores at 12 mo postoperatively compared to the preoperative period(P < 0.001).The mean preoperative Merle d'Aubigne score was 13.0(SD: 1.8).The respective score at 12 mo improved to 17.0(SD: 2.0).The 12-mo mean score was retained at 5 years.CONCLUSION: The modified porous tantalum rod technique presented here showed encouraging outcomes.The survival rates based on conversion to THA are the lowest reported in the published literature. | Emilios E Pakos Panayiotis Megas Nikolaos K Paschos Spyridon A Syggelos Antonios Kouzelis Georgios Georgiadis Theodoros A Xenakis | 2015 | World Journal of Orthopedics2015,6,10: | 20 |
| 2 | Effect of Different Rootstocks on the Salt Stress Tolerance in Watermelon Seedlings显示文摘The selection of salt-tolerant watermelon rootstocks requires the evaluation of its underlying mechanism. The present study investigated the effect of applying 200 mmol·L-1 NaCl on various physiological and biochemical parameters of Citrullus lanatus ‘Jingxin No.2'(abbreviated as W) seedlings grafted onto W, Cucurbita moschata ‘Quanneng Tiejia'(P1), ‘Kaijia No.1'(P2), and Lagenaria siceraria ‘Hanzhen No.3'(G), which were grown hydroponically. All the measured growth parameters were significantly inhibited by salt stress, with W/P2 exhibiting superior growth. Salt stress increased Na+uptake, which in turn disrupted K+/Na+homeostasis in all grafting combinations. The photosynthetic capacity, chlorophyll concentration, and photochemical efficiency of photosystem II of all grafts significantly decreased with salt stress. However, W grafted onto G and P2 showed higher resistance than W grafted onto W and P1. Electrolyte leakage(EL) superoxide dismutase, catalase, peroxidase, and ascorbate peroxidase activity were significantly affected by both grafting and salt stress. Minimum EL was observed in W/P2, which also exhibited superior antioxidative capacity. Physiological and biochemical assessment indicated that W grafted onto the P2 rootstock displayed the greatest salt tolerance. | 3 | Gender gap in acute coronary heart disease:Myth or reality?显示文摘AIM: To investigate potential gender differences in the prevalence of cardiovascular risk factors, cardiovascular disease (CVD) management, and prognosis in acute coronary syndrome (ACS). METHODS: A systematic literature search was performed through Medline using pre-specified key words . An additional search was performed, focusing specifically on randomized controlled clinical trials in relation to therapeutic intervention and prognosis. In total, 92 relevant articles were found. RESULTS: Women with CVD tended to have more hypertension and diabetes at the time of presentation, whereas men were more likely to smoke. Coronary angiography and revascularization by percutaneous coronary intervention were performed more often in men. Women were at a greater risk of short-term mortality and complications after revascularization. Interestingly, women under 40 years presenting with ACS were athighest risk of cardiovascular death compared with men of the same age, irrespective of risk factors. This disadvantage disappeared in older age. The long-term mortality risk of ACS was similar in men and women, and even in favor of women. | 6 | Comprehensive Mineral Nutrition Analysis of Watermelon Grafted onto Two Different Rootstocks显示文摘Grafting is a widely used practice in fruit-bearing vegetables. However, why grafting affects plant growth, fruit yield, and quality, especially from the aspect of mineral nutrition, remains unclear. In this study, watermelon cultivar ‘Zaojia 8424' was grafted onto bottle gourd ‘Jingxinzhen1'(Lagenaria siceraria) and pumpkin ‘Qingyanzhen 1'(Cucurbita maxima × C. moschata). Non-grafted plants were used as the control. Results show that rootstock grafting significantly increases plant growth and single fruit weight of watermelon. Watermelon grafted onto rootstocks, especially pumpkin, exhibits significantly higher root volume, root surface area, and number of root tips and forks in comparison with non-grafted plants. Fruit flesh, rind firmness, and rind thickness were enhanced by grafting. However, fruit soluble solids and taste significantly decreased in plants grafted onto pumpkin. The total uptake(mg · plant^(-1)) and concentration(mg · g^(-1)DW) of N, K, Ca, Fe, Mg, and Mn in root, stem, leaf,fruit rind, and flesh were generally higher in grafted plants compared to non-grafted ones, especially for N of pumpkin rootstock-grafted plants.The total uptake of nutrients of plants grafted onto bottle gourd and pumpkin was increased by 30.41% and 49.14% at fruit development stage and by 21.33% and 47.46% at fruit maturation stage, respectively, compared with non-grafted plants. | 9 | Applications of stem cells in orthodontics and dentofacial orthopedics:Current trends and future perspectives显示文摘A simple overview of daily orthodontic practice involves use of brackets, wires and elastomeric modules. However, investigating the underlying effect of orthodontic forces shows various molecular and cellular changes. Also, orthodontics is in close relation with dentofacial orthopedics which involves bone regeneration. In this review current and future applications of stem cells(SCs) in orthodontics and dentofacial orthopedics have been discussed. For craniofacial anomalies, SCs have been applied to regenerate hard tissue(such as treatment of alveolar cleft) and soft tissue(such as treatment of hemifacial macrosomia). Several attempts have been done to reconstruct impaired temporomandibular joint. Also, SCs with or without bone scaffolds and growth factors have been used to regenerate bone following distraction osteogenesis of mandibular bone or maxillary expansion. Current evidence shows that SCs also have potential to be used to regenerate infrabony alveolar defects and move the teeth into regenerated areas. Future application of SCs in orthodontics could involve accelerating tooth movement, regenerating resorbed roots and expanding tooth movement limitations. | 11 | Application of virtual histological intravascular ultrasound in plaque composition assessment of saphenous vein graft diseases显示文摘Objective:Saphenous vein grafts disease(SVGD)is a common complication after coronary artery bypass graft(CABG)and normally treated by percutaneous coronary intervention(PCI).The most common complication after SVG-PCI is slow or no-reflow.It is known that the no-reflow phenomenon occurs in up to 15%of the SVG-PCI and is associated with high risk of major adverse cardiac events(MACEs)and mortality,therefore,it is important to investigate the factors that could predict the clinical outcome of PCI for risk stratification and guiding interventions.In recent years,the spectral analysis of intravascular ultrasound(IVUS)radiofrequency data(virtual histology-IVUS[VH-IVUS])has been used to provide quantitative assessment on both plaque compositions and morphologic characteristics.Data sources:The PubMed,Embase,and Central databases were searched for possible relevant studies published from 1997 to 2018 using the following index keywords:“Coronary artery bypass grafting,”“Saphenous venous graft disease,”“Virtual histologyintravascular ultrasound,”“Virtual histology-intravascular ultrasound,”and“Percutaneous coronary intervention.”Study selection:The primary references were Chinese and English articles including original studies and literature reviews,were identified and reviewed to summarize the advances in the application of VH-IVUS techniques in situ vascular and venous graft vascular lesions.Results:With different plaque components exhibiting a defined spectrum,VH-IVUS can classify atherosclerotic plaque into four types:fibrous tissue(FT),fibro fatty(FF),necrotic core(NC),and dense calcium(DC).The radiofrequency signal is mathematically transformed into a color-coded representation,including lipid,fibrous tissue,calcification,and necrotic core.Several studies have demonstrated the independent relationship between VH-IVUS-defined plaque classification or plaque composition and MACEs,but a significant association between plaque components and no-reflow after PCI in acute coronary syndrome.In recent years,VH-IVUS are applied to assess the plaque composition of SVGD,based on the similarity of pathophysiological mechanisms between coronary artery disease(CAD)and SVGD,further studies with the larger sample size,the long-term follow-up,multicenter clinical trials may be warranted to investigate the relationship between plaque composition of saphenous vein graft(SVG)by VH-IVUS and clinical outcomes in patients with SVGD undergoing PCI.Conclusions:In degenerative SVG lesions,VH-IVUS found that plaque composition was associated with clinical features,future studies need to explore the relationship between VH-IVUS defined atherosclerotic plaque components and clinical outcomes in SVGD patients undergoing PCI,an innovative prediction tool of clinical outcomes can be created. | Jing Gao Yue-Ying Wang Yin Liu | 2019 | Chinese Medical Journal2019,,8: | 5 |
| 12 | Improvement in affinity and thermostability of a fully human antibody against interleukin-17A by yeast-display technology and CDR grafting显示文摘Monoclonal antibodies(m Abs) are widely used in many fields due to their high specificity and ability to recognize a broad range of antigens. IL-17 A can induce a rapid inflammatory response both alone and synergistically with other proinflammatory cytokines. Accumulating evidence suggests that therapeutic intervention of IL-17 A signaling offers an attractive treatment option for autoimmune diseases and cancer. Here, we present a combinatorial approach for optimizing the affinity and thermostability of a novel anti-h IL-17 A antibody. From a large na?ve phage-displayed library, we isolated the anti-IL-17 A m Ab 7 H9 that can neutralize the effects of recombinant human IL-17 A. However, the modest neutralization potency and poor thermostability limit its therapeutic applications. In vitro affinity optimization was then used to generate 8 D3 by using yeast-displayed random mutagenesis libraries.This resulted in four key amino acid changes and provided an approximately 15-fold potency increase in a cell-based neutralization assay. Complementarity-determining regions(CDRs) of 8 D3 were further grafted onto the stable framework of the hu Fv 4 D5 to improve thermostability. The resulting hybrid antibody9 NT/S has superior stabilization and affinities beyond its original antibody. Human fibrosarcoma cellbased assays and in vivo analyses in mice indicated that the anti-IL-17 A antibody 9 NT/S efficiently inhibited the secretion of IL-17 A-induced proinflammatory cytokines. | 13 | Use of demineralized bone matrix in the extremities显示文摘Autologous bone graft is considered as the gold standard for all indications for bone grafting procedures but the limited availability and complications in donor site resulted in seeking other options like allografts andbone graft substitutes. Demineralized bone matrix(DBM) is an allograft product with no quantity limitation. It is an osteoconductive material with osteoinductive capabilities, which vary among different products, depending on donor characteristics and differences in processing of the bone. The purpose of the present review is to provide a critical review of the existing literature concerning the use of DBM products in various procedures in the extremities. Clinical studies describing the use of DBM alone or in combination with other grafting material are available for only a few commercial products. The Level of Evidence of these studies and the resulting Grades of Recommendation are very low. | 14 | Impact of gout on in-hospital outcomes of acute coronary syndrome-related hospitalizations and revascularizations: Insights from the national inpatient sample显示文摘BACKGROUND Previous studies have established a role of gout in predicting risk and prognosis of cardiovascular diseases. However, large-scale data on the impact of gout on inpatient outcomes of acute coronary syndrome (ACS)-related hospitalizations and post-revascularization is inadequate. AIM To evaluate the impact of gout on in-hospital outcomes of ACS hospitalizations, subsequent healthcare burden and predictors of post-revascularization inpatient mortality. METHODS We used the national inpatient sample (2010-2014) to identify the ACS and goutrelated hospitalizations, relevant comorbidities, revascularization and postrevascularization outcomes using the ICD-9 CM codes. A multivariable analysis was performed to evaluate the predictors of post-revascularization in-hospital mortality. RESULTS We identified 3144744 ACS-related hospitalizations, of which 105198 (3.35%) also had gout. The ACS-gout cohort were more often older white males with a higher prevalence of comorbidities. Coronary artery bypass grafting was required more often in the ACS-gout cohort. Post-revascularization complications including cardiac (3.2% vs 2.9%), respiratory (3.5% vs 2.9%), and hemorrhage (3.1% vs 2.7%) were higher whereas all-cause mortality was lower (2.2% vs 3.0%) in the ACSgout cohort (P < 0.001). An older age (OR 15.63, CI: 5.51-44.39), non-elective admissions (OR 2.00, CI: 1.44-2.79), lower household income (OR 1.44, CI: 1.17- 1.78), and comorbid conditions predicted higher mortality in ACS-gout cohort undergoing revascularization (P < 0.001). Odds of post-revascularization inhospital mortality were lower in Hispanics (OR 0.45, CI: 0.31-0.67) and Asians (OR 0.65, CI: 0.45-0.94) as compared to white (P < 0.001). However, postoperative complications significantly raised mortality odds. Mean length of stay, transfer to other facilities, and hospital charges were higher in the ACS-gout cohort. | 17 | Preparation and Characterization of UV-absorbing PVDF Membranes via Pre-irradiation Induced Graft Polymerization显示文摘Herein, excellent UV-absorbing poly(vinylidene fluoride)(PVDF) membranes were fabricated through the pre-irradiation induced graft polymerization method. The PVDF chains irradiated with ^(60)Co γ-ray were modified with the polymerizable UV absorber 2-[2-hydroxy-5-[2-(methacryloyloxy)ethyl]phenyl]-2 H-benzotriazole(RUVA-93). The influences of irradiation dose and monomer concentration on the prepared PVDF-g-PRUVA-93 membranes were investigated, and the optimal condition was eventually obtained. The chemical structures of the films were studied by ~1H-NMR, FTIR, and XRD. UV light transmittance and DSC tests were used to characterize the UV-absorbing performance and thermal property of the PVDF films before and after modification. The results proved that the PRUVA-93 side chains were successfully incorporated into the PVDF main chains and the obtained PVDF-g-PRUVA-93 films possessed remarkable UV-absorbing property. The modified membrane made under the optimized experiment condition could completely block the UV light in the range of 200-387 nm. | 18 | A universal pipeline for mobile mRNA detection and insights into heterografting advantages under chilling stress显示文摘Heterografting has long been used to enhance the chilling tolerance of temperature-sensitive crops,including watermelon,whose mechanism is known to involve bidirectional long-distance mRNA movements.Despite several studies reporting on mobile mRNA(mb-mRNA)profiles in plants,accurate identification of mb-mRNAs is challenging owing to an array of technical problems.Here,we developed a bioinformatical pipeline that took most of the known technical concerns into consideration and is considered to be a universal tool for mb-mRNA detection in heterografts.By applying this pipeline to a commercial watermelon–bottle gourd heterografting system,we detected 130 and 1144 mb-mRNAs upwardly and 167 and 1051 mb-mRNAs downwardly transmitted under normal and chilling-stress conditions,respectively.Quantitative real-time PCR indicated a high accuracy rate(88.2%)of mb-mRNA prediction with our pipeline.We further revealed that the mobility of mRNAs was not associated with their abundance.Functional annotation and classification implied that scions may convey the stress signal to the rootstock,subsequently triggering energy metabolism reprogramming and abscisic acid-mediated stress responses by upward movement of effective mRNAs,ultimately leading to enhanced chilling tolerance.This study provides a universal tool for mb-mRNA detection in plant heterografting systems and novel insights into heterografting advantages under chilling stress. | Ying Wang Lingping Wang Nailin Xing Xiaohua Wu Xinyi Wu Baogen Wang Zhongfu Lu Pei Xu Ye Tao Guojing Li Yuhong Wang | 2020 | Horticulture Research2020,7,1: | 3 |
| 19 | Synthesis of Inorganic Silica Grafted Three-arm PLLA and Their Behaviors for PLA Matrix显示文摘The low melt strength and poor crystallization behavior severely limit the processing and application of poly(lactic acid)(PLA) as biodegradable film materials. In this work, three-arm poly(L-lactic acid)(3A-PLLA) grafted silica nanoparticles with two kinds of topology structures were introduced into PLA matrix to improve the biodegradation performance. Different methods were used to characterize the structure of the grafted 3A-PLLA chains, the grafting density, and the thermal decomposition behavior of the nanoparticles. By varying the mass ratios of raw materials and altering the order of dropping solution in the reaction, high grafting densitytangled 3A-PLLA grafted SiO_2 was synthesized as '3A-PLLA grafting to SiO_2'(GTS), while low grafting density-stretched 3A-PLLA grafted SiO_2 was obtained as 'SiO_2 grafting to 3A-PLLA'(GTA). Topology of nanoparticles as well as the filler-matrix interaction is critically important to structure bio-nanocomposites with desirable properties. Thus, the GTS and GTA nanoparticles were introduced into PLA matrix to assess the effect. The SEM images showed the uniform dispersion of the modified nanoparticles, while the shear rheology results revealed that GTA nanoparticles made a more significant contribution on the melt-strengthening and relaxation time-extension of PLA. Moreover, it is suggested that GTA nanoparticles were more effective to act as a nucleating agent for PLA, which was proved by differential scanning calorimetry(DSC) and polarized optical microscopy(POM) researches. | 20 | Contralateral C7 transfer combined with acellular nerve allografts seeded with differentiated adipose stem cells for repairing upper brachial plexus injury in rats显示文摘Nerve grafting has always been necessary when the contralateral C7 nerve root is transferred to treat brachial plexus injury. Acellular nerve allograft is a promising alternative for the treatment of nerve defects, and results were improved by grafts laden with differentiated adipose stem cells. However, use of these tissue-engineered nerve grafts has not been reported for the treatment of brachial plexus injury. The aim of the present study was to evaluate the outcome of acellular nerve allografts seeded with differentiated adipose stem cells to improve nerve regeneration in a rat model in which the contralateral C7 nerve was transferred to repair an upper brachial plexus injury. Differentiated adipose stem cells were obtained from Sprague-Dawley rats and transdifferentiated into a Schwann cell-like phenotype. Acellular nerve allografts were prepared from 15-mm bilateral sections of rat sciatic nerves. Rats were randomly divided into three groups: acellular nerve allograft, acellular nerve allograft + differentiated adipose stem cells, and autograft. The upper brachial plexus injury model was established by traction applied away from the intervertebral foramen with micro-hemostat forceps. Acellular nerve allografts with or without seeded cells were used to bridge the gap between the contralateral C7 nerve root and C5–6 nerve. Histological staining, electrophysiology, and neurological function tests were used to evaluate the effect of nerve repair 16 weeks after surgery. Results showed that the onset of discernible functional recovery occurred earlier in the autograft group first, followed by the acellular nerve allograft + differentiated adipose stem cells group, and then the acellular nerve allograft group;moreover, there was a significant difference between autograft and acellular nerve allograft groups. Compared with the acellular nerve allograft group, compound muscle action potential, motor conduction velocity, positivity for neurofilament and S100, diameter of regenerating axons, myelin sheath thickness, and density of myelinated fibers were remarkably increased in autograft and acellular nerve allograft + differentiated adipose stem cells groups. These findings confirm that acellular nerve allografts seeded with differentiated adipose stem cells effectively promoted nerve repair after brachial plexus injuries, and the effect was better than that of acellular nerve repair alone.
