The improved algorithm demonstrates better relationship between the data term and the confidence term for the optimization of the priority than the classical Criminisi algorithm. | 5 | Gestational diabetes: weight gain during pregnancy and its relationship to pregnancy outcomes显示文摘Background:Weight gain during pregnancy reflects the mother's nutritional status.However, it may be affected by nutritional therapy and exercise interventions used to control blood sugar in gestational diabetes mellitus (GDM).This study aimed to evaluate weight gain during gestation and pregnancy outcomes among women with GDM.Methods:A retrospective study involving 1523 women with GDM was conducted between July 2013 and July 2016.Demographic data, gestational weight gain (GWG), blood glucose, glycated-hemoglobin level, and maternal and fetal outcomes were extracted from medical records.Relationships between GWG and pregnancy outcomes were investigated using multivariate logistic regression.Results:In total, 451 (29.6%) women showed insufficient GWG and 484 (31.8%) showed excessive GWG.Excessive GWG was independently associated with macrosomia (adjusted odds ratio [aOR] 2.20, 95% confidence interval [CI] 1.50-3.52, P<0.001), large for gestational age (aOR 2.06, 95% CI 1.44-2.93, P<0.001), small for gestational age (aOR 0.49, 95% CI 0.25-0.97, P=0.040), neonatal hypoglycemia (aOR 3.80, 95% CI 1.20-12.00, P=0.023), preterm birth (aOR 0.45, 95% CI 0.21-0.96, P=0.040), and cesarean delivery (aOR 1.45, 95% CI 1.13-1.87, P=0.004).Insufficient GWG increased the incidence of preterm birth (aOR 3.53, 95% CI 1.96-6.37, P<0.001).Conclusions:Both excessive and insufficient weight gain require attention in women with GDM.Nutritional therapy and exercise interventions to control blood glucose should also be used to control reasonable weight gain during pregnancy to decrease adverse pregnancy outcomes. | Bao-Hua Gou Hui-Min Guan Yan-Xia Bi Bing-Jie Ding | 2019 | Chinese Medical Journal2019,,2: | 28 |

| 7 | Intestinal microbiota and type 2 diabetes:from mechanism insights to therapeutic perspective显示文摘The incidence of type 2 diabetes(T2DM)is rapidly increasing worldwide.However,the pathogenesis of T2DM has not yet been well explained.Recent evidence suggests that the intestinal microbiota composition is associated with obesity and T2DM.In this review,we provide an overview about the mechanisms underlying the role of intestinal microbiota in the pathogenesis of T2DM.There is clear evidence that the intestinal microbiota influences the host through its effect on body weight,bile acid metabolism,proinflammatory activity and insulin resistance,and modulation of gut hormones.Modulating gut microbiota with the use of probiotics,prebiotics,antibiotics,and fecal microbiota transplantation may have benefits for improvement in glucose metabolism and insulin resistance in the host.Further studies are required to increase our understanding of the complex interplay between intestinal microbiota and the host with T2DM.Further studies may be able to boost the development of new effective therapeutic approaches for T2DM. | Jun-Ling Han Hui-Ling Lin | 2014 | World Journal of Gastroenterology2014,20,47: | 26 |

| 10 | Beneficial effects of antidepressant mirtazapine in functional dyspepsia patients with weight loss显示文摘AIM: To explore the effects and mechanism of action of antidepressant mirtazapine in functional dyspepsia(FD) patients with weight loss.METHODS: Sixty depressive FD patients with weight loss were randomly divided into a mirtazapine group(MG), a paroxetine group(PG) or a conventional therapy group(CG) for an 8-wk clinical trial. Adverse effects and treatment response were recorded. The Nepean Dyspepsia Index-symptom(NDSI) checklist and the 17-item Hamilton Rating Scale of Depression(HAMD-17) were used to evaluate dyspepsia and depressive symptoms, respectively. The body composition analyzer was used to measure body weight and fat. Serum hormone levels were measured by ELISA.RESULTS:(1) After 2 wk of treatment, NDSI scores were significantly lower for the MG than for the PG and CG;(2) After 4 or 8 wk of treatment, HAMD-17 scores were significantly lower for the MG and PG than for the CG;(3) After 8 wk of treatment, patients in the MG experienced a weight gain of 3.58 ± 1.57 kg, which was significantly higher than that observed for patients in the PG and CG. Body fat increased by 2.77 ± 0.14kg, the body fat ratio rose by 4%, and the visceral fat area increased by 7.56 ± 2.25 cm2; and(4) For the MG, serum hormone levels of ghrelin, neuropeptide Y(NPY), motilin(MTL) and gastrin(GAS) were significantly upregulated; in contrast, those of leptin, 5-hydroxytryptamine(5-HT) and cholecystokinin(CCK) were significantly downregulated. CONCLUSION: Mirtazapine not only alleviates symptoms associated with dyspepsia and depression linked to FD in patients with weight loss but also significantly increases body weight(mainly the visceral fat in body fat). | 13 | Consequences of gestational and pregestational diabetes on placental function and birth weight显示文摘Maternal diabetes constitutes an unfavorable environment for embryonic and fetoplacental development. Despite current treatments, pregnant women with pregestational diabetes are at increased risk for congenital malformations, materno-fetal complications, placental abnormalities and intrauterine malprogramming. The complications during pregnancy concern the mother (gravidic hypertension and/or preeclampsia, cesarean section) and the fetus (macrosomia or intrauterine growth restriction, shoulder dystocia, hypoglycemia and respiratory distress). The fetoplacental impairment and intrauterine programming of diseases in the offspring's later life induced by gestational diabetes are similar to those induced by type 1 and type 2 diabetes mellitus. Despite the existence of several developmental and morphological differences in the placenta from rodents and women, there are similarities in the alterations induced by maternal diabetes in the placenta from diabetic patients and diabetic experimental models. From both human and rodent diabetic experimentalmodels, it has been suggested that the placenta is a compromised target that largely suffers the impact of maternal diabetes. Depending on the maternal metabolic and proin ammatory derangements, macrosomia is explained by an excessive availability of nutrients and an increase in fetal insulin release, a phenotype related to the programming of glucose intolerance. The degree of fetal damage and placental dysfunction and the availability and utilisation of fetal substrates can lead to the induction of macrosomia or intrauterine growth restriction. In maternal diabetes, both the maternal environment and the genetic background are important in the complex and multifactorial processes that induce damage to the embryo, the placenta, the fetus and the offspring. | 14 | POLYMER CHARGE AND MOLECULAR WEIGHT EFFECTS ON TREATED IRRIGATION FURROW PROCESSES显示文摘Application of 5-10 mg L-1 water soluble anionic polyacrylamide (PAM) to furrow irrigation water during flow advance substantially reduces sediment loss and increases net infiltration. We hypothesized that PAM_s solvated molecular conformation influences its irrigation-management efficacy. The study was conducted in Kimberly, Idaho, on Portneuf silt loam (Durinodic Xeric Haplocalcids), under furrowirrigated beans (Phaseolus vulgaris ) at a 1.5% slope. Polyacrylamides with contrasting molecular weight (anionic; 4-7, 12- 15 and 14-17 MDa, i.e. Mg mol-1 ), charge type (neutral, anionic, cationic), and charge density (8, 19, 35 mol %) were tested in two studies. Inflow rate was 23 L min-1 during furrow advance, and 15 L min-1 for the remaining set. Anionic and neutral PAMs were twice as effective as cationic PAMs for controlling sediment loss in new furrows. The order of effectiveness for overall soilloss control was : anionic >neutral >cationic PAM, and efficacy increased with increasing charge density and/or molecular weight. Net furrow infiltration increased by 14 to 19% when PAM treatment molecular weight was reduced from 17 to 4 MDa. General trends suggested that medium and high charge density anionic and neutral PAM produced the greatest increase in infiltration compared with controls.Compared with untreated furrows, neutral PAM gave the greatest season-long net infiltration gains (5%),while charged PAMs tended to increase net infiltration early in the season on new furrows but decreased infiltration on repeat-irrigated furrows later in the season. | R. | 15 | Finger vein recognition using weighted local binary pattern code based on a support vector machine显示文摘Finger vein recognition is a biometric technique which identifies individuals using their unique finger vein patterns. It is reported to have a high accuracy and rapid processing speed. In addition, it is impossible to steal a vein pattern located inside the finger. We propose a new identification method of finger vascular patterns using a weighted local binary pattern (LBP) and support vector machine (SVM). This research is novel in the following three ways. First, holistic codes are extracted through the LBP method without using a vein detection procedure. This reduces the processing time and the complexities in detecting finger vein patterns. Second, we classify the local areas from which the LBP codes are extracted into three categories based on the SVM classifier: local areas that include a large amount (LA), a medium amount (MA), and a small amount (SA) of vein patterns. Third, different weights are assigned to the extracted LBP code according to the local area type (LA, MA, and SA) from which the LBP codes were extracted. The optimal weights are determined empirically in terms of the accuracy of the finger vein recognition. | 16 | Impact of built environment on physical activity and obesity among children and adolescents in China:A narrative systematic review显示文摘Background: Neighborhood built environment may profoundly influence children's physical activity(PA) and body weight. This study systematically reviewed scientific evidence regarding the impact of built environment on PA and obesity among children and adolescents in China.Methods: A keyword and reference search was conducted in Active Living Research, Cochrane Library, PubMed, and Web of Science. Studies that met all of the following criteria were included in the review:(1) study designs—experimental studies, observational studies, and qualitative studies;(2) study subjects—Chinese children and/or adolescents aged 17 years;(3) exposures—neighborhood built environment;(4)outcomes—PA and/or body weight status;(5) article type—peer-reviewed publications;(6) time window of search—from the inception of an electronic bibliographic database to May 31, 2018;(7) country—China; and(8) language—articles written in English.Results: A total of 20 studies, including 16 cross-sectional studies, 3 longitudinal studies, and 1 descriptive study, met the predetermined selection criteria and were included in the review. A total of 13 studies adopted subjective built environment measures reported by parents and/or children, 2 adopted objective measures(e.g., geographical information system, field observations), and 5 adopted both objective and subjective measures. PA behaviors included PA, physical inactivity, sedentary behavior, active/passive commuting from/to school, and park visits. Among the 16 studies that provided some quantitative estimates of the influence of built environment on PA and body weight status, all reported a statistically significant relationship in the expected direction. Availability and accessibility in proximity to greenspaces, parks, recreational facilities,and sidewalks were found to be associated with increased PA levels, reduced sedentary behaviors, and/or active commuting among Chinese children and adolescents. In contrast, the absence of bike lanes and living in higher density residential areas were associated with increased likelihood of childhood overweight and obesity.Conclusion: Neighborhood built environment plays an important role in Chinese children's PA engagement and weight outcomes. Building new exercise facilities and enhancing the accessibility of existing facilities hold the potential to enhance PA engagement among Chinese children and adolescents. | 18 | Association between serum estradiol level on the human chorionic gonadotrophin administration day and clinical outcome显示文摘Background: Estradiol, as an important hormone in follicular development and endometrial receptivity, is closely related to clinical outcomes of fresh in vitro fertilization embryo transfer (IVF-ET) cycles. The aim of this retrospective study was to evaluate the association between elevated serum estradiol (E2) levels on the day of human chorionic gonadotrophin (hCG) administration and IVF-ET pregnancy and birth outcomes. Methods: A total of 1771 infertile patients with their first fresh IVF-ET cycles were analyzed retrospectively between January 2011 and January 2016 in Peking University First Hospital. Patients were categorized by serum E2 levels on the day of hCG administration into six groups: group 1 (serum E2 levels < 1000 pg/mL, n = 205), group 2 (serum E2 levels 1001-2000 pg/mL, n = 457), group 3 (serum E2 levels 2001-3000 pg/mL, n = 425), group 4 (serum E2 levels 3001-4000 pg/mL, n = 310), group 5 (serum E2 levels 4001- 5000 pg/mL, n = 237), and group 6 (serum E2 levels > 5000 pg/mL, n — 137). The retrieved oocyte and Mil oocyte numbers and implantation and clinical pregnancy rates of the groups were compared as the first objective of the study. For the 360 women with singleton births among all patients, the area under the corresponding receiver operating characteristic curve (ROC curve) was calculated to assess the predictive value of the E2 change for the probability of low birth weight (LBW) infants as the second objective. Results: The retrieved oocyte and Mil oocyte numbers and implantation and clinical pregnancy rates gradually increased from groups 1 to 5 but decreased in group 6. The parameters of group 1 were statistically worse than those of the other groups, from group 2 to group 6 (the number of retrieved oocytes, t = 13.096, t = 23.307, t = 23.086, t = 26.376, t = 19.636, P < 0.003;the number of retrieved Mil oocytes, t = 10.856, t = 20.868, t = 21.874, t = 23.374, t = 19.092, P < 0.003;the implantation rate,χ^2= 12.179,χ^2= 22.239,χ^2= 23.993,χ^2= 23.344,χ^2= 16.758, P < 0.003;the clinical pregnancy rate,χ^2= 16.415,χ^2= 28.074,χ^2= 35.387,χ^2= 37.025,χ^2= 24.590, P < 0.003).
