| 2 | Parenchyma-sparing pancreatectomies for benign or border-line tumors of the pancreas显示文摘Standard pancreatic resections, such as pancreaticoduodenectomy, distal pancreatectomy, or total pancreatectomy, result in an important loss of normal pancreatic parenchyma and may cause impairment of exocrine and endocrine function. Whilst these procedures are mandatory for malignant tumors, they seem to be too extensive for benign or border-line tumors, especially in patients with a long life expectancy. In recent years, there has been a growing interest in parenchyma-sparing pancreatic surgery with the aim of achieving better functional results without compromising oncological radicality in patients with benign, border-line or low-grade malignant tumors. Several limited resections have been introduced for isolated or multiple pancreatic lesions, depending on the location of the tumor: central pancreatectomy, duodenum-preserving pancreatic head resection with or without segmental duodenectomy, inferior head resection, dorsal pancreatectomy, excavation of the pancreatic head, middle-preserving pancreatectomy, and other multiple segmental resections. All these procedures are technically feasible in experienced hands,with very low mortality, although with high morbidity rate when compared to standard procedures. Pancreatic endocrine and exocrine function is better preserved with good quality of life in most of the patients, and tumor recurrence is uncommon. | 3 | 为十二指肠的胃肠的 stromal 肿瘤的有限切除术:外科的管理和临床的结果显示文摘 AIM:To analyze our experience in patients with duodenal gastrointestinal stromal tumors(GIST) and review the appropriate surgical approach.METHODS:We retrospectively reviewed the medical records of all patients with duodenal GIST surgically treated at our medical institution between 2002 and 2011.Patient files,operative reports,radiological charts and pathology were analyzed.For surgical therapy open and laparoscopic wedge resections and segmental resections were performed for limited resection(LR).For extended resection pancreatoduodenectomy was performed.Age,gender,clinical symptoms of the tumor,anatomical localization,tumor size,mitotic count,type of resection resectional status,neoadjuvant therapy,adjuvant therapy,risk classification and follow-up details were investigated in this retrospective study.RESULTS:Nine patients(5 males/4 females) with a median age of 58 years were surgically treated.The median follow-up period was 45 mo(range 6-111 mo).The initial symptom in 6 of 9 patients was gastrointestinal bleeding(67%).Tumors were found in all four parts of the duodenum,but were predominantly located in the first and second part of the duodenum with each 3 of 9 patients(33%).Two patients received neoadjuvant medical treatment with 400 mg imatinib per day for 12 wk before resection.In one patient,the GIST resection was done by pancreatoduodenectomy.The 8 LRs included a segmental resection of pars 4 of the duodenum,5 wedge resections with primary closure and a wedge resection with luminal closure by Roux-Y duodeno-jejunostomy.One of these LRs was done minimally invasive;seven were done in open fashion.The median diameter of the tumors was 54 mm(14-110 mm).Using the Fletcher classification scheme,3/9(33%) tumors had high risk,1/9(11%) had intermediate risk,4/9(44%) had low risk,and 1/9(11%) had very low risk for aggressive behaviour.Seven resections showed microscopically negative transsection margins(R0),two showed positive margins(R1).No patient developed local recurrence during follow-up.The one patient who underwent pancreatoduodenectomy died due to progressive disease with hepatic metastasis but without evidence of local recurrence.Another patient died in complete remission due to cardiac disease.Seven of the nine patients are alive disease-free.CONCLUSION:In patients with duodenal GIST,limited surgical resection with microscopically negative margins,but also with microscopically positive margins,lead to very good local and systemic disease-free survival. | Jens Hoeppner Birte Kulemann Goran Marjanovic Peter Bronsert Ulrich Theodor Hopt | 2013 | World Journal of Gastrointestinal Surgery2013，5，2： | 11 |

| 5 | Application of internal fixation of steel-wire limited loop in early Achilles tendon rupture显示文摘Objective:To explore the clinical effect and safety of internal fixation of steel-wire limited loop in early Achilles tendon rupture.Methods:Seventy-six patients respectively with early transected and avulsed types of Achilles tendon rupture were selected and treated with internal fixation of steel-wire limited loop.The patients began to take exercise for their lower limbs through continous passive motion as early as possible after surgical repair,and the loops were removed after 3-5 months.Six months later,the condition of complications including Achilles tendon re-rupture,wound fistula,wound infection and skin necrosis,cutaneous sensation in sural nerve dominance region,time back to preinjury work or learning as well as time to physical activities were observed.One year later,the therapeutic effect was evaluated,and the maximum circumferences of bilateral legs and raptured plane circumferences of Achilles tendon were measured.Results:The wound of all patients healed well,no complications like Achilles tendon re-rupture,wound fistula,wound infection and skin necrosis occured,and the cutaneous sensation in sural nerve dominance region was normal.The mean time back to preinjury work or learning as well as to pysical activities of all patients were respectively 10 and 22 weeks.Seventy out of 76 patients(92.1%) achieved an excellent effect,and 6(7.9%) good effect.The excellent and good rate came up to 100%.The maximum circumference in the affected leg decreased to 2 mm averagely compared with the offside,while the ruptured plane circumferences of Achilles tendon in the affected side increased to 2.2 mm compared with the offside.Conclusions:Kor early Achilles tendon rupture,internal fixation of steel-wire limited loop can recover the ankle function better,return to the preinjury state in the shortest lime,and has few complications. | Zhe Chen Jia-Sen Wei Zhao-Yang Hou Jiong Hu Yan-Guang Cao Qi-Xin Chen | 2013 | Asian Pacific Journal of Tropical Medicine2013，6，11： | 10 |

| 7 | Combined treatment of phacoemulsification and single-port limited pars plana vitrectomy in acute angle-closure glaucoma显示文摘AIM: To evaluate the efficacy of combined treatment of phacoemulsification(PE) and micro-incisional single-port transconjunctival limited pars plana vitrectomy(PPV) in acute angle-closure glaucoma(AACG).METHODS: A retrospective study included 26 patients who underwent PE diagnosed with AACG. Among them, 16 patients(16 eyes) underwent PE alone, 10 patients(10 eyes) underwent combined limited vitrectomy and PE. Then we compared intraocular pressure(IOP), anterior chamber angle, anterior chamber depth, central corneal thickness and corneal endothelial cell count before and after surgery, and effective PE time during cataract surgery.RESULTS: Effective PE time was shorter in the combined surgery group than in the single surgery group(P=0.040). There was no statistically significant difference in IOP and best-corrected visual acuity between the two groups postoperatively. At 6 mo postoperatively, there was no difference in the anterior chamber angle, anterior chamber depth, and central corneal thickness between two groups, but corneal endothelial cell count was higher in the combined surgery group than in the single surgery group(P=0.046). | 9 | Assessment of soil erosion in a tropical mountain river basin of the southern Western Ghats,India using RUSLE and GIS显示文摘Revised Universal Soil Loss Equation(RUSLE) model coupled with transport limited sediment delivery(TLSD) function was used to predict the longtime average annual soil loss, and to identify the critical erosion-/deposition-prone areas in a tropical mountain river basin, viz., Muthirapuzha River Basin(MRB; area=271.75 km^2), in the southern Western Ghats, India. Mean gross soil erosion in MRB is 14.36 t ha^(-1) yr^(-1), whereas mean net soil erosion(i.e., gross erosion-deposition) is only 3.60 t ha^(-1) yr^(-1)(i.e., roughly 25% of the gross erosion). Majority of the basin area(~86%) experiences only slight erosion(<5 t ha^(-1) yr^(-1)), and nearly 3% of the area functions as depositional environment for the eroded sediments(e.g., the terraces of stream reaches, the gentle plains as well as the foot slopes of the plateau scarps and the terrain with concordant summits). Although mean gross soil erosion rates in the natural vegetation belts are relatively higher, compared to agriculture, settlement/built-up areas and tea plantation, the sediment transport efficiency in agricultural areas and tea plantation is significantly high,reflecting the role of human activities on accelerated soil erosion. In MRB, on a mean basis, 0.42 t of soil organic carbon(SOC) content is being eroded per hectare annually, and SOC loss from the 4th order subbasins shows considerable differences, mainly due to the spatial variability in the gross soil erosion rates among the sub-basins. | 10 | Gastrointestinal stromal tumors of the duodenum:Surgical management and survival results显示文摘AIM:To provide long-term survival results of operable duodenal gastrointestinal stromal tumors(DGISTs)in a tertiary center in China.METHODS:In this retrospective study,the pathological data of 28 patients with DGISTs who had been treated surgically at the Second Department of General Surgery,Sir Run Run Shaw Hospital(SRRSH)from June1998 to December 2006 were reviewed.All pathological slides were examined by a single pathologist to confirm the diagnosis.In patients whose diagnosis was not confirmed by immunohistochemistry at the time of resection,representative paraffin blocks were reassembled,and sections were studied using antibodies against CD117(c-kit),CD34,smooth muscle actin(SMA),vimentin,S-100,actin(HHF35),and desmin.Operative procedures were classified as wedge resection(WR,local resection with pure closure,without duodenal transection or anastomosis),segmental resection[SR,duodenal transection with Roux-Y or BillrothⅡgastrojejunostomy(G-J),end-to-end duodenoduodenostomy(D-D),end-to-end or end-to-side duodenojejunostomy(D-J)],and pancreaticoduodenectomy(PD,Whipple operation with pancreatojejunostomy).R0 resection was pursued in all cases,and at least R1 resection was achieved.Regional lymphadenectomy was not performed.Clinical manifestations,surgery,medical treatment and follow-up data were retrospectively analyzed.Related studies in the literature were reviewed.RESULTS:There were 12 males and 16 females patients,with a median age of 53 years(20-76 years).Their major complaints were'gastrointestinal bleeding'(57.2%)and'nonspecific discomfort'(32.1%).About14.3%,60.7%,17.9%,and 7.1%of the tumors originated in the first to fourth portion,respectively,with a median size of 5.8 cm(1.6-20 cm).Treatment was by WR in 5 cases(17.9%),SR in 13 cases(46.4%),and by PD in 10 cases(35.7%).The morbidity and mortality rates were 35.7%and 3.6%,respectively.The median post-operative stay was 14.5 d(5-47 d).During a follow-up of 61(23-164)mo,the 2-year and 5-year relapse-free survival was 83.3%and 50%,respectively.Eighty-four related articles were reviewed.CONCLUSION:Surgeons can choose to perform limited resection or PD for operable DGISTs if clear surgical margins are achieved.Comprehensive treatment is necessary. | Xiao Liang Hong Yu Lin-Hua Zhu Xian-Fa Wang Xiu-Jun Cai | 2013 | World Journal of Gastroenterology2013，19，36： | 6 |
| 11 | Late Paleozoic sedimentation and tectonics of rift and limited ocean basin at southern margin of the Qinling显示文摘The rifting and occurrence of limited oceanic basin at southern margin of Qinling separated south Qinling from Yangtze Block in late Paleozoic. Detailed sedimentary studies were carried out at two localities at southern flank of this rift-limited oceanic basin zone. and depositional architectures were then established accordingly. The results show that tectono-sedimentary stories are differing in different positions but clearly demonstrate spatial development of the rift. It is obvious that the rift in west part of Mianlue underwent two developmentary phases, early rapid subsidence and late slow subsidence, and there occurred breakup unconformity between the seccessions of the two subsidence phases. Combined with the data of structural, geochemical and chronological studies on the ophiolitic complex within the Mianlue zone. it is demonstrated that the rifting-drifting transition occurred, leading to the development of limited oceanic basin during the late Paleazoic. | 12 | Seasonal dependency of controlling factors on the phytoplankton production in Taihu Lake, China显示文摘In this study, 44 profiles of gross primary productivity(GPP) and sunlight, along with water temperature, Chlorophyll-a(Chla) and nutrients, were observed in Meiliang Bay of Taihu Lake, China, in the spring, summer, and fall seasons. Effects of water temperature, light,and nutrient concentration were examined in relation to the GPP-unit-Chla(GPP of algae per Chla). The results showed that the optimum temperature for the GPP of phytoplankton was 27.9°C, the optimal PNA-unit-Chla(photon number absorbed by phytoplankton per Chla) was 0.25(mol), and the HSCN-unit-Chla and HSCP-unit-Chla(half-saturation constants of nitrogen and phosphorus of algae per Chla) were 0.005(mg/L) and 0.0004(mg/L), respectively. The seasonal dependency of the effect of different factors on the GPP was analyzed. Compared with temperature and nutrients, light was found to be the most important factor affecting the GPP during the three seasons. The effect of temperature and nutrients on the GPP of phytoplankton has obvious seasonal change. In spring, temperature was the secondary factor affecting the GPP of phytoplankton, and the effect of nutrients may be negligible in the eutrophic lake on account of temperature limit, which showed that the GPP of algae was only affected by the physical process. In summer and fall, temperature didn't affect the GPP of algae, and the presence of nutrients was the secondary factor affecting the GPP of phytoplankton. From the natural and attenuation evolution condition of the earth and terrestrial planets, through the limit condition got from the combination of the mathematics logic and the geological significance, the limited expanding earth evolution model is established as follows:in which R0= 4.651×106 m, A =1.86×106 m, β = -6.01?10-10 a and ts=3×10 a. From this model the maximum radius Rmax of the limited expanding earth evolution should be 6511 km. | 19 | Bilateral sequential femoral neck stress fractures in young adult with HIV infection on antiretroviral therapy: A case report显示文摘BACKGROUND Femoral neck stress fractures are rarely encountered among young adults and are often associated with either repetitive excessive loading or underlying bone pathology.Preliminary research has indicated human immunodeficiency virus(HIV)/antiretroviral therapy(ART)as predisposing agents to osteopenia and osteoporosis related complications.We report a case of HIV/ART induced insufficiency fracture in a resource limited setting in Central India.Our aim is to increase awareness and promote screening of HIV/ART related osteopenia and osteoporosis in order to prevent catastrophic orthopaedic complications.CASE SUMMARY A 35-year-old HIV positive male presented with a stress fracture of left femoral neck.The patient was on ART and reported no comorbidities.He went on to be successfully managed surgically.However,during work-up osteopenia of the contralateral proximal femur was recognised using Singh’s Index.Six months post-op the patient presented with right-sided femoral-neck stress fracture.At this stage the patient was nonconcordant with ART and denied surgical fixation.CONCLUSION In the absence of co-morbidities,several mechanisms of HIV/antiretroviral therapy may have played a role in predisposing our patient towards such a presentation.We recommend routine screening all HIV-infected patients for osteopenia,especially in younger individuals.In low resource settings and district hospitals,pelvis radiograph&Singh’s index can be used for screening. | Saisunder Shashank Chaganty Deeptiman James | 2019 | World Journal of Orthopedics2019，10，6： | 3 |
| 20 | Predicting the effects of sediment based internal nutrient loads on eutrophication in Kücükcekmece Lagoon for rehabilitation planning显示文摘In deep stratified coastal lagoons, hypoxic waters that result from phytoplankton decomposition in the stratified bottom waters are often associated with eutrophication. Decomposing biomass reaches the bottom sediments and enriches them with nutrients and organic matter. Nutrients trapped in sediments are released with time and promote excessive phytoplankton growth in the surface water. Because eutrophication in lentic ecosystems progresses in a self-fuelling cycle, outflow is the only available process for exporting excess nutrients to recover from eutrophication. Thus, rehabilitation of eutrophic coastal lagoons that have limited seawater interactions is a long term process. The importance of nutrient release from sediments on eutrophication and the delay effect of internal nutrient loading on the rehabilitation of a eutrophic coastal lagoon with limited seawater exchange were analysed in this study.An ecological model that couples the water column and the sediment diagenesis processes, was developed for water quality management purposes. Our findings indicate that the recovery of the Lagoon from eutrophication will be taken decades even in the absence of external nutrient loading. Therefore, we suggest applying rehabilitation strategies that control the nutrient fluxes from sediments for a faster recovery from heavily eutrophic conditions.
