1Overview of immunosuppression in liver transplantation显示文摘Continued advances in surgical techniques and immunosuppressive therapy have allowed liver transplantation to become an extremely successful treatment option for patients with end-stage liver disease.Beginning with the revolutionary discovery of cyclosporine in the 1970s,immunosuppressive regimens have evolved greatly and current statistics confirm one-year graft survival rates in excess of 80%. Immunosuppressive regimens include calcineurin inhibitors,anti-metabolites,mTOR inhibitors,steroids and antibody-based therapies.These agents target different sites in the T cell activation cascade,usually by inhibiting T cell activation or via T cell depletion.They are used as induction therapy in the immediate periand post-operative period,as long-term maintenance medications to preserve graft function and as salvage therapy for acute rejection in liver transplant recipients. 2Enhanced Antibacterial Activity of Bifunctional Fe_(3)O_(4)-Ag Core-Shell Nanostructures显示文摘We describe a simple one-pot thermal decomposition method for the production of a stable colloidal suspension of narrowly dispersed superparamagnetic Fe_(3)O_(4)-Ag core-shell nanostructures.These biocompatible nanostructures are highly toxic to microorganisms.Antimicrobial activity studies were carried out on both Gram negative(Escherichia coli and Proteus vulgaris)and Gram positive(Bacillus megaterium and Staphylococcus aureus)bacterial strains.Efforts have been made to understand the underlying molecular mechanism of such antibacterial actions.The effect of the core-shell nanostructures on Gram negative strains was found to be better than that observed for silver nanoparticles.The minimum inhibitory concentration(MIC)values of these nanostructures were found to be considerably lower than those of commercially available antibiotics.We attribute this enhanced antibacterial effect of the nanostructures to their stability as a colloid in the medium,which modulates the phosphotyrosine profile of the bacterial proteins and arrests bacterial growth.We also demonstrate that these core-shell nanostructures can be removed from the medium by means of an external magnetic field which provides a mechanism to prevent uncontrolled waste disposal of these potentially hazardous nanostructures.Bhupendra Chudasama Anjana K.Vala Nidhi Andhariya R.V.Upadhyay R.V.Mehta 2009Nano Research2009,2,12:17
3Bone mineral density and disorders of mineral metabolism in chronic liver disease显示文摘AIM:To estimate the prevalence and identify the risk factors for metabolic bone disease in patients with cirrhosis.METHODS:The study was performed on 72 Indian patients with cirrhosis(63 male, 9 female;aged < 50 years).Etiology of cirrhosis was alcoholism(n = 37), hepatitis B(n = 25) and hepatitis C(n = 10).Twenty-three patients belonged to Child class A, while 39 were in class B and 10 in class C.Secondary causes for metabolic bone disease and osteoporosis were ruled out.Sunlight exposure, physical activity and dietary constituents were calculated.Complete metabolic profiles were derived, and bone mineral density(BMD) was measured using dual energy X ray absorptiometry.Low BMD was defined as a Z score below-2.RESULTS:Low BMD was found in 68% of patients.Lumbar spine was the most frequently and severely affected site.Risk factors for low BMD included low physical activity, decreased sunlight exposure, and low lean body mass.Calcium intake was adequate, with unfavorable calcium:protein ratio and calcium:phosphorus ratio.Vitamin D deficiency was highly prevalent(92%).There was a high incidence of hypogonadism(41%).Serum estradiol level was elevated significantly in patients with normal BMD.Insulin-like growth factor(IGF) 1 and IGF binding protein 3 levels were below the age-related normal range in both groups.IGF-1 was signiflcantly lower in patients with low BMD.Serum osteocalcin level was low(68%) and urinary deoxypyridinoline to creatinine ratio was high(79%), which demonstrated low bone formation with high resorption.CONCLUSION:Patients with cirrhosis have low BMD.Contributory factors are reduced physical activity, low lean body mass, vitamin D def iciency and hypogonadism and low IGF-1 level.Joe George Hosahithlu K Ganesh Shrikrishna Acharya Tushar R Bandgar Vyankatesh Shivane Anjana Karvat Shobna J Bhatia Samir Shah Padmavathy S Menon Nalini Shah 2009World Journal of Gastroenterology2009,15,28:15
4Histone modifications dictate specific biological readouts显示文摘The basic unit of chromatin is the nucleosomal core particle, containing 147 bp of DNA that wraps twice around an octamer of core histones.The core histones bear a highly dynamic N-terminal amino acid tail around 20?35 residues in length and rich in basic amino acids.These tails extending from the surface of nucleosome play an important role in folding of nucleosomal arrays into higher order chromatin structure, which plays an important role in eukaryotic gene regulation.The amino terminal tails protruding from the nuclesomes get modified by the addition of small groups such as methyl, acetyl and phosphoryl groups.In this review, we focus on these complex modification patterns and their biological functions.Moreover, these modifications seem to be part of a complex scheme where distinct histone modifications act in a sequential manner or in combination to form a'histone code'read by other proteins to control the structure and/or function of the chromatin fiber.Errors in this histone code may be involved in many human diseases especially cancer, the nature of which could be therapeutically exploited.Increasing evidence suggests that many proteins bear multiple, distinct modifications, and the ability of one modification to antagonize or synergize the deposition of another can have significant biological consequences.Anjana Munshi Gowhar Shafi Nishat Aliya Akka Jyothy 2009Journal of Genetics and Genomics2009,36,2:10
5Acute portal vein thrombosis after liver transplant presenting with subtle ultrasound abnormalities:A case report and literature review显示文摘BACKGROUND Portal vein thrombosis(PVT) after liver transplantation(LT) is an uncommon complication with potential for significant morbidity and mortality that transplant providers should be cognizant of.Recognizing subtle changes in postoperative ultrasounds that could herald but do not definitively diagnose PVT is paramount.CASE SUMMARY A 30-year-old female with a history of alcohol-related cirrhosis presented with painless jaundice and received a deceased donor orthotopic liver transplant.On the first two days post-operatively,her liver Doppler ultrasounds showed a patent portal vein,increased hepatic arterial diastolic flows,and reduced hepatic arterial resistive indices.She was asymptomatic with improving labs.On postoperative day three,her resistive indices declined further,and computed tomography of the abdomen revealed a large extra-hepatic PVT.The patient then underwent emergent percutaneous venography with tissue plasminogen activator administration,angioplasty,and stent placement.Aspirin was started to prevent stent thrombosis.Follow-up ultrasounds showed a patent portal vein and improved hepatic arterial resistive indices.Her graft function improved to normal by discharge.Although decreased hepatic artery resistive indices and increased diastolic flows on ultrasound are often associated with hepatic arterial stenosis post-LT,PVT can also cause these findings.CONCLUSION Reduced hepatic arterial resistive indices on ultrasound can signify PVT post-LT,and thrombolysis,angioplasty,and stent placement are efficacious treatments.Thomas Couri Carla Harmath Talia Baker Anjana Pillai 2019World Journal of Hepatology2019,11,2:8
6Hierarchical highly ordered SnO_(2) nanobowl branched ZnO nanowires for ultrasensitive and selective hydrogen sulfide gas sensing显示文摘Highly sensitive and selective hydrogen sulfide(H_(2)S)sensors based on hierarchical highly ordered SnO_(2) nanobowl branched ZnO nanowires(NWs)were synthesized via a sequential process combining hard template processing,atomic-layer deposition,and hydrothermal processing.The hierarchical sensing materials were prepared in situ on microelectromechanical systems,which are expected to achieve high-performance gas sensors with superior sensitivity,long-term stability and repeatability,as well as low power consumption.Specifically,the hierarchical nanobowl SnO_(2)@ZnO NW sensor displayed a high sensitivity of 6.24,a fast response and recovery speed(i.e.,14 s and 39 s,respectively),and an excellent selectivity when detecting 1 ppm H_(2)S at 250°C,whose rate of resistance change(i.e.,5.24)is 2.6 times higher than that of the pristine SnO_(2) nanobowl sensor.The improved sensing performance could be attributed to the increased specific surface area,the formation of heterojunctions and homojunctions,as well as the additional reaction between ZnO and H_(2)S,which were confirmed by electrochemical characterization and band alignment analysis.Moreover,the well-structured hierarchical sensors maintained stable performance after a month,suggesting excellent stability and repeatability.In summary,such well-designed hierarchical highly ordered nanobowl SnO_(2)@ZnO NW gas sensors demonstrate favorable potential for enhanced sensitive and selective H_(2)S detection with long-term stability and repeatability.Li-Yuan Zhu Kai-Ping Yuan Jia-He Yang Cheng-Zhou Hang Hong-Ping Ma Xin-Ming Ji Anjana Devi Hong-Liang Lu David Wei Zhang 2020Microsystems & Nanoengineering2020,6,1:7
7Genetics of Nonalcoholic Fatty Liver Disease:An Overview显示文摘Nonalcoholic fatty liver disease(NAFLD) is the most common liver disease in the world today.Its incidence in adults and children is rising rapidly due to the ongoing epidemics of obesity and type 2 diabetes.Hence,it has become a global public health issue.Environmental factors have been found to play a major role in the etiology of NAFLD,especially for genetically susceptible populations. 10Comparative effectiveness of Biobrane^(■),RECELL^(■)Autologous skin Cell suspension and Silver dressings in partial thickness paediatric burns:BRACS randomised trial protocol显示文摘Background:Mixed partial thickness burns are the most common depth of burn injury managed at a large Australian paediatric hospital specialty burns unit.Prolonged time until re-epithelialisation is associated with increased burn depth and scar formation.Whilst current wound management approaches have benefits such as anti-microbial cover,these are not without inherent limitations including multiple dressing changes.The Biobrane^(■)RECELL^(■)Autologous skin Cell suspension and Silver dressings(BRACS)trial aims to identify the most effective wound management approach for mixed partial thickness injuries in children.Methods:All children presenting with an acute burn injury to the study site will be screened for eligibility.This is a single-centre,three-arm,parallel group,randomised trial.Children younger than 16 years,with burns≥5%total body surface area involving any anatomical location,up to 48 h after the burn injury,and of a superficial partial to mid-dermal depth,will be included.A sample size of 84 participants will be randomised to standard silver dressing or a Regenerative Epithelial Suspension(RES^(TM))with Biobrane^(■)or Biobrane^(■)alone.The first dressing will be applied under general anaesthesia and subsequent dressings will be changed every 3 to 5 days until the wound is≥95%re-epithelialised,with re-epithelialisation time the primary outcome.Secondary outcomes of acute pain,acute itch,scar severity,health-related quality of life,treatment satisfaction,dressing application ease and healthcare resource use will be assessed at each dressing change and 3,6 and 12 months post-burn injury.Discussion:The findings of this study can potentially change the wound management approach for superficial partial to mid-dermal burns in children locally and worldwide.Trial registration:The Australian New Zealand Clinical Trials Registry(ACTRN12618000245291)approved prospective registration on 15 February 2018.Registration details can be viewed at https://www.anzctr.org.au/Trial/Registration/TrialReview.aspx?id=374272&isReview=true.Anjana Bairagi Bronwyn Griffin Zephanie Tyack Dimitrios Vagenas Steven M.McPhail Roy Kimble 2019Burns & Trauma2019,7,1:3

12Obstructive jaundice due to a rare periampullary tumor显示文摘Gangliocytic paraganglioma is a rare neuroendocrine tumor predominantly arising in the second part of the duodenum with rare local recurrence or metastasis to regional lymph nodes.A 92-year-old female presented with obstructive jaundice.On exam she had pale conjunctiva and icteric sclera.Abdominal examination revealed tenderness in the upper abdomen.Laboratory data was consistent with obstructive jaundice.Computed tomography of the abdomen revealed a dilated gall bladder and a common bile duct(CBD)with no evidence of liver lesions or pancreatic head mass.Endoscopic ultrasonography revealed a 1 cm isoechoic submucosal nodule at the periampullary area,dilated CBD(9 mm),a prominent pancreatic duct(4.1 mm)and a hydropic gall bladder with no stones.Endoscopic retrograde cholangiopancreaticography was performed to relieve obstruction and showed a 1 cm periampullary mass which underwent an en-bloc snare resection.Histopathology analyses with immunohistochemical stains were positive for cytokeratin,synaptophysin,S-100 protein,neuron specific enolase and negative for actin and desmin consistent with periampullary gangliocytic paraganglioma.Periampullary gangliocytic paraganglioma is a rare benign tumor of the small bowel.Common presentation includes abdominal pain and obstructive jaundice which should be included in differential diagnosis of obstructive jaundice.Endoscopic resection is a curative therapy in the absence of local invasion or distant metastasis.Anjana Sathyamurthy Abhishek Choudhary Dennis Ng Shuaib Okponobi Alberto Diaz-Arias Ajitinder Grewal Ghassan M Hammoud 2013World Journal of Gastrointestinal Oncology2013,5,10:2

14Effect of Ocimum sanctum on the development of protective immunity against Salmonella typhimurium infection through cytokines显示文摘Objective:To investigate the protective role of Ocimum sanctum(O.sanctum) leaves against Salmonella typhim.urium(S.typhimurium) infection in rats by inducing TNF-α,IFN-γ& IL-2 cytokines.Methods:Wistar albino rats were fed with aqueous extract of 0.sanctum leaves using 250 mg/kg body weight dose once a day for 20 consecutive days.Control rats were fed with placebo.Rats were infected with LD50 dose of 5.typhimurium infection and monitored for their survival.Bacterial blood burden in both the groups was compared and numbers of activated peritoneal macrophages were counted.Concentration of TNF-α,IFN-γand IL-2 cytokines in serum during different time intervals was assayed by sandwich ELISA.Results:Rats of control group showed a high mortality rate and had higher bacterial blood burden when compared with O.sanctum extract fed rats.There was a significant increase in the number of S.typhimurium engulfed peritoneal macrophages in the peritoneal fluid of O.sanctum fed animals.The protective control against bacterial infection in O,sanctum fed rats was associated with elevated level of TNF-α,IFN-γand IL-2 cytokines in serum.Conclusions:These findings suggest that orally administered O.sanctum leaves extract effectively enhanced activation in macrophage and lymphocytes,depicted by the elevated serum concentration of TNF-α,IFN-γand IL-2 cytokines,leading to induce a protective resistance against Salmonella typhimurium infection.Anjana Goel Sandeep Kumar Ashok Kumar Bhatia 2010Asian Pacific Journal of Tropical Medicine2010,3,9:2
