18对晚期慢性心力衰竭患者行心脏再同步化治疗致NYHA分级改善的临床预测因素和时机:多中心InSync随机临床评价(MIRACLE)和多中心InSyncICD随机临床评价(MIRACLE-ICD)试验的结果显示文摘Background: Based on current patient selection criteria, a significant proportion of recipients of cardiac resynchronization therapy(CRT) do not respond to treatment. The purpose of this analysis is to identify predictors and characterize the timing of response to CRT in patients with advanced heart failure. Methods: Patients randomized to receive CRT in the MIRACLE and MIRACLE-ICD trials, designed to assess the benefit of CRT compared with standard medical therapy in patients with advanced heart failure, left ventricular ejection fraction< 0.35, and QRS ≥130 milliseconds, were included for this analysis. Patients with an improvement of ≥1 New York Heart Association(NYHA)class from baseline to the 6-month follow-up were considered responders and those who had no change or worse NYHA class or died were classified as nonresponders. Responders were subdivided into early(within 1-3 months) and late(6 months). Results: One hundred forty-three(64%) of 224 and 190(61%) of 313 patients in the MIRACLE and MIRACLEICD trials, respectively, responded to therapy, with 81(57%) of 143 and 100(53%) of 190 responding early. Several but differing factors predicted CRT response and timing in the two trials with a high sensitivity(89%-90%) but, owing to a low specificity(31%-49%), a modest predictive accuracy(66%-75%). Conclusions: Based on improvement of ≥1 NYHA class, less than two thirds of patients enrolled in the MIRACLE or MIRACLE-ICD trials responded to CRT, with just more than half responding within the first month. Several factors predicted CRT response and timing, but given their modest predictive accuracy, comparable for both studies, additional methods for selecting candidates most likely to benefit from CRT are needed.Pires L.A. 19坚持以循证医学为基础的指导方针制定非甾体类抗炎药处方的全国性状况显示文摘Background &Aims: Our objective was to assess adherence to evidence-based guidelines by providers of the Department of Veterans Affairs nationwide. Methods: This was a cross-sectional study among veterans prescribed a nonsteroidal anti-inflammatory drug (NSAID) from January 1, 2002, to December 31, 2002. Prescrip tion data were linked to inpatient and outpatient medical records and death file s. The population was characterized as high risk based on the following: age 65 years or older, concurrent corticosteroid or anticoagulant use, history of pepti c ulcer, and high average daily dose of NSAIDs. Adherence was defined as the pre scription of a traditional NSAID with gastroprotection or a coxib in high-risk NSAID users. Univariate and multivariate analyses assessed the potential predict ors of adherence. Results: Three hundred three thousand seven hundred eighty-se ven met our definition of high risk. Most (97.3%) were male; 55.6%were white, 9.6%black, and 34.8%of other/unknown race. Age 65 years or older was the large st high-risk subset (87.1%). Overall, only 27.2%of highrisk veterans (n = 82, 766) were prescribed an adherent strategy. Among veterans with at least 2 risk f actors, adherence was 39.7%; among those with 3 risk factors, adherence was 41. 8%. Predictors of adherence included history of upper gastrointestinal events, anticoagulant use, rheumatologic disease, high Deyo comorbidity index score, use of low-dose salicylates, and concurrent corticosteroid use. Predictors of nona dherence included prescriptions≥90 days and high average daily dose of NSAIDs. Conclusions: Adherence to evidence-based guidelines for safe prescription of NS AIDs in the Department of Veterans Affairs is low (27.2%). 20Assessment of Knowledge,Attitude,and Practice in Forensic Dental Anatomy and Histology among Dental Practitioners显示文摘Dental anatomy and histology is one of the fundamental subjects and a key to comprehend the act of dentistry.Dental features such as tooth morphology and variations in shape and size play a vital role in the area of forensic odontology.Forensic odontology is a challenging field that involves the application of such knowledge of dental sciences in the identification of deceased individuals.The aim and objective was to assess and evaluate the knowledge,attitude,and practice regarding dental anatomy and histology in forensic investigation among dental practitioners.A cross-sectional survey was carried out among 217 dental practitioners from Maharashtra who in their day-to-day life may experience instances of forensic interest.The information was collected through telephonic interview by means of a pretested,close-ended questionnaire.In our stud%the general awareness about forensic odontology was found to be more among MDS dental practitioners than BDS dental practitioners.Overall,there is a lack of adequate legal knowledge and forensic practices among the dentists of Mumbai and Nagpur.However,the practitioners did have a positive attitude toward the field and were keen to learn more about it.The study is the first of its kind to be conducted in Maharashtra state of India regarding dental anatomy and histology with respect to forensic odontology.The dental practitioners were also aware about the dental anatomy and dental histology with respect to forensic investigation.Supriya Soni Payal Shirpure Ujwal Shahu Abraham Johnson 2020Journal of Forensic Science and Medicine2020,6,3:0
