2椎体间融合器的单节段植入对腰椎多节段椎弓根固定装置中螺钉应力的影响显示文摘目的通过体外生物力学实验评价椎体间融合器(cage)的单节段植入对腰椎多节段固定装置中椎弓根钉应力的影响。方法10具牛新鲜尸体腰骶椎标本L2-S1用于实验。经后方行L3-L4、L4-L5和L5-L6椎板和椎间盘部分切除,用椎弓根钉和一对碳纤维cage行以下3种三节段固定:①无cage植入;②L4-L5cage植入;③L5-L6cage植入。每组分别行载荷量为600N的轴向压缩试验,记录各节段椎弓根钉的折曲应变量,并加以比较。结果在无cage植入组中,L3和L6椎弓根钉的折曲应变量显著高于L4和L5;在L4-L5cage植入组中,L4和L5椎弓根钉的折曲应变量显著低于L3和L6;在L5-L6cage植入组中,L5和L6椎弓根钉的折曲应变量显著低于L3和L4。结论多节段椎弓根固定结构可导致上下两端椎弓根钉的应力增大;Cage的植入可降低上下相邻椎弓根钉的应力,但使相邻以远的椎弓根钉的应力增大。于滨生 刘少喻 ABUMI Kuniyoshi 李佛保 梁春祥 陈柏龄 龙厚清 2006中山大学学报(医学科学版)2006,27,1:6
3腰椎弓根钉固定后两种融合术的生物力学比较显示文摘【目的】对侧后方融合与经后路椎体间融合进行生物力学比较,探讨腰椎重建术式的选择方法。【方法】10具小牛尸体腰椎标本(L3-L6)用于实验。从L4-L5后方破坏腰椎稳定结构,并用椎弓根钉固定(PSF)和椎体间cage行以下重建:①完整脊柱+PSF,②两侧关节突关节内侧切除+PSF,③两侧关节突关节全切+PSF,④椎间盘部分切除+PSF,⑤椎间盘部分切除+PSF/cage。各组均行载荷量为5N·m的前-后屈曲测试,记录固定椎间(L4-L5)的刚度值及L4螺钉的应变量,并比较。【结果】除椎间盘部分切除+PSF外所有重建组的脊柱刚度均显著大于完整脊柱组,椎间盘部分切除+PSF/cage组在所有的固定组中获得最大的脊柱刚度。椎间盘部分切除+PSF组在所有的重建组中螺钉的应变量最大,而椎间盘部分切除+PSF/cage组的螺钉的应变量在所有重建组中最小。【结论】若脊柱前方的承载能力尚存,使用PSF的侧后方融合可获得充分的力学稳定性。当脊柱前方丧失其承载能力,单纯PSF的脊柱刚度低,螺钉的应力显著增大。对此种病例,cage的使用可增加重建脊柱的初期稳定性,有利于降低后方固定器械的应力。于滨生 刘少喻 李佛保 ABUMI Kuniyoshi 陈柏龄 龙厚清 2005中山大学学报(医学科学版)2005,26,5:3
