Pyogenic liver abscess: Changing patterns in approach显示文摘AIM: To define optimum management of the pyogenic liver abscess and assess new trends in treatment. METHODS: One hundred and sixty nine patients with pyogenic liver abscess managed at Sher-i-Kashmir In- stitute of Medical Sciences, Srinagar, Kashmir (India) from July 2001 to August 2006 were studied to evaluate and define the optimum treatment. RESULTS: Mortality in the surgically treated group of patients was 9.4% (12/119), while those treated non-surgically had a fatality rate of 16.66% (7/42). Multiple liver abscesses treated surgically had a surprisingly low mortality of 30%. The biliary tract (64.97%) was the most common cause of liver abscess. Multiple abscesses, mixed organisms and abscess complications are all associated with a significantly increased mortality. However, the lethality of the primary disease process was the most important factor in determining survival.CONCLUSION: Transperitoneal surgical drainage and antibiotics are the mainstay of treatment. Percutaneous drainage is recommended for high risk patients only. | Ajaz A Malik Shams UL Bari Khawaja Abdul Rouf Khurshid Alam Wani | 2010 | World Journal of Gastrointestinal Surgery2010,2,12: | 15
Cytokine- and chemokine-induced inflammatory colorectal tumor microenvironment: Emerging avenue for targeted therapy显示文摘Colorectal cancer(CRC)is a predominant life-threatening cancer, with liver and peritoneal metastases as the primary causes of death.Intestinal inflammation,a known CRC risk factor, nurtures a local inflammatory environment enriched with tumor cells, endothelial cells, immune cells,cancer-associated fibroblasts, immunosuppressive cells, and secretory growth factors. The complex interactions of aberrantly expressed cytokines, chemokines, growth factors, and matrix-remodeling enzymes promote CRC pathogenesis and evoke systemic responses that affect disease outcomes.Mounting evidence suggests that these cytokines and chemokines play a role in the progression of CRC through immunosuppression and modulation of the tumor microenvironment, which is partly achieved by the recruitment of immunosuppressive cells. These cells impart features such as cancer stem cell-like properties, drug resistance, invasion, and formation of the premetastatic niche in distant organs, promoting metastasis and aggressive CRC growth.A deeper understanding of the cytokineand chemokine-mediated signaling networks that link tumor progression and metastasis will provide insights into the mechanistic details of disease aggressiveness and facilitate the development of novel therapeutics for CRC.Here,we summarized the current knowledge of cytokine-and chemokine-mediated crosstalk in the inflammatory tumor microenvironment, which drives immunosuppression, resistance to therapeutics, and metastasis during CRC progression. We also outlined the potential of this crosstalk as a novel therapeutic target for CRC.The major cytokine/chemokine pathways involved in cancer immunotherapy are also discussed in this review. | Ajaz A.Bhat Sabah Nisar Mayank Singh Bazella Ashraf Tariq Masoodi Chandra P.Prasad Atul Sharma Selma Maacha Thasni Karedath Sheema Hashem Syed Besina Yasin Puneet Bagga Ravinder Reddy Michael P.Frennaux Shahab Uddin Punita Dhawan Mohammad Haris Muzafar A.Macha | 2022 | Cancer Communications2022,42,8: | 8
在良性的胆汁的阻塞的 choledochoduodenostomy 的长期的结果显示文摘 AIM:To determine the long-term results of choledochodudenostomy in patients with benign billiary obstruction.METHODS:This prospective study was conducted at Sheri Kashmir Institute of Medical Sciences Srinagar Kashmir,India over a period of 10 years from January 1997 to December 2007.The total number of patients who underwent choledochoduodenostomy during this period was 270.On the basis of etiology of biliary tract obstruction, patients were divided into a calculus group,an oriental cholangiohepatitis group,a benign biliary stricture group and others.Patients were followed for a variable period of 13 mo to 15 years.RESULTS:Choledochoduodenostomy(CDD) with duodenotomy was performend in four patients.CDD with removal of T-tube,CDD with left hepatic lobectomy and CDD with removal of intra biliary ruptured hydatid was performed in three patients each.In the remaining patients only CDD was performed.Immediate post operative complications were seen in 63(23%) patients, while long-term complications were seen in 28(11%) patients, which were statistically significant.Three patients died during hospitalization while four patients died in the late post-operative period.CONCLUSION:Our conclusion is that CDD is safe and produces good long term results when a permanent biliary drainage procedure is required. | Ajaz A Malik Shiraz A Rather Shams UL Bari Khursheed Alam Wani | 2012 | World Journal of Gastrointestinal Surgery2012,4,2: | 7
Portal biliopathy显示文摘Portal biliopathy refers to cholangiographic abnormalities which occur in patients with portal cavernoma. These changes occur as a result of pressure on bile ducts from bridging tortuous paracholedochal, epicholedochal and cholecystic veins. Bile duct ischemia may occur due prolonged venous pressure effect or result from insufficient blood supply. In addition, encasement of ducts may occur due fibrotic cavernoma. Majority of patients are asymptomatic. Portal biliopathy is a progressive disease and patients who have long standing disease and more severe bile duct abnormalities present with recurrent episodes of biliary pain, cholangitis and cholestasis. Serum chemistry, ultrasound with color Doppler imaging, magnetic resonance imaging with magnetic resonance cholangiopancreatography and magnetic resonance portovenography are modalities of choice for evaluation of portal biliopathy. Endoscopic retrograde cholangiography being an invasive procedure is indicated for endotherapy only. Management of portal biliopathy is done in a stepwise manner. First, endotherapy is done for dilation of biliary strictures, placement of biliary stents to facilitate drainage and removal of bile duct calculi. Next portal venous pressure is reduced by formation of surgical porto-systemic shunt or transjugular intrahepatic portosystemic shunt. This causes significant resolution of biliary changes. Patients who persist with biliary symptoms and bile duct changes may benefit from surgical biliary drainage procedures(hepaticojejunostomy or choledechoduodenostomy). | mohammad s khuroo ajaz a rather naira s khuroo mehnaaz s khuroo | 2016 | World Journal of Gastroenterology2016,22,35: | 6
Surgical management of complicated hydatid cysts of the liver显示文摘AIM: To review the clinical presentation and surgical management of complicated hydatid cysts of the liver and to assess whether conservative surgery is adequate in the management of complicated hydatid cysts of liver.METHODS:The study was carried out at Sher-i-Kashmir Institute of Medical Science,Srinagar,Kashmir,India.Sixty nine patients with hydatid disease of the liver were surgically managed from April 2004 to October 2005 with a follow up period of three years. It included 27 men and 42 women with a median age of 35 years. An abdominal ultrasound, computed tomography and serology established diagnosis.Patients with jaundice and high suspicion of intrabiliary rupture were subjected to preoperative endoscopic retrograde cholangiography.Cysts with infection, rupture into the biliary tract and peritoneal cavity were categorized as complicated cysts.Eighteen patients(26%)had complicated cysts and formed the basis for this study.RESULTS:Common complications were infection(14%),intrabiliary rupture(9%)and intraperitoneal rupture(3%).All the patients with infected cysts presented with pain and fever. All the patients with intrabiliary rupture had jaundice, while only four with intrabiliary rupture had pain and only two had fever.Surgical procedures performed in complicated cysts were:infection-omentoplasty in three and external drainage in seven;intrabiliary rupture-omentoplasty in two and internal drainage in four patients. Two patients with intraperitoneal rupture underwent external drainage. There was no mortality. The postoperative morbidity was 50%in complicated cysts and 16%in uncomplicated cysts.CONCLUSION:Complicated hydatid cyst of the liver can be successfully managed surgically with good long term results. | Ajaz A Malik Shams UL Bari Ruquia Amin Masooda Jan | 2010 | World Journal of Gastrointestinal Surgery2010,2,3: | 4
Hepatobiliary and pancreatic ascariasis显示文摘Hepatobiliary and pancreatic ascariasis(HPA) was described as a clinical entity from Kashmir,India in 1985. HPA is caused by invasion and migration of nematode,Ascaris lumbricoides,in to the biliary tract and pancreatic duct. Patients present with biliary colic, cholangitis, cholecystitis, hepatic abscesses and acute pancreatitis. Ascarides traverse the ducts repeatedly, get trapped and die, leading to formation of hepatolithiasis. HPA is ubiquitous in endemic regions and in Kashmir, one such region,HPA is the etiological factor for 36.7%,23%,14.5% and 12.5% of all biliary diseases, acute pancreatitis, liver abscesses and biliary lithiasis respectively. Ultrasonography is an excellent diagnostic tool in visualizing worms in gut lumen and ductal system. The rational treatment for HPA is to give appropriate treatment for clinical syndromes along with effective anthelmintic therapy. Endotherapy in HPA is indicated if patients continue to have symptoms on medical therapy or when worms do not move out of ductal lumen by 3 wk or die within the ducts. The worms can be removed from the ductal system in most of the patients and such patients get regression of symptoms of hepatobiliary and pancreatic disease. | Mohammad S Khuroo Ajaz A Rather Naira S Khuroo Mehnaaz S Khuroo | 2016 | World Journal of Gastroenterology2016,22,33: | 4

GC/MS-based differential metabolic profiling of human peptic ulcer disease to study Helicobacter pylori-induced metabolic perturbations显示文摘Helicobacter pylori infection has been significantly linked to Peptic Ulcer Disease and Gastric Cancer.Metabolomic fingerprinting may offer a principal way of early diagnosis and to understand the molecular mechanism of H.pylori-induced pathogenicity. The rationale of the study is to explore the underlying distinct metabolic mechanisms of H.pylori-induced PUD and to identify potential biomarkers for disease diagnosis and associated risks using Gas chromatography/mass spectrometry.GC/MS-based analytical method was used to compare metabolic profiles of healthy controls(N=20)and peptic ulcer patients(N=45).Acquired metabolomic data were analyzed by constructing a diagnostic model using principal component analysis and a non-parametric two-tailed paired Wilcoxon analysis to identify disease-specific metabolic biomarkers.A total of 75 low-molecular-weight endogenous metabolites were detected during comparative metabolomic analysis of PUD vs.healthy gut tissues, among which 16 metabolites are being proposed to be diagnostic markers of Human PUD.Perturbations related to amino acids, carbohydrates, fatty acids, organic acids, and sterol metabolism were significantly revealed during this differential metabolomic profiling. Results convincingly suggest that metabolic profiles can contribute immensely in early diagnosis of the disease and understanding molecular mechanisms of disease progression for predicting novel drug targets for prophylactic and anaphylactic measures. | GAGANJOT GUPTA DEEPAK BANSAL ANSHULA SHARMA TAWSEEF AHMAD ATUL SACHDEV AJAZ AHMAD HAMED A.EL-SEREHY BALJINDER KAUR | 2021 | BIOCELL2021,45,5: | 2
Sinapic acid ameliorates D-galactosamine/lipopolysaccharideinduced fulminant hepatitis in rats:Role of nuclear factor erythroidrelated factor 2/heme oxygenase-1 pathways显示文摘BACKGROUND Sinapic acid(SA)has been shown to have various pharmacological properties such as antioxidant,antifibrotic,anti-inflammatory,and anticancer activities. Its mechanism of action is dependent upon its ability to curb free radical production and protect against oxidative stress-induced tissue injuries.AIM To study the hepatoprotective effects of SA against lipopolysaccharide(LPS)/Dgalactosamine(D-GalN)-induced acute liver failure(ALF)in rats.METHODS Experimental ALF was induced with an intraperitoneal(i.p.)administration of 8μg LPS and 800 mg/kg D-GalN in normal saline.SA was administered orally once daily starting 7 d before LPS/D-GalN treatment.RESULTS Data showed that SA ameliorates acute liver dysfunction, decreases serum levels of alanine transaminase(ALT),and aspartate aminotransferase(AST),as well as malondialdehyde(MDA)and NO levels in ALF model rats. However, pretreatment with SA(20 mg/kg and 40 mg/kg)reduced nuclear factor kappalight-chain-enhancer of activated B cells(NF-κB)activation and levels of inflammatory cytokines(tumor necrosis factor-αand interleukin 6).Also,SA increased the activity of the nuclear factor erythroid-related factor 2/heme oxygenase-1(Nrf2/HO-1)signaling pathway.CONCLUSION In conclusion,SA offers significant protection against LPS/D-GalN-induced ALF in rats by upregulating Nrf2/HO-1 and downregulating NF-κB. | Mushtaq Ahmad Ansari Mohammad Raish Yousef A Bin Jardan Ajaz Ahmad Mudassar Shahid SheikhFayaz Ahmad Nazrul Haq Mohammad Rashid Khan Saleh A Bakheet | 2021 | World Journal of Gastroenterology2021,27,7: | 2

We verified the significant and regular optical photometric variability to be present in all three stars. The strong photometric variability and emission of Hα and Ca II H & K using high resolution optical spectroscopy with the Hanle Echelle Spectrograph(HESP),which operates in conjunction with the Himalayan Chandra Telescope, verify the strong chromospheric activity which is present in RS CVn stars. The photometric studies of II Peg, UX Ari and IM Peg were subjected to light curve analysis for spot parameters using a two-starspot model. | Ajaz Ahmad Dar Padmakar Singh Parihar Manzoor Ahmad Malik | 2018 | Research in Astronomy and Astrophysics2018,18,9: | 1 |
Sustainable Utilization and Economic Development of Some Plant Resources in Northern Pakistan显示文摘Overshadowed by the vast impact that habitat loss has on targeted plant species survival, it is easy to overlook the fact that utilization and harvesting of species can often result in their genetic depletion and in an often rapid downward trend of their population status. In this regard an ethnobotanical study on the medicinal plants of Shawar valley, District Swat was conducted with the aims to document medicinal plants and to examine the current status of the medicinal plants trade as well as the market chain starting from collectors to consumers. The survey reported 50 species of plants belonging to 33 families as ethnomedicinally important. These species were used as crud drugs by the local people and folk-lore for treating diseases in traditional system of medicine. The detailed local uses, recipe preparation along with their local names and diseases treated were recorded for each species.Market survey revealed that the structure of medicinal plant trade is complex and heterogeneous, involving many players. The collectors are often not aware of the high market prices and medicinal values, and most of the collected material is sold to local middlemen at a very low price. There was an increase of 3 to 5 folds in prices from collectors to the national market.Recommendations were given in the spheres of training sustainable harvesting and post harvesting of wild medicinal plant resources, trade monitoring, equitable sharing of benefits of wild resources, improved control on harvesting and trade for the conservation of resources, enhancement of cultivation efforts, future research into trade in wild harvested plants, community participation in natural resource management and value addition in the herbal products to maximize the benefits. | Hassan Sher M Ajaz Hazrat Sher | 2007 | 云南植物研究2007,29,2: | 1

Succinate-bonded pullulan: An efficient and reusable super-sorbent for cadmium-uptake from spiked high-hardness groundwater显示文摘Chemically modified pullulan was evaluated for its sorption efficiency and selectivity to remove cadmium(Cd) from spiked high-hardness groundwater(GW). Pullulan esterified with succinic anhydride using dimethylaminopyridine showed a fairly high degree of substitution value as confirmed by1 H NMR spectroscopy. Pullulan succinate(Pull-Suc) was converted into the sodium salt(Pull-Suc-Na). The effect of contact time(5–200 min) and p H(2–8) on Cd-uptake by the sorbent(Pull-Suc-Na) was investigated. The sorbent showed more than 90% Cd-removal in first 15 min from distilled water(DW) and GW solution, respectively. Comparison of Pull-Suc-Na with other polysaccharidal sorbents suggested its high efficiency(DW 476.2 mg/g and GW 454.5 mg/g) and selectivity for the removal of Cd by an ion exchange mechanism, which is further supported by the negative Gibbs free energy values calculated from Langmuir isotherms. A Langmuir isotherm kinetic model provided the best fit for the sorption of Cd using Pull-Suc-Na. The sorbent showed a negligible decrease in Cd-uptake over three regeneration cycles. The thermal stability testing of the sorbents indicated that Pull-Suc-Na(sorbent) is more stable than Pull-Suc. | Azhar Abbas Muhammad Ajaz Hussain Muhammad Amin Muhammad Sher Muhammad Nawaz Tahir Wolfgang Tremel | 2015 | Journal of Environmental Sciences2015,27,11: | 1
