| 1 | Upper gastrointestinal bleeding in Scotland 2000-2010: Improved outcomes but a significant weekend effect显示文摘AIM: To assess numbers and case fatality of patients with upper gastrointestinal bleeding(UGIB),effects of deprivation and whether weekend presentation affected outcomes.METHODS: Data was obtained from Information Services Division(ISD) Scotland and National Records of Scotland(NRS) death records for a ten year period between 2000-2001 and 2009-2010. We obtained data from the ISD Scottish Morbidity Records(SMR01) database which holds data on inpatient and daycase hospital discharges from non-obstetric and nonpsychiatric hospitals in Scotland. The mortality data was obtained from NRS and linked with the ISD SMR01 database to obtain 30-d case fatality. We used 23 ICD-10(International Classification of diseases) codes which identify UGIB to interrogate database. We analysed these data for trends in number of hospital admissions with UGIB,30-d mortality over time and assessed effects of social deprivation. We compared weekend and weekday admissions for differences in 30-d mortality and length of hospital stay. We determined comorbidities for each admission to establish if comorbidities contributed to patient outcome. RESULTS: A total of 60643 Scottish residents were admitted with UGIH during January,2000 and October,2009. There was no significant change in annual number of admissions over time,but there was a statistically significant reduction in 30-d case fatality from 10.3% to 8.8%(P < 0.001) over these 10 years. Number of admissions with UGIB was higher for the patients from most deprived category(P < 0.05),although case fatality was higher for the patients from the least deprived category(P < 0.05). There was no statistically significant change in this trend between 2000/01-2009/10. Patients admitted with UGIB at weekends had higher 30-d case fatality compared with those admitted on weekdays(P < 0.001). Thirty day mortality remained significantly higher for patients admitted with UGIB at weekends after adjusting for comorbidities. Length of hospital stay was also higher overall for patients admitted at the weekend when compared to weekdays,although only reached statistical significance for the last year of study 2009/10(P < 0.0005). 结果载脂蛋白B100(ApoB)/载脂蛋白A1(ApoA1)比值的PAR最高(54%),其每一标准差差别的OR最高(1.59,95%CI1.53~1.64)。LDL胆固醇/HDL-胆固醇比值的PAR为37%;总胆固醇/HDL-胆固醇的PAR为32%,其显著低于ApoB/A1比值的PAR(P〈0.0001)。这些结果在所有种族组、男女两性组及所有年龄组均一致。

| 4 | Efficacy of selective laser trabeculoplasty following incisional glaucoma surgery显示文摘AIM: To evaluate the efficacy of selective laser trabeculoplasty(SLT) in glaucomatous eyes with previous incisional glaucoma surgery. METHODS: A retrospective cohort of eyes that underwent SLT at a single institution from 2013-2015 were followed for 1 y. Reduction in intraocular pressure(IOP) following SLT was evaluated in eyes with prior trabeculectomy with Ex Press mini shunt(Alcon, Ft Worth, TX, USA), Ahmed valve(New World Medical, Cucamonga, CA, USA), or combined phacoemulsification-trabeculectomy. A control group was included with eyes without prior surgery that underwent SLT. Success was defined as >20% drop in IOP from pre-SLT baseline. RESULTS: One-hundred and six eyes were included with 53 in both the prior glaucoma surgery(PGS) and no prior glaucoma surgery(NPGS) groups. Mean pre-SLT IOP was 19.2±4.3 and 20.6±6.0 mm Hg for PGS and NPGS groups, respectively(P=0.17). Both groups produced statistically significant IOP reductions at 1 and 6 mo(P<0.04). At 6 mo, mean IOP reduction reached 7.3% and 10.8% for the PGS and NPGS groups, respectively(P=0.42). Overall, 27.9% and 31.7% of eyes in PGS and NPGS groups met success criteria at 1 y(P=0.70). In the PGS group, eyes with baseline IOP ≥21 mm Hg had IOP reductions of 18.1%(P<0.001), 16.7%(P<0.01), and 8.4%(P=0.31) compared to eyes with baseline IOP <21 mm Hg who had IOP reductions of 2.3%(P=0.39), 3.4%(P=0.19), and 1.1%(P=0.72) at 1, 6 mo, and 1 y, respectively. CONCLUSION: SLT is efficacious in eyes with prior incisional glaucoma surgery and results in similar IOP reductions compared to eyes without PGS.
