1Role of dental plaque,saliva and periodontal disease in Helicobacter pylori infection显示文摘Helicobacter pylori(H.pylori)infection is one of the most common bacterial infections in humans.Although H.pylori may be detected in the stomach of approximately half of the world’s population,the mechanisms of transmission of the microorganism from individual to individual are not yet clear.Transmission of H.pylori could occur through iatrogenic,fecal-oral,and oral-oral routes,and through food and water.The microorganism may be transmitted orally and has been detected in dental plaque and saliva.However,the role of the oral cavity in the transmission and recurrence of H.pylori infection has been the subject of debate.A large number of studies investigating the role of oral hygiene and periodontal disease in H.pylori infection have varied significantly in terms of their methodology and sample population,resulting in a wide variation in the reported results.Nevertheless,recent studies have not only shown that the microorganism can be detected fairly consistently from the oral cavity but also demonstrated that the chances of recurrence of H.pylori infection is more likely among patients who harbor the organism in the oral cavity.Furthermore,initial results from clinical trials have shown that H.pylori-positive dyspeptic patients may benefit from periodontal therapy.This paper attempts to review the current body of evidence regarding the role of dental plaque,saliva,and periodontal disease in H.pylori infection.Pradeep S Anand Kavitha P Kamath Sukumaran Anil 2014World Journal of Gastroenterology2014,20,19:15
2Liver transplantation in alcoholic liver disease current status and controversies显示文摘Alcoholic cirrhosis remains the second most common indication for liver transplantation.A comprehensive medical and psychosocial evaluation is needed when making a decision to place such patients on the transplant list.Most transplant centers worldwide need a minimum of 6 mo of alcohol abstinence for listing these patients.Patients with alcohol dependence are at high risk for relapse to alcohol use after transplantation(recidivism).These patients need to be identified and require alcohol rehabilitation treatment before transplantation.Recidivism to the level of harmful drinking is reported in about 15%-20%cases.Although,recurrent cirrhosis and graft loss from recidivism is rare,occurring in less than 5%of all alcoholic cirrhosis-related transplants,harmful drinking in the post-transplant pe-riod does impact the long-term outcome.The development of metabolic syndrome with cardiovascular events and de novo malignancy are important contributors to non liver-related mortality amongst transplants for alcoholic liver disease.Surveillance protocols for earlier detection of de novo malignancy are needed to improve the long-term outcome.The need for a minimum of 6 mo of abstinence before listing makes transplant a nonviable option for patients with severe alcoholic hepatitis who do not respond to corticosteroids.Emerging data from retrospective and prospective studies has challenged the 6 mo rule,and beneficial effects of liver transplantation have been reported in select patients with a first episode of severe alcoholic hepatitis who are unresponsive to steroids.Ashwani K Singal Khushdeep S Chaha Khalid Rasheed Bhupinderjit S Anand 2013World Journal of Gastroenterology2013,19,36:12
3Management of hepatitis C virus infection in HIV/HCV co-infected patients: Clinical review显示文摘Nearly one fourth of individuals with human immunodeficiency virus (HIV) infection have hepatitis C virus (HCV) infection in the US and Western Europe. With the availability of highly active antiretroviral therapy and the consequent reduction in opportunistic infections, resulting in the prolongation of the life span of HIV-infected patients, HCV co-infection has emerged as a signif icant factor influencing the survival of HIV patients. Patients with HIV/HCV co-infection have a faster rate of fibrosis progression resulting in more frequent occurrences of cirrhosis, end-stage liver disease, and hepatocellular carcinoma. However, the mechanism of interaction between the two viruses is not completely understood. The treatment for HCV in co-infected patients is similar to that of HCV monoinfection; i.e., a combination of pegylated interferon and ribavirin. The presence of any barriers to antiHCV therapy should be identified and eliminated in order to recruit all eligible patients. The response to treatment in co-infected patients is inferior compared to the response in patients with HCV mono-infection. The sustained virologic response rate is only 38% for genotype-1 and 75% for genotype-2 and -3 infections. Liver transplantation is no longer considered a contraindication for end-stage liver disease in coinfected patients. However, the 5 year survival rate is lower in co-infected patients compared to patients with HCV mono-infection (33% vs 72%, P = 0.07). 5Influence of heat treatment on the machinability and corrosion behavior of AZ91 Mg alloy显示文摘In the present study,AZ91 Mg alloy was heat treated at 410℃ for 6,12 and 24 h to investigate the influence of heat treatment on machinability and corrosion behavior.The effect of soaking time on the amount and distribution of Mg 17 Al 12(β-phase)was analyzed under the optical microscope.Microhardness measurements demonstrated the increased hardness with increased heat treatment soaking time,which can be attributed to the solid solution strengthening.The influence of super saturatedα-grains on reducing the cutting force(F z)with respect to increased cutting speed was observed as prominent.The corrosion behavior of the heat treated specimens was studied by conducting electrochemical tests.Surprisingly,corrosion rate of heat treated samples was observed as increased compared with the base material.From the results,it is evident that the machinability of AZ91 Mg alloy can be improved by producing super saturatedα-grains through heat treatment but at the cost of losing corrosion resistance.Swetha Chowdary V Ravikumar Dumpala Anand Kumar S Kondaiah VV Ratna Sunil B 2018Journal of Magnesium and Alloys2018,6,1:7

8Biosynthesis of silver nanoparticles by using mangrove plant extract and their potential mosquito larvicidal property显示文摘Objective:To identify the larvicidal activities of silver nano particles synthesised with Rhizophora mucronata(R.mucronata) leaf extract against the larvae of Aedes aegypti(Ae.aegypti) and Culex quinquefasciatus(Cx.quinquefasciatus).Methods:In vitro larvicidal activities such as LC50 and LC90 were assessed for the Ae.aegypti and Cx.quinquefasciatus larval species.Further, characterisation such as UV,XRD,FTIR and AFM analysis were carried out for the synthesised silver nano particles.Results:The LC50 value of the synthesised silver nano particle was identified as 0.585 and 0.891 mgg/L for Ae.aegypti and Cx.quinquefasciatus larvae respectively. Further,the LG90 values are also identified as 2.615 and 6.291 mg/L for Ae.aegypti and Cx. quinquefasciatus species respectively.The synthesised silver nanoparticles have maximum absorption at 420 nm with the average size of 60-95 nm.The XRD data showed 20 intense values with various degrees such as 37.10°,47.66°,63.97°and 70.01°.The FTIR data showed prominent peaks in(3 426.89,2 925.49,2 869.56,2 346.95,1 631.49,1 031.73,669.18 and 455.12) different ranges.Conclusions:The biosynthesis of silver nanoparticles with leaf aqueous extract of R. mucronata provides potential source for the larvicidal activity against mosquito borne diseases.M Gnanadesigan M Anand S Ravikumar M Maruthupandy V Vijayakumar S Selvam M Dhineshkumar AK Kumaraguru 2011Asian Pacific Journal of Tropical Medicine2011,4,10:4
9Laparoscopic liver resection:Wedge resections to living donor hepatectomy, are we heading in the right direction?显示文摘Despite inception over 15 years ago and over 3000 completed procedures, laparoscopic liver resection has remained mainly in the domain of selected centers and enthusiasts. Requirement of extensive open liver resection(OLR) experience, in-depth understanding of anatomy and considerable laparoscopic technical expertise may have delayed wide application. However healthy scepticism of its actual benefits and presence of a potential publication bias; concern about its safety and technical learning curve, are probably equally responsible. Given that a large proportion of our work, at least in transplantation is still OLR, we have attempted to provide an entirely unbiased, mature opinion of its pros and cons in the current invited review. We have dividedthis review into two sections as we believe they merit separate attention on technical and ethical grounds. The first part deals with laparoscopic liver resection(LLR) in patients who present with benign or malignant liver pathology, wherein we have discussed its overall outcomes; its feasibility based on type of pathology and type of resection and included a small section on application of LLR in special scenarios like cirrhosis. The second part deals with the laparoscopic living donor hepatectomy(LDH) experience to date, including its potential impact on transplantation in general. Donor safety, graft outcomes after LDH and criterion to select ideal donors for LLR are discussed. 10Inflammatory bowel disease:Therapeutic limitations and prospective of the stem cell therapy显示文摘Inflammatory bowel disease(IBD),consisting primarily of ulcerative colitis and Crohn’s disease,is a group of debilitating auto-immune disorders,which also increases the risk of colitis-associated cancer.However,due to the chronic nature of the disease and inconsistent treatment outcomes of current anti-IBD drugs(e.g.,approximately 30%non-responders to anti-TNFαagents),and related serious side effects,about half of all IBD patients(in millions)turn to alternative treatment options.In this regard,mucosal healing is gaining acceptance as a measure of disease activity in IBD patients as recent studies have correlated the success of mucosal healing with improved prognosis.However,despite the increasing clinical realization of the significance of the concept of mucosal healing,its regulation and means of therapeutic targeting remain largely unclear.Here,stemcell therapy,which uses hematopoietic stem cells or mesenchymal stem cells,remains a promising option.Stem cells are the pluripotent cells with ability to differentiate into the epithelial and/or immune-modulatory cells.The overreaching concept is that the stem cells can migrate to the damaged areas of the intestine to provide curative help in the mucosal healing process.Moreover,by differentiating into the mature intestinal epithelial cells,the stem cells also help in restoring the barrier integrity of the intestinal lining and hence prevent the immunomodulatory induction,the root cause of the IBD.In this article,we elaborate upon the current status of the clinical management of IBD and potential role of the stem cell therapy in improving IBD therapy and patient’s quality of life.Rangnath Mishra Punita Dhawan Anand S Srivastava Amar B Singh 2020World Journal of Stem Cells2020,12,10:4
11Alcoholic hepatitis and concomitant hepatitis C virus infection显示文摘Hepatitis C virus(HCV)infection and alcohol abuse are two most important causes of chronic liver disease in the United States.Alcoholic hepatitis is a unique clinical syndrome among patients with chronic and active alcohol abuse with a potential for high short-term mortality.About 20%of patients presenting with alcoholic hepatitis have concomitant HCV infection.Mortality from alcoholic hepatitis is increased in the presence of concomitant hepatitis C due to synergistic interaction between HCV and alcohol in causing hepatocellular damage.Large prospective randomized studies are needed to develop guidelines on the use of corticosteroids among patients with alcoholic hepatitis and concomitant HCV infection.The impact of antiviral therapy on mortality and outcome in the setting of alcoholic hepatitis remains a novel area for future research.Mohamed Shoreibah Bhupinderjit S Anand Ashwani K Singal 2014World Journal of Gastroenterology2014,20,34:3
12Dysphagia after vertical sleeve gastrectomy: Evaluation of risk factors and assessment of endoscopic intervention显示文摘AIM To evaluate the risks of medical conditions, evaluate gastric sleeve narrowing, and assess hydrostatic balloon dilatation to treat dysphagia after vertical sleeve gastrectomy(VSG).METHODS VSG is being performed more frequently worldwide as a treatment for medically-complicated obesity, and dysphagia is common post-operatively. We hypothesize that post-operative dysphagia is related to underlying medical conditions or narrowing of the gastric sleeve. This is a retrospective, single institution study of consecutive patients who underwent sleeve gastrectomy from 2013 to 2015. Patients with previous bariatric procedures were excluded. Narrowing of a gastric sleeve includes: inability to pass a 9.6 mm gastroscope due to stenosis or sharp angulation or spiral hindering its passage.RESULTS Of 400 consecutive patients, 352 are included; the prevalence of dysphagia is 22.7%; 33 patients(9.3%) have narrowing of the sleeve with 25(7.1%) having sharp angulation or a spiral while 8(2.3%) have a stenosis. All 33 patients underwent balloon dilatation of the gastric sleeve and dysphagia resolved in 13 patients(39%); 10 patients(30%) noted resolution of dysphagia after two additional dilatations. In a multivariate model, medical conditions associated with post-operative dysphagia include diabetes mellitus, symptoms of esophageal reflux, a low whole blood thiamine level, hypothyroidism, use of non-steroidal anti-inflammatory drugs, and use of opioids.CONCLUSION Narrowing of the gastric sleeve and gastric sleeve stenosis are common after VSG. 13Expression of genes that control core fucosylation in hepatocellular carcinoma: Systematic review显示文摘BACKGROUND Changes in N-linked glycosylation have been observed in the circulation of individuals with hepatocellular carcinoma. In particular, an elevation in the level of core fucosylation has been observed. However, the mechanisms through which core fucose is increased are not well understood. We hypothesized that a review of the literature and related bioinformatic review regarding six genes known to be involved in the attachment of core fucosylation, the synthesis of the fucosylation substrate guanosine diphosphate(GDP)-fucose, or the transport of the substrate into the Golgi might offer mechanistic insight into the regulation of core fucose levels.AIM To survey the literature to capture the involvement of genes regulating core Nlinked fucosylation in hepatocellular carcinoma METHODS The PubMed biomedical literature database was searched for the association of hepatocellular carcinoma and each of the core fucose-related genes and their protein products. We also queried The Cancer Genome Atlas Liver hepatocellular carcinoma(LIHC) dataset for genetic, epigenetic and gene expression changes for the set of six genes using the tools at cBioportal.RESULTS A total of 27 citations involving one or more of the core fucosylation-related genes(FPGT, FUK, FUT8, GMDS, SLC35 C1, TSTA3) and hepatocellular carcinoma were identified. The same set of gene symbols was used to query the371 patients with liver cancer in the LIHC dataset to identify the frequency of m RNA over or under expression, as well as non-synonymous mutations, copy number variation and methylation level. Although all six genes trended to moresamples displaying over expression relative to under-expression, it was noted that a number of tumor samples had undergone amplification of the genes of the de novo synthesis pathway, GMDS(27 samples) and TSTA3(78 samples). 14Anticoagulation in simultaneous pancreas kidney transplantation - On what basis?显示文摘BACKGROUND Despite technical refinements,early pancreas graft loss due to thrombosis continues to occur.Conventional coagulation tests(CCT)do not detect hypercoagulability and hence the hypercoagulable state due to diabetes is left untreated.Thromboelastogram(TEG)is an in-vitro diagnostic test which is used in liver transplantation,and in various intensive care settings to guide anticoagulation.TEG is better than CCT because it is dynamic and provides a global hemostatic profile including fibrinolysis.AIM To compare the outcomes between TEG and CCT(prothrombin time,activated partial thromboplastin time and international normalized ratio)directed anticoagulation in simultaneous pancreas and kidney(SPK)transplant recipients.METHODS A single center retrospective analysis comparing the outcomes between TEG and CCT-directed anticoagulation in SPK recipients,who were matched for donor age and graft type(donors after brainstem death and donors after circulatory death).Anticoagulation consisted of intravenous(IV)heparin titrated up to a maximum of 500 IU/h based on CCT in conjunction with various clinical parameters or directed by TEG results.Graft loss due to thrombosis,anticoagulation related bleeding,radiological incidence of partial thrombi in the pancreas graft,thrombus resolution rate after anticoagulation dose escalation,length of the hospital stays and,1-year pancreas and kidney graft survival between the two groups were compared.RESULTS Seventeen patients who received TEG-directed anticoagulation were compared against 51 contemporaneous SPK recipients(ratio of 1:3)who were anticoagulated based on CCT.No graft losses occurred in the TEG group,whereas 11 grafts(7 pancreases and 4 kidneys)were lost due to thrombosis in the CCT group(P=0.06,Fisher’s exact test).The overall incidence of anticoagulation related bleeding(hematoma/gastrointestinal bleeding/hematuria/nose bleeding/re-exploration for bleeding/post-operative blood transfusion)was 17.65%in the TEG group and 45.10%in the CCT group(P=0.05,Fisher’s exact test).The incidence of radiologically confirmed partial thrombus in pancreas allograft was 41.18%in the TEG and 25.50%in the CCT group(P=0.23,Fisher’s exact test).All recipients with partial thrombi detected in computed tomography(CT)scan had an anticoagulation dose escalation.The thrombus resolution rates in subsequent scan were 85.71%and 63.64%in the TEG group vs the CCT group(P=0.59,Fisher’s exact test).The TEG group had reduced blood product usage{10 packed red blood cell(PRBC)and 2 fresh frozen plasma(FFP)}compared to the CCT group(71 PRBC/10 FFP/2 cryoprecipitate and 2 platelets).The proportion of patients requiring transfusion in the TEG group was 17.65%vs 39.25%in the CCT group(P=0.14,Fisher’s exact test).The median length of hospital stay was 18 days in the TEG group vs 31 days in the CCT group(P=0.03,Mann Whitney test).The 1-year pancreas graft survival was 100%in the TEG group vs 82.35%in the CCT group(P=0.07,log rank test)and,the 1-year kidney graft survival was 100%in the TEG group vs 92.15%in the CCT group(P=0.23,log tank test).CONCLUSION TEG is a promising tool in guiding judicious use of anticoagulation with concomitant prevention of graft loss due to thrombosis,and reduces the length of hospital stay.Jeevan Prakash Gopal Frank JMF Dor Jeremy S Crane Paul E Herbert Vassilios E Papalois Anand SR Muthusamy 2020World Journal of Transplantation2020,10,7:2
