| 18 | 代谢综合征溶栓治疗抵抗性有性别差异显示文摘背景和目的:临床研究证实代谢综合征(MetS)增加女性脑卒中的危险性高于男性;西班牙研究组最近应用TCD证实MetS可使急性大脑中动脉(MCA)缺血性脑卒中患者对静脉溶栓治疗(t-PA)出现较高的抵抗性,该研究目的是进一步确定急性MCA缺血性脑卒中患者中,MetS对静脉溶栓治疗的反应是否也有性别的差别。方法:该研究系前瞻性研究,应用TCD连续观察MCA闭塞患者静脉t-PA治疗(脑卒中发病3h,给予标准0.9ms/kg的t-PA)前后闭塞动脉再通情况,分析其有无性别差异。诊断MetS按照AHA/NHLBI-2005标准。TCD应用脑缺血溶栓血流分级(TIBI)评分系统(0~5级),动脉完全再通定义为舒张末期流速改善至正常或TIBI值提高至4或5级;血块溶解抵抗(溶栓抵抗)定义为t-PA给药24h后,TCD检查无动脉完全再通出现。结果:(1)总共125例患者[男75例,女50例,平均(67.6±11.0)岁]中,诊断为MetS者76例(61%),溶栓治疗24h后,溶栓抵抗者53例(42%)。(2)多元一调整,Logistic回归统计学分析确认MetS伴有较高的t-PA溶栓抵抗,其独立于其他统计学意义的基线变量(OR值=9.8;95%CI3.5~27.8;P=0.0001),也独立于MetS单项组成成分。(3)MetS伴有的溶栓抵抗发生概率,在女性高于男性,有统计学意义[OR值(女性):17.5;95%CI1.9~163.1;OR值(男性)=5.1;95%CI1.6~15.6]。结论:MetS对急性MCA缺血性脑卒中静脉溶栓的抵抗作用,女性比男性表现得更显著。 | Arenillas JF Sandoval P de la Ossa NP 李新 王纪佐 | 2009 | 中华神经科杂志2009,42,8: | 0 |
| 19 | Decline in subarachnoid haemorrhage volumes associated with the first wave of the COVID- 19 pandemic显示文摘Background During the COVID-19 pandemic,decreased volumes of stroke admissions and mechanical thrombectomy were reported.The study’s objective was to examine whether subarachnoid haemorrhage(SAH)hospitalisations and ruptured aneurysm coiling interventions demonstrated similar declines.Methods We conducted a cross-sectional,retrospective,observational study across 6 continents,37 countries and 140 comprehensive stroke centres.Patients with the diagnosis of SAH,aneurysmal SAH,ruptured aneurysm coiling interventions and COVID-19 were identified by prospective aneurysm databases or by International Classification of Diseases,10th Revision,codes.The 3-month cumulative volume,monthly volumes for SAH hospitalisations and ruptured aneurysm coiling procedures were compared for the period before(1 year and immediately before)and during the pandemic,defined as 1 March-31 May 2020.The prior 1-year control period(1 March-31 May 2019)was obtained to account for seasonal variation.Findings There was a significant decline in SAH hospitalisations,with 2044 admissions in the 3 months immediately before and 1585 admissions during the pandemic,representing a relative decline of 22.5%(95%CI−24.3%to−20.7%,p<0.0001).Embolisation of ruptured aneurysms declined with 1170-1035 procedures,respectively,representing an 11.5%(95%CI−13.5%to−9.8%,p=0.002)relative drop.Subgroup analysis was noted for aneurysmal SAH hospitalisation decline from 834 to 626 hospitalisations,a 24.9%relative decline(95%CI−28.0%to−22.1%,p<0.0001).A relative increase in ruptured aneurysm coiling was noted in low coiling volume hospitals of 41.1%(95%CI 32.3%to 50.6%,p=0.008)despite a decrease in SAH admissions in this tertile.Interpretation There was a relative decrease in the volume of SAH hospitalisations,aneurysmal SAH hospitalisations and ruptured aneurysm embolisations during the
