| 1 | Liver transplantation in alcoholic liver disease current status and controversies显示文摘Alcoholic cirrhosis remains the second most common indication for liver transplantation.A comprehensive medical and psychosocial evaluation is needed when making a decision to place such patients on the transplant list.Most transplant centers worldwide need a minimum of 6 mo of alcohol abstinence for listing these patients.Patients with alcohol dependence are at high risk for relapse to alcohol use after transplantation(recidivism).These patients need to be identified and require alcohol rehabilitation treatment before transplantation.Recidivism to the level of harmful drinking is reported in about 15%-20%cases.Although,recurrent cirrhosis and graft loss from recidivism is rare,occurring in less than 5%of all alcoholic cirrhosis-related transplants,harmful drinking in the post-transplant pe-riod does impact the long-term outcome.The development of metabolic syndrome with cardiovascular events and de novo malignancy are important contributors to non liver-related mortality amongst transplants for alcoholic liver disease.Surveillance protocols for earlier detection of de novo malignancy are needed to improve the long-term outcome.The need for a minimum of 6 mo of abstinence before listing makes transplant a nonviable option for patients with severe alcoholic hepatitis who do not respond to corticosteroids.Emerging data from retrospective and prospective studies has challenged the 6 mo rule,and beneficial effects of liver transplantation have been reported in select patients with a first episode of severe alcoholic hepatitis who are unresponsive to steroids. | Ashwani K Singal Khushdeep S Chaha Khalid Rasheed Bhupinderjit S Anand | 2013 | World Journal of Gastroenterology2013,19,36: | 12 |
| 2 | Management of hepatitis C virus infection in HIV/HCV co-infected patients: Clinical review显示文摘Nearly one fourth of individuals with human immunodeficiency virus (HIV) infection have hepatitis C virus (HCV) infection in the US and Western Europe. With the availability of highly active antiretroviral therapy and the consequent reduction in opportunistic infections, resulting in the prolongation of the life span of HIV-infected patients, HCV co-infection has emerged as a signif icant factor influencing the survival of HIV patients. Patients with HIV/HCV co-infection have a faster rate of fibrosis progression resulting in more frequent occurrences of cirrhosis, end-stage liver disease, and hepatocellular carcinoma. However, the mechanism of interaction between the two viruses is not completely understood. The treatment for HCV in co-infected patients is similar to that of HCV monoinfection; i.e., a combination of pegylated interferon and ribavirin. The presence of any barriers to antiHCV therapy should be identified and eliminated in order to recruit all eligible patients. The response to treatment in co-infected patients is inferior compared to the response in patients with HCV mono-infection. The sustained virologic response rate is only 38% for genotype-1 and 75% for genotype-2 and -3 infections. Liver transplantation is no longer considered a contraindication for end-stage liver disease in coinfected patients. However, the 5 year survival rate is lower in co-infected patients compared to patients with HCV mono-infection (33% vs 72%, P = 0.07). | 5 | Corticosteroids and pentoxifylline for the treatment of alcoholic hepatitis:Current status显示文摘The treatment of choice for patients with severe alcoholic hepatitis (AH) is use of corticosteroids.Many randomized well designed studies have been reported from all over the world on the use of corticosteroids in the treatment of AH.However,the data on the efficacy of corticosteroids in these patients have been conflicting.Initial meta-analyses also failed to show beneficial effects of corticosteroids.Based on individual data meta-analysis showing clear benefit of corticosteroids amongst patients with severe AH (modified discriminant function of 32 or more),led American College of Gastroenterology to recommend use of corticosteroids as the first line treatment option amongst patients with severe AH.However,corticosteroids are relatively contraindicated amongst patients with severe AH and coexistent sepsis,gastrointestinal bleeding,and acute pancreatitis.These patients may be candidates for second line treatment with pentoxifylline.Further,specific treatment of AH with corticosteroids far from satisfactory with as many as 40%-50% of patients failing to respond to steroids,thus classified as nonresponsive to steroids.The management of these patients is a continuing challenge for physicians.Better treatment modalities need to be developed for this group of patients in order to improve the outcome of patients with severe AH.This article describes at length the available trials on use of corticosteroids and pentoxifylline with their current status.Route of administration,dosage,adverse effects,and mechanisms of action of these two drugs are also discussed.Finally,an algorithm with clinical approach to management of patients who present with clinical syndrome of AH is described. | Ashwani K Singal Ishmeet Walia Anjna Singal Roger D Soloway | 2011 | World Journal of Hepatology2011,3,8: | 7 |

| 9 | Liver transplantation in acute alcoholic hepatitis:Current status and future development显示文摘Acute alcoholic hepatitis (AH) is a distinct clinical entity amongst patients with chronic alcohol abuse.Patients with severe AH are at risk of dying in about 20%-25% cases despite specific treatment with corticosteroids and/or pentoxifylline.Clearly,a need for an additional more effective treatment option is unmet currently.Liver transplantation (LT),a definitive treatment option for alcoholic cirrhosis requires 6 mo abstinence.However,this rule cannot be applied to patients with AH as these patients are actively drinking prior to their presentation.Shortage of donors,ethical issues,and fear of recidivism after transplantation with less than 6 mo pretransplant abstinence are some of the reasons behind this rule of 6 mo of abstinence and hesitancy of transplanting patients with AH.These issues are debated at length in this manuscript.Further,retrospective studies have shown that patients undergoing transplantation for alcoholic cirrhosis and having histological changes of AH have been shown to fare as well when compared to patients without these histological changes.Recently,French workers have reported a case matched prospective study showing encouraging data on the usefulness of LT for patients who are non-responders to cortico-steroid and/or pentoxifylline therapy.Future studies are needed to identify patients with severe AH who are going to benefit most with LT.In the light of emerging data on the efficacy of LT in improving survival of patients with severe acute AH who do not respond to corticosteroids,the time is ripe to re-evaluate our policy of LT in patients with AH. | Ashwani K Singal Andrea Duchini | 2011 | World Journal of Hepatology2011,3,8: | 4 |

| 14 | Multi-objective energy management in microgrids with hybrid energy sources and battery energy storage systems显示文摘Microgrid with hybrid renewable energy sources is a promising solution where the distribution network expansion is unfeasible or not economical.Integration of renewable energy sources provides energy security,substantial cost savings and reduction in greenhouse gas emissions,enabling nation to meet emission targets.Microgrid energy management is a challenging task for microgrid operator(MGO)for optimal energy utilization in microgrid with penetration of renewable energy sources,energy storage devices and demand response.In this paper,optimal energy dispatch strategy is established for grid connected and standalone microgrids integrated with photovoltaic(PV),wind turbine(WT),fuel cell(FC),micro turbine(MT),diesel generator(DG)and battery energy storage system(ESS).Techno-economic benefits are demonstrated for the hybrid power system.So far,microgrid energy management problem has been addressed with the aim of minimizing operating cost only.However,the issues of power losses and environment i.e.,emission-related objectives need to be addressed for effective energy management of microgrid system.In this paper,microgrid energy management(MGEM)is formulated as mixedinteger linear programming and a new multi-objective solution is proposed for MGEM along with demand response program.Demand response is included in the optimization problem to demonstrate it’s impact on optimal energy dispatch and techno-commercial benefits.Fuzzy interface has been developed for optimal scheduling of ESS.Simulation results are obtained for the optimal capacity of PV,WT,DG,MT,FC,converter,BES,charging/discharging scheduling,state of charge of battery,power exchange with grid,annual net present cost,cost of energy,initial cost,operational cost,fuel cost and penalty of greenhouse gases emissions.The results show that CO_(2) emissions in standalone hybrid microgrid system is reduced by 51.60%compared to traditional system with grid only.Simulation results obtained with the proposed method is compared with various evolutionary algorithms to verify it’s effectiveness. | V.V.S.N.Murty Ashwani Kumar | 2020 | Protection and Control of Modern Power Systems2020,5,1: | 3 |

| 16 | Lycopene modulates cellular proliferation, glycolysis and hepatic ultrastructure during hepatocellular carcinoma显示文摘AIM To investigate the effect of lycopene extracted from tomatoes(LycT) on ultrastructure, glycolytic enzymes, cell proliferation markers and hypoxia during N-Nitrosodiethylamine(NDEA)-induced hepatocarcinogenesis. METHODS Female BALB/c mice were randomly divided into four groups: The Control, NDEA(200 mg NDEA/kg b.w. given i.p.), LycT(5 mg/kg b.w. given orally on alternate days) and Lyc T + NDEA group. The m RNA and protein expression of various cell proliferation markers(PCNA, Cyclin D1, and p21) were assessed by reverse transcription-polymerase chain reaction and enzyme linked immunosorbent assay, respectively. The ultrastructure of hepatic tissue was analyzed using scanning and transmission electron microscopy. The enzymatic activity of glycolytic enzymes was estimated using standardized protocols, while glucose-6-phosphate dehydrogenase activity level was estimated using a kit obtained from Reckon Diagnostic P. Ltd.(India). RESULTS Uncontrolled proliferation in the liver of NDEA(P ≤ 0.001) mice was evident from the high expression of cell-proliferation associated genes(PCNA, Cyclin D1, and p21) when compared to control and Lyc T mice. In addition, enhanced activities of hexokinase, phosphoglucoisomerase, aldolase, glucose-6-phosphatedehydrogenase and hypoxia-inducible factor-1α were observed in NDEA mice as compared to control(P ≤ 0.001) and Lyc T(P ≤ 0.001) mice. The alterations in hepatic ultrastructure observed in the NDEA group correlated with the changes in the above parameters. Lyc T pre-treatment in NDEA-challenged mice ameliorated the investigated pathways disrupted by NDEA treatment. Moreover, hepatic electron micrographs from the Lyc T + NDEA group showed increased macrophages, apoptotic bodies and well-differentiated hepatocellular carcinoma(HCC) in comparison to undifferentiated HCC as observed in the NDEA treated group. The present study was designed to evaluate the antianxiety and antidepressant potential of M. paniculata leaves using elevated plus maze model and forced swim test, respectively.METHODS: Extracts of M. paniculata made with petroleum ether(60–80 °C), chloroform, ethanol and water were evaluated for antianxiety and antidepressant activity. The anxiolytic chloroform extract was subjected to column chromatography, yielding five fractions(F_1–F_5). Fraction F_5(100 mg/kg), which showed notable anxiolytic activity, was further chromatographed to get four subfractions(F_(5.1)–F_(5.4)). Simultaneously, the ethanol extract was partitioned with ethyl acetate to obtain ethyl acetate soluble fraction(EASF) and ethyl acetate insoluble fraction. Phytochemical screening of bioactive extracts/fractions and detection of mahanimbine in M. paniculata leaf extract by thin-layer chromatography was also carried out.RESULTS: Fraction F_(5.3)(25 mg/kg) and EASF(20 mg/kg) showed significant anxiolytic and antidepressant activity, respectively. Thin-layer chromatography and phytochemical screening demonstrated the absence of mahanimbine in M. paniculata leaves.
