1Acute renal dysfunction in liver diseases显示文摘Renal dysfunction is common in liver diseases,either as part of multiorgan involvement in acute illness or secondary to advanced liver disease.The presence of renal impairment in both groups is a poor prognostic indicator.Renal failure is often multifactorial and can present as pre-renal or intrinsic renal dysfunction.Obstructive or post renal dysfunction only rarely complicates liver disease.Hepatorenal syndrome(HRS)is a unique form of renal failure associated with advanced liver disease or cirrhosis,and is characterized by functional renal impairment without significant changes in renal histology.Irrespective of the type of renal failure,renal hypoperfusion is the central pathogenetic mechanism,due either to reduced perfusion pressure or increased renal vascular resistance.Volume expansion,avoidance of precipitating factors and treatment of underlying liver disease constitute the mainstay of therapy to prevent and reverse renal impairment.Splanchnic vasoconstrictor agents,such as terlipressin,along with volume expansion,and early placement of transjugular intrahepatic portosystemic shunt(TIPS)may be effective in improving renal function in HRS.Continuous renal replacement therapy(CRRT)and molecular absorbent recirculating system(MARS)in selected patients may be life saving while awaiting liver transplantation.Alex P Betrosian Banwari Agarwal Emmanuel E Douzinas 2007World Journal of Gastroenterology2007,13,42:14
2Human albumin solution for patients with cirrhosis and acute on chronic liver failure: Beyond simple volume expansion显示文摘To provide an overview of the properties of human serum albumin(HSA), and to review the evidence for the use of human albumin solution(HAS) in critical illness, sepsis and cirrhosis. A MEDLINE search was performed using the terms 'human albumin', 'critical illness', 'sepsis' and 'cirrhosis'. The references of retrieved articles were reviewed manually. Studies published between 1980 and 2014 were selected based on quality criteria. Data extraction was performed by all authors. HSA is the main plasma protein contributing greatly to its oncotic pressure. HSA demonstrates important binding properties for endogenous and exogenous toxins, drugs and drug metabolites that account for its anti-oxidant and anti-inflammatory properties. In disease states, hypoalbuminaemia is secondary to decreased HSA production, increased loss or transcapillary leakage into the interstitial space. HSA function can be also altered in disease with reduced albumin binding capacity and increased production of modified isoforms. HAS has been used as volume expander in critical illness, but received criticism due to cost and concerns regarding safety. More recent studies confirmed the safety of HAS, but failed to show any survival benefit compared to the cheaper crystalloid fluids, therefore limiting its use. On the contrary, in cirrhosis there is robust data to support the efficacy of HAS for the prevention of circulatory dysfunction post-large volume paracentesis and in the context of spontaneous bacterial peritonitis, and for the treatment of hepato-renal syndrome and hypervolaemic hyponatraemia. It is likely that not only the oncotic properties of HAS are beneficial in cirrhosis, but also its functional properties, as HAS replaces the dysfunctional HSA. 3Effects of 42-year long-term fertilizer management on soil phosphorus availability, fractionation,adsorption–desorption isotherm and plant uptake in flooded tropical rice显示文摘Soil phosphorus(P) fractionation, adsorption, and desorption isotherm, and rice yield and P uptake were investigated in flooded tropical rice(Oryza sativa L.) following 42-year fertilizer and manure application. The treatments included low-input [unfertilized control without N, P, or K(C0N0)], farmyard manure(FYM)(C1N0), NP(C0NP), NPK(C0NPK), FYM + NP(C1NP), and high-input treatment, FYM + NPK(C1NPK). Grain yield was increased significantly by 74%over the control under the combined application of FYM + NPK. However, under low- and high-input treatments, yield as well as P uptake was maintained at constant levels for 35 years.During the same period, high yield levels and P uptake were maintained under the C0 NP, C0 NPK,and C1 NPK treatments. These are unique characteristics of a tropical flooded ecosystem, which is a self-sustaining system for rice production. The Fe–P fraction was highest compared to the Ca–P and Al–P fractions after 42 years of fertilizer application and was significantly higher under FYM + NPK treatment. The P adsorption capacity of soil was highest under the low-input treatment and lowest under long-term balanced fertilization(FYM + NPK). In contrast, P desorption capacity was highest under NPK and lowest in the control treatment. 4Difficulties in diagnosing acute kidney injury post liver transplantation using serum creatinine based diagnostic criteria显示文摘Renal function in patients with advanced cirrhosis is an important prognostic factor for survival both prior to and following liver transplantation. The importance of renal function is reflected by the introduction of the model for end stage liver disease(MELD) score, which includes serum creatinine. The MELD score has been shown to predict the short term risk of death for transplant wait listed patients and is currently used by many countries to allocate liver transplants on the basis of severity of underlying illness. Changes in serum creatinine are also used to stage acute kidney injury. However prior to liver transplantation the serum creatinine typically over estimates underlying renal function, particularly when a colorimetric Jaffe based assay is used, and paradoxically then under estimates renal function post liver transplantation, particularly when immunophyllins are started early as part of transplant immunosuppression. As acute kidney injury is defined by changes in serum creatinine, this potentially leads to over estimation of the incidence and severity of acute kidney injury in the immediate post-operative period.Banwari Agarwal Andrew Davenport 2014World Journal of Hepatology2014,6,10:4
5Can non-invasive measurements aid clinical assessment of volume in patients with cirrhosis?显示文摘AIM:To evaluate the non-invasive assessments of volume status in patients with cirrhosis.METHODS:Echocardiography and multifrequency bioimpedance analysis measurements and short synacthen tests were made in 20 stable and 25 acutely decompensated patients with cirrhosis.RESULTS:Both groups had similar clinical assessments,cortisol response and total body water(TBW),however the ratio of extracellular water(ECW)/TBW was significantly greater in the trunk(0.420±0.004 vs0.404±0.005),and limbs(R leg 0.41±0.003 vs 0.398±0.003,P<0.05,and L leg 0.412±0.003 vs 0.399±0.003)with decompensated cirrhosis compared to stable cirrhotics,P<0.05).Echocardiogram derived right atrial and ventricular filling and end diastolic pressures and presence of increased left ventricular end diastolic volume and diastolic dysfunction were similar in both groups.The decompensated group had lower systemic blood pressure,mean systolic 101.8±4.3 vs122.4±5.3 and diastolic 58.4±4.1 mmHg vs 68.8±3.1 mmHg respectively,P<0.01,and serum albumin30(27-33)vs 32(31-40.5)g/L,P<0.01.CONCLUSION:Decompensated cirrhotics had greater leg and truncal ECW expansion with lower serum albumin levels consistent with intravascular volume depletion and increased vascular permeability.Andrew Davenport Banwari Argawal Gavin Wright Konstantinos Mantzoukis Rumyana Dimitrova Joseph Davar Panayota Vasianopoulou Andrew K Burroughs 2013World Journal of Hepatology2013,5,8:3

7Efficacy of natural biocide on control of microbial induced corrosion in oil pipelines mediated by Desulfovibrio vulgaris and Desulfovibrio gigas显示文摘We compared the efficacy of a natural biocide with four chemical tetrakishydroxymethyl phosphonium sulfonate,benzyl trimethyl ammonium chloride,and formaldehyde,glutaraldehyde,to control microbial induced corrosion in oil pipelines.The efficacy of biocides were monitored against Desulfovibrio vulgaris and Desulfovibrio gigas in experimental pipes by measuring cell counts,H 2 S production,Fe(Ⅱ) production,production of extracellular polymeric substances and structure of biofilm.The treatment with cow urine hadminimum planktonic cell counts of 3×10 2 CFU/mL as well as biofilm cell counts of 9×10 1 CFU/mL as compared with tetrakishydroxyl methyl phosphonium sulfonate,benzyl trimethyl ammonium chloride,formaldehyde and glutaraldehyde.Sulfide production was the lowest with cow urine (0.08 mmol/L),followed by tetrakishydroxymethyl phosphonium sulfonate 0.72 mmol/L.On day 90 of treatment,Fe(Ⅱ) production was also found to be the lowest with cow urine.The scanning electron microscopic studies indicated that the biofilm bacteria were killed by cow urine.These results demonstrate the cow urine mediated control of microbially induced corrosion,and this is indicative of its potential as a viable substitute of toxic biocides.To the best of our knowledge,this seems to be the first report which screens possible biocidal activity by cow urine as compared to the most common biocides which oil industry is currently using.Meeta Lavania Priyangshu M.Sarma Ajoy K.Mandal Simrita Cheema Banwari Lal 2011Journal of Environmental Sciences2011,23,8:2
