| 1 | Comparison of visual and topographic outcomes of deep-anterior lamellar keratoplasty and penetrating keratoplasty in keratoconus显示文摘AIM:To compare visual,surgical and topographic outcomes of deep anterior lamellar keratoplasty(DALK)and penetrating keratoplasty(PK)for keratoconus(KC).METHODS:In this multicenter,prospective,randomized clinical trial 76 eyes of 71 KC patients operated between January 2011 and July 2014 in 2 tertiary referral hospitals were included. Consecutive patients were alternately selected to receive one of the two surgical methods. Thirty eight eyes underwent DALK with the big-bubble technique and 38 eyes underwent PK.RESULTS:Mean best spectacle corrected visual acuity(BSCVA)at the first postoperative week(P=0.012)and the first postoperative month(P<0.001)was statistically significantly higher in DALK group. The mean BSCVA at12 mo was not significantly different for DALK(0.30±1.99 log MAR)versus PK(0.40±0.33 log MAR)(P=0.104). The76.3% of the eyes had a BSCVA over 0.5 in DALK and 47.4%in PK group(P=0.009). The 7.9% of the eyes had a BSCVA of 1.0 in DALK and 5.3% in PK group(P=0.644). Mean spherical equivalent was-2.94 D in DALK and-3.09 D in PK group.Mean topographic astigmatism was 4.62 D and 4.18 D respectively. Regular topographic patterns were observed in 31(81.6%)of DALK and 29(76.3%)of PK(P=0.574). The most frequent topographic pattern was oblate asymmetric bow tie,seen in 39.5% in DALK and 23.7% in PK. CONCLUSION:Big bubble DALK provides an earlier visual improvement compare to PK. However,visual and topographic outcomes are similar to those in PK at 1y. Postoperative complications including rejection and intraocular pressure elevation are more frequent in PK. DALK is a safer alternative to PK for KC. | 2 | Renoportal anastomosis in living donor liver transplantation with prior proximal splenorenal shunt显示文摘For transplant surgeons, end-stage liver disease with portal venous thrombosis and a previous splenorenal shunt(SRS) is a significant challenge during liver transplantation. Thrombosis of the portal vein can be corrected by surgical interventions, such as portal venous thrombectomy or surgical removal of the thrombosed portal vein. Even also placement of a graft between the mesenteric vein and the graft portal vein can be performed. If these maneuvers fail, a renoportal anastomosis(RPA) can be performed to achieve adequate graft inflow. A 51-year-old male patient who had a history of proximal SRS and splenectomy underwent living donor liver transplantation(LDLT) due to cryptogenic cirrhosis. LDLT was performed with RPA using a cadaveric iliac vein graft. The early postoperative course of the patient was completely uneventful and he was discharged 20 d after transplantation. | 3 | 原发性房角关闭眼激光虹膜切除术后UBM测量参数比较(英文)显示文摘目的:比较激光虹膜切开术前后原发性房角关闭(primary angle closure,PAC)眼超声生物显微镜(ultrasound biomicroscopy,UBM)参数。方法:对46例74眼PAC患者进行了一项前瞻性临床试验。平均年龄58,04±11.33(24.0-82.0)岁。所有病例行完整的眼科检查,前房角镜A超生物测量和超声生物显微镜检查(Vu-max,SonomedInc.,NY,USA)及24-2标准无色差视野检查(SAP,Humphrey Visual Field Analyzer-II i,Carl Zeiss Meditec Inc.,Dublin,CA,USA)。使用配对t检验评估基准和随访的平均值之间的变化。结果:激光周边虹膜切除术(laser peripheral iridotomy,LPI)术后,平均房角从8.02±4.61(0.10-19.60)度增加到17.66±6.39(0.10-32.70)度(P=0.000),房角开放距离500(angle opening distance 500,AOD500)从0.11±0.06(0.01-0.30)mm增加到0.23±0.07(0.13-0.50)mm(P=0.000),平均虹膜厚度(iris thickness,IT)从0.58±0.11(0.33-0.99)mm下降到0.52±0.10(0.25-0.77)mm(P=0.000)。小梁睫状突距离,虹膜睫状突距离和睫状体厚度均未发现显著统计学差异。结论:LPI术后PAC的白种人眼睛的AOD500,虹膜晶体距离和前房角增加,IT和虹膜晶体高度减少。这说明LPI解除了瞳孔阻滞,加深了前房,扩大了房角,减少了虹膜前凸和增厚。 | Bora Yüksel Tümay (O)rsel Memed Acar Sevgi Onat | 2012 | 国际眼科杂志2012，12，1： | 2 |
