| 10 | Anal canal gastrointestinal stromal tumors:Case report and literature review显示文摘Gastrointestinal stromal tumors(GIST)are an uncommon group of tumors of mesenchymal origin.GIST of the anal canal is extremely rare.At present,only 10cases of c-kit positive anal GIST have been reported in the literature.There is no widely accepted treatment approach for this neoplasia.Literature is sparse on imaging evaluation of anal canal GIST,usually described as a lesion in the intersphincteric space.We describe the case of a 73-year-old man with a mass in the anal canal,and no other symptoms.Endoanal ultrasound and magnetic resonance imaging showed a well circumscribed solid nodule in the intersphincteric space.The patient was treated by local excision.Gross pathological examination showed a 7 cm×3.5 cm×3cm mass,and histological examination showed a proliferation of spindle cells,with prominent nuclear palisading.The mitotic count was of 12 mitoses/50 HPF.The tumor was positive for KIT protein,CD34 and vimentin in the majority of cells,and negative for desmin and S100.A diagnosis of GIST,with high risk aggressive behavior was made.An abdomino-perineal resection was discussed,but refused.The follow-up included clinical evaluation and anal ultrasound.After 5 years the patient is well,with maintained continence and no evidence of local recurrence. | Nuno Carvalho Diogo Albergaria Rui Lebre Joo Giria Vitor Fernandes Helena Vidal Maria José Brito | 2014 | World Journal of Gastroenterology2014,20,1: | 1 |

| 14 | Tertiary peritonitis: A disease that should not be ignored显示文摘Intra-abdominal infections can be classified into uncomplicated or complicated(peritonitis). Peritonitis is divided into primary, secondary, and tertiary. Tertiaryperitonitis is the less common but the most severe among peritonitisstratifications, being defined as a recurrent intra-abdominal infection that occurs48 h after a well-succeeded control of a secondary peritonitis. This disease has acomplex pathogenesis that is closely related to the capacity of the peritonealcavity to activate immunological processes. Patients who progress to persistentperitonitis are at an increased risk of developing several infectious complicationssuch as sepsis and multiple organ failure syndrome. Moreover, tertiary peritonitisremains an important cause of hospital death mainly among patients withassociated risk factors. The microbiological profile of organisms causing tertiaryperitonitis is often different from that observed in other types of peritonitis. Inaddition, there is a high prevalence of multidrug-resistant pathogens causing thiscondition, and an appropriate and successful clinical management depends on anearly diagnosis, which can be made easier with the use of clinical scorespresenting a good prediction value during the intensive care unit admission.Complementarily, immediate therapy should be performed to control theinfectious focus and to prevent new recurrences. | 15 | A soil quality index for subtropical sandy soils under different Eucalyptus harvest residue managements显示文摘Eucalyptus harvest residues are attractive energy production resources for the forestry industry.However,their removal can have adverse impacts on soil quality and forest productivity,especially in sandy soils.In this study,we assessed the effects of Eucalyptus harvest residue managements with variable intensity on forest productivity and on physical,chemical,and biological indicators of the soil quality.The experiment was conducted in a Quartzipsamment(33 g kg-1clay)planted with Eucalyptus saligna in Barra do Ribeiro in southern Brazil.Before the Eucalyptus was planted,residues from the previous rotation were subjected to five different management treatments:(1)FRM,in which all forest residues(bark,branches,leaves,and litter)were allowed to remain on the soil and only trunk wood was removed;(2)FRMB,in which was identical to FRM except that bark was also removed;(3)FRMBr,in which only trunk wood and branches were removed;(4)FRR,which involved removing all types of residues(bark,branches,leaves,and litter);and,(5)FRRs,in which all forest residues from the previous rotation were removed,and leaves and branches from the new plantation were prevented from falling onto the soil surface using a shade net.Six years after planting,soil samples were collected at four different depths(0-2.5,2.5-5,5-10,and 10-20 cm)to determine 17 soil chemical,physical,and biological indicators.The results were combined into a soil quality index(SQI)using the principal component analysis approach.The SQI reduced by 30%,in the 0-20 cm layer,due to removal of harvest residues from the previous rotation,and collection of litter before it falls on the ground.The main drivers of SQI reduction were the principal components associated with soil organic matter and biological activity.Furthermore,the SQI was positively linearly related to tree height at P<0.01 and to tree diameter at breast height at P=0.07.The adverse impact on soil quality and forest productivity in our study indicates that removal of Eucalyptus harvest residues from sandy soils should be avoided. | Jackson Freitas Brilhante de Sao José Maurício Roberto Cherubin Luciano Kayser Vargas Bruno Brito Lisboa Josiléia Acordi Zanatta Elias Frank Araújo Cimélio Bayer | 2023 | Journal of Forestry Research2023,34,1: | 0 |

| 17 | Maternal low protein diet induces persistent expression changes in metabolic genes in male rats显示文摘BACKGROUND Perinatal exposure to a poor nutritional environment predisposes the progeny to the development of metabolic disease at the adult age,both in experimental models and humans.Numerous adaptive responses to maternal protein restriction have been reported in metabolic tissues.However,the expression of glucose/fatty acid metabolism-related genes in adipose tissue and liver needs to be described.AIM To evaluate the metabolic impact of perinatal malnutrition,we determined malnutrition-associated gene expression alterations in liver and adipose tissue.METHODS In the present study,we evaluated the alterations in gene expression of glycolytic/Krebs cycle genes(Pyruvate dehydrogenase kinase 4 and citrate synthase),adipogenic and lipolytic genes and leptin in the adipose tissue of offspring rats at 30 d and 90 d of age exposed to maternal isocaloric low protein(LP)diet throughout gestation and lactation.We also evaluated,in the livers of the same animals,the same set of genes as well as the gene expression of the transcription factors peroxisome proliferator-activated receptor gamma coactivator 1,forkhead box protein O1 and hepatocyte nuclear factor 4 and of gluconeogenic genes.RESULTS In the adipose tissue,we observed a transitory(i.e.,at 30 d)downregulation of pyruvate dehydrogenase kinase 4,citrate synthase and carnitine palmitoyl transferase 1b gene expression.Such transcriptional changes did not persist in adult LP rats(90 d),but we observed a tendency towards a decreased gene expression of leptin(P=0.052).The liver featured some gene expression alterations comparable to the adipose tissue,such as pyruvate dehydrogenase kinase 4 downregulation at 30 d and displayed other tissue-specific changes,including citrate synthase and fatty acid synthase upregulation,but pyruvate kinase downregulation at 30 d in the LP group and carnitine palmitoyl transferase 1b downregulation at 90 d.These gene alterations,together with previously described changes in gene expression in skeletal muscle,may account for the metabolic adaptations in response to maternal LP diet and highlight the occurrence of persistent transcriptional defects in key metabolic genes that may contribute to the development of metabolic alterations during the adult life as a consequence of perinatal malnutrition.CONCLUSION We conclude that perinatal malnutrition relays long-lasting transcriptional alterations in metabolically active organs,i.e.,liver and adipose tissue. | Allan de Oliveira Lira JoséLuiz de Brito Alves Mariana Pinheiro Fernandes Diogo Vasconcelos David Filipe Santana João Henrique da Costa-Silva Béatrice Morio Carol Góis Leandro Luciano Pirola | 2020 | World Journal of Diabetes2020,11,5: | 0 |
