| 1 | Dysregulation of iron and copper homeostasis in nonalcoholic fatty liver显示文摘Elevated iron stores as indicated by hyperferritinemiawith normal or mildly elevated transferrin saturation a n d m o s t l y m i l d h e p a t i c i r o n d e p o s i t i o n a r e a characteristic finding in subjects with non-alcoholic fatty liver disease(NAFLD). Excess iron is observed in approximately one third of NAFLD patients and is commonly referred to as the 'dysmetabolic iron overload syndrome'. Clinical evidence suggests that elevated body iron stores aggravate the clinical course of NAFLD with regard to liver-related and extrahepatic disease complications which relates to the fact that excess iron catalyses the formation of toxic hydroxylradicals subsequently resulting in cellular damage. Iron removal improves insulin sensitivity, delays the onset of type 2 diabetes mellitus, improves pathologic liver function tests and likewise ameliorates NAFLD histology. Several mechanisms contribute to pathologic iron accumulation in NAFLD. These include impaired iron export from hepatocytes and mesenchymal Kupffer cells as a consequence of imbalances in the concentrations of iron regulatory factors, such as hepcidin, cytokines, copper or other dietary factors. | 2 | All-dielectric silicon metalens for two-dimensional particle manipulation in optical tweezers显示文摘Dynamic control of compact chip-scale contactless manipulation of particles for bioscience applications remains a challenging endeavor,which is restrained by the balance between trapping efficiency and scalable apparatus.Metasurfaces offer the implementation of feasible optical tweezers on a planar platform for shaping the exerted optical force by a microscale-integrated device.Here we design and experimentally demonstrate a highly efficient silicon-based metalens for two-dimensional optical trapping in the near-infrared.Our metalens concept is based on the Pancharatnam–Berry phase,which enables the device for polarization-sensitive particle manipulation.Our optical trapping setup is capable of adjusting the position of both the metasurface lens and the particle chamber freely in three directions,which offers great freedom for optical trap adjustment and alignment.Two-dimensional(2D)particle manipulation is done with a relatively low-numerical-aperture metalens(NA(ML)=0.6).We experimentally demonstrate both 2D polarization-sensitive drag and drop manipulation of polystyrene particles suspended in water and transfer of angular orbital momentum to these particles with a single tailored beam.Our work may open new possibilities for lab-on-a-chip optical trapping for bioscience applications and microscale to nanoscale optical tweezers. | TEANCHAI CHANTAKIT CHRISTIAN SCHLICKRIEDE BASUDEB SAIN FABIAN MEYER THOMAS WEISS NATTAPORN CHATTHAM AND THOMAS ZENTGRAF | 2020 | Photonics Research2020，8，9： | 6 |

| 4 | Single port laparoscopy in gastroenterology and hepatology: A fine step forward显示文摘Single incision laparoscopy(SIL) has become an emerging technology aiming at a further reduction of abdominal wall trauma in minimally invasive surgery. Available data is encouraging for the safe application of standardized SIL in a wide range of procedures in gastroenterology and hepatology. Compared to technically simple SIL procedures, the merit of SIL in advanced surgeries, such as liver or colorectal interventions, compared to conventional laparsocopy is self-evident without any doubt. SIL has already passed the learning curve and is routinely utilized in expert centers. This minimized approach has allowed to enter a new era of surgical management that can not be acceded without a fruitful combination of prudent training, consistent day-to-day work and enthusiastic motivation for technical innovations. Both, basic and novel technical specifics as well as particular procedures are described herein. | 20 | Successful combination of direct antiviral agents in livertransplanted patients with recurrent hepatitis C virus显示文摘AIM To analyze the safety and efficiency of direct-actingantiviral(DAA) regimens in liver-transplanted patients with hepatitis C virus(HCV) reinfection.METHODS Between January 2014 and December 2016, 39 patients with HCV reinfection after liver transplantation were treated at our tertiary referral center with sofosbuvir(SOF)-based regimens, including various combinations with interferon(IFN), daclatasvir(DAC), simeprivir(SIM) and/or ledipasvir(LDV). Thirteen patients were treated with SOF + IFN ± RBV. Ten patients were treated with SOF + DAC ± RBV. Fiveteen patients were treated with fixed-dose combination of SOF + LDV ± RBV. One patient was treated with SOF + SIM + RBV. Three patients with relapse were retreated with SOF + LDV + RBV. The treatment duration was 12-24 wk in all cases. The decision about the HCV treatment was made by specialists at our transplant center, according to current available or recommended medications.RESULTS The majority of patients were IFN-experienced(29/39, 74.4%) and had a history of hepatocellular carcinoma(26/39, 66.7%) before liver transplantation. Sustained virological response at 12 wk(SVR12) was achieved in 10/13(76.9%) of patients treated with SOF + IFN ± RBV. All patients with relapse were treated with fixed-dose combination of SOF + LDV + RBV. Patients treated with SOF + DAC + RBV or SOF + LDV + RBV achieved 100% SVR12. SVR rates after combination treatment with inhibitors of the HCV nonstructural protein(NS)5 A and NS5 B for 24 wk were significantly higher, as compared to all other therapy regimens(P = 0.007). Liver function was stable or even improved in the majority of patients during treatment. All antiviral therapies were safe and well-tolerated, without need of discontinuation of treatment or dose adjustment of immunosuppression. No serious adverse events or any harm to the liver graft became overt. No patient experienced acute cellular rejection during the study period.
