| 1 | Injection drug use and HIV/AIDS transmission in China显示文摘After nearly three decades of being virtually drug free, use of heroin and other illicit drugs has re-emerged in China as a major public health problem. One result is that drug abuse, particularly heroin injection, has come to play a predomi- nant role in fueling China’s AIDS epidemic. The first outbreak of HIV among China’s IDUs was reported in the border area of Yunnan province between China and Myanmar where drug trafficking is heavy. Since then drug-related HIV has spread to all 31 provinces, autonomous regions and municipalities. This paper provides an overview to HIV/AIDS transmission through injection drug use in China. It begins with a brief history of the illicit drug trade in China, followed by a discussion of the emergence of drug related AIDS, and a profile of drug users and their sexual partners who have contracted the virus or who are vulnerable to infection. | 2 | How we can improve patients’comfort after Milligan- Morgan open haemorrhoidectomy显示文摘AIM:To demonstrate the value of Diosmin(flavonidic fraction)in the management of post-haemorhoidectomic symptoms.METHODS:Eighty-six consecutive patients with grades ⅢandⅣacute mixed hemorrhoids admitted to the Anorectal Surgical Department of First Affiliated Hospital,Xinjiang Medical University from April 2009 to April 2010,were enrolled in this study.An observerblinded,randomized trial was conducted to compare post-haemorhoidectomic symptoms with use of Diosmin flavonidic fraction vs placebo.Eighty-six patients were randomly allocated to receive Diosmin flavonidic fraction 500 mg for 1 wk(n=43)or placebo(n=43).The Milligan-Morgan open haemorrhoidectomy was performed by a standardized diathermy excision method.Pain,bleeding,heaviness,pruritus,wound edema and mucosal discharge were observed after surgery.The postoperative symptoms and hospitalization time were recorded.RESULTS:The mean age of the Diosmin group and controls was 53.2 and 51.3 years,respectively.In Diosmin group,haemorrhoid piles were of the third degree in 33 patients and the fourth degree in 10;and in the control group,29 were of the third degree and 14 were of the fourth degree.There was no statistically significance in age,gender distribution,degree and number of excised haemorrhoid piles,and the mean duration of haemorrhoidal disease between the two groups.There was a statistically significant improvement in pain,heaviness,bleeding,pruritus from baseline to the 8th week after operation(P<0.05).Patients taking Diosmin had a shorter hospitalization stay after surgery(P< 0.05).There was also a significant improvement on the proctoscopic appearance(P<0.001).However,there was no statistical difference between the two groups in terms of wound mucosal discharge.Two patients experienced minor bleeding at the 8th week in Diosmin group,and underwent surgery.CONCLUSION:Diosmin is effective in alleviating postoperational symptoms of haemorrhoids.Therefore,it should be considered for the initial treatment after haemorrhoid surgery.However,further prospective randomized trials are needed to confirm the findings of this study. | Ma-Mu-Ti-Jiang A ba-bai-ke-re Hong-Guo Huang Wen-Ni Re Kai Fan Hui Chu Er-Ha-Ti Ai Mai-Mai-Ti-Tu-Er-Xun KE Li-Mu Yi-Rui Wang Hao Wen | 2011 | World Journal of Gastroenterology2011，17，11： | 13 |

| 6 | Age and menopausal status are important factors influencing the serum human epididymis secretory protein 4 level:a prospective cross-sectional study in healthy Chinese people显示文摘Background::Human epididymis secretory protein 4(HE4)is a new ovarian cancer biomarker.The factors influencing HE4 levels are not clear,and the reference data in China are limited.Here,we aim to evaluate the effects of menopause and age on HE4 levels and to provide a possible reference value for HE4 in healthy Chinese people.Methods::A total of 2493 healthy females aged 40 years or older were recruited from March 2013 to March 2017 with the cooperation of four medical institutions across Beijing,China.The serum levels of HE4 and cancer antigen 125(CA125)were measured by enzyme-linked immunosorbent assay.The Wilcoxon rank-sum test of variance and a stratified analysis were used to analyze the relationships among age,menopausal status,and levels of HE4 or CA125.Confidence intervals(5%-95%)were determined for reference ranges in different populations.Results::There was a statistically significant difference in median HE4 levels between the post-menopausal(n=2168)and premenopausal groups(n=325)(36.46 vs.24.04 pmol/L,Z=-14.41,P<0.001).HE4 increased significantly with age in the post-menopausal groups(H=408.18,P<0.001)but not in the pre-menopausal subjects(Z=-0.43,P=0.67).The upper 95th percentile of HE4 levels were 44.63 pmol/L for pre-menopausal women,78.17 pmol/L for post-menopausal women,and 73.3 pmol/L for all women.In the post-menopausal population,the HE4 reference ranges were 13.15 to 47.31,14.31 to 58.04,17.06 to 73.51,24.50 to 115.25,and 35.71 to 212.37 pmol/L for different age groups from forty divided by decade.The CA125 level was affected mainly by menopausal status and not age.Conclusions::Menopausal status and age were both important factors influencing the level of HE4,and age affected HE4 levels mainly in post-menopausal women.The HE4 level was higher in the post-menopausal population than in the pre-menopausal population and increased with age. | Hong-Yan Cheng Lin Zeng Xue Ye Rui-Qiong M a Zhi-Jian Tang Hong-Ling Chu Yi-M ing Zhao Li-Rong Zhu Yu-Nong Gao Xiao-Hong Chang Heng Cui | 2020 | Chinese Medical Journal2020，，11： | 6 |

| 13 | Soil microbial activity and community structure as affected by exposure to chloride and chloride-sulfate salts显示文摘Mixed or chloride salty ions dominate in saline soils, and exert wide-ranging adversely affect on soil biological processes and soil functions. The objectives of this study were to(1) explore the impacts of mixed(0, 3, 6, 10, 20 and 40 g Cl~–/SO_4^(2–)salt/kg dry soil) and chloride(0, 1.5, 3, 5, 8 and 15 g Cl~– salt/kg dry soil) salts on soil enzyme activities, soil physiological functional(Biolog) profiles and microbial community structure by using soil enzymatic, Biolog-Eco microplates as well as denaturing gradient gel electrophoresis(DEEG) methods, and(2) determine the threshold concentration of soil electronic conductivity(EC_(1:5)) on maintaining the functional and structural diversity of soil microbial community. The addition of either Cl~– or mixed Cl~–/SO_4^(2–)salt obviously increased soil EC, but adversely affected soil biological activities including soil invertase activity, soil microbial biomass carbon(MBC) and substrate-induced respiration(SIR). Cl~– salt showed a greater deleterious influence than mixed Cl~–/SO_4^(2–)salt on soil enzymes and MBC, e.g., the higher soil MBC consistently appeared with Cl~–/SO_4^(2–)instead of Cl– treated soil. Meanwhile, we found that SIR was more reliable than soil basal respiration(SBR) on explaining the changes of soil biological activity responsive to salt disturbance. In addition, microbial community structures of the soil bacteria, fungi, and Bacillus were obviously affected by both salt types and soil EC levels, and its diversity increased with increasing of mixed Cl~–/SO_4^(2–)salt rates, and then sharply declined down after it reached critical point. Moreover, the diversity of fungal community was more sensitive to the mixed salt addition than other groups. The response of soil physiological profiles(Biolog) followed a dose-response pattern with Cl~–(R^2=0.83) or mixed Cl~–/SO_4^(2–)(R^2=0.89) salt.
