| 1 | Gastro-intestinal toxicity of chemotherapeutics in colorectal cancer:The role of inflammation显示文摘Chemotherapy-induced diarrhea(CID)is a common and often severe side effect experienced by colorectal cancer(CRC)patients during their treatment.As chemotherapy regimens evolve to include more efficacious agents,CID is increasingly becoming a major cause of dose limiting toxicity and merits further investigation.Inflammation is a key factor behind gastrointestinal(GI)toxicity of chemotherapy.Different chemotherapeutic agents activate a diverse range of pro-inflammatory pathways culminating in distinct histopathological changes in the small intestine and colonic mucosa.Here we review the current understanding of the mechanisms behind GI toxicity and the mucositis associated with systemic treatment of CRC.Insights into the inflammatory response activated during this process gained from various models of GI toxicity are discussed.The inflammatory processes contributing to the GI toxicity of chemotherapeutic agents are increasingly being recognised as having an important role in the development of anti-tumor immunity,thus conferring added benefit against tumor recurrence and improving patient survival.We review the basic mechanisms involved in the promotion of immunogenic cell death and its relevance in the treatment of colorectal cancer.Finally,the impact of CID on patient outcomes and therapeutic strategies to prevent or minimise the effect of GI toxicity and mucositis are discussed. | Chun Seng Lee Elizabeth J Ryan Glen A Doherty | 2014 | World Journal of Gastroenterology2014,20,14: | 10 |
| 2 | Two-year outcomes of ab interno trabeculectomy with the Trabectome for Chinese primary open angle glaucoma: a retrospective multicenter study显示文摘AIM: To evaluate the 2-year efficacy and safety of ab interno trabeculectomy with the Trabectome in Chinese primary open angle glaucoma(POAG) patients.METHODS: This was a multicenter, retrospective, observational study and included POAG patients with or without visually-significant cataracts. The Chinese patients were enrolled from three glaucoma centers and a group of comparable Japanese POAG patients was analyzed from our international Trabectome database. The patients received Trabectome or a combined surgery with phacoemulsification and intraocular lens implantation. The primary outcome was intraocular pressure(IOP) reduction. Secondary outcomes included reduction of glaucoma medications, surgical complications, and success at 2 y. Success was defined as: 1) IOP≤21 mm Hg and at least 20% IOP reduction from baseline after 3 mo at any two consecutive visits; 2) no additional glaucoma surgery required.RESULTS: A total of 42 Chinese POAG patients from three glaucoma centers were enrolled. Twelve patients underwent Trabectome surgery combined with phacoemulsification and intraocular lens implantation while the remainder underwent Trabectome surgery alone. Thirteen patients had a history of failed glaucoma surgery and were considered as complicated cases. In China data, the mean preoperative IOP was 21.4±1.23 mm Hg. The Trabectome lowered IOP to 17.9±1.8 mm Hg at 2 y(P=0.05). The number of glaucoma medications also decreased significantly from a baseline of 2.0±0.9 to 1.1±0.8 at 2 y post-surgery(P=0.04). The overall 2-year success rate was 78%, with patients undergoing combined surgery having a higher success rate compared with those undergoing Trabectome surgery alone(100% vs 76%). In Japan data, the mean preoperative IOP was 20.8±7.7 mm Hg. The Trabectome lowered IOP to 12.20±2.0 mm Hg at 2 y. The number of glaucoma medications also decreased significantly from a baseline of 2.1±0.9 to 3.4±0.6 at 2 y post-surgery. | 3 | Physiological responses of artificial moss biocrusts to dehydration-rehydration process and heat stress on the Loess Plateau, China显示文摘Ex-situ cultivation of biological soil crusts(biocrusts) is a promising technology to produce materials that can induce the recovery of biocrusts in the field for the purposes of preventing soil erosion and improving hydrological function in degraded ecosystems. However, the ability of artificially cultivated biocrusts to survive under adverse field conditions, including drought and heat stresses, is still relatively unknown. Mosses can bolster biocrust resistance to the stresses(e.g., drought and heat) and the resistance may be introduced prior to field cultivation. In this study, we subjected the well-developed artificial moss biocrusts(dominant species of Didymodon vinealis(Brid.) Zand.) that we cultivated in the phytotron to a dehydration-rehydration experiment and also a heat stress experiment and measured the activities of protective enzymes(including peroxidase(POD), superoxide dismutase(SOD), and catalase(CAT)) and the contents of osmoregulatory substances(including soluble proteins and soluble sugars) and malondialdehyde(MDA, an indicator of oxidative stress) in the stem and leaf fragments of mosses. The results showed that, during the dehydration process, the activities of protective enzymes and the contents of osmoregulatory substances and MDA gradually increased with increasing duration of drought stress(over 13 days). During the rehydration process, values of these parameters decreased rapidly after 1 d of rehydration. The values then showed a gradual decrease for 5 days, approaching to the control levels. Under heat stress(45°C), the activities of protective enzymes and the content of soluble proteins increased rapidly within 2 h of heat exposure and then decreased gradually with increasing duration of heat exposure. In contrast, the contents of soluble sugars and MDA always increased gradually with increasing duration of heat exposure. This study indicates that artificial moss biocrusts possess a strong drought resistance and this resistance can be enhanced after a gradual dehydration treatment. This study also indicates that artificial moss biocrusts can only resist short-term heat stress(not long-term heat stress). | 4 | Ipsilateral breast tumor recurrence in early stage breast cancer patients treated with breast conserving surgery and adjuvant radiation therapy: Concordance of biomarkers and tumor location from primary tumor to in-breast tumor recurrence显示文摘BACKGROUND Patients with an in-breast tumor recurrence(IBTR)after breast-conserving therapy have a high risk of distant metastasis and disease-related mortality.Classifying clinical parameters that increase risk for recurrence after IBTR remains a challenge.AIM To describe primary and recurrent tumor characteristics in patients who experience an IBTR and understand the relationship between these characteristics and disease outcomes.METHODS Patients with stage 0-II breast cancer treated with lumpectomy and adjuvant radiation were identified from institutional databases of patients treated from 2003-2017 at our institution.Overall survival(OS),disease-free survival,and local recurrence-free survival(LRFS)were estimated using the Kaplan Meier method.We identified patients who experienced an isolated IBTR.Concordance of hormone receptor status and location of tumor from primary to recurrence was evaluated.The effect of clinical and treatment parameters on disease outcomes was also evaluated.RESULTS We identified 2164 patients who met the eligibility criteria.The median follow-up for all patients was 3.73[interquartile range(IQR)2.27-6.07]years.Five-year OS was 97.7%(95%CI:96.8%-98.6%)with 28 deaths;5-year LRFS was 98.0%(97.2-98.8)with 31 IBTRs.We identified 37 patients with isolated IBTR,19(51.4%)as ductal carcinoma in situ and 18(48.6%)as invasive disease,of whom 83.3%had an in situ component.Median time from initial diagnosis to IBTR was 1.97(IQR:1.03-3.5)years.Radiotherapy information was available for 30 of 37 patients.Median whole-breast dose was 40.5 Gy and 23 patients received a boost to the tumor bed.Twenty-five of thirty-two(78.1%)patients had concordant hormone receptor status,HER-2 receptor status,and estrogen receptor(ER)(P=0.006)and progesterone receptor(PR)(P=0.001)status from primary to IBTR were significantly associated.There were no observed changes in HER-2 status from primary to IBTR.The concordance between quadrant of primary to IBTR was 10/19[(62.2%),P=0.008].Tumor size greater than 1.5 cm(HR=0.44,95%CI:0.22-0.90,P=0.02)and use of endocrine therapy upfront(HR=0.36,95%CI:0.18-0.73,P=0.004)decreased the risk of IBTR.CONCLUSION Among patients with early stage breast cancer who had breast conserving surgery treated with adjuvant RT,ER/PR status and quadrant were highly concordant from primary to IBTR.Tumor size greater than 1.5 cm and use of adjuvant endocrine therapy were significantly associated with decreased risk of IBTR. | Juhi M Purswani Fauzia Shaikh S Peter Wu Jennifer Chun Kim Freya Schnabel Nelly Huppert Carmen A Perez Naamit K Gerber | 2020 | World Journal of Clinical Oncology2020,11,1: | 2 |
