| 1 | Update on the applications and limitations of alpha-fetoprotein for hepatocellular carcinoma显示文摘Alpha-fetoprotein(AFP)is an oncofetal glycoprotein that has been used as a tumor marker for hepatocellular carcinoma(HCC)in combination with ultrasound and other imaging modalities.Its utility is limited because of both low sensitivity and specificity,and discrepancies among the different methods of measurements.Moreover,its accuracy varies according to patient characteristics and the AFP cut-off values used.Combination of AFP with novel biomarkers such as AFP-L3,Golgi specific membrane protein(GP73)and des-gamma-carboxyprothrombin significantly improved its accuracy in detecting HCC.Increased AFP level could also signify severity of hepatic destruction and subsequent regeneration and is commonly observed in patients with acute and chronic liver conditions and cirrhosis.Hereditary and other non-hepatic disorders can also cause AFP elevation. | Hira Hanif Mukarram Jamat Ali Ammu T Susheela Iman Waheed Khan Maria Alejandra Luna-Cuadros Muzammil Muhammad Khan Daryl Tan-Yeung Lau | 2022 | World Journal of Gastroenterology2022，28，2： | 9 |
| 2 | Intrahepatic distribution of hepatitis B virus antigens in patients with and without hepatocellular carcinoma显示文摘AIM: To study the intrahepatic expression of hepatitis B surface antigen(HBs Ag) and hepatitis B core antigen(HBc Ag) in chronic hepatitis B patients with and without hepatocellular carcinoma. METHODS: A total of 33 chronic hepatitis B patients(mean age of 40.3 ± 2.5 years), comprising of 14 HBe Ag positive and 19 HBe Ag negative patients; and 13 patients with hepatitis B virus related hepatocellular carcinoma(mean age of 49.6 ± 4.7 years), were included in our study. Immunohistochemical staining for HBc Ag and HBs Ag was done using standard streptavidin-biotin-immunoperoxidase technique on paraffin-embedded liver biopsies. The HBc Agand HBs Ag staining distributions and patterns were described according to a modified classification system. RESULTS: Compared to the HBe Ag negative patients, the HBe Ag positive patients were younger, had higher mean HBV DNA and alanine transaminases levels. All the HBe Ag positive patients had intrahepatic HBc Ag staining; predominantly with 'diffuse' distribution(79%) and 'mixed cytoplasmic/nuclear ' pattern(79%). In comparison, only 5% of the HBe Ag-negative patients had intrahepatic HBc Ag staining. However, the intrahepatic HBs Ag staining has wider distribution among the HBe Ag negative patients, namely; majority of the HBe Ag negative cases had 'patchy' HBs Ag distribution compared to 'rare' distribution among the HBe Ag positive cases. All but one patient with HCC were HBe Ag negative with either undetectable HBV DNA or very low level of viremia. Intrahepatic HBc Ag and HBs Ag were seen in 13(100%) and 10(77%) of the HCC patients respectively. Interestingly, among the 9 HCC patients on anti-viral therapy with suppressed HBV DNA, HBc Ag and HBs Ag were detected in tumor tissues but not the adjacent liver in 4(44%) and 1(11%) patient respectively. | 14 | Simple non-invasive biomarkers of advanced fibrosis in the evaluation of non-alcoholic fatty liver disease显示文摘Background and aims.Non-alcoholic fatty liver disease(NAFLD)is a common,morbid disease with profound implications for the overall health of the patient.We set out to determine the clinical predictors of advanced histology in the referral population.Methods.We performed a retrospective review of all biopsy-proven NAFLD patients,including 358 unique patients first seen between 1996 and 2009.Liver histology and ultrasound images were reviewed prospectively by clinicians who were blinded to clinical information and test indication.Results.Compared with men,women tended to present at an older age(51.4-10.6 vs 45.3-11.2 years,P<0.001),were more likely to be Caucasian(P=0.003),less likely to present with an elevated alanine aminotransferase(ALT)(75.2%vs 88.8%),and more likely to have advanced non-alcoholic steatohepatitis(NASH)(44.7%vs 29.9%;P=0.04)and advanced fibrosis(23.3%vs 14.1%;P=0.03).In multivariate logistic regression,body mass index(BMI)-30 kg/m^(2)(odds ratio(OR)2.21;95%confidential interval(CI):1.23–4.08),female gender(OR 1.76;95%CI:1.01–3.10)and aspartate aminotransferase(AST)>40 IU/L(OR 2.00;95%CI:1.14–3.55)were associated with a NAFLD activity score>4.The sensitivity and specificity of an AST to platelet ratio index(APRI)>1 for significant fibrosis was 30.0%(95%CI:17.2–45.4%)and 92.8%(95%CI:88.2-95.8%),respectively;the likelihood ratio is 4.2.In multivariate logistic regression,APRI>1 was the most significant predictor of advanced fibrosis(OR 3.85;95%CI:1.55–9.59).In patients without ultrasound-detected steatosis,20%had advanced fibrosis and 16.7%had active NASH.Conclusion.Patients with suspected NAFLD should routinely be evaluated for advanced liver disease,including non-invasive indices of fibrosis such as APRI,and serious consideration given to liver biopsy. | Elliot B.Tapper Katherine Krajewski Michelle Lai Tracy Challies Robert Kane Nezam Afdhal Daryl Lau | 2014 | Gastroenterology Report2014，2，4： | 0 |
