| 1 | Serum levels of undercarboxylated osteocalcin are related to cardiovascular risk factors in patients with type 2 diabetes mellitus and healthy subjects显示文摘AIM To determine a potential relationship between serum undercarboxylated(uc OC) concentration and cardiovascular risk factors in type 2 diabetes(T2D) patients and healthy subjects(HS).METHODS A cross-sectional study was conducted on 140 subjects classified into two groups, 70 with T2D and 70 HS. Medical history and physical examination with anthropometric measurements were obtained from all subjects. Body fat percentage was determined by bioelectrical impendency analysis. Serum uc OC concentration was determined by enzyme immunoassay,while serum levels of insulin and hsC RP were obtained using high sensitivity enzyme-linked immunosorbent assay. Insulin resistance was determined using the homeostasis model assessment-IR. Lipid profile [triglycerides,total cholesterol(TC), high-density lipoproteins(HDL-c),low density lipoproteins(LDL-c), very low-density lipoproteins] was determined by spectrophotometry and standard formulas when applicable. RESULTS The T2D patient group showed significantly higher values of waist circumference, waist-to-hip ratio, systolic blood pressure(SBP), diastolic blood pressure(DBP),current smoking, and alcohol use when compared to the HS group(P < 0.05). We observed a significantly lower serum ucO C concentration in T2D than in HS(1.5 ± 1.4vs 2.3 ± 1.8, P < 0.05). In the whole study population,ucO C concentration was inversely correlated with body mass index(BMI)(r =-0.236, P < 0.05), fasting plasma glucose(r =-0.283, P < 0.01) and HDL-c(r =-0.255,P < 0.05); and positively correlated with LDL-c/HDL-c ratio(r = 0.306, P < 0.05) and TC/HDL-c ratio(r =0.284, P < 0.05). In the T2D group, serum uc OC concentration was inversely correlated with BMI(r =-0.310, P < 0.05) and body-fat percentage(r =-0.311,P < 0.05), and positively correlated with DBP(r = 0.450,P < 0.01). In HS group a positive correlation between serum levels of uc OC and SBP(r = 0.277, P < 0.05)was observed. | 2 | Liver transplantation for metastatic neuroendocrine tumor: A case report and review of the literature显示文摘Neuroendocrine tumors are divided into gastrointestinal carcinoids and pancreatic neuroendocrine tumors. The WHO has updated the classification of these lesions and has abandoned the term 'carcinoid'. Both types of tumors are divided into functional and non-functional tumors. They are characterized by slow growth and frequent metastasis to the liver and may be limited to the liver for long periods. The therapeutic approach to hepatic metastases should consider the number and distribution of the liver metastases as well as the severity of symptoms related to hormone production and tumor bulk. Surgery is generally considered as the first line therapy. In patients with unresectable liver metastases,alternative treatments are dependent on the type and the growth rate. Initial treatments consist of long acting somatostatin analogs and/or interferon. Streptozocinbased chemotherapy is usually reserved for symptomatic patients with rapidly advancing disease, but generally the therapy is poorly tolerated and its effects are short-lived.Locoregional therapy directed such as hepatic-artery embolization and chemoembolization, radiofrequency thermal ablation and cryosurgery, is often used instead of systemic therapy, if the disease is limited to the liver.However, liver transplantation should be considered in patients with neuroendocrine metastases to the liver that are not accessible to curative or cytoreductive surgery and if medical or locoregional treatment has failed and if there are life threatening hormonal symptoms. | 3 | Long-term results of endosurgical and open surgical approach for Zenker diverticulum显示文摘AIM: To assess the effectiveness of minimally invasive versus traditional open surgical approach in the treatment of Zenker diverticulum. METHODS: Between 1976 and 2006, 297 patients underwent transoral stapling (n = 181) or stapled diverticulectomy and cricopharyngeal myotomy (n = 116). Subjective and objective evaluations of the outcome of the two procedures were made at 1 and 6 mo after operation, and then every year. Long-term follow-up data were available for a subgroup of patients at a minimum of 5 and 10 years. RESULTS: The operative time and hospital stay were markedly reduced in patients undergoing the endosurgical approach. Overall, 92% of patients undergoing the endosurgical approach and 94% of those undergoing the open approach were symptom-free or were significantly improved after a median follow-up of 27 and 48 mo, respectively. At a minimum follow-up of 5 and 10 years, most patients were asymptomatic after both procedures, except for those individuals undergoing an endosurgical procedure for a small diverticulum (< 3 cm). CONCLUSION: Both operations relieve the outflow obstruction at the pharyngoesophageal junction, indicating that cricopharyngeal myotomy has an important therapeutic role in this disease independent of the resection of the pouch and of the surgical approach. | 5 | Erosion problem in tool steel using cold box core-making process显示文摘This paper presents the erosion results of the AISI H13 steel impinged by resin-bonded silica sand, using a testing rig that closely simulates the real blowing conditions during industrial core-making. Steel specimens were heat treated to obtain hardness of 294, 445 and 595 HV200 (29, 45 and 55 HRC). Erosion tests were carried out at impingement angles from 20° to 90° and air drag pressures of 1.38, 2.07 and 2.76 bar (20, 30 and 40 psi). The main results are summarized as follows:(i) The harder material, the lower erosion;(ii) the maximum erosion rate is at 30°;(iii) Little difference in erosion rate at impact angle of 60° and 90° for a constant pressure tested regardless of the hardness level;(iv) As the pressure increases, so does the erosion rate, being more sensitive for low impact angles. | 7 | Hepatitis B vaccination in patients with inflammatory bowel disease显示文摘AIM:To determine the prevalence for hepatitis B virus(HBV) and HBV screening and vaccination practices for inflammatory bowel disease(IBD).METHODS:This study is a retrospective,cross-sectional observational study.A retrospective chart review was performed in 500 patients who have been consecutively treated for IBD between September 2008 and January 2013 at the Rush University Medical Center Gastroenterology section.The patients were identified through the electronic medical record with the criteria that they attended the gastroenterology clinic,and that they had a diagnosis of IBD at the time of visit discharge.Once identified,each record was analyzed to determine whether the subject had been infected with HBV in the past,already been vaccinated against HBV,or advised to get vaccinated and followed through with the recommended vaccination.RESULTS:About 254 out of 500 patients(51%) had HBV screening ordered.Among those ordered to have screening tests,86% followed through with HBV serology.Gastroenterology physicians had significantly different screening ratios from each other(P < 0.001).There were no significant differences in the ratios of HBV screening when IBD specialists were compared to other gastroenterology physicians(0.505 ± 0.023 vs 0.536 ± 0.066,P = 0.66).Of those 220 patients screened,51% of IBD patients were found not to be immune against HBV.Approximately 50% of gastroenterology physicians recommended HBV vaccinations to their patients in whom serology was negative for antibodies against HBV.IBD specialists recommended vaccinations to a higher percentage of their patients compared to other gastroenterology physicians(0.168 ± 0.019 vs 0.038 ± 0.026,P = 0.015).Present and/or past HBV infection was found in 3.6% of the patients who had serology checked.There was no statistically significant difference in the prevalence of hepatitis B surface antigen(HBsAg) between our study and that reported in previous studies done in Spain(4/220 vs 14/2076 respectively,P = 0.070); and in France(4/220 vs 3/315 respectively,P = 0.159).But,the prevalence of anti-HBcAb in this study was less than that reported in the study in Spain(7/220 vs 155/2076 respectively,P = 0.006); and was not sig-nificantly different from that reported in the study in France(7/220 vs 8/315 respectively,P = 0.313).CONCLUSION:The prevalence of HBsAg in our IBD patients was not higher than previously reported European studies.Most IBD patients are not routinely screened or vaccinated against HBV at a tertiary referral center in the United States. | Ruwaida Ben Musa Anuhya Gampa Sanjib Basu Ali Keshavarzian Garth Swanson Michael Brown Rana Abraham Keith Bruninga John Losurdo Mark DeMeo Sohrab Mobarhan David Shapiro Ece Mutlu | 2014 | World Journal of Gastroenterology2014，20，41： | 2 |
