1Adjuvant therapy sparing in rectal cancer achieving complete response after chemoradiation显示文摘AIM:To evaluate the long-term results of conventional chemoradiotherapy and laparoscopic mesorectal excision in rectal adenocarcinoma patients without adjuvant therapy.METHODS:Patients with biopsy-proven adenocarcinoma of the rectum staged cT3-T4 by endoscopic ultrasound or magnetic resonance imaging received neoadjuvant continuous infusion of 5-fluorouracil for five weeks and concomitant radiotherapy.Laparoscopic surgery was planned after 5-8 wk.Patients diagnosed with ypT0N0 stage cancer were not treated with adjuvant therapy according to the protocol.Patients with ypT1-2N0 or ypT3-4 or N+were offered 5-fluorouracil-based adjuvant treatment on an individual basis.An external cohort was used as a reference for the findings.RESULTS:One hundred and seventy six patients were treated with induction chemoradiotherapy and 170underwent total mesorectal excision.Cancer staging of ypT0N0 was achieved in 26/170(15.3%)patients.After a median follow-up of 58.3 mo,patients withypT0N0 had five-year disease-free and overall survival rates of 96%(95%CI:77-99)and 100%,respectively.We provide evidence about the natural history of patients with localized rectal cancer achieving a complete response after preoperative chemoradiation.The inherent good prognosis of these patients will have implications for clinical trial design and care of patients.CONCLUSION:Withholding adjuvant chemotherapy after complete response following standard neoadjuvant chemoradiotherapy and laparoscopic mesorectal excision might be safe within an experienced multidisciplinary team.Xabier García-Albéniz Rosa Gallego Ralf Dieter Hofheinz Gloria Fernández-Esparrach Juan Ramón Ayuso-Colella Josep Antoni Bombí Carles Conill Miriam Cuatrecasas Salvadora Delgado Angels Ginés Rosa Miquel Mario Pagés Estela Pineda Verónica Pereira Aarón Sosa Oscar Reig Iván Victoria Luis Feliz Antonio María de Lacy Antoni Castells Iris Burkholder Andreas Hochhaus Joan Maurel 2014World Journal of Gastroenterology2014,20,42:5

6HP0953-hypothetical virulence factor overexpresion and localization during Helicobacter pylori infection of gastric epithelium显示文摘BACKGROUND The high prevalence and persistence of Helicobacter pylori(H. pylori) infection, as well as the diversity of pathologies related to it, suggest that the virulence factors used by this microorganism are varied. Moreover, as its proteome contains 340hypothetical proteins, it is important to investigate them to completely understand the mechanisms of its virulence and survival. We have previously reported that the hypothetical protein HP0953 is overexpressed during the first hours of adhesion to inert surfaces, under stress conditions, suggesting its role in the environmental survival of this bacterium and perhaps as a virulence factor.AIM To investigate the expression and localization of HP0953 during adhesion to an inert surface and against gastric(AGS) cells.METHODS Expression analysis was performed for HP0953 during H. pylori adhesion. HP0953 expression at 0,3, 12, 24, and 48 h was evaluated and compared using the Kruskal-Wallis equality-of-populations rank test. Recombinant protein was produced and used to obtain polyclonal antibodies for immunolocalization. Immunogold technique was performed on bacterial sections during adherence to inert surfaces and AGS cells, which was analyzed by transmission electron microscopy. HP0953 protein sequence was analyzed to predict the presence of a signal peptide and transmembrane helices, both provided by the ExPASy platform, and using the GLYCOPP platform for glycosylation sites. Different programs, via, I-TASSER, RaptorX, and HHalign-Kbest, were used to perform three-dimensional modeling.RESULTS HP0953 exhibited its maximum expression at 12 h of infection in gastric epithelium cells.Immunogold technique revealed HP0953 localization in the cytoplasm and accumulation in some peripheral areas of the bacterial body, with greater expression when it is close to AGS cells.Bioinformatics analysis revealed the presence of a signal peptide that interacts with the transmembrane region and then allows the release of the protein to the external environment. The programs also showed a similarity with the Tip-alpha protein of H. pylori. Tip-alpha is an exotoxin that penetrates cells and induces tumor necrosis factor alpha production, and HP0953 could have a similar function as posttranslational modification sites were found;modifications in turn require enzymes located in eukaryotic cells. 8治疗用单克隆抗体的超敏反应:表型和内型显示文摘背景单克隆抗体的广泛的应用导致了超敏反应(HSRs)发生率的增加,这阻碍了单克隆抗体作为一线治疗方法在临床上的应用。HRSs的症状、诊断方法及定向治疗方法均尚未标准化。目的我们基于临床表型、潜在的内型和生物标志物进行脱敏的治疗方法提出一种新的基于循证医学的m Ab HSR分类方法。方法描述了104例患者对16种m Abs HSR的表型、内型和生物标志物,并与526次皮下及静脉脱敏治疗结果进行比较。结果初始反应呈现4种模式:Ⅰ型样反应占63%,细胞因子释放反应占13%,混合反应占21%,Ⅳ型迟发反应占3%。与之形成鲜明对比的是,在脱敏治疗期间突发的HSRs为23%,其中52%为细胞因子释放反应,32%为Ⅰ型反应,12%为混合型反应,4%为Ⅰ型伴Ⅳ型迟发超敏反应。58例患者对10种m Abs进行皮肤试验,41%的患者出现阳性。在脱敏治疗期间,1例患者血清类胰蛋白酶水平升高,8例患者IL-6水平升高,均为细胞因子释放表型。结论 m Abs引起的HSRs可界定为Ⅰ型、细胞因子释放型、混合(Ⅰ型/细胞因子释放型)型和Ⅳ型反应,可通过生物标志物如皮肤试验、类胰蛋白酶和IL-6进行鉴别分型。当确诊为m Abs HSR时,这些表型有助于患者接受个性化和精准医学治疗,并为脱敏治疗的实施提供依据。脱敏治疗是一种安全有效的治疗手段,可使出现过变态反应的m Abs仍能作为一线治疗方法。Ghislaine Annie C.Isabwe Marlene Garcia Neuer Leticia de las Vecillas Sanchez Donna-Marie Lynch FNP-BC,Kathleen Marquis Mariana Castells 顾建青(翻译) 王良录(审校) 2018中华临床免疫和变态反应杂志2018,12,5:1
