| 2 | Should we resect colorectal cancer in patients over the age of 85?显示文摘BACKGROUND The prevalence of colorectal cancer in the elderly is rising,with increasing numbers of older patients undergoing surgery.However,there is a paucity of information on the surgical outcomes and operative techniques used in this population.AIM To evaluate the post-operative outcomes for patients≥85 years old following colorectal cancer resection as well as evaluating the outcomes of laparoscopic resection of colorectal cancer in patients over 85.METHODS Patients who underwent colorectal cancer resection at our institution between January 2010 and December 2018 were included.The study was divided into two parts.For part one,patients were divided into two groups based on age:Those age≥85 years old(n=48)and those aged 75-84 years old(n=136).Short term surgical outcomes and clinicopathological features were compared using appropriate parametric and non-parametric testing.For part two,patient’s over 85 years old were divided into two groups based upon operative technique:Laparoscopic(n=37)vs open(n=11)colorectal resection.Short-term postoperative outcomes of each approach were assessed.RESULTS The median length of stay between patients over 85 and those aged 75-85 was eight days,with no statistically significant difference between the groups(P=0.29).No significant difference was identified between the older and younger groups with regards to severity of complications(P=0.93),American Society of Anaesthesiologists grading(P=0.43)or 30-d mortality(2%vs 2%,P=0.96).Patients over 85 who underwent laparoscopic colorectal resection were compared to those who underwent an open resection.The median length of stay between the groups was similar(8 vs 9 d respectively)with no significant difference in length of stay(P=0.18).There was no significant difference in 30-d mortality rates(0%vs 9%,P=0.063)or severity of complication grades(P=0.46)between the laparoscopic and open surgical groups.CONCLUSION No significant short term surgical differences were identified in patients≥85 years old when compared to those 75-85 years old.There is no difference in short term surgical outcomes between laparoscopic or open colorectal resections in patients over 85. | David E Flynn Derek Mao Stephanie Yerkovich Robert Franz Harish Iswariah Andrew Hughes Ian Shaw Diana Tam Manju Chandrasegaram | 2021 | World Journal of Gastrointestinal Oncology2021，13，3： | 0 |
| 3 | The Hong Kong consensus statements on unresectable hepatocellular carcinoma:narrative review and update for 2021显示文摘Background and Objective:Hong Kong,like many parts of Asia,faces a high burden of hepatocellular carcinoma(HCC)caused by high endemic rates of hepatitis B virus infection.Hong Kong clinicians have developed a high level of expertise in HCC treatment across surgical,transarterial,ablative,radiotherapeutic and systemic modalities.This publication summarizes the latest evidence-based recommendations on how these modalities should be used.Methods:In two meetings held in 2020,a multidisciplinary panel of surgeons,oncologists and interventional radiologists performed a narrative review of evidence on the management of HCC,with an emphasis on treatment of HCC not amenable to surgical resection.Close attention was paid to new evidence published since the previous version of these statements in 2018.Key Content and Findings:The expert panel has formulated 60 consensus statements to guide the staging and treatment of unresectable HCC.Since the previous version of these statements,considerable additions have been made to the recommendations on use of targeted therapies and immunotherapies because of the large volume of new evidence.Conclusions:Our consensus statements offer guidance on how to select HCC patients for surgical or non-surgical treatment and for choosing among non-surgical modalities for patients who are not candidates for resection.In particular,there is a need for more evidence to aid physicians in the selection of second-line systemic therapies,as currently most data are limited to patients with disease progression on first-line sorafenib. | Tan-To Cheung Simon Chun-Ho Yu Stephen L.Chan Ronnie T.P.Poon Philip Kwok Ann-Shing Lee Anna Tai Derek Tam Chin-Cheung Cheung Tak-Wing Lai Nam-Hung Chia Ada Law Tracy Shum Yim-Kwan Lam Vince Lau Victor Lee Charing Chong Chung-Ngai Tang Thomas Yau | 2023 | Hepatobiliary Surgery and Nutrition2023，12，3： | 0 |
| 4 | Dual ultra-wideband(UWB)radar-based sleep posture recognition system:Towards ubiquitous sleep monitoring显示文摘Sleep posture monitoring is an essential assessment for obstructive sleep apnea(OSA)patients.The objective of this study is to develop a machine learning-based sleep posture recognition system using a dual ultra-wideband radar system.We collected radiofrequency data from two radars positioned over and at the side of the bed for 16 patients performing four sleep postures(supine,left and right lateral,and prone).We proposed and evaluated deep learning approaches that streamlined feature extraction and classification,and the traditional machine learning approaches that involved different combinations of feature extractors and classifiers.Our results showed that the dual radar system performed better than either single radar.Predetermined statistical features with random forest classifier yielded the best accuracy(0.887),which could be further improved via an ablation study(0.938).Deep learning approach using transformer yielded accuracy of 0.713. | Derek Ka-Hei Lai Li-Wen Zha Tommy Yau-Nam Leung Andy Yiu-Chau Tam Bryan Pak-Hei So Hyo-Jung Lim Daphne Sze Ki Cheung Duo Wai-Chi Wong James Chung-Wai Cheung | 2023 | Engineered Regeneration2023，4，1： | 0 |
