1Th17 involvement in nonalcoholic fatty liver disease progression to non-alcoholic steatohepatitis显示文摘The nonalcoholic fatty liver disease(NAFLD) is the hepatic manifestation of the metabolic syndrome. NAFLD encompasses a wide histological spectrum ranging from benign simple steatosis to non-alcoholic steatohepatitis(NASH). Sustained inflammation in the liver is critical in this process. Hepatic macrophages, including liver resident macropaghes(Kupffer cells), monocytes infiltrating the injured liver, as well as specific lymphocytes subsets play a pivotal role in the initiation and perpetuation of the inflammatory response, with a major deleterious impact on the progression of fatty liver to fibrosis. During the last years, Th17 cells have been involved in the development of inflammation not only in liver but also in other organs, such as adipose tissue or lung. Differentiation of a na?ve T cell into a Th17 cell leads to pro-inflammatory cytokine and chemokine production with subsequent myeloid cell recruitment to the inflamed tissue. Th17 response can be mitigated by T regulatory cells that secrete anti-inflammatory cytokines. Both T cell subsets need TGF-β for their differentiation and a characteristic plasticity in their phenotype may render them new therapeutic targets. 2Clinical expression of insulin resistance in hepatitis C and B virus-related chronic hepatitis:Differences and similarities显示文摘AIM:To investigate the prevalence of the clinical parameters of insulin resistance and diabetes in patients affected by chronic hepatitis C(CHC) or chronic hepatitis B(CHB) . METHODS:We retrospectively evaluated 852 consecutive patients(726 CHC and 126 CHB) who had undergone liver biopsy.We recorded age,sex,ALT,type 2 diabetes and/or metabolic syndrome(MS) ,body mass index(BMI) ,and apparent disease duration(ADD) . RESULTS:Age,ADD,BMI,prevalence of MS and diabetes in patients with mild/moderate liver fibrosis were significantly higher in CHC.However,the degree of steatosis and liver fibrosis evaluated in liver biopsies did not differ between CHC and CHB patients.At multivariate analysis,age,sex,BMI,ALT and diabetes were independent risk factors for liver fibrosis in CHC,whereas only age was related to liver fibrosis in CHB. We also evaluated the association between significant steatosis(>30%) and age,sex,BMI,diabetes,MS and liver fibrosis.Diabetes,BMI and liver fibrosis were associated with steatosis>30%in CHC,whereas only age and BMI were related to steatosis in CHB. 3Gastrointestinal and liver infections in children undergoing antineoplastic chemotherapy in the years 2000显示文摘AIM: To review gastrointestinal and liver infections in children undergoing antineoplastic chemotherapy. To look at gut microflora features in oncology children.METHODS: We selected studies published after year 2000, excluding trials on transplanted pediatric patients. We searched English language publications in MEDLINE using the keywords: 'gastrointestinal infection AND antineoplastic chemotherapy AND children', 'gastrointestinal infection AND oncology AND children', 'liver infection AND antineoplastic chemotherapy AND children', 'liver abscess AND chemotherapy AND child', 'neutropenic enterocolitis AND chemotherapy AND children', 'thyphlitis AND chemotherapy AND children', 'infectious diarrhea AND children AND oncology', 'abdominal pain AND infection AND children AND oncology', 'perianal sepsis AND children AND oncology', 'colonic pseudo-obstruction A N D o n c o l o g y A N D c h i l d A N D c h e m o t h e r a p y ', 'microflora AND children AND malignancy', 'microbiota AND children AND malignancy', 'fungal flora AND children AND malignancy'. We also analysed evidence from several articles and book references.RESULTS: Gastrointestinal and liver infections represent a major cause of morbidity and mortality in children undergoing antineoplastic chemotherapy. Antineoplastic drugs cause immunosuppression in addition to direct toxicity, predisposing to infections, although the specific risk is variable according to disease and host features. Common pathogens potentially induce severe diseases whereas opportunistic microorganisms may attack vulnerable hosts. Clinical manifestations can be subtle and not specific. In addition, several conditions are rare and diagnostic process and treatments are not standardized. Diagnosis may be challenging, however early diagnosis is needed for quick and appropriate interventions. Interestingly, the source of infectionin those children can be exogenous or endogenous. Indeed, mucosal damage may allow the penetrance of endogenous microbes towards the bowel wall and their translocation into the bloodstream. However, only limited knowledge of intestinal dysbiosis in oncology children is available. 4Management of portopulmonary hypertension:New perspectives显示文摘Portopulmonary hypertension(PPHTN)is a known complication of cirrhosis.Moderate-to-severe PPHTN implies an extremely poor prognosis.It occurs in 5%-10%of patients referred for liver transplantation(LT),and probably with an higher incidence in patients with large portosystemic shunts.Patients with moderate-tosevere pulmonary hypertension have been previously excluded from LT because of the extremely high surgical risk and since the post-transplant outcome reported was poor.Recently,new perspectives in the management of patients with portopulmonary hypertension are emerging.In fact,some pulmonary vasoactive drugs have become routine in the treatment of patients with idiopathic pulmonary hypertension.These drugs,particularly epoprostenol,have been recently introduced in the treatment of patients with PPHTN,and have been shown to be effective in reducing pulmonary artery pressure as well as pulmonary vascular resistances.Furthermore,recent studies seem to demonstrate that treatment with pulmonary vasoactive drugs could allow liver transplantation with acceptable surgical risks and excellent survival.Although there are not large series nor prospective studies addressing this topic,the clinical scenario of patients with PPHTN seems to be positively changing.Luigi Mancuso Francesca Scordato Michela Pieri Eliana Valerio Andrea Mancuso 2013World Journal of Gastroenterology2013,19,45:5

6Novel fibrin-fibronectin matrix accelerates mice skin wound healing显示文摘Plasma fibrinogen(F1)and fibronectin(pFN)polymerize to form a fibrin clot that is both a hemostatic and provisional matrix for wound healing.About 90%of plasma F1 has a homodimeric pair ofγchains(γγF1),and 10%has a heterodimeric pair ofγand more acidicγ′chains(γγ′F1).We have synthesized a novel fibrin matrix exclusively from a 1:1(molar ratio)complex ofγγ′F1 and pFN in the presence of highly active thrombin and recombinant Factor XIII(rFXIIIa).In this matrix,the fibrin nanofibers were decorated with pFN nanoclusters(termedγγ′F1:pFN fibrin).In contrast,fibrin made from 1:1 mixture ofγγF1 and pFN formed a sporadic distribution of“pFN droplets”(termedγγF1+pFN fibrin).Theγγ′F1:pFN fibrin enhanced the adhesion of primary human umbilical vein endothelium cells(HUVECs)relative to theγγF1+FN fibrin.Three dimensional(3D)culturing showed that theγγ′F1:pFN complex fibrin matrix enhanced the proliferation of both HUVECs and primary human fibroblasts.HUVECs in the 3Dγγ′F1:pFN fibrin exhibited a starkly enhanced vascular morphogenesis while an apoptotic growth profile was observed in theγγF1+pFN fibrin.Relative toγγF1+pFN fibrin,mouse dermal wounds that were sealed byγγ′F1:pFN fibrin exhibited accelerated and enhanced healing.This study suggests that a 3D pFN presentation on a fibrin matrix promotes wound healing.Carlos Poblete Jara Ou Wang Thais Paulino do Prado Ayman Ismail Frank Marco Fabian Han Li Licio A.Velloso Mark A.Carlson William Burgess Yuguo Lei William H.Velander Eliana P.Araújo 2020Bioactive Materials2020,5,4:5

8Iron Mobilization and Mineralogical Alterations Induced by Iron-Deficient Cucumber Plants (Cucumis sativus L.) in a Calcareous Soil显示文摘Dicotyledons cope with ion(Fe) shortage by releasing low-molecular-weight organic compounds into the rhizosphere to mobilize Fe through reduction and complexation mechanisms. The effects induced by these root exudates on soil mineralogy and the connections between Fe mobilization and mineral weathering processes have not been completely clarified. In a batch experiment, we tested two different kinds of organic compounds commonly exuded by Fe-deficient plants, i.e., three organic acids(citrate, malate, and oxalate)and three flavonoids(rutin, quercetin, and genistein), alone or in combination, for their ability to mobilize Fe from a calcareous soil and modify its mineralogy. The effect of root exudates on soil mineralogy was assessed in vivo by cultivating Fe-deficient and Fe-sufficient cucumber plants(Cucumis sativus L.) in a RHIZOtest device. Mineralogical analyses were performed by X-ray powder diffraction. The batch experiment showed that citrate and, particularly, rutin(alone or combined with organic acids or genistein)promoted Fe mobilization from the soil. The combinations of rutin and organic acids modified the soil mineralogy by dissolving the amorphous fractions and promoting the formation of illite. These mineralogical alterations were significantly correlated with the amount of Fe mobilized from the soil. The RHIZOtest experiment revealed a drastic dissolution of amorphous components in the rhizosphere soil of Fe-deficient plants, possibly caused by the intense release of phenolics, amino acids, and organic acids, but without any formation of illite. 10Relationship between head and neck cancer therapy and some genetic endpoints显示文摘Head and neck cancer(HNC) is the sixth most common human malignancy worldwide. The main forms of treat ment for HNC are surgery, radiotherapy(RT) and che motherapy(CT). However, the choice of therapy de pends on the tumor staging and approaches, which are aimed at organ preservation. Because of systemic RT and CT genotoxicity, one of the important side effects is a secondary cancer that can result from the activity of radiation and antineoplastic drugs on healthy cells Ionizing radiation can affect the DNA, causing single and double-strand breaks, DNA-protein crosslinks and oxidative damage. The severity of radiotoxicity can be directly associated with the radiation dosimetry and the dose-volume differences. Regarding CT, cisplatin is stil the standard protocol for the treatment of squamous cell carcinoma, the most common cancer located in theoral cavity. However, simultaneous treatment with cisplatin, bleomycin and 5-fluorouracil or treatment with paclitaxel and cisplatin are also used. These drugs can interact with the DNA, causing DNA crosslinks, double and single-strand breaks and changes in gene expression. Currently, the late effects of therapy have become a recurring problem, mainly due to the increased survival of HNC patients.
