1半胱氨酰白三烯拮抗剂治疗儿童哮喘(英文)显示文摘哮喘是一种梗阻性肺疾病,以支气管收缩、持续性气道炎症和气道重塑为特征。临床上常用吸入皮质激素和β2受体拮抗剂辅佐治疗儿童哮喘,但这些药物疗法的治疗效果不是很理想,而且不适当的大剂量皮质激素会导致骨质疏松,生长迟缓和青光眼等严重副作用。口服抗白三烯剂是一种治疗儿童哮喘的新方法。白三烯是由花生四烯酸通过5 脂肪氧化酶途径由肥大细胞、嗜酸粒细胞和肺泡巨噬细胞合成的脂质。它通过与气道平滑肌上的CysLT1受体结合参与哮喘的炎症反应而引起支气管收缩。最近的研究表明,半胱氨酰白三烯可以介导上皮下胶原沉着和平滑肌肥大增生,从而导致不可逆的气道重塑和气道阻塞。实验研究和初步的临床研究显示,白三烯拮抗剂孟鲁司特辅助治疗儿童哮喘,可以减少皮质激素和β2拮抗剂的用量,预防气道重塑、减轻哮喘症状。然而,抗白三烯药物的疗效还需通过随机、双盲和多中心的临床研究得到进一步证实。Robert KLEINMAN Marco MURA Ming-Yao LIU 2005中国当代儿科杂志2005,7,5:13
2Therapeutic and clinical aspects of portal vein thrombosis in patients with cirrhosis显示文摘Portal vein thrombosis(PVT) is a frequent complication in cirrhosis, particularly in advanced stages of the disease. As for general venous thromboembolism, risk factors for PVT are slow blood flow, vessel wall damage and hypercoagulability, all features of advanced cirrhosis. Actually, the old dogma of a hemorrhagic tendency in cirrhosis has been challenged by new laboratory tools and the clinical evidence that venous thrombosis also occurs in cirrhosis. The impaired hepatic synthesis of both pro- and anticoagulants leads to a rebalanced hemostasis, more liable to be tipped towards thrombosis or even bleeding. Conventional anticoagulant drugs(low molecular weight heparin or vitamin K antagonists) may be used in cirrhosis patients with PVT, particularly in those eligible for liver transplantation, to prevent thrombosis progression thus permitting/facilitating liver transplant. However, several doubts exist on the level of anticoagulation achieved as estimated by coagulation tests, on the efficacy of treatment monitoring and on the correct timing for discontinuation in non-transplant candidates, while in transplant candidates there is expert consensus on continuing anticoagulation until transplantation. The recent introduction of direct acting oral anticoagulant drugs(DOACs) in other clinical settings generates much interest on their possible application in patients with cirrhosis and PVT. 3Cirrhosis and portal hypertension: The importance of risk stratification, the role of hepatic venous pressure gradient measurement显示文摘Portal hypertension is the main prognostic factor in cirrhosis. The recent emergence of potent antiviral drugs and new algorithm of treatment for the management of complications due to portal hypertension have sensibly changed our perception of cirrhosis that can be now considered as a multistage liver disease whose mortality risk can be reduced by a tailored approachfor any stage of risk. Experts recommend to move toward a pathophysiological classification of cirrhosis that considers both structural and functional changes. The hepatic venous pressure gradient HVPG, is the reference gold standard to estimate the severity of portal hypertension in cirrhosis. It correlates with both structural and functional changes that occur in cirrhosis and carries valuable prognostic information to stratify the mortality risk. This article provides a general overview of the pathophysiology and natural course of cirrhosis and portal hypertension. We propose a simplified classification of cirrhosis based on low, intermediate and high mortality stage. The prognostic information provided by HVPG is presented according to each stage. 4Clinical expression of insulin resistance in hepatitis C and B virus-related chronic hepatitis:Differences and similarities显示文摘AIM:To investigate the prevalence of the clinical parameters of insulin resistance and diabetes in patients affected by chronic hepatitis C(CHC) or chronic hepatitis B(CHB) . METHODS:We retrospectively evaluated 852 consecutive patients(726 CHC and 126 CHB) who had undergone liver biopsy.We recorded age,sex,ALT,type 2 diabetes and/or metabolic syndrome(MS) ,body mass index(BMI) ,and apparent disease duration(ADD) . RESULTS:Age,ADD,BMI,prevalence of MS and diabetes in patients with mild/moderate liver fibrosis were significantly higher in CHC.However,the degree of steatosis and liver fibrosis evaluated in liver biopsies did not differ between CHC and CHB patients.At multivariate analysis,age,sex,BMI,ALT and diabetes were independent risk factors for liver fibrosis in CHC,whereas only age was related to liver fibrosis in CHB. We also evaluated the association between significant steatosis(>30%) and age,sex,BMI,diabetes,MS and liver fibrosis.Diabetes,BMI and liver fibrosis were associated with steatosis>30%in CHC,whereas only age and BMI were related to steatosis in CHB. 5Intrahepatic vascular changes in non-alcoholic fatty liver disease: potential role of insulin-resistance and endothelial dysfunction显示文摘Metabolic syndrome is a cluster of several clinical conditions characterized by insulin-resistance and high cardiovascular risk. Non-alcoholic fatty liver disease is the liver expression of the metabolic syndrome, and insulin resistance can be a frequent comorbidity in several chronic liver diseases, in particular hepatitis C virus infection and/or cirrhosis. Several studies have demonstrated that insulin action is not only relevant for glucose control, but also for vascular homeostasis. Insulin regulates nitric oxide production, which mediates to a large degree the vasodilating, antiinflammatory and antithrombotic properties of a healthy endothelium, guaranteeing organ perfusion. The effects of insulin on the liver microvasculature and the effects of IR on sinusoidal endothelial cells have been studied in animal models of non-alcoholic fatty liver disease. 6The “Brain Stress Timing” phenomenon and other misinterpretations of randomized clinical trial on aneurysmal subarachnoid hemorrhage显示文摘Clipping and coiling are currently the two alternatives in treatment of ruptured cerebral aneurysms. In spite of some meritorious analysis, further discussion is helpful to understand the actual state of art. Retreatment and rebleeding rates clearly favors clipping, although short-term functional outcome seems to be beneficial for clipping, while this different is not such if we perform the comparison at a longer follow up. Longterm follow ups and cost analysis are mandatory to have a clear view of the current picture in treatment of subarachnoid hemorrhage. 7Numerical study on fatigue crack propagation behaviors in lubricated rolling contact显示文摘The surface-initiated Rolling Contact Fatigue(RCF)including pitting and micro-pitting is one of the key issues affecting the reliability of tribological components such as gears and bearings used in various devices.In this work,a surface-initiated crack Finite Element(FE)model which considers the effect of lubricant on crack faces was developed to investigate surface-initiated RCF using an automatic crack propagating Python script.Different lubricating states,initial crack parameters and loading conditions were simulated to analyze the evolution of crack propagation and the Stress Intensity Factors(SIFs).The RCF crack propagation path and life were predicted by employing the Maximum Tangential Stress(MTS)criterion coupled with the Paris’s law.A typical RCF failure is predicted in the numerical simulation.Results reveal that the lubricating pressurization dominates the surface-initiated RCF.In addition,the initial crack angle has a significant effect on the RCF crack propagation path and the fatigue life.Haifeng HE Huaiju LIU Caichao ZHU Andrea MURA 2021Chinese Journal of Aeronautics2021,34,9:4
8Use of non-selective beta blockers in cirrhosis:The evidence we need before closing(or not) the window显示文摘Non selective beta blockers(NSBBs)are used in primary and secondary prophylaxis of portal hypertensionrelated bleeding in patients with cirrhosis.The efficacy of NSBBs treatment is predicted by hemodynamic response in term of reduction of the hepatic venouspressure gradient(HVPG)below 12 mm Hg or at least20%of the basal value.Nevertheless a relevant number of patients who do not achieve this HVPG reduction during NSBBs therapy do not bleed during follow up;this evidence suggests an additional non-hemodynamic advantage of NSBBs treatment to modify the natural history of cirrhosis.Recent studies have questioned the efficacy and safety of NSBBs in patients with advanced stage of liver disease characterized by refractory ascites and/or spontaneous bacterial peritonitis.These studies have suggested the existence of a defined and limited period to modify the natural history of cirrhosis by NSBBs:the'window hypothesis'.According with this hypothesis,patients with cirrhosis benefit from the use of NSBBs from the appearance of varices up to the development of an advanced stage of cirrhosis.Indeed,in patients with refractory ascites and/or spontaneous bacterial peritonitis the hemodynamic effects of NSBBs may expose to a high risk of further complications such as renal insufficiency and/or death.Methodological concerns and contrasting results counterbalance the evidence produced up to now on this issue and are the main topic of this editorial.Vincenzo La Mura Giulia Tosetti Massimo Primignani Francesco Salerno 2015World Journal of Gastroenterology2015,21,8:4

12Evidence to suggest adoption of water exchange deservesbroader consideration:Its pain alleviating impact occurs in90%of investigators显示文摘AIM: To determine variations in colonoscopy real-time insertion pain among investigators using three different insertion techniques.METHODS: From March 2013 through June 2014, 18-85-year-old diagnostic and 50-70-year-old screening patients were enrolled at each center to on-demand sedation colonoscopy with water exchange(WE), water immersion(WI) and insufflation with air or CO2 for insertion and withdrawal [air or carbon dioxide(AICD)]. Data were aggregated for analysis. Primary outcome: Variations in real-time maximum insertion pain(0 = none, 1-2 = discomfort, 10 = worst).RESULTS: One thousand and ninety-one cases analyzed: WE(n = 371); WI(n = 338); AICD(n = 382). Demographics and indications were comparable. The WE group had the lowest real-time maximum insertion pain score, mean(95%CI): WE 2.8(2.6-3.0), WI 3.8(3.5-4.1) and AICD 4.4(4.1-4.7), P < 0.0005. Ninety percent of the colonoscopists were able to use water exchange to significantly decrease maximum insertion pain scores. One investigator had high insertion pain in all groups, nonetheless WE achieved the lowest real-time maximum insertion pain score. WE had the highest proportions of patients with painless unsedated colonoscopy(vs WI, P = 0.013; vs AICD, P < 0.0005); unsedated colonoscopy with only minor discomfort(vs AICD, P < 0.0005), and completion without sedation(vs AICD, P < 0.0005).CONCLUSION: Aggregate data confirm superiority of WE in lowering colonoscopy real-time maximum insertion pain and need for sedation. Ninety percent of investigators were able to use water exchange to significantly decrease maximum insertion pain scores.
