| 3 | Impact of enclosure management on soil properties and microbial biomass in a restored semi-arid rangeland, Kenya显示文摘Rangeland degradation is a serious problem throughout sub-Saharan Africa and its restoration is a challenge for the management of arid and semi-arid areas. In Lake Baringo Basin of Kenya, communities and individual farmers are restoring indigenous vegetation inside enclosures in an effort to combat severe land degradation and address their livelihood problems. This study evaluated the impact of enclosure management on soil properties and microbial biomass, being key indicators of soil ecosystem health. Six reseeded communal enclosures using soil embankments as water-harvesting structures and strictly regulated access were selected, varying in age from 13 to 23 years. In six private enclosures, ranging from 3 to 17 years in age, individual farmers emulated the communal enclosure strategy and restored areas for their exclusive use. Significant decreases in bulk density, and increases in the soil organic carbon, total nitrogen and microbial biomass contents and stocks were found in the enclosures as compared with the degraded open rangeland. In the private enclosures, the impact of rehabilitation on the soil quality was variable, and soil quality was in general lower than that obtained under communal management. The significant increase of absolute stocks of carbon, nitrogen and microbial biomass compared to the degraded open rangeland indicates the potential for the restoration of soil quality through range rehabilitation. | 4 | Developing a donation after cardiac death risk index for adult and pediatric liver transplantation显示文摘AIM To identify objective predictive factors for donor after cardiac death(DCD) graft loss and using those factors, develop a donor recipient stratification risk predictive model that could be used to calculate a DCD risk index(DCD-RI) to help in prospective decision making on organ use.METHODS The model included objective data from a single institute DCD database(2005-2013, n = 261). Univariate survival analysis was followed by adjusted Cox-regressional hazard model. Covariates selected via univariate regression were added to the model via forward selection, significance level P = 0.3. The warm ischemic threshold was clinically set at 30 min. Points were given to each predictor in proportion to their hazard ratio. Using this model, the DCD-RI was calculated. | 9 | 策略将在肝移植以后减少丙肝病毒复发显示文摘 Hepatitis C virus (HCV) is a major health problem that leads to chronic hepatitis, cirrhosis and hepatocellular carcinoma, being the most frequent indication for liver transplantation in several countries. Unfortunately, HCV re-infects the liver graft almost invariably following reperfusion, with an accelerated history of recurrence, leading to 10%-30% of patients progressing to cirrhosis within 5 years of transplantation. In this sense, some groups have even advocated for not retransplanting this patients, as lower patient and graftoutcomes have been reported. However, the management of HCV recurrence is being optimized and several strategies to reduce post-transplant recurrence could improve outcomes, decrease the rate of re-transplantation and optimize the use of available grafts. Three moments may be the focus of potential actions in order to decrease the impact of viral recurrence: the pretransplant moment, the transplant environment and the post-transplant management. In the pre-transplant setting, it is not well established if reducing the pre transplant viral load affects the risk for HCV progression after transplant. Obviously, antiviral treatment can render the patient HCV RNA negative post transplant but the long-term benefit has not yet been fully established to justify the cost and clinical risk. In the transplant moment, factors as donor age, cold ischemia time, graft steatosis and ischemia/reperfusion injury may lead to a higher and more aggressive viral recurrence. After the transplant, discussion about immunosuppression and the moment to start the treatment (prophylactic, pre-emptive or once-confirmed) together with new antiviral drugs are of interest. | 10 | Paediatric retinal detachment:aetiology,characteristics and outcomes显示文摘AIM: To provide contemporary data on the aetiology, clinical features and outcomes of paediatric retinal detachment.METHODS: A retrospective review of all those under 16 y who underwent surgical repair for retinal detachment at a single centre between the years 2008 and 2015 inclusive was performed. In each case the cause of retinal detachment, the type of detachment, the presence or absence of macular involvement, the number and form of reparative surgeries undertaken, and the surgical outcome achieved was recorded.RESULTS: Twenty-eight eyes of 24 patients, 15(62.5%) of whom were male and 9(37.5%) of whom were female, their mean age being 11.6 y and range 2-16y developed retinal detachment over the eight year period studied. Trauma featured in the development of retinal detachment in 14(50.0%) cases. Retinal detachment was associated with other ocular and/or systemic conditions in 11(39.3%) cases. A mean of 3.0 procedures with a range of 1-9 procedures per patient were undertaken in the management of retinal detachment. Complex vitrectomy combined with scleral buckling or complex vitrectomy alone were those most frequently performed. Mean postoperative visual acuity was 1.2 log MAR with range 0.0-3.0 log MAR. In 22 of 26(84.6%) cases which underwent surgical repair the retina was attached at last follow-up.CONCLUSION: Aggressive management of paediatric retinal detachment including re-operation increases the likelihood of anatomical success.
